일개 보건소에서 시행한 불면증에 대한 한약치료의 효과분석 (Clinical Effect of Herbal Medicine Treatment on Insomnia in a Public Health Center)

  • Objectives: We conducted a public health promotion program in Korean medicine for participants suffering from chronic insomnia in 2016~2017. A significant improvement effect was obtained through the 8-week herbal medicine treatment program. The object of this study is to report the results of this public health promotion program. Methods: Seventy patients diagnosed with chronic insomnia participated in the program, 49 of whom completed the program. Participants were treated with the herbal medicine for insomnia treatment for 8 weeks. A sleep diary was used to assess the total sleep time, sleep latency, and number of times the participants woke. The treatment satisfaction was then evaluated. Results: Through the 8-week herbal medicine treatment program, the total sleep time was significantly increased by $109.59{\pm}70.77$ minutes, the sleep latency was significantly decreased by $63.53{\pm}47.79$ minutes, and the number of times the participants woke was significantly decreased by $1.54{\pm}1.19$ times compared to the baseline (all, p=0.000). Among the 24 patients who were examined for the treatment satisfaction, the average score was 4.33 points of a total of 5 points. Six patients had adverse events. Except for one patient, the symptoms of other patients were mitigated by changing or discontinuing the herbal medicine. Conclusions: For patients with chronic insomnia, the 8-week herbal medicine treatment program was effective, with few side effects and high treatment satisfaction. This study has significance as this is the first report in Korea on the effect of a herbal medicine treatment program for chronic insomnia through Korean medicine public health promotion.

Clinical Efficacy of herbal medicine for Chemo-Radiotherapy-induced oral mucositis

  • Objectives: The purpose of this systematic review was to evaluate the effects of herbal medicine on Chemo-Radiotherapy-induced oral mucositis. Methods: Electronic databases were used to search for studies published through 10 years until October 2022, and a randomized controlled study was conducted to evaluate efficacy of herbal medicine on chemo-radiotherapy-induced oral mucositis. Study quality was assessed using the Cochran's risk bias tool. Results: Two-hundred and three articles were initially searched, and 11 studies (head and neck cancer, breast cancer, colorectal cancer, esophageal cancer etc. undergoing radio-chemotherapy were included in analysis. The effect of herbal medicine on chemo-radiotherapy-induced oral mucositis, 9 studies reported that herbal medicine was more effective than the placebo group or conventional treatment. One study reported that the effect of the herbal compound treatment group was similar to that of the conventional herbal medicine, and one study reported that there was no difference in effect between the two herbal medicines and the group without treatment. Conclusion: This study suggests that herbal medicine effectively relieves the symptoms of chemo-radiotherapy-induced oral mucositis. However, there is limited evidence that herbal medicine may relief chemo-radiotherapy-induced oral mucositis, so further investigation is needed.

소아의 난치성 신증후군의 한약 치료에 대한 최신 임상연구 동향 (Review of Clinical Research for Herbal Medicine Treatment on Refractory Nephrotic Syndrome in Children)

  • Objectives The purpose of this study is to analyze clinical studies on effectiveness of herbal medicine in refractory nephrotic syndrome (RNS). Methods We searched the randomized controlled trials (RCTs) with herbal medicine treatment on RNS from the Pubmed, CNKI, OASIS, NDSL, J-stage, and CiNii. The demographic data, duration of illness, intervention, treatment period, outcome, adverse events, and composition of herbal medicine were analyzed for this study. Results 11 RCT studies were selected and analyzed. The children in the control group were given western medicine therapy, and the treatment group was given herbal medicine along with the same western medicine of the control group. The most commonly used herbal medicines were the prescriptions to treat 'Kidney Deficiency with Blood Stasis (腎虛兼瘀血)' which composed of 'Promoting blood circulation (化瘀)', 'Diuresis-inducing (利水)', or 'Heating Yang (溫陽)' medicine based on 'Replenishing Kidney or Spleen (補腎, 補脾)' medicine. In the treatment group, proteinuria and serum lipid was significantly decreased, serum albumin was significantly increased, and total effective rate was significantly higher than the control group. Hypercoagulation and relapse rate was also significantly reduced. Adverse events were significantly lower in the treatment group. Conclusions Herbal medicine treatment on pediatric RNS can be suggested as a new treatment for children who have less response to the conventional therapy. It can also supplement the limitations of the western medicine by reducing adverse events from the steroids and immuno-suppressive agents, and lower the relapse rate as well.

  • There are some controversies about the safety of herbal medicine. In order to examine the safety of herbal medicine, this study investigated the level of liver function test(AST/ALT) in patients taking herbal medicine for more than 6 months We checked liver function in 121 patients who took herbal medicine for more than 6 months. AST/ALT were measured before treatment and every 3 months (from 3 month to 9 month). In 121 patients taking herbal medicine for more than 6 months, mean AST level after 6 months was lower than before treatment and mean ALT level after 6 months was lower than before treatment. In 20 patients with abnormal AST/ALT before herbal treatment, 18 patients's AST/ALT changed to normal after 6 month herbal treatment. 2 patients's AST/ALT was slightly higher than normal. One woman patient met the criteria for herb-induced liver injury(HILI) with RUCAM scores 4 after taking herbal medicine for 6 months. Although her RUCAM score decreased to zero after taking herbal medicine for 9 months. This study suggests that long-term herbal medicine for 6 months or longer is very unlikely to injury liver function. Individual-specific liver damage may occur, it can be recovered.

