Cost of Illness due to Maternal Disorders in Korea (우리나라 모성 관련 질환의 사회적 비용)

  • Objectives: Maternal morbidity and mortality are important healthcare issues. However there have been few studies on cost of illness (COI) from maternal disorders. This study aimed to estimate the COI due to maternal disorders in Korea. Methods: By reviewing previous studies and consulting expert we determined the scope of maternal disorders. We operationally defined maternal disorders as maternal hemorrhage, maternal sepsis, hypertensive disorders of pregnancy, obstructed labor, and abortion for maternal disorders. The reference period of this study is the year 2015. Main source of data were the National Health Insurance Service claims data, cause of death statistics from the Korea National Statistical Office, and the Korea Health Panel study. We classified the total costs into direct and indirect costs. The direct costs cover healthcare costs and non-healthcare costs. The indirect costs consist of productivity losses due to morbidity and premature death. Results: The cost of maternal disorders in 2015 was 229.7 billion won. The direct and indirect costs of maternal disorders were 165.2 billion won and 64.5 billion won respectively. The largest cost item for maternal disorders was healthcare cost (138.3 billion won, 60.2%). By age groups, the COI in 30-39 years old women were the highest (165.1 billion won, 71.9%). Abortion was the disorder with the highest COI among maternal disorders (71.9 billion won, 31.3%). Conclusion: The COI due to maternal disorders in Korea is quite substantial. Economic burden of maternal disorder increased when being compared with the year 2012 data despite the continued low birth rate in Korea. Therefore, it is necessary to continuously monitor the social costs of the maternal disorders in Korea.

Incidence and management of mandibular fractures in a low-resource health facility in Ghana

  • Objectives: The mandible and other parts of the maxillofacial region suffer significant morbid injuries following road traffic accidents. Our study gives epidemiological description of mandibular fractures in Ghana and also evaluates the relevance of closed reduction and indirect fixation for managing mandibular fractures in low-resource health facilities in low-income countries like Ghana. Patients and Methods: This is a retrospective study involving 268 patients who reported to the Department of Oral and Maxillofacial Surgery of the Sunyani Regional Hospital with mandibular fractures from January 2010 to December 2019. Patient medical records were assessed for information on age, sex, fracture etiology, anatomic location of fracture, time of day of road traffic accident, and other associated injuries. Results: A total of 268 patients were included in this study (males, 216 [80.6%]; females, 52 [19.4%]). Motor vehicular accident (MVA) was the leading cause of mandibular fractures (202 injuries, 75.4%). Other etiologies included assault (39, 14.6%), gunshot (13, 4.9%), falls (12, 4.5%), and industrial accidents (2, 0.7%). Of the 161 male cases caused by MVA, 121 (75.2%) occurred at night and in the evening while the remaining 40 (24.8%) occurred in the morning and afternoon. Among all managed 222 patients, 212 (79.1%) were treated with closed reduction and indirect fixation technique while 10 (3.7%) were treated with open reduction and direct fixation. Conclusion: Closed reduction with indirect fixation could successfully be used to manage mandibular fractures in low resourced health facilities, especially in low-income countries. The poor lightening system on roads in Ghana is a major contributory factor to motor vehicular accidents.

Path Analysis of Factors Limiting Crop Yield in Rice Paddy and Upland Corn Fields (벼와 옥수수 재배 포장에서 경로분석을 이용한 작물 수확량 제한요인 분석)

