HISTOMORPHOMETRIC STUDY OF DENTAL IMPLANTS WITH DOUBLE ACID-ETCHED AND ANODIC OXIDIZED SURFACE IN THE RABBIT TIBIA (토끼 경골에서 치과용 임프란트의 이중 산부식 및 양극 산화 표면처리에 따른 조직계측학적 연구)

  • This study was performed to evaluate the effects of three different implant surface treatments to the bone formation during osseous healing period under unloading conditions. Machined, double-acid etched and anodic oxidized implants were inserted into tibia of 3.0 - 3.5 kg NZ white male rabbits and 2 animals of each group were sacrificed at 2, 4 and 8 weeks. The specimens containing implant was dehydrated and embedded into hard methylmethacrylate plastic. After grinding to $50{\mu}m$, the specimens were stained with Villanueva bone stain. From each specimen, histomorphometric evaluation and the bone implant contact rate were analysed with optical microscope. The results were as follows; 1. In the scanning electronic microscopic examination, machined surface implant had several shallow and paralleled scratches on plain surface, double acid-etched implant had lots of minute wrinkles, rough valley and also irregularly located craters that looked like waves, anodic oxidized surface implant had porosity that minute holes were wholly distributed on the surface. 2. After 2 weeks of implantation, the percentages of bone-to-implant contact in the machined implant, double acid-etched implant and anodic oxidized implant were 26.85%, 62.64% and 59.82%, after 4 weeks of implantation they were 64.29%, 77.85% and 75.23%, and after 8 weeks they were 82.66%, 85.34% and 86.39%. 3. After 2 weeks of implantation, the percentages of bone area between threads in the machined implant, double acid-etched implant and anodic oxidized implant were 21.55%, 42.81%, and 40.33%, after 4 weeks of implantation they were 49.32%, 62.60% and 75.56%, and after 8 weeks they were 71.62%, 87.73% and 83.94%. In summary, percentages of implant surface contacted to bone trabeculae and bone formation area inside threads in double acid-etched implants and anodic oxidized implants were greater than machined implants in early healing stage. These results suggest that double acid-etched and anodic oxidized surface implants could reduce the healing period for osseointegration and may enable to do early function.

  • Titanium for dental implant application has the superior properties of biocompatibility, specific strength, and corrosion resistance. However, it is extremely difficult to find a suitable surface treatment method for sufficient osseointegration with biological tissue/bone cell and implant surface. Surface treatment technology using laser has been researched as the way to increase surface area of implant. In this study, to develop the surface treatment process with improved adhesion between implant and bone cell at the same time for superior biocompatibility, pulsed laser beam was overlapped continuously for scribed surface morphology and determination of friction coefficient. As the results, surface area and friction coefficient was increased over 2 times by the comparison with sand blasting, which is used for the conventional method. In this time, the optimal condition for laser beam power and beam irradiation speed was 13 watt and 50 mm/sec, respectively.

  • Statement of problem. Surface alteration of the implant screws after function may be associated with mechanical failure. Theses metal fatigue appears to be the most common cause of structural failure. Purpose. The purpose of this study was to evaluate surface alteration of the implant screws after function through the examination of used and unused implant screws in SEM(scanning electron microscope). Materials and methods. In this study, abutment screws(Steri-oss, 3i), gold retaining screw(3i) and titanium retaining screw(3i) were retrieved from patients. New, unused abutment and retaining screws were prepared for control group. Each of the old, used screws was retrieved with a screwdriver. And retrieved implant complex of Steri-oss system was prepared for this study. Then, SEM investigation and EDS analysis of abutment and retaining screws were performed. And SEM investigation of cross-sectioned sample of retrieved implant complex was performed. Results. In the case of new, unused implant screws, as maunfactured circumferential grooves are regularly examined and screw thread are sharply remained. Before ultrasonic cleansing of old, used implant screw, a lot of accumulation and corrosion products were existed. After ultrasonic cleansing of old, used implant screws, circumferential grooves as examined before function were randomly deepened and scratches increased. Also, dull screw thread was examined. More surface alterations after function were examined in titanium screw than gold screw. And more surface alteration was examined when retrieved with driver than retrieved without driver. Conclusions. These surface alteration after function may result in the screw instability. Regularly cleansing and exchange of screws was recommended. We recommend the use of gold screw rather than titanium screw, and careful manipulation of the driver.