한약치료로 호전된 폐경 여성 환자의 질염 1례 (A Case Report of Vaginitis in Menopausal Woman Treated by Herbal Medicine)

  • The purpose of this study is to report the effects of Korean medicine treatment, especially herbal medicine on vaginitis in menopausal woman. The patient suffered from vaginitis was treated with herbal medicine such as Yongdamsagan-tang and Jibaekjihwang-tang. The effect of treatment was evaluated through questionnaire. After treatment, symptoms of vaginitis were reduced. This case report shows that herbal medicine treatment may be effective for vaginitis.

무증상성 단백뇨에 대한 단기 한약 투약 경과 : 증례보고 (Progress of Short-Term Herbal Medicine Administration for Asymptomatic Proteinuria: Case Report)

  • Objectives: This case reports a certain level of improvement for asymptomatic proteinuria with short-term administration of herbal medicine. Methods: In the first treatment phase, the patient was treated with Dangguisu-san (DGSS) for his rib fracture. In the second treatment phase, the patient was treated with a herbal formulation of Astragali and Angelicae Sinensis (A&As particle) for asymptomatic proteinuria. Results: Treatment with herbal medicine resulted in a decrease in the patient's complaints regarding symptoms. Also, proteinuria-related items in hematology and urinalysis were improved. Conclusions: Herbal medicine therapy may be effective for proteinuria treatment.

임신성 피부질환의 한약 치료에 대한 국내 연구 동향 (A Review of the Domestic Study Trends on Dermatosis Gravidarum Treated with Herbal Medicine)

  • Objectives: The purpose of this study is to investigate the domestic study trends on dermatosis gravidarum treated with herbal medicine. Methods: We searched for studies of dermatosis gravidarum treated with herbal medicine published until September 2019, through 6 Korean web databases. After searching studies, we analyzed selected studies. Results: As a result, eight studies were included in this study, all of which were case reports. Three studies used herbal medicine alone, three studies used herbal medicine with acupuncture, and two studies used herbal medicine with other treatment. In all of eight studies, herbal medicine treatment was effective for dermatosis gravidarum. Conclusion: This study shows that herbal medicine could be helpful in treating dermatosis gravidarum. However, in order to consolidate the foundation of herbal medicine treatment for dermatosis gravidarum, more case reports and high-quality clinical researches are needed.

골관절염 환자의 한약 양약 투여 현황 및 안전성 연구 (A Study of the Co-Administration of Herbal and Western Medicines to Hospitalized Patients with Osteoarthritis)

  • Objectives: The combined use of herbal and Western medicine is increasing. However, herbal medicine is highly likely to interact with Western medicine making it important to understand the effects of co-administration. This study investigates the ratio of patients who take Western medicine with herbal medicine, the types of medicines commonly prescribed together, and the results of hospital examinations. Methods: We investigated patients who were hospitalized at Kyung Hee University Korean Medical Hospital for at least one day from January 1, 2010 to December 31, 2017. There were some inclusion criteria. First, we chose patients aged 19 and over. Second, we chose patients who were diagnosed with osteoarthritis (OA) with diagnosis codes M13, M15, M17 according to KCD-7. Third, patients had liver function tests, renal function tests, and general hematology tests performed at least two times during hospitalization. Results: Among a total of 131 OA patients, 32 (24.4%) patients were treated with herbal-Western medicine combination therapy. The most commonly prescribed herbal medicine was Daegalwhal-tang, and the most commonly prescribed Western medicine was celecoxib. In the laboratory findings, all liver function tests, renal function tests, and general hematology tests showed no difference compared to admission day. There were also no differences between herbal medicine single treatment and herbal-Western medicine combination treatment. Conclusions: From these results, we suggest that herbal medicine single treatment and herbal-Western medicine combination treatment for OA patients does not cause adverse effects.

이명(耳鳴)의 한의학적 치료에 대한 임상적 고찰 (Acupuncture Combined with Herbal Medicine for the Treatment of Tinnitus;Clinical Study in 11 Cases)

  • Background and Objectives : This case report aims to determine the effect of acupuncture and herbal medicine as a treatment for tinnitus. Subjects and Methods : 11 patients with tinnitus were treated by acupuncture combined with herbal medicine. Patients were evaluated with verbal numerical rating scale scores for loudness, annoyance, and awareness of tinnitus before and after treatment. Results : 9 patients reported reduction of loudness, annoyance, and awareness of tinnitus after treatment. Among them, complete suppression of the tinnitus was obtained in 1 case. 2 patients reported no improvement. Conclusions : This case study presents clinical possibility of acupuncture and herbal medicine for the treatment of tinnitus.

  • Objectives The purpose of this study is to analyze Chinese randomized controlled trials (RCTs), and to evaluate the effectiveness and safety of the oral herbal medicine treatment for pediatric chickenpox. Methods We searched RCTs after 2000s from the China National Knowledge Infrastructure (CNKI). Afterwards, the year of publication, demographic information, duration of chickenpox, intervention, treatment duration, outcome measure, results and adverse events were investigated and analyzed for this study. Results 21 RCTs out of 219 studies were collected and analyzed. Although each composition of the herbal medicine was different, they have common ingredients such as 清熱解毒, 散結消腫, 疏散風熱, 涼血解毒, 解表散風 in order to make efficacy of 清熱解毒, 散結消腫, 疏散風熱, 涼血解毒, 解表散風. The oral herbal medicine showed better efficacy and safety to improve clinical symptoms such as total efficacy, cure rate, antipyretic time, antipruritic time, scab time, anti-rash time, hospitalization period and herpes recovery time compared to the western medicine treatment. Conclusions These results show that the oral herbal medicine treatment on chickenpox in children may be more effective in reducing of clinical symptoms compared to the western medicine treatment.