  • Knowledge of the relationship between crop yield and yield-limiting factors is essential for precision farming. However, developing this knowledge is not easy because these yield-limiting factors are interrelated and affect crop yield in different ways. In this study, data for grain yield and yield-limiting factors, including crop chlorophyll content, soil chemical properties, and topography were collected for a small (0.3 ha) rice paddy field in Korea and a large (36 ha) upland corn field in the USA, and relationships were investigated with path analysis. Using this approach, the effects of limiting factors on crop yield could be separated into direct effects and indirect effects acting through other factors. Path analysis provided more insight into these complex relationships than did simple correlation or multiple linear regression analysis. Results of correlation analysis for the rice paddy field showed that EC, Ca, and $SiO_2$ had significant (P<0.1) correlations with rice yield, while pH, Ca, Mg, Na, $SiO_2,\;and\;P_2O_5$ had significant correlations with the SPAD chlorophyll reading. Path analysis provided additional information about the importance and contribution paths of soil variables to rice yield and growth. Ca had the highest direct effect (0.52) and indirect effect via Mg (-0.37) on rice yield. The indirect effect of Mg through Ca (0.51) was higher than the direct effect (-0.38). Path analysis also enabled more appropriate selection of important factors limiting crop yield by considering cause-and-effect relationships among predictor and response variables. For example, although pH showed a positive correlation (r=0.35) with SPAD readings, the correlation was mainly due to the indirect positive effects acting through Mg and $SiO_2$, while pH not only showed negative direct effects, but also negatively impacted indirect effects of other variables on SPAD readings. For the large upland Missouri corn field, two topographic factors, elevation and slope, had significant (P<0.1) direct effects on yield and highly significant (P<0.01) correlations with other limiting factors. Based on the correlation analysis alone, P and K were determined to be nutrients that would increase corn yield for this field. With the help of path analysis, however, increases in Mg could also be expected to increase corn yield in this case. In general, path analysis results were consistent with published optimum ranges of nutrients for rice and com production. We conclude that path analysis can be a useful tool to investigate interrelationships between crop yield and yield limiting factors on a site-specific basis.

Study on the Donation and Consumer Attitude for the Retail Brand Management (소매 브랜드 관리를 위한 기부 및 소비자 태도에 관한 연구)

  • Purpose - The purpose of this study is to provide managerial implications for retail brand managers. Specifically, current study divide donation type into direct donation(e.g., corporate philanthropy) and indirect donation(cause-related marketing) and investigate the donation type on the purchase intention. Also, this research intends to provide the mechanism between the donation type on the purchase intention by showing the mediating role of the consumers' perceived warmth. Moreover, the moderating role of the brand type(non-luxury vs. luxury brand) between the donation type, the warmth perception, and the purchase intention will be examined. Research design, data, and methodology - A total of 174 undergraduate students from a university in Korea were recruited and were randomly assigned to the conditions of a 2(donation type: indirect vs. direct) X 2 (brand type: non-luxury vs. luxury) between-subjects design. The hypotheses were tested using SPSS 21.0. Two-way ANOVA and multiple regression analysis were performed. Results - Empirical results showed that the consumers' perceived warmth was higher in the direct donation condition than the indirect donation condition and moreover this influence of the donation type on the perceived warmth was moderated by the brand type. Specifically, the influence of donation type on the perceived warmth was significant only for luxury brand. And there is an interaction effect of donation type and brand type on the purchase intention. More specifically, the purchase intention for luxury brand was higher in the direct donation condition than the indirect donation condition whereas the purchase intention for non-luxury brand was higher in the indirect donation condition than in the direct donation condition. Conclusions - This research contributed to the CSR literature of retail brand management by showing that the influence of the donation type and the brand type on the purchase intention. Moreover, this study enriched CSR literature by introducing Stereotype Content Model and showing the mediating role of the consumers' perceived warmth. Managerially, these results suggested retail brand managers of non-luxury brand and luxury brand how to select an appropriate type of donation and conduct CSR strategies.

A Study on the Variations of the Body Trunk Temperature by the Drug-Pad Moxibustion Method (약패드 뜸 방식을 이용한 체간온도변화에 대한 연구)

