The Coping Experience in Hypertensive Clients (고혈압 환자의 대처경험)

  • To find the experience about clients with hypertension and to help them to care for themselves in the community. Method: All data was collected from August 1999 to October 1999, through in-depth interviews, observation, and telephone interview with 7 participants who have been diagnosed with hypertension for 1 to 10 years. According to Strauss and Corbin's Methodology, the data was continuously coded into concepts and categories, and then new data was analyzed simultaneously by a constant comparative method. Results: There are 171 concepts, and then they were grouped into 34 the lower categories and 15 to the upper categories. The course of the coping of fear in hypertensive client consisted of 6 processes. The awareness of seriousness was context, and the fear was core phenomenon about the coping experience. We also found that hypertensive clients have 3 patterns, depending on the awareness degree of seriousness and the fear about hypertension. Conclusion: Our nurses should recognize the importance of effective management and seriousness about hypertension, offer clients the importance of family support and the information of etiology, symptoms and signs of hypertension, and provide the correct information on hypertensive medication. We should be able to guide their fear about hypertension to positive self-management, so that they may manage their disease thoroughly and effectively.

The Effects of an Internet Based Coaching Program for Obesity Management in Hypertensive Patients (고혈압 환자의 비만관리를 위한 인터넷 기반 코칭 프로그램의 효과)

  • Purpose: The purpose of this study was to investigate the effects of the internet based coaching program for obesity management on weight control related knowledge, self-efficacy, eating habits, physical activity and obesity related physiological indexes in hypertensive patients. Methods: The study design was non-equivalent control group pre-test-post-test design. A total of 43 subjects were overweight or obese hypertensive patients aged 30 or older, who were divided into experimental group (n=23) and control group (n=20). This program lasting 12 weeks consisted of a weight control related education, internet based individual coaching for diet, exercise and behavior modification, offering internet community and health counseling. The variables were measured three times (before the treatment, after 6 weeks, and after 12 weeks) and the collected data were analyzed by SPSS/WIN 17.0 program. Results: The experimental group had significantly improved weight control related knowledge, self efficacy, eating habits, body composition, and systolic blood pressure with time. Conclusion: The internet based coaching program was effective in improving obesity management, also this program will help to prevent cardiovascular disease of obese hypertensive patients.

  • Periodontal disease is a common inflammatory disorder that is being considered as a risk factor for atherosclerotic complication. Recent epidemiological evidence also supports that its potential association with increased blood pressure levels and hypertensive prevalence. Data from cross-sectional studies suggest that in hypertensive patients periodontal disease may enhance the risk and degree of target organ damage. So dental infections have been associated with cardiovascular diseases. There are potential pathophysiologic links between hypertension and periodontits. The role of the inflammatory pathway include C-reactive protein(CRP). CRP is an inflammatory mediator that has been shown to predict the development of hypertension independently of baseline BP and traditional risk factors, has been consistently reported as at least mildly elevated in patients with periodontal disease. Reactive oxygen species produced by locally infiltrating neutrophils participate in periodontal tissue destruction. Periodontits can lead to inflammatory responses in the atrial myocardium, which disturbs the structural and electrophysiologic properties of the atrium and facilitates atrial fibrillation in the animal experiment.

Effects of Chunmagudeungeumgamibang on Hypertension (천마구등음가미방(天麻鉤藤飮加味方)이 고혈압에 미치는 영향)

  • Chunmagudeungeumgamibang(CGE) has been used for many years as a therapeutic agent for acute stage of cerebrovascular disease and hypertension in oriental medicine. But the effect of CGE on hypertension and vascular system is not well-known. This study was done to investigate the effects of CGE on hypertension. The results were obtained as follow : CGE showed a safety in cytotoxicity and toxicity of liver. CGE showed scavenging activity on DPPH free radical. CGE showed the inhibitory effect on ROS and ACE. CGE significantly decreased the blood pressure and pulse in DOCA-salt hypertensive rat. CGE significantly decreased the levels of aldosterone in DOCA-salt hypertensive rat. CGE significantly decreased the levels of dopamine, epinephrine in DOCA-salt hypertensive rat. CGE significantly decreased the levels of potassium and chloride in DOCA-salt hypertensive rat. CGE significantly increased the levels of calcium in DOCA-salt hypertensive rat. These results suggest that CGE might be effective in treatment and prevention of hypertension

