The effect of oral glucose tolerance testing on changes in arterial stiffness and blood pressure in elderly women with hypertension and relationships between the stage of diabetes and physical fitness levels

  • [Purpose] The purpose of this study was to assess changes in blood glucose level, blood pressure, and arterial stiffness after a 75 g oral glucose tolerance test (OGTT) in elderly women aged over 65 years with hypertension and either normal glycemic control, impaired fasting glucose tolerance, or diabetes mellitus. We also wished to investigate the relationship between stages of diabetes and physical fitness. [Methods] A total of 24 elderly women with hypertension were assigned to a control group (CON; n=7), impaired fasting glucose group (IFG; n=9), and diabetes mellitus group (DM; n=8). In each group, blood glucose level, brachial ankle pulse wave velocity (PWV), and blood pressure were measured at baseline as well as 60 and 120 minutes after a 75 g OGTT. Physical fitness factors such as hand grip strength, balance test, 4 m gait speed test, chair stand test, short physical performance battery, and 6-minute walking test were subsequently assessed. [Results] In all three groups, blood glucose levels were significantly increased at 60 and 120 minutes after a 75 g OGTT. In the DM group, blood glucose levels were significantly higher before and after a 75 g OGTT than in the CON group. In the CON group, PWV was significantly increased at 60 minutes after a 75 g OGTT; however, there were no changes in other groups after glucose ingestion. In the CON group, systolic and diastolic blood pressures were significantly decreased at 60 and 120 minutes after a 75 g OGTT compared to baseline. However, there was no change in blood pressure after ingestion in the DM group. The IFG group had greater grip strength than the CON group; however, there were no differences in other variables between the groups. [Conclusion] After a 75 g OGTT, elderly women with hypertension and diabetes maintain higher blood glucose levels compared to those with hypertension alone. Unlike elderly women with hypertension alone, those with hypertension and diabetes did not show changes in arterial stiffness and blood pressure after a 75 g OGTT. Therefore, elderly women with hypertension and diabetes may not be able to control their blood vessels following a 75 g OGTT due to impaired vascular endothelial function. Moreover, there was no association between diabetes stage and physical fitness in elderly women with hypertension.

일 농촌지역 성인의 고혈압과 당뇨병 관리 행태 (Management Behavior in Hypertension and Diabetes of Rural Adults in Korea)

  • 연구의 목적은 일 농촌지역의 전수조사를 통해 고혈압과 당뇨병 환자들의 관리 행태를 분석하기 위해 시도되었다. 조사는 2014년 5월 19일부터 9월 21까지 구조화된 설문지를 이용한 자기기입식으로 진행되었다. 연구결과, 고혈압으로 진단 받은 사람 355명(36.6%) 중 현재 치료를 받고 있는 사람은 337명(94.9%)이었으며, 당뇨병으로 진단을 받은 사람 109명(11.2%) 중에서 현재 치료를 받고 있는 사람은 96명(88.1%)이었다. 고혈압 잠재위험군 총 211명 중 고혈압 진단을 받은 사람은 89명(42.2%)이었으며, 당뇨병 잠재위험군 356명 중에서 당뇨병 진단을 받은 사람은 78명(22.0%)이었다. 위 결과를 바탕으로 고혈압과 당뇨병 예방을 포함한 효과적인 관리 전략을 국가 차원에서 수립하고 실행하는 방안을 제안하고자 한다.

고혈압과 당뇨병이 우울과 자살생각에 미치는 융합적인 영향 요인 (Convergence Effect Factors of Hypertension and Diabetes Mellitus on Depression and Suicidal Impulse)

  • 본 연구는 강원도 일 농촌지역의 주민들을 대상으로 고혈압과 당뇨병이 우울증과 자살의도에 미치는 영향력을 분석하고자 시도하였다. 설문조사는 2014년 5월19일부터 9월 30일까지 진행되었으며, 30세이상의 성인 1200명의 대상자 중 970명이 참여하였다. 조사 도구는 고혈압과 당뇨병 현황과 대상자들의 정신 건강 상태를 파악하기 위해 우울감과 자살생각 수준을 조사하였고, 건강생활 행태를 보기 위해 흡연과 운동여부를 조사하였다. 연구 결과, 고혈압과 당뇨병으로 진단된 환자들의 우울감과 자살의도가 유의미하게 높았다(p<0.001>). 또한 자살의도에 가장 영향을 미치는 요인은 고혈압과 우울감이었다. 이러한 결과를 바탕으로 지역사회 중심의 고혈압, 당뇨병 관리사업에 정신보건서비스를 반드시 포함시켜 포괄적이고 효율적인 운영이 되도록 노력하여야 한다.

