Korean government is preparing the long-term care financing and delivery system in order to cope with rapid population aging. The system should be designed to provide demented patients with an appropriate services that the patients want to take, and considered to be necessary for them. In this regard, this study aims to analyse empirically a relationship between the types of long-term care services that demented patients wanted to take and they actually received during 2004. The caregivers of 609 dementia patients, who were randomly selected in a manner of proportional allocation from a nationwide claim database of the Korean National Health Insurance Corporation, were interviewed in September, 2005. Independent variables include socio-demographic characteristics, Activities of Daily Living(ADL) and Instrumental Activities of Daily Living(IADL). To explore the correspondence of the types of long-term care services that demented patients wanted to take and that they actually received, and its affecting factors, we conducted chi-square test and logistic regression analysis. Main findings are as follows. First, while only 20% of study subjects wanted home services as a long-term care services, those who wanted to use the long-term care facilities and general hospital were 37%, 43% respectively. Second, the correspondence rate was just 38% on average, and extremely low in the demented patients who wanted to use long-term care facilities. Third, the demented patients who resided in urban areas and received relatively high level of education showed high correspondence rate. Fourth, the high ADL score was closely related to low correspondence rate.

Despite its universal coverage of health insurance, the rural health insurance program(RHIP) stands at the crossroads in Korea. The RHIP has weaknesses in stability of financing, problems of inequities in the provision of health services and has suffered from high cost of running the program. The author has analyzed these problems from the perspective of health insurance policy and presented several options for improvement. First of all, this study urged the importance of a firm Governmental commitment of RHIP with the 50% subsidization of contributions as the Government had promised, instead of the current 40%. This can be justified from the 20% subsidization by the Government for the contributions of private school teachers and their dependents, who belong to richer segments of the population. Second, various cost containment measures ought to be sought curbing the rising demand for medical through strengthening health education and increasing individual responsibility, and tightening the claim review process. Third, this study requires the Government to run a demonstration project on the introduction of case payment system for primary health care. Fourth introducing an income-related cost sharing scheme is another possibility. Reforming the cost sharing formula for large medical expenditures is recommendable for a beginning. This measure can take the form of tax credit for medical expenditures of the poor. Fifth, the degree of financial adjustment among health insurance plans should be levelled up for enhancing stability of RHIP and social solidarity. Sixth, health policy should be redirected toward development of rural health resources and higher priority should be put on relieving difficulties in access to care. Seventh. the insurance plan owned-hospital needs to be developed or provision of health services in the medically underserved areas, and the need of such facilities is particularly acute for geriatric care, rehabilitation and renal dialysis, etc. Eighth, more generous insurance benefits are required of the elderly who are suffering the most : elimination of the maximum 180 days of benefit period and provision of glasses and artificial dentures, etc. Ninth. the economies of scale principle is working for the operating expenses of regional self-employed insurance plan. Thus, measures should be instituted to pursue an optimum size of health insurance plans. Lastly, excessive dependence on exclusion items is an evil so that some radical remedies are urgently required to cut them.

This study investigates factors affecting the number of insurance solicitor's turnovers of life insurance companies based on questionnaire about them. Since the response variable which is the number of insurance solicitor's turnovers is count data, it is analyzed by Poisson regression which is one of generalized regression. When work year in current company, which is direct influential factor on the number of insurance solicitor's turnovers, is controlled, affiliated corporation has been found to be the most influential factor. In addition, age, motivation to work as financial planner, monthly income, a number of average new contract per month, and final education have been identified to be important factors. If insurance solicitor's occupant organization is large company, the number of turnovers becomes small, but if the organization is general agent(GA), it becomes larger. When insurance solicitor's age is high, the number of insurance solicitor's turnovers are reduced. If the motivation to become a financial planner is due to acquaintance such as family and relatives, the number of turnovers becomes small.
This study focused on the change of environment and cognition of dental hygienists about dental calculus removal after the national health insurance. We conducted online and offline surveys for 290 dental hygienists working in dental clinics in Seoul, Gyeonggi and Incheon areas. Differences in perceptions were assessed by independent t-test and ANOVA. 62.1% answered that the health insurance coverage of dental calculus removal was appropriate, and 49.6% said that the desired number of health insurance coverage about the dental calculus removal should be applied twice a year. 54.3% said that the age after 20 years-old was not appropriate in national health insurance coverage of dental calculus removal, and 49.3% said that the appropriate starting age of dental calculus removal should be applied from high school students. 26.3% said that the number of national health insurance applications should be increased yearly, 20.5% said that oral care education should be added. Most of the dental hygienists said that the number of scaling patients increased, but that the quality of the scaling did not deteriorate. According to general characteristics, in the recognition of the removal of calculus, the dental hygienists having a career for 7~8 years felt less change. The dental hygienist wanted to expand the scope of national health insurance about scaling removal so that more subjects could remove dental calculus removal. Dental hygienists wanted that national health insurance should be systematically supplemented in order to contribute to the promotion of oral health of the people.

