Clinical Features and the Risk Factors of Distant Metastasis in Locally Advanced Advanced Head and Neck Cancer Patients after Induction Chemotherapy Followed Locoregional Control Therapy (유도화학요법 및 국소 치료 후 원격전이를 보인 국소 진행성 두경부암 환자군의 임상 특징 및 위험인자에 관한 연구)

  • Backgrounds : Head and neck cancer is one of the most prevalent cancers in the world. It tends to remain localized at the primary site and regional lymph nodes, but if distant metastasis occurs, it has a poor prognosis. This study was performed to evaluate the prevalence of distant metastasis and to determine the risk factor in locally advanced head and neck cancer after induction chemotherapy followed locoregional control therapy. Methods : A retrospective review was performed in 420 patients with locally advanced head and neck cancer who treated with induction chemotherapy followed locoregional control therapy from January 2001 to December 2010. Among them, 31 patients who had distant metastasis as first relapse within 2 years after termination of therapy were analyzed for clinical features and the risk factors of distant metastasis. Results : The overall incidence of distant metastasis was 7.3%. The bone, lung, and liver were the most frequent metastatic organs. In univariate analysis, nodal stage, nasopharyngeal cancer, laryngeal cancer, G3/G4 neutropenia during induction chemotherapy, and concurrent chemoradiotherapy were the influencing factors for distant metastasis. In multivariate analysis, advanced N stage and nasopharynx were the risk factors of distant metastasis, and grade 3/4 neutropenia during induction chemotherapy was considered to decrease distant metastasis. Conclusion : This study suggests that the advanced N stage is the risk factor of distant metastasis and Grade 3/4 neutropenia during induction chemotherapy can be beneficial against distant metastasis in locally advanced head and neck cancer patients treated with induction chemotherapy followed locoregional control therapy.

  • Current immunosuppressants have nonspecific immuosuppressive effects, and are not helpful for tolerance induction. Consequently, transplant patients cannot discontinue using them, and their nonspecific immunosuppressive effects result in many side effects, including infection and malignancy. However, most of cellular immunotherapy can have donor antigen-specific immunsuppressive effects. Therefore, cell therapy could be an alternative or adjunctive to nonspecific immunosuppressants. Polyclonal or antigen-specific Foxp3+ regulatory T cells have been actively tried for prevention of acute rejection, treatment of chronic rejection, or tolerance induction in clinical trials. Regulatory macrophages are also under clinical trials for kidney transplant patients. IL-10-secreting type 1 regulatory T cells and donor- or recipient-derived tolerogenic dendritic cells will also be used for immunoregulation in clinical trials of kidney transplantation. These cells have antigen-specific immunoregulatory effects. Mesenchymal stromal cells (MSCs) have good proliferative capacity and immunosuppressive actions independently of major histocompatibility complex; therefore, even third-party MSCs can be stored and used for many patients. Cell therapy using various immunoregulatory cells is now promising for not only reducing side effects of nonspecific immunosuppressants but also induction of immune tolerance, and is expected to contribute to better outcomes in transplant patients.

The Effects of Task-Oriented Training on Motor and Cognitive Function in Focal Ischemic Brain Injury Model of Rat