한국성인의 고혈압, 당뇨 환자의 구강건강행태가 잔존치아 수에 미치는 영향 (Effects of oral health behavior on remaining teeth in Korean adults older than 45 years with hypertension and diabetes)

  • Objectives: The purpose of this study was to investigate the effects of oral health behaviors of patients with hypertension and diabetic patients on the number of remaining teeth in Korean adults over 45 years of age, furthermore, we also aimed to develop oral health care programs and health promotion for patients with hypertension and diabetes. Methods: Altogether, 1,526 subjects with hypertension and diabetes from the 6th Korean National Health and Nutrition Survey (2013-2015) were analyzed. Results: A multiple logistic regression analysis was performed to determine the effects of oral health behavior and periodontal disease on residual teeth in patients with hypertension and diabetes after adjusting for age. We found 1.53 times the risk of having less than 20 remaining teeth in the group of brushing more than 3 times a day (95% CI: 1.00,2.35, p<0.001), 2.27 times (95% CI: 1.23-4.17, p<0.05) in the group not using the dental floss group 0.05), compared to the group undergoing regular dental check-up (95% CI: 0.93-1.85, p>0.05), compared to the non-smoking group 0.77 times (95% CI: 0.51-1.14, p>0.05), 1.33 times (95% CI: 0.98-1.80, p>0.05) in the drinking group compared to the non-drinking group, in the periodontal disease group, the risk of having less than 20 remaining teeth was 2.19 times higher (95% CI: 1.48-3.24, p>0.001) than in the normal group. Conclusions: As a super-aging society, develops in Korea, the prevalence of hypertension and diabetes will increase gradually. Therefore, an oral health policy should be implemented at the national level for the prevention and treatment of periodontal disease as well as the management of oral health the elderly.

Effect of diabetes, hypertension, and obesity management on visual acuity in geriatric cataract surgery patients

  • Method : Using annual statistics of major surgeries provided by the National Health Insurance Corporation, it figured out what changes come in visual acuity by the number of cataract surgeries nationwide plus age, gender, and geriatric disease every 3 years from 2013 to 2019 through joint point regression for statistics. Objective : This study is intended to identify the relationship between geriatric diseases (diabetes, hypertension, obesity) and visual acuity in geriatric cataract surgery patients. Result : First, geriatric diseases of cataract surgery patients were closely related to diabetes, hypertension, obesity, smoking, and drinking. In particular, diabetes, hypertension, and smoking had a high prevalence rate. There was no difference in gender and age. Second, 72% of all geriatric cataract surgeries were performed at the clinic level, and intraocular lens that was used after geriatric cataract surgery accounted for the majority of monofocal intraocular lens as 96.6%. Third, the visual acuity in geriatric cataract surgery patients improved from an average of 0.40 before surgery to 0.06 after surgery, and visual acuity improvement was found in 95% of them. These results suggest that geriatric cataract patients can expect visual acuity stabilization and positive visual acuity improvement through early surgery.

식이 칼슘, 비타민 D의 섭취와 골다공증, 고혈압 및 당뇨병의 상관관계 (The Correlation between Dietary Intakes of Calcium and Vitamin D and Osteoporosis, Hypertension and Diabetes Mellitus)

  • In the light of the increasing concern over the prevalence of osteoporosis, hypertension, diabetes mellitus, we performed this study to review the correlation between the dietary calcium and vitamin D intake and these diseases. To this end, we investigated the effects of dietary calcium and vitamin D on these diseases. We observed that the intake of dietary calcium and vitamin D had a negative correlation with the incidences of osteoporosis, hypertension, and diabetes mellitus. Further, the intake of these two nutrients is expected to improve related mechanisms such as the renin-angiotensin system. Therefore, we suggest that dietary calcium and vitamin D have a beneficial effect on these diseases.

지역사회 고혈압·당뇨병 예방관리 인력의 건강증진 핵심역량 교육요구도 (Educational Needs for Health Promotion Core Competencies among Personnels of Hypertension and Diabetes Control in Community)

  • Objectives: This study was for analysis the educational needs of personnels who works for hypertension and diabetes centers in community in order to serve as the basis for the development of the future educational programs. Methods: To analyze the educational needs of personnels who wokrs for hypertension and diabetes centers in community, a survey based on CompHP core competencies framework for health promotion was conducted in September 2016. 102 personnels were answered to the online questionnaires and collected data were analyzed through SPSS. Results: The highest point of educational needs was 'how to use verbal and non-verbal effective communication skills' among 46 core competencies for health promotion and 'self-management practical skills for hypertension and diabetes patients'. There were some differences of educational needs between leaders and general staff members of centers. Most wanted educational subjects of leaders are 'contribute to the development and dissemination of health promotion evaluation and research process', and 'use evaluation findings to refine and improve health promotion action'. On the other hand, most general staff members of centers wanted 'use effective communication skills including written, verbal, non-verbal, and listening skills' and 'facilitate the development of personal skills that will maintain and improve health. Conclusions: Evidence-based and long-term educational programs should be developed for personnels who works for hypertension and diabetes centers in community.