Objectives: This study was conducted to investigate the incidence rate of noise-induced hearing loss (NIHL) in Korean youths using the National Health Insurance Data Methods: We obtained the case of NIHL with the National Health Insurance Data and calculated the incidence rate of NIHL after classifying them by year (from 2005 through 2008), age, gender, birth cohort. Results: The age-adjusted incidence rate by year of NIHL per 100,000 population increased by 40.6% from 13.8 persons in 2005 to 19.4 persons in 2008, which showed a increasing trend every year. In particular, the incidence rate per 100,000 population among the adolescents (15-19 years of age) shot up by 78.2% from 8.7 persons in 2005 to 15.5 persons in 2008. According to the incidence rate by birth cohort of NIHL, the birth cohorts showed the gradually increase in incidence rate as time passed by. Adolescents in early 20s (20-24 years of age) had a higher incidence rate than teenage and men had a higher incidence rate than women. Conclusions: The results suggest that incidence rate of NIHL in Korean youths has rapidly increased among adolescents. Therefore, it is required to educate adolescents about the risk of hearing loss due to noise, establish program to prevent NIHL among adolescents and provide them with health education on hearing loss prevention.
This study deals with characteristics and risk of a PM based on user survey result, road driving test and data analysis of PM accident. Text mining method is applied to extract PM accident data from Big Data, which are claim data of private insurance company. Road driving test and survey on safety, convenience, noise, overtake ability, steering ability, and climbing ability of PM are performed to evaluate user's safety and convenience considering domestic road condition. As the result of claim data analysis, annual average increase rate of PM accident is 47.4% and average compensation of personal mobility is higher than that of bicycle by maximum 1.5 times. 79.8% of PM accident is self-caused accident due to unskilled driving and age-specific diagnosis rate of driver over 60 is higher than that of under 60. Diagnosis rate of over 60 at lower limb, foot, rib and spine is especially higher than that of under 60. As the result of road driving test and user survey, satisfaction level on safety and convenience of PM is evaluated as close to that of bicycle and satisfaction level of PM is increased after boarding. Overtake ability, steering ability, and climbing ability of PM are evaluated as same or better than that of bicycle but warning equipment to pedestrian or bike such as horn is required because noise level of PM during driving is too low. Finally, user survey result shows that bicycle road is suitable for PM and safety standard, advance-education and insurance are required for PM. It is suggested that drivers' license for PM can be replaced by advance-education. Results of this study can be used to prepare safety measures and legal basis for PM operation.
The ageing problem of the population has been emerging in Korea since 1970's so that it is expecting the elderly 65 years and over among the total population from $4.5\%$ in 1988 to be $6.3\%$ in year 2000. This study was conducted to provide secure policy development in coming years for the aged on medical and health care aspects based on the examining current status of the aged problems and health care policies and systems. The study divided into four parts; The first part examined the medical insurance program and public assistance program of the health services in relation to the aged. The second part emphasized on reduction of medical care cost for the aged. The third part studied the regular health check-up program and health education for the aged. The fourth part examined the chronic disease management programs for the aged and strategies of the health care service quality improvement and specialized programs. The following recommendations made as the results of the study. 1. At present, the medical insurance program and public assistance program for the medically indigent is not appropriate to the elderly because it is a part of general medical insurance program so that Health Security Law for the Aged is proposed. 2. Medical cost will be increased due to the high occupancy rate of hospital beds and long stay of the elderly so that it is recommended to develop an early discharge program, home health care program, Health hospice and an althernative programs. 3. At present, a regular health check-ups for the elderly is not included in medical insurance program so that it is recommended to be included in the insurance program and at the same time health education program thoroughly developed for the aged. 4. To make proper medical and nursing services on chronic diseases for the elderly, it is recommended manpower development, specialized clinics or hospitals, nursing homes and an equivalent long term care facilitices should be established on the community based and a research institutions also to be related to supper the care programs.

This paper reviews the uncovered people of the Korean Employment Insurance System (EIS) and analyzes policy options for minimizing the dead zone of the EIS. There are several policy options such as subsidizing insurance premium to employers and employees of small companies, extending coverage of excluded groups, relaxing qualifications of unemployment benefits and increasing benefit period and level, introducing the unemployment assistance system, introducing the unemployment insurance savings account system, extending coverage to non-wage workers and individualizing package services. According to the survey to the specialists and comparative evaluation criteria, the best policy option to minimize the dead zone of the EIS was to activate individualizing package services of intensive consultation, job place services, tailored vocational training, income support, daycare services, etc. to cure complex employment barriers of job seekers.

Purpose: The purposes of this study were to examine the effects of the Cooperative Clinical Nursing Education Program on the competency and satisfaction of nursing students in clinical practice. Method: The research design was a quasi-experimental pretest-posttest design with a convenience sample of 107 nursing students who were at one College of Nursing located in Seoul. Data were collected using a self-administered questionnaire. Results: Nursing students participated in the program reported higher nursing skills and total satisfaction than control group after completing education program. Conclusion: Cooperative clinical nursing education program for nursing students had positive effects on nursing skills and satisfaction of clinical practice.
Objectives: The purpose of the study was to find determinants for participation in the National health screening program participation according to sex in Korean. Methods: Data for 5,355 from the Korea National Health and Nutrition Examination Survey VI was included. Impact factors were analyzed by sex using logistic regression. Results: The participation in men rates(65.6%) higher than women(60.6%). In the case of men, those who have higher age (OR=0.33, 95% CI 0.21-0.52), higher income(OR=0.72, 95% CI 0.54-0.97), higher education (OR=0.54, 95% CI 0.36-0.81), occupation(OR=1.80, 95% CI 1.38-2.36), spouse(OR=1.95, 95% CI 1.45-2.62) were shown to have health examination more frequently. In the case of women, those who live in more developed city(OR=1.42, 95% CI 1.1-1.76), have higher age(OR=0.17, 95% CI 0.11-0.26), higher income(OR=0.17, 95% CI 0.11-0.26), higher education(OR=0.68, CI 0.46-0.99), occupation(OR=1.54, CI 1.32-1.80) and health insurance(OR=2.68, 95% CI 1.17-6-15) were shown to have health examination more frequently insurance were shown to have health examination more frequently. Conclusions: As a result, for both, age, income, education and occupation influenced health examination rate. Additionally, for men, marital status and for women, residence and the type of their health insurance was influenced. Individualized intervention is needed to increase participation rate.