  • The purpose of this study was to investigate the effects of the task-oriented training according to the application time with the change of motor and cognition function. Focal ischemic brain injury was produced in Sprague-Dawley rats (20 rats, $250{\pm}50$ g) through middle cerebral artery occlusion (MCAo). Before MCAo induction, all rats were trained in treadmill training and Morris water maze training for 1 week. Then they were randomly divided into groups: Group I : MCAo induction ($n_1$=5), Grop II: the application for simple treadmill task training after. MCAo induction ($n_2$=5). Group III: the application for Morris water maze cognitive task training after MCAo induction ($n_3$=5). Group IV: the application for progressive treadmill task training and Morris water maze cognitive task training after MCAo induction ($n_4$=5). Modified limb placing tests (MLPTs) and motor tests (MTs) were performed to test motor function and then Morris water maze acquisition test (MWMAT) and Morris water maze retention test (MWMRT) were performed to test cognitive function. For MTs, there were significant interactions among the groups with the time (p<.001). Group IV showed the steeper increasing pattern than those in other Groups on the 7th and 14th day. For MLPTs, there were significant interactions among the groups with the time (p<.001). The scores in Group III. IV had showed the more decreasing pattern than those in Group I, II since the 7th day and 14th day. For MWMAT, there were significant interactions among the groups with the time (p<.001). Group II found the Quadrant circular platform showed the steeper decreasing pattern than that in Group I on the 9th, 10th, 11th and 12th day. Group III. IV found the quadrant circular platform showed the slower decreasing pattern than that in Group I, II, For MWMRT, there were significant differences among the four groups (p<.001). The time to dwell on quadrant circular platform in Group IV on the 13th day was the longest compared with other groups. These results suggested that the combined task training was very effective to improve the motor and cognition function for the rats affected on their focal ischemic brain injury.

Treatment Outcome after Induction Chemotherapy Followed by Radiation Therap in Locally Advanced Hypopharyngeal Cancer (진행된 하인두암에서 유도항암화학요법 후 방사선치료의 결과)

  • Purpose:To know the results after induction chemotherapy followed by curative radiation therapy for locally advanced hypopharyngeal cancer Methods and Materials:From August 1990 to December 2003, forty patients who were treated with induction chemotherapy and curative radiation therapy were analyzed retrospectively. Median age of patients was 60 years(range:40-78 years) and clinical stage was wholly stage 3 or 4. Induction chemotherapy used cisplatin with 5-FU or docetaxel, and its interval was 3 weeks. Irradiated radiation dose was 70 to 78Gy (median:70.8Gy). Results:Median follow-up time was 39.4 months(range:8-115 months). Treatment failures were observed in 52.5% patients, and main failure pattern was local recurrence in 16 patients. 3 and 5 year disease free survival were 52.6%, 48.2% respectively and values of overall survival were 60.0, 43.9% and median survival time was 44.7 months. Treatment response was only a prognostic factor for survival. Laryngeal preservation was observed in twenty-four(60.0%) patients. Conclusion:Initial primary tumor stage was a significant prognostic factor for laryngeal preservation, and response after radiation therapy was a prognostic factor for long-term survival after induction chemotherapy followed by curative radiation therapy for locally advanced hypopharyngeal cancer.

Effect of Induction Chemotherapy Followed by Radiation Therapy of Locally Advanced Hypopharyngeal Carcinoma (국소진행된 하인두암에서 선행 항암화학요법과 방사선치료의 효과)

  • Purpose:Hypopharyngeal carcinoma is usually diagnosed as an advanced disease after an asymptomatic beginning, and it is related to a high frequency of lymph node metastases. An eventual negative outcome may occur not only because of possible locoregional failures but also for frequent distant metastases. Thus, the efficacy of induction chemotherapy followed by radiation therapy, with regards to the response, survival rate and complications for locally advanced hypopharyngeal carcinoma patients, was examined. Methods and Materials:Since July 1998 to February 2001, 18 patients having locally advanced hypopharyngeal carcinoma were treated with induction chemotherapy followed by radiation therapy, and the results were retrospectively analyzed. The regimen of the induction chemotherapy was the 5-flurouracil(5-FU, 1,000mg/$m^2$ daily for 5 consecutive days) and cisplatin(100mg/$m^2$ on day 1) combination at 3-week intervals for 2 cycles. The total radiation dose for the primary tumor and metastatic lymph nodes was 68.4-72.0Gy(median:70.2Gy) Results:The 3-year overall survival rate and disease free survival rate were 31.3% and 22.2%, respectively. In 6 patients(33.3%), preservation of the larynx for over 3 years was possible. After the induction chemotherapy and radiotherapy, a complete response was noted in 14 patients(77.8%), and a partial response in 4 patients (22.2%), with an overall response rate of 100%. Conclusion:Induction chemotherapy followed by radiation therapy is an effective treatment and larynx preservation rate was 33% in patients with locally advanced hypopharyngeal carcinoma in our report.

