Design Features and Operating Characteristics of the MC-50 Cyclotron (MC-50 싸이클로트론의 설계 특징과 동작 특성)

  • The MC-50 cyclotron at Korea Canter Center Hospital is now operational for neutron therapy and medical radioisotope production. Design features, mechanical structures and operating characteristics of the MC-50 are described in this paper. Optimum operating condition for this cyclotron has been determined by the repetitive running, and the performances of the internal beam have been investigated through the measurements of intensity and spatial distribution of the internal beam as a function of the radius of the cyclotron. Routinely, the 40 $\mu$A of 50 MeV protons have been obtained at first Faraday cup with a extraction efficiency of 61%.

Endoscopic features aiding the diagnosis of gastric mucosa-associated lymphoid tissue lymphoma

  • The incidence of gastric mucosa-associated lymphoid tissue (MALT) lymphoma is increasing worldwide, but the diagnosis is difficult. Most patients are asymptomatic or complain of nonspecific gastrointestinal symptoms. As the endoscopic features of gastric MALT lymphoma are variable and nonspecific, the possibility of this condition may be overlooked during esophagogastroduodenoscopy, and it remain undiagnosed. Therefore, this condition needs to be considered when an abnormal mucosa is observed during this procedure. Biopsy performed during endoscopy is the primary diagnostic test, but false negative results are possible; large numbers of samples should be collected from both normal and abnormal mucosae. Endoscopic ultrasonography is useful to assess the depth of invasion and to predict the treatment response. After treatment, follow-up tests are required every 3 months until complete remission is achieved, and annually thereafter. Early diagnosis of gastric MALT lymphoma is difficult, and its diagnosis and follow-up require wide experience and competent endoscopic technique.

Computed tomography and magnetic resonance imaging features of suspected transitional cell carcinoma lesions involving the bladder, prostate, and urethra in a dog: a case report

  • A 14-year-old, spayed female, poodle was presented with dysuria and hematuria. A mass that appeared hypoechoic on ultrasound and hypoattenuating on computed tomography (CT) extended from the bladder neck to the urethra. Magnetic resonance imaging (MRI) showed the mass invading the muscular layer of the bladder, urethra, and prostate with distinct margins. Transitional cell carcinoma (TCC) was confirmed with the CADET-BRAF test. This study describes the CT and MRI features of suspected TCC lesions involving the bladder, prostate, and urethra. MRI showed superior soft tissue contrast resolution, enabling evaluation of invasion of the muscular layer of the bladder and urethra.

  • Hypersensitivity pneumonitis (HP) is an inflammatory and fibrosing interstitial lung disease caused by inhaled antigens in susceptible individuals. Various environmental materials can serve as inciting agents of HP. Recent guidelines have defined typical radiologic and histopathologic findings of HP, classifying it into nonfibrotic and fibrotic HP categories, as this classification better correlates with clinical outcomes. Diagnosing HP poses challenges, necessitating multidisciplinary discussions based on clinical assessments and radiologic or histopathological features. The cornerstone of treatment lies in avoiding the causative antigen. Oral glucocorticoids serve as the initial treatment for symptomatic HP. Immunosuppressants may be employed in patients who do not respond to initial treatment, and antifibrotics could be considered for patients displaying progressive fibrosis. This review outlines the clinical features, diagnostic evaluation, and treatment of HP.

Comparative Study on the Long-Acting Thyroid Stimulator in Graves' Disease (Graves 병(病)에 있어 Long-Acting Thyroid Stimulator(LATS)에 관(關)한 연구(硏究))

  • In order to study the role of LATS in the pathogenesis of the Graves' disease, the serum activity of the LATS was determined by the bioassay of the modified McKenzie method. The subjects examined in the study consisted of 76 individuals including 12 cases of normal control, 54 cases with typical Graves' disease and 10 cases of chronic thyroiditis. The data observed in the patients with the Graves' disease were analyzed in comparison with the clinical features, laboratory findings, and responsiveness to the treatment. The results obtained are as follows: 1. None of the subjects which did not have the Graves' disease showed a positive LATS activity, except one case with the chronic thyroiditis. 2. Twenty-two out of the 54 cases with the Graves' disease showed positive results for LATS(40.7%). The positivity was significantly higher in the patients who had been treated with anti. thyroid regimen but still showed hyperthyroidism than in the patients who had not been treated. 3. The activity of LATS was gradually decreased or even became absent as the hyperthyroidism was corrected after the treatment. 4. No significant difference was noticed in age & sex between the positive and negative groups of LATS. 5. There was no evidence of significant correlation between the LATS activity and clinical features.