Effects of Blasting and Acidic Treatment on the Corrosion Characteristics of Dental Implant Fabricated with Cp-Ti and Ti-6Al-4V Alloy (Cp-Ti와 Ti-6Al-4V 합금으로 제조된 치과용 임플란트의 부식특성에 관한 블라스팅과 산세처리의 영향)

  • The effects of blasting and acidic treatment on the corrosion characteristics of dental implant fabricated with Cp-Ti and Ti-6Al-4V alloy have been researched by using electrochemical methods. The fabricated implants were cleaned and sandblasted by $Al_2O_3$ powder and then acidic treatment was carried out in nitric acid solution. The surface morphology were observed using scanning electron microscope. The corrosion behaviors were investigated using potentiosat and EIS in 0.9% NaCl solution at $36.5{\pm}1^{\circ}C$. The potentio-dynamic test in 0.9% NaCl indicated that the corrosion potential of blasting and acidic treated implant was lower than that of non treated implant, but current density was higher than that of non treated implant. From the cyclic potentiodynamic test results of Ti implant, the passivation current density of blasting and acidic treated implant slightly higher than that of non treated implant. From A.C. impedance test results in 0.9% NaCl solution, polarization resistance($R_p$) value of blasting and acidic treated implant was lower than that of non treated implant. In case of blasting and acidic treated implant surface, the pits were observed in valley and crest of implant surface.

Bone-to-Implant Contact according to the Surface Roughness of the Implants (임플란트의 표면조도가 골융합에 미치는 영향)

  • This study was performed to evaluate the difference of the bone-to-implant contact according to the surface roughness of the implants. Two beagles were used in the experiment. Extraction of the all premolars was performed in the mandible. In 3 months of healing, screw-shaped pure titanium machined surface implants (Implantium(R), Dentium Co. Korea), implants blasted with 45${\mu}m$ $TiO_2$ particles, (Implantium(R), Dentium Co. Korea) and implants blasted with 100${\mu}m$ $TiO_2$ particles (Implantium(R), Dentium Co. Korea) in diameter 3.4mm and length 6mm were installed in the edentulous mandible. Each dog was sacrificed at 4, 12 weeks after placement and then nondecalcified specimens were prepared for histologic analysis. The results of this study were as follows. 1. At 4 , 12 weeks after the surgery, bone-to-implant contact in the surface blasted with $TiO_2$ particles was higher than that in the pure titanium machined surface respectively. 2. Osseointegration in the surface blasted with 45${\mu}m$ $TiO_2$ particles was more increased than that in the surface blasted with 100${\mu}m$ $TiO_2$ particles. 3. Bone-to-implant contact was increased with time independent of surface roughness. 4. Bone formation was in the outfolded area more than inside the threads independent of surface roughness. From the above results, we were able t o find the most bone-to-implant contact in 45${\mu}m$ $TiO_2$ blasted implant.

The effect of erbium-doped: yttrium, aluminium and garnet laser irradiation on the surface microstructure and roughness of double acid-etched implants

  • Purpose: One of the most frequent complications related to dental implants is peri-implantitis, and the characteristics of implant surfaces are closely related to the progression and resolution of inflammation. Therefore, a technical modality that can effectively detoxify the implant surface without modification to the surface is needed. The purpose of this study was to evaluate the effect of erbium-doped: yttrium, aluminium and garnet (Er:YAG) laser irradiation on the microstructural changes in double acid-etched implant surfaces according to the laser energy and the application duration. Methods: The implant surface was irradiated using an Er:YAG laser with different application energy levels (100 mJ/pulse, 140 mJ/pulse, and 180 mJ/pulse) and time periods (1 minute, 1.5 minutes, and 2 minutes). We then examined the change in surface roughness value and microstructure. Results: In a scanning electron microscopy evaluation, the double acid-etched implant surface was not altered by Er:YAG laser irradiation under the condition of 100 mJ/pulse at 10 Hz for any of the irradiation times. However, we investigated the reduced sharpness of the specific ridge microstructure that resulted under the 140 mJ/pulse and 180 mJ/pulse conditions. The reduction in sharpness became more severe as laser energy and application duration increased. In the roughness measurement, the double acid-etched implants showed a low roughness value on the valley area before the laser irradiation. Under all experimental conditions, Er:YAG laser irradiation led to a minor decrease in surface roughness, which was not statistically significant. Conclusions: The recommended application settings for Er:YAG laser irradiation on double acid-etched implant surface is less than a 100 mJ/pulse at 10 Hz, and for less than two minutes in order to detoxify the implant surface without causing surface modification.

