Titanium and titanium alloys are the most common materials used for dental and biomedical implants, owing to their biocompatibility and favourable mechanical properties. However infection of the region surrounding a dental implant by pathogenic microorganisms is a significant factor in implant failure. Prevention and control of microbial colonization of implant surfaces is considerable interest to the biomedical community. One important strategy is to render the implant surface antibacterial by impeding the formation of biofilm. A number of approaches have been proposed for this purpose. Therefore, we reviewed the researches of antibacterial coatings on titanium implants in this articles.
After stage-two surgery, the highest incidence of failure has been attributed to implant overload. However, the biomechanical factors cited inthe literature that contribute to implant overload, such as bone type, cuspal inclination, horizontal offset, maxillary compared to mandibular arch, the inclusion of natural teeth within the prosthesis, and occlusal harmony are superimposed on physiologic variations. Following two cases, including reduction of cuspal inclination and usage of modified incisal pin, showed a method of contouring occlusal surface for reduction of unpreditable implant overload.
OBJECTIVE The purpose of clinical study was to evaluate the survival of dental implants used in restoring patients with implant supported removable partial dentures (ISRPDs) of different configurations. MATERIALS AND METHODS The sample consisted of 20 consecutively treated partially edentulous patients, who, between 2003 and 2008, had a total of 84implants placed in different arch sites and who were treated with ISRPDs. The mean age was 57 years. Mean follow up time from delivery of ISRPDs was 3 years 2 months (range, 1 to 6 years). Osseointegration failure, postoperative complication of dental prosthesis, and the success rate of ISRPDs were retrospectively evaluated using clinical and radiographic examination. RESULT The overall implant survival rate was 100%. During follow up, the one clasp of removable partial denture was broken in 1 year11 months after prosthesis delivery. All patients were satisfied with their prosthesis. CONCLUSIONS Implant supported removable partial dentures could serve as favorable prognosis. Careful patient selection, with an appropriate maintenance and recall system, is recommended to obtain satisfactory results.

The purpose of this paper was to investigate the significance of splinted and non-splinted implant-supported restorations with an internal connection for multiple consecutively missing teeth. Upon examination of the effects of fixture-abutment connection, the distribution of occlusal load was favorable in splinted implant-prosthesis with an external connection, but effect of strain distribution was not significant in splinted implant-prosthesis with an internal connection. In splinted implant-prostheses for short implants, strain distribution was not affected by the method of retention. For cement-retained prostheses, the effect of strain distribution due to splinting was not significant. In clinical studies, non-splinted prostheses with an internal connection for multiple consecutively missing teeth showed high survival rate, mild marginal bone loss, and stable periodontal condition. However, failure to achieve optimal proximal contact between single-unit prostheses may lead to food impaction, and veneer fracture may be inevitable when the framework provides inadequate support in the proximal region. In conclusion, splinted implant-prosthesis is not an indication in all cases, and clinical consideration of its use should be based on the patient's oral condition, such as location and number of implants, formation of proximal contact, canine guidance, existence of parafunctional habit, and oral hygiene, when multiple consecutively missing teeth are replaced by internal connection type implant.
연구배경: 전신질환을 보유한 대상자를 위한 임상치과위생사 주도의 치위생관리과정에서의 중요성을 강조하고자 임상적 자료를 근거로 치과 임플란트에 영향을 미치고 있는 전신질환을 파악하고자 하였다. 연구방법: 치과 임플란트에 영향을 미치는 전신질환을 파악하고자 2023년 3월 1일부터 5월 31일까지 문헌고찰을 실시하였으며, 검색 시기는 2000년 1월~2020년 12월까지 국내·외 학술지에 게재된 연구 논문을 대상으로 하였다. 검색에 활용된 국내 database는 학술정보서비스(RISS), 누리미디어(DBpia), 한국학술정보(http://www.papersearch.net;KISS)를 활용하였으며, 국외 database는 Pubmed에서 '치과 임플란트 실패'와 '임플란트와 전신질환' 그리고 Dental implant failures와 Dental implant and systemic disease를 검색하여 전체 383편 중 최종적으로 13편의 자료를 선별하여 분석하였다. 결과: 임플란트의 누적생존률은 평균 94.3%이었으며 실패율은 5.7%이었다. 임상적으로 임플란트와 관련된 전신 질환에 대한 분석결과는 당뇨에 대한 연구가 13편(100.0%)으로 가장 높은 빈도를 차지하였으며 고혈압과 흡연에 대한 연구가 각각 8편(61.5%), 심혈관질환 7편(53.8%), 골관련 질환은 5편(38.5%) 순으로 위험요인이 조사되었다. 그 외 간질환 및 갑상선 이상 그리고 혈액이상 질환과 장기이식 및 감염성 질환 등이 확인되었다. 결론: 임상 치과위생사는 조절되지 않는 전신질환은 임플란트 실패의 위험요인이므로 전신질환이 임플란트에 미치는 영향에 대한 인식과 치위생 사정-판단-수행-평가 등의 주기적인 예방적 치위생관리 과정에서 환자와의 정보공유로 지속적으로 건강한 구강상태를 유지하도록 하여야 할 것이다.
연구 목적: 부분 무치악 환자에서 Mg titanate implant (M Implant system, Shinhung, Korea)의보철 후 1년간의 방사선사진을 이용한 변연골 흡수량과 Osstell$^{(R)}$을 이용한 임플란트 안정성 평가 결과를 분석하여 임상적인 성공률에 대해 알아보고자 한다. 연구 재료 및 방법: 38명의 환자에 79개의 임플란트를 식립하여 보철 후 1년 동안 변연골 흡수량, 임플란트의 동요도, 임플란트 식립 위치에 따른 분포 및 성공률, 보철 전후에 따른 성공률을 분석했다. 결과: 변연골의 흡수량은 보철물 장착 후 1년간의 평가에서 상악에서는 평균 1.537 mm, 하악에서는 평균 1.172 mm의 변연골 흡수가 관찰되었으며, 전체적인 상하악의 평균 변연골 흡수량은 1.255 mm로 관찰되었다. 수술 후 시간이 경과함에 따라 ISQ값은 미약하게 감소하였다가 증가하는 경향을 보였으나, 상악과 하악 모두에서 식립 당일 이후 예비 인상일, 최종 보철물의 장착일과 주기적인 평가 기간 동안 ISQ값의 큰 증가나 감소를 나타내지는 않았다. 성공률은 상악에서의 94.12%였고, 하악에서의 98.39%였다. 결론: 본 실험의 결과로 미루어 Mg titanate implant는 임상적으로 양호한 결과를 보여주었다.