  • The purpose of this study was to investigate the spontaneous therapy, TENS(transcutaneous electrical nerve stimulation) and ice therapy after induction of DOMS(delayed onset muscle soreness) through application of eccentrically exercise in ralation to biceps brachii muscle of opposite-dominance. And to explain this, we analyzed VAS(visual analogue scales), peak torque, RMS(roots mean square) when exercising elbow joint. The subjects were 30 adults without any physical defect. Subjects were divided into three groups that spontaneous therapy, TENS therapy and ice therapy after DOMS induction, choosen movement could get the maximal peak torque in angular velocity of $60^{\circ}$/sec, and mearsured peak torque, RMS and VAS. 1. The VAS was significantly decreased after 3 days application of TENS than other groups(p<.05). 2. The peak torque was significantly increased after 3 days application of TENS than other groups(p<.05). 3. The RMS was significantly increased after 3 days application of TENS than other groups(p<.05). This study showed that TENS therapy application were effective treatment strategy on DOMS than spontaneous therapy and ice therapy. Therefore, it could be considered as a treatment method in the patients with DOMS.

Effect of Percutaneous Electrical Nerve Stimulation on the Spinal Neuron Excitability in STZ-induced Diabetic Rats. (피하신경전기자극이 STZ-유도 당뇨 쥐의 척수신경원 흥분성에 미치는 효과)

  • This study aimed at examining the effects of percutaneous electrical nerve stimulation(PENS) applied to different parts of the streptozotocin(STZ) induced diabetic rats on the change of glucose level and spinal neuron excitability. A total of twenty-eight SD rats, divided into four groups, were used as experiment animal. Experiment group I, the normal control group, was composed of normal rats without diabetes induction. Experiment group was composed II of the rats without any treatment after experimental diabetes induction. Experiment group III was composed of the rats with 2 Hz and $200\;{\mu}s$ of PENS to the acupuncture points related with diabetes for 20 minutes after diabetes induction. Experiment group IV was composed of the rats with 2 Hz and $200\;{\mu}s$ of PENS to the parts unrelated with diabetes for 20 minutes after diabetes induction. The results can be summarized as follow: As for glucose level, the group I showed no change within normal range, and the group III showed significant increase, compared with other groups (p<0.05). As for the change of H latency, M and H amplitude, the group III showed significant differences in decrease of latency and amplitude (p<0.05). As for Hmax/Mmax ratio, the normal and other groups showed no significant differences in decrease of amplitude. It can be concluded from the above results that PENS to the acupuncture points of the STZ-induced diabetic rats was effective for spinal neurone excitability, in particular, for those of the group with PENS to the acupuncture points. This study was conducted in the period of acute diabetes induction, and therefore, further study should be conducted in the period of chronic diabetes to research both acute and chronic diabetes.

An Influence Stretching Exercise with Ultrasound and Microwave Application Having on Knee Joint Position Sense after Induction to Muscle Fatigue (근피로 유발 후 신장운동시 심부투열치료가 슬관절 위치감각에 미치는 영향)