Acute disseminated encephalomyelitis caused by Epstein-Barr virus infection in an immunocompetent adult woman

  • Epstein-Barr virus(EBV) infection is common and usually asymptomatic in young infants and children. However, EBV infections in transplant recipients and other immunosuppressed patients can be fatal. EBV-related neurological complications in immunocompetent adults are extremely rare and self-limited. Acute disseminated encephalomyelitis(ADEM) may also follow EBV infection; ADEM is characterized by abrupt onset and rapid progression. We report an immunocompetent adult patient who developed diffuse meningoencephalitis with ADEM-like features caused by EBV infection. A 35-year-old Vietnamese woman was admitted presenting with urinary retention, altered mental status, and paraplegia. PCR of the patient's cerebrospinal fluid showed positive results for EBV. Brain and spine magnetic resonance imaging showed ADEM-like features. She was treated with acyclovir, steroid, and immunoglobulins. We report the case of an immunocompetent adult Vietnamese woman who presented with rapidly progressive diffuse meningoencephalitis associated with EBV infection and was treated with antivirals, corticosteroids, and immunoglobulins.

Fabrication of Microfibrous Structures with Rolled-Up Forms using a Bilayer Self-Assembly Process (이중층 자가조립 공정을 활용한 롤형태의 생체의료용 마이크로섬유 구조체 제작)

  • Numerous fabrication techniques have been used to mimic cylindrical natural tissues, such as blood vessels, tendons, ligaments, and skeletal muscles. However, most processes have limitations in achieving the biomimetic properties of multilayered and porous architectures. In this study, to embrace both features, a novel self-assembly method was proposed using electrospun microfibrous sheets. A bilayer microfibrous structure, comprising two sheets with different internal stresses, was fabricated by electrospinning a polycaprolactone (PCL) sheet on a uniaxially stretched thermoplastic polyurethane (TPU) sheet. Then, by removing the stretching tension, the sheet was rolled into a hollow cylindrical structure with a specific internal diameter. The internal diameter could be quantitatively controlled by adjusting the thickness of the PCL sheet against that of the TPU sheet. Through this self-assembly method, biomimetic cylindrical structures with multilayer and porous features can be manufactured in a stable and controllable manner. Therefore, the resulting structures may be applied to various tissue engineering scaffolds, especially vascular and connective tissues.

Bilateral Ovarian Metastases from ALK Rearranged Non-Small Cell Lung Cancer

  • Anaplastic lymphoma kinase (ALK) rearrangement, is a kind of driver mutation, accounts for 3%-5% of non-small cell lung cancer (NSCLC). NSCLC patients harboring ALK fusion genes have distinct clinical features and good response to ALK inhibitors. Metastasis from lung cancer to the ovary has rarely been known. We report a case of a 54-year-old woman with bilateral ovarian metastases from ALK rearranged NSCLC. She underwent bilateral salpingo-oophorectomy for ovary masses, which were progressed after cytotoxic chemotherapy although primary lung mass was decreased. Histopathological examination of the ovary tumor showed characteristic adenocarcinoma patterns of the lung and ALK rearrangement.

Influence of Internal Resonance on Responses of an Autoparametric Vibration Absorber under Random Excitation (불규칙 가진력을 받는 동흡진기의 내부공진효과)

  • The main objectives of this study are to examine the random response of a vibration absorber system with autoparametric coupling in the neighborhood of internal resonance by Gaussian closure and to compare the results with those obtained by Monte Carlo simulation. The numerical simulation is found to support the main features of the nonlinear modal interaction in the neighborhood of internal resonance conditions. While the Gaussian closure exhibits regions of multiple solutions in the neighborhood of internal resonance, the numerical simulation gives only one solution depending on the assigned initial conditions. The on-off intermittency phenomena of the cantilever mode is observed in the Monte Carlo simulation over a small range of parameter.

The Observational Evidence for the Internal Excitation of Umbral Velocity Oscillations

  • The umbral oscillations of velocity are commonly observed in the chromosphere of a sunspot. Their sources are considered to be either the external p-mode driving or the internal excitation by magnetoconvection. Even though the possibility of the p-mode driving has been often considered, the internal excitation has been rarely investigated. We report the observational evidence for the internal excitation obtained by analyzing velocity oscillations in the temperature minimum region of a sunspot umbra. The velocity oscillations in the temperature minimum region were determined from Fe I $5435{\AA}$ line data taken by the Fast Imaging Solar Spectrograph (FISS) of the 1.6 m Goode solar Telescope (GST) at the Big Bear Solar Observatory. As a result, we discovered 4 events of oscillations which appear to be internally excited. We analyze their characteristics and relation to photospheric features. Based on these results, we estimate the contribution of the internal excitation for umbral oscillations and discuss their importance.