전남지역 일부 고혈압·당뇨병 등록·관리센터 이용자의 일반특성, 자기효능감 및 식생활관리 실태 연구 (General Characteristics, Self-Efficacy, and Diet Control of Hypertension Patients at a Diabetes Admission Control Center in the Jeollanma-do Area)

  • This study investigates general environmental factors influencing hypertension and diabetes patients and their disease control methods, self-efficacy, nutritional risk, diagnosis of eating styles according to nutritional risk, accountability in eating habit instructions, and nutritional intake and provides basic data for eating habit control in hypertension and diabetes patients. For this, 70 patients who visited a hypertension and diabetes admission center were interviewed using a questionnaire. According to the results, the implementation of self-efficacy in hypertension and diabetes was higher in female subjects. The diagnosis of eating type with nutritional risk was higher in female subjects, and there was a significant difference between male and female subjects (p<0.05). Both sexes had scores above 6 in nutritional risk and were diagnosed to have a "high-risk nutritional status." In the diagnosis of eating habits with nutritional risk, diet quality was higher for female patients (p<0.05), and the nutritional intake of subjects was low for most nutrients. In particular, the intake of calcium, vitamin A, riboflavin, and folic acid was low, indicating a need to improve eating habits for the balanced intake of nutrients because of the increasing importance of eating habits for controlling chronic diseases.

일 지역 만성질환관리 건강포인트사업이 고혈압 및 당뇨병관리 지식에 미치는 효과 (Hypertension and Diabetes mellitus Management Effects of Community based Chronic Disease Management Program)

  • Purpose: The aim of this study is to evaluate the effects of community based chronic disease management program for managing hypertension and diabetes mellitus. Method: The subjects of this study were 1,000 clients, who participated in a city community based chronic disease management program. The data were collected by a phone survey directly, and selected from DB of community based chronic disease management program, indirectly. The data were analyzed by the STATA program, using descriptive statistics, t-test, and Multiple regression analysis. Results: The mean score of knowledge of hypertension was 4.33 and diabetes mellitus was 2.52. The affecting factors of knowledge of hypertension were the clinic visiting point and blood sugar. However, the affecting factors of knowledge of diabetes mellitus was core lab point, diastolic blood pressure, and blood sugar. Conclusion: the results of this study showed that knowledge of hypertension increased with higher level of education, and younger ages, while knowledge of diabetes mellitus increased with higher level education, more core lab point, younger ages, and in males. Therefore, to control the chronic diseases more effectively in a community, it is important to develop a more systematic education and self- managing programs, using collaboration of health centers and local clinics.

만성질환 교육이 고혈압, 당뇨병 환자의 지식수준에 미치는 영향 (Effects of Chronic Disease Education for Hypertension, Diabetes Patients's Knowledge)

  • Objectives: This study is based on Public Care Service for Chronic Diseases in Incheon Metropolitan City featuring Health-Point Service. It is objective to evaluate whether the service could improve self-management ability of hypertension and diabetes patients. Methods: The knowledge about diabetes and hypertension, which can be improved by educational intervention, is considered as one of the most important factor for self-management ability. While the education service have been conducted, 194 patients who have been diagnosed as hypertension and diabetes within 3 months were included. They completed questionnaires two times within 4 months period, which give information about the knowledge, service registration and education experiences. For further inspection, 396 patients who had been diagnosed formerly were studied by cross-sectional analysis. Results: Regardless of service registration or educational intervention, there wasn't statistically meaningful difference between the initial and the follow-up assessments on the knowledge. However, when investigating formerly diagnosed patients, educated patients had better knowledge on hypertension compared with non-educated patients and registered patients, educated patients had the better knowledge on diabetes compared with non-registered patients, non-educated patients. Additionally, analyzing each questions, educated patients got higher grades in questions about definition of hypertension, diabetes managements which they can practice in daily lives. Conclusions: It is probable that long-term service registration and education experiences could improve the knowledge and self-management ability.