  • Purpose : This study will recognize about the influences that stretching exercise with ultrasound and microwave application has on a knee joint position sense after induction to muscle fatigue. Methods : The object by each ten people divided at random thirty physical healthy adult men to constant group 1, group 2, group 3 to the objects. Treadmill exercised for induction to muscle fatigue, and let an exercise intensity decided on 85% of the maximum heart beat number with the goal heart beat number, and you exercise. Goal heart beat number measured as used heat rate monitor, and measured a oneself at the scale in order to recognize own physical state after end treadmill exercise to all objects to criteria to evaluate an ability shown in case of sports. Knee joint position sense used a N-K table, and experimenter did so as adaptation got passively arbitrary three angle done, and to order an original position. The object carried in person out one angle that experimenter designated, and measured repeatedly an error along him to this three times after being so. Rear before induction to muscle fatigue a position sense and measured after stretching exercise application, and played. Stretching exercise after induction applied stretching exercise, stretching exercise with ultrasound, stretching exercise with microwave, and applied to each group 1, group 2 and group 3 to muscle fatigue. Results : The result each group the difference which considers does not come out it was not but, improvement of the position sense is the possibility of knowing was from group 1, group 2 and group 3. Conclusion : These findings stretching exercise and stretching exercise with ultrasound and microwave after induct ion give help to a position sense elevation, and execute stretching exercise, and be effective against damage prevention by physical insecurity to muscle fatigue, and look.

Effects of Environmental Reinforcement Through Motivation on Motor and Cognitive Function in Rats With Focal Ischemic Brain Injury

  • It is known that individual factors as cognitive, perception, emotion, and motivation may greatly influence on recovery from neurologic region. This study was to investigate the effects of environmental reinforcement through motivation to perform the tasks voluntarily on motor and cognition function in rats with focal ischemic brain injury. Focal ischemic brain injury was induced in Sprague-Dawley rats (15 rats, $250{\pm}50$ g) through middle cerebral artery occlusion (MCAo). And then, experiment groups were randomly divided into three groups; The control group: MCAo induction ($n_1$=5), the environmental reinforcement (ER) group: the application for ER after MCAo induction ($n_2$=5), the environmental reinforcement through motivation (ERM) group: the application for ERM after MCAo induction ($n_3$=5). The climbing test (CT) and the modified limb placing tests (MLPTs) to measure the motor function and the Morris water maze acquisition test (MWMAT) and the Morris water maze retention test (MWMRT) to measure the cognitive function were performed. For the CT, the ERM group was significantly larger than the ER group. For the MLPTs, the ERM group was significantly decreased compared to other groups. For the MWMAT, the time to find the circular platform in the ERM group significantly decreased compared to other groups. For the MWMRT, the time to dwell on the quadrant circular platform in the ERM group was significantly increased compared to other groups. These results suggested that the ERM could improve the motor and cognitive functions in the rats with focal ischemic brain injury.

Induction of Ovulation with Clomiphene Citrate (Clomiphene Citrate를 이용한 배란유도에 관한 연구)

  • Therapy for anovulation represents one of the more gratifying and successful type of infertility management. Despite the introduction of bromocriptine, human menopausal gonadotropin (HMG), and gonadotropin releasing hormone (GnRH), clomiphene citrate remains the mainstay of ovulation induction therapy. There is wide variability in reported rates of ovulation induction (57-91%) and conception (25-43%) following clomiphene therapy. Factors contributing to this variability among different reports are the differences in dosage and duration of therapy, different criteria utilized in selecting patients for clomiphene therapy and different luteal phase parameters of presumptive ovulation. A review of recent experience with clomiphene citrate in the Department of Obstetrics and Gynecology in Seoul National University from January, 1983 to May, 1985 yields the following conclusions: 1. Ovulation rate per total patients treated was, 69.0% 2. Pregnancy rate per total patients treated was 31.7%, and that per total patients ovulated was 45.9%. 3. Ovulation rate at the dosage level up to 150 mg/day (50.3-53.8%) was somewhat higher than that at 200 mg/day or more (33.3-34.6%), and pregnancy rate per total patients treated was comparable at each dosage level. 4. Ovulation rate per total patients ovulated at each dosage level, where ovulation and conception occurred, showed a decreasing tendency as the dosage increased, but pregnancy rate per total patients conceived was comparable except at 200 mg/day. 5. Cumulative pregnancy rate per total patients conceived in each ovulatory cycle was 68.9% in 3 cycles, 88.9% in 4 cycles, and 100% in 6 cycles.