Quantitative Analysis of Renogram (Renogram의 정량분석(定量分析)에 관(關)한 연구(硏究))

  • Radioisotope renography was carried out in 564 cases consisting of 150 normal controls, 140 hypertensives, 102 hypertensive nephropathys, 62 chronic renal diseases, non-functioning kidneys. It was aimed to study which parameter of the renogram is most applicable to any definite disease of the kidney. The analytical methods adopted were; Tobe, Spencer, Krueger, Matchida and Takeuchi. In the non-functioning kidney groups, the hemograms and serum nitrogen series were also studied to evaluate the relationships between the renograms and renal anemia. The parameters were; time of maximum amplitude (Tmax), half-time of maximum amplitude ($T\frac{1}{2}$), Kac value calculated from these two parameters in Tobe's method, slopes of Band C phase, B/A and B/C values in Spencer's method, total concentration (T.C.), minute concentration (M.C.) and minute excretion (M.E.) in Krueger's method, Matchida's K value and Takeuchi's renal function Index (R.F.I.). Following were the results: 1. In general, marked differences in the patterns of the renogram were observed between the normal controls and nephropathys. In Tobe's method, each parameter showed statistically significant delay or decrease in patients with hypertensive nephropathys and chronic renal diseases. In Spencer's method, slopes of B and C phase and B/C, also showed the statistically significant decrease in patients with hypertension, hypertensive nephropathys and chronic renal diseases. In Krueger's method, M.C. and ME showed the statistically significant differences between the control and patients with hypertension, hypertensive nephropathys and chronic renal diseases, In Matchida's method, K value showed the statistically significant differences between the control and patients with hypertensive nephropathys and chronic renal diseases. 2. It appeared, therefore, that Tobe's $T\frac{1}{2}$, Kac value, Spencer's slopes of Band C phase, B/A, B/C values, Krueger's T.C., M.C., and M.E. values, Matchida's K value are useful for the differentiation of various renal diseases, however, qualitative analysis of the renogram with one or two parameters is not accurate. 3. In bilateral non-functioning kidney groups, a positive correlation between anemia and nitrogen retention was observed, although the quantitative assessment of the degree of non-functioning was impossible.

Scientific Study of Magnetic Acu-point Therapy on Eight Extraordinary Meridian for Lowering Blood Pressure (기경팔맥 자석침의 혈압 강하 효과에 대한 과학적 검증 연구)

  • This study assessed effects of magnetic acu-point therapy as an add-on to conventional antihypertensive managements such as medication or lifestyle modification for hypertensive or pre-hypertensive subjects. This study was conducted with the purpose to evaluate the decreasing effect of the blood pressure on the hypertensive patients who were given to magnetic acu-point therapy. A single-blind, placebo-controlled trial by cross matching was conducted at Daejeon University Oriental Hospital. The washout periods of cross matching are 10 days. 22 hypertensive (systolic BP>140 mmHg or diastolic BP>90 mmHg) volunteers were recruited and treatments were donemagnetic acu-points on Oegwan(TE 5), Jogimeup(GB 41), Naegwan(PC 6), Gongson(SP 4), Hugye(SI 3), Sinmaek(BL 62). The treated group was statistically significant decrease of the blood pressure, while control group show no statistically significant difference of BP. From the results, magnetic acu-point therapy seems to offer a benefit to the treatment of hypertensive patients.

Effects of Kamigingansikpung-tang on Hypertension Model by DOCA-salt (가미진간식풍탕(加味鎭肝熄風湯)이 DOCA-salt로 유발된 고혈압(高血壓) 병태(病態) 흰쥐 모델에 미치는 영향)

  • Kamigingansikpung-tang(KGST) has been used for many years as a therapeutic agent for acute stage of cerebrovascular disease and hypertension in oriental medicine. KGST significantly decreased the levels of potassium(K+) and chloride(Cl-) in DOCA-salt induced hypertensive rat. KGST significantly decreased the levels of uric acid and creatine in DOCA-salt induced hypertensive rat. KGST has an effect on inhibiting cell damage of the heart, liver, kidney, and adrenal gland. results suggest that KGST might be effective in treatment and prevention of hypertension.