  • We implemented the Drug-Pad Moxibustion Method in order to improve the conventional moxibustion therapy. This method is aimed to eliminate burning wounds and smoke, which are the defects of conventional moxibustion therapy. And we performed to verify the efficiency by comparing the Drug-Pad Moxibustion Method with the conventional Indirect Moxibustion Therapy. We measured the body heat and the lasting time of blood circulation improvement using thermography. The moxibustion therapy has two kinds of effects: The formers are pharmacological effects of the Moxa's vasodilators and antioxidants. The latters are thermal effects which cause improvement of the blood circulation. To remove the demerits without omission of above therapeutic effects, we extracted the vasodilators and antioxidant compounds from the Moxa-$CH_2Cl_2$ fraction Moxa-EtOAc and composed the moxibustion kit with $(Ba_{0.8}\;Sr_{0.2})_{0.996}\;Y_{0.004}\;TiO_2+0.5_{WT}\;SiO_2%$ Positive Temperature Coefficients Thermistor. The experimental demonstrations have been made by the stimulating the spot which is CV4(Kwan-Won), CV8(Shin-Guel), CV12(Jung-Wan) acupuncture points of the conception vessel meridian(CV). And stimulating time was one hour. We divided the subjects into 5 groups such as no stimulation group, conventional Indirect Moxibustion group, only Drug-Pad stimulation group, only heat stimulation group, and Drug-Pad Moxibustion group. In the different cases, we have measured the body heat in pre-stimulation, just after stimulation, 2 hours after, and 4 hours after. The body heats of the group who were stimulated by the Drug-Pad Moxibustion Method were increased by over the $2^{\circ}C$. And the body heats of the group who were stimulated by the Indirect Moxibustion Method were increased by average the $1^{\circ}C$. We have evaluated that the Drug-Pad Moxibustion Method is improvement on the conventional Indirect Moxibustion Method by the heat-increasing rate is 200% and the lasting time is 150% with the body heat of the abdominal region. In the conclusions, We have implemented the Drug-Pad Moxibustion Method and evaluated the efficiency of the Drug-Pad Moxibustion Method comparing with the conventional Indirect Moxibustion Method.

A Study on the Stern Bearing Damage and Shaft Alignment for 37K DWT Product/Chemical Tanker (37K DWT 석유화학제품 운반선의 선미관 베어링 발열 사고 및 축계정렬에 대한 연구)

  • Together with the emerging of the Eco-ship, the application of large-diameter and high-efficiency propeller required more careful attention than before in the design of the shafting system. After the adoption of Environmentally Acceptable Lubricants (EAL) to the stern tube lubrication oil, a number of aft stern tube bearing accidents have been reported, and a variety of institutions have actively conducted research on the cause relationship. This study attempted to find the cause of the accident by measuring the alignment of the shafting system of a medium-sized product/chemical tanker with aft stern tube bearing damage and analyzing the reaction force of each bearing. In addition, a reasonable solution to the correction of the shaft alignment was suggested and the feasibility was reviewed. Through various measured data and analysis, the actual installation of shafting system was slightly different from the design drawing condition, but it was found that each bearing load distribution was within the allowable range. Therefore, it was confirmed that the cause of this accident was due to the dissatisfaction the misalignment slope of aft stern tube bearing rather than the effect of the bearing overload. As a solution to this cause, countermeasures such as double slope were suggested in the aft stern tube bearing, and the characteristics of EAL also seem to have an indirect effect.

An Analysis of the Causes of Mathematics Anxiety in the Elementary School Students According to the Grades and Sex (학년 및 성별에 따른 초등학생의 수학불안 요인 분석)

  • The purposes of this study are to find out differences of the causes of mathematics anxiety in elementary school students according to the Grades and sex, and to find out which causes have more influence on achievement of mathematics and how much it is. For this purposes, the problems of this study are defined as follows: First, are there any differences in the influences of each super-causes and sub-causes of mathematics anxiety according to the Grades? Second, are there any differences in the influences of each super-causes and sub-causes of mathematics anxiety according to the sex? Third, what relation do the super-causes of mathematics anxiety have to the achievement of mathematics? The conclusions of this study are as follows: First, mathematics anxiety is much more affected by the internal cause like the cause of student attitude than the external cause like the causes of circumstance and the cause of teacher. Second. mathematics anxiety is much more affected by a direct experience like the causes of a shortage of time and the causes of student interest than indirect experience like the causes of teacher's authority and the causes of the application of daily life. Third, the causes of circumstance and parent's attitude. its sub-cause, have greater influence on the female group than on the male group. Fourth, in the middle Gradess, the female group is more affected by the cause of student attitude and the cause of circumstance than the male group, but in the higher Grades. the differences disappear and those two become common causes of anxiety. Fifth, As the students go up to the next Grades in school, the cause of teacher, the characteristic of the curriculum and the cause of prejudice have more influence on the mathematics anxiety. Sixth, the causes of teacher, the causes of mathematical curriculum and the causes of student attitude among super-causes of mathematics anxiety have a negative effect on the achievement of mathematics. But the causes of circumstance have a positive effect on it. And also, the causes of mathematical curriculum among super-causes is much related to the causes of teacher and the causes of circumstance.