  • In this study, surface characteristics of dental implant fixture with various manufacturing process have been researched using electrochemical methods. The dental implant fixture was selected with 5 steps by cleaning, surface treatment and sterilization with same size and screw structure; the 1st step-machined surface, 2nd step-cleaned by thinner and prosol solution, 3th step-surface treated by RBM (resorbable blasting media) method, 4th step-cleaned and dried, 5th step-sterilized by gamma-ray. The electrochemical behavior of dental implant fixture has been evaluated by using potentiostat (EG&G Co, 2273A) in 0.9% NaCl solution at $36.5{\pm}1^{\circ}C$. The corrosion surface was observed using field-emission scanning electron microscopy (FE-SEM) and energy dispersive x-ray spectroscopy (EDS). The step 5 sample showed the cleaner and rougher surface than step 3 sample. The step 5 sample of implant fixture treated by RBM and gamma sterilization showed the low corrosion current density compared to others. Especially, the step 3 sample of implant fixture treated by RBM was presented the lowest value of corrosion resistance and the highest value of corrosion current density. The step 3 sample showed the low value of polarization resistance compared to other samples. In conclusion, the implant fixture treated with RBM and gamma sterilization has the higher corrosion resistance, and corrosion resistance depends on the step of manufacturing process.

  • Typical implants are not specified the screw structure and the surface process according to the bone quality of the human body. The purpose of this study is to complement the shortcomings of the existing implant surface processing method. It is to propose a surface processing method that increases the adhesion between the implant and bone tissue by increasing the surface area per unit area of the implant. It is very important to establish precise and systematic process parameters when surface treatment of implants using lasers. Therefore, we intend to develop a process so that the implant can be a biocompatible structure using a Q-switching Nd:YAG laser with a wavelength of 1.06um. Implant surface treatment technology through this study will be used in the industry.

The Effects of various Regeneration techniques on Bone Regeneration around Dental Implant (수종의 재생 술식 시행이 매식체 근원심부의 골재생에 미치는 영향)

  • The successful implantation necessitate tissue regeneration m site of future implant placement, there being severe bone defect. Therapeutic approaches to tissue regeneration in the site have used bone grafts, root surface treatments, barrier membranes, and growth factors, the same way being applied to periodontal tissue regeneration. Great interest in periodontal tissue regeneration has lead to research in bone graft, guided-tissue regeneration, and the administration of growth factors as possible means of regenerating lost periodontal tissue. The blood component separated by centrifuging the blood is the platelet-rich plasma. There are growth factors, PDGF, $TGF{beta}1$, $TGF{beta}2$ and IGF in the platelet-rich plasma. The purpose of this study was to study the histopathological correlation between the use of platelet-rich plasma and the healing of bone defect around implant fixture site. Implant fixtures were inserted and graft materials were placed into the left femur of in the experimental group, while the control group received only implant fixtures. In the first experimental group, platelet-rich plasma and BBP xenograft were placed at the implant fixture site, and the second experimental group had platelet-rich plasma, BBP xenograft, and the e-PTFE membrane placed at the fixture site. The degree of bone regeneration adjacent to the implant fixture was observed and compared histopathologically at 2, 4, and 8 weeks after implant fixture insertion. The results of the experiment were as follows: 1. Bone remodeling in acid etched surface near the implant fixture of all experimental groups was found to be greater than new bone formation. 2. Bone remodeling in acid etched surface distant to the implant fixture of all experimental groups was decreased and new bone formation was not changed. 3. Significant new bone formation in machined surface near the implant fixture of bothl experimental groups was observed in 2 weeks. 4. New bone formation in machined surface distant to the implant fixture of both experimental groups was observed. Bone remodeling was significant in near the implant fixture and not in distant to the implant fixture. The results of the experiment suggested that the change of bone formation around implant. Remodeling in machined surface distant to the implant fixture of both experimental groups, and new bone formation and remodeling near the implant fixture were significant.

  • The present study was performed to evaluate the effect of tetracycline-BCL on the change of implant surface microstructure according to application time. Implant with pure titanium machined surface, double coated $FBR^{(R)}$ surface and oxidized CellNest surface were utilized. Implant surface was rubbed with $50mg/m{\ell}$ tetracycline-BCL solution for ${\frac{1}{2}}$, 1, $1{\frac{1}{2}}$, 2 and $2{\frac{1}{2}}$min. respectively in the test group. Then, specimens were processed for scanning electron microscopic observation. The results of this study were as follows. 1. Both test and control group showed a few shallow grooves and ridges in pure titanium machined surface implants. There were not significant differences between two groups. 2. The double coated $FBR^{(R)}$ surfaces showed fine crystalline structures. The roughness of surfaces conditioned with tetracycline-BCL was lessened relative to the application time. 3. The oxidized CellNest surfaces showed the porous structures. The surface conditioning with tetracycline-BCI influenced on its micro-morphology. In conclusion, the detoxification of the affected implant surface with $50mg/m{\ell}$ tetracycline-BCL should be applied respectively with different time according to various implant surfaces.