The Effect of Individual Education for Hypertensives at Home on Knowledge of Hypertension, Attitude about Chronic Disease, Self-care Management And Blood Pressue. (개별접촉 교육이 고혈압 환자의 지식$\cdot$태도와 자기건강관리 이행 및 혈압 변화에 미치는 영향)

  • This is the quasi experimental study to evaluate the effect of individual health education for hypertensive patients at home on knowledge of hypertesnion, attitude about chronic disease, self-care management. The individual health education program was performed at each patient's home every one month through, 1 years. The first data collection was carried out in May 1991. and the last was done in July 1992 through questionaires. The study results were as follows; 1) The subjects were 22 hypertensive patients who agreed the participation of study among registered patients at a public health center in Incheon. They were consisted of thirteen males and nine females. And their duration of illness were average 5 years, their mean age were 65 years. The over all living conditions were poor and the average monthly income was 50 thousdand won. 2) The effect of individual health education through home visit was statistically significant. The Knowledge of hypertension (t= -4.40, p<.001), attitude about chronic disease (t=­2.65, p<.05), self-care management of the subjects were significantly improved. (t=-3.76, p<.001), and their blood pressure were decreased. 3) Between the knowledge of hypertesnion and the attitude about chronic disease showed significant positive relationship. But the self-care management had not relationship with these two factors. unexpectedly. 4) The knowledge of hypertension, attitude about chronic disease, and self-care management had not evenly influenced the control of hypertension. These results suggested that the effort needed to find out the other factors influencing self-care management and develop the self-care management measuring tool. And the health education programs for chronic patients were developed, systematically. And the standardized health education model was developed for home health care nursing intervention in community based.

Genetic Variations in Six Candidate Genes for Insulin Resistance in Korean Essential Hypertensives

  • Hypertension is a complex disease with strong genetic influences. Essential hypertension has been shown to be associated with insulin resistance. To clarify the genetic basis of insulin resistance in Hypertension, case-control association studies were performed to examine candidate genes for insulin resistance in hypertension. Polymorphisms investigated were the BstO I polymorphism of the $\beta$3-adrenergic receptor (ADRB3) gene, the Xba I Polymorphism of the glycogen synthase (GSY) gene, the Dde I polymorphism of the protein phosphatase 1 G subuit (PP1G) gene, the BstE II polymorphism of the glucagon receptor (GCG-R) gene, the Pst 1 polymorphism of the insulin (INS) gene and the Acc I polymorphism of the glucokinase (GCK) gene. No significant differences were observed in the distribution of alleles and genotypes of the ADRB3, GSY PP1G, GCG-R, INS, and GCK genes between hypertensive and normotensive groups. Although the frequencies in each of these polymorphisms were not significantly different between essential hypertensive and normotensive individuals, our results may provide additional information for linkage analysis and associative studies of disorders in carbohydrate metabolism or in cardiovascular disease.

Expression of caveolin-3 as positive intracellular signaling regulator on the development of hypertrophy in cardiac tissues

  • We have examined distribution and expression of caveolin-3 (cav-3), one of three caveolin isoforms from 16-wks-old spontaneously hypertensive rats (SHR) compared with age-matched control wistar-kyoto (WKY) rats. The expression of cav-3 was increased, whereas expression of PKB/Akt and calcineurin (Cn) was not changed in cardiac tissues of SHR compared to WKY rats. Interestingly, expression of cav-3, PKB/Akt and Cn were decreased in plasma membrane fraction in SHR compared to WKY rats. In H9c2 cardiomyoblast cells treated with phenylephrine ($50{\mu}M$, 48hr) or isoproterenol ($10{\mu}M$, 48hr), the expression of cav-3 was markedly enhanced compared to nontreated cells. Upon immunofluorescence analysis, cav-3 was localized in plasma membrane of control H9c2 cells. However phenylephrine or isoproterenol treatment caused translocation of cav-3 to perinuclear region. These results suggest that cav-3 plays as positive regulators in the development of hypertrophy in cardiac tissues of SHR rats.