A Study on the Available Countermeasures through a Practical Analysis of China involving Trade Claims (중국 클레임 실태분석을 통한 대응방안에 관한 연구)

  • Since China joined the WTO, the trade between Korea and China has continued to grow by leaps and bounds, making both nations become mutually important trading partners to each other. The volume of trades with China showed a rapid upward trend with an average annual increase rate of 20% and the resultant dispute is also rising. Thus, this study was aimed to make some suggestions and come up with ways of responding claims by surveying Korean trading firms which have experience in making transactions with China to investigate the current status and the actual condition of their occurrence. As a result of this study, firstly, the main cause of such dispute was shown to be the failure to perform the agreement. Secondly, there were malty verbal agreements and due to a deterioration of trust in Chinese firms, the method of a letter of credit was adopted by many Korean firms. Thirdly, the payment and credit appeared to be most important in transactions with China. A credit inquiry is a must in future transactions with China. Fourthly, the indirect cause of the occurrence of trade claims was the difference in commercial practices and as for the way of settling disputes, the agreement through negotiations between the parties involved was the most. Fifthly, small and medium firms should make more of their credit and English proficiency than large ones do.

  • In this study, we inquire what is the rules which is implicitly used by the adolescent group about bullying phenomenon. We inquire how to interpret the phenomenon from the adolescents'viewpoint and their opinions of how appearance and clothing determine the extent of bullying. After we discuss this problem, we propose an indirect solution to the cause of bullying. We selected a middle school boy by reason that boys have been more experienced than others according to a previous study and are at a keen adolescent stage of development. Moreover, 'ichime'phenomenon is seriously reported at a middle school student stage in the case of Japan. The purpose of the study was to analyze the following : 1. Research on an actual case of bullying in a middle school student. 2. the adolescents'viewpoint on characteristics of school bullying. 3. The cause of violence and injury from school bullying. 4. How appearance and clothing affect bullying. 5. The preference and uses of clothing worn by adolescents. The process of this study brought a substantially between quantitative and qualitative research. Theses questionnaires were administered to 320 middle school students in Seoul from the 14/sup th/ to the 25/sup th/ of June. The data totaled up to 280 subjects, which consist 121 male students and 159 female students except for invalid data.

Carotid Cavernous Sinus Fistula with Abducens Nerve Palsy after Le Fort I Osteotomy : A Case Report (상악골절단술 후 외전신경마비를 동반한 경동맥 해면정맥동루)

  • Carotid cavernous sinus fistula(CCSF) is an abnormal communication at the base of the skull between the internal carotid artery and the cavernous sinus. Fistula is almost associated with extensive facial trauma as a result of direct or indirect forces. Most fistulas of traumatic origin develop as a result of fractures through the base of the skull, which cause the laceration of the internal carotid artery near the cavernous sinus. The signs and symptoms of CCSF are pulsating exophthalmosis, orbital headache, pain, orbital or frontal bruit, loss of visual acuity, diplopia and ophthalmoplegia. Angiography reveals a definite CCSF and a detachable balloon embolization is known to be the treatment of choice. Even though carotid cavernous sinus fistula is an uncommon complication after orthognathic surgery, several cases of CCSF due to congenital anomalies, pre-existing aneurysms and abnormally thickened maxillary posterior wall have been reported in the literature. We have experienced a case of CCSF after Le Fort I osteotomy for maxillary advancement in skeletal class III patient and the cause, pathogenesis, diagnosis and treatment of this case.
