A Study on Curriculum Development For Community Health Practitioners (보건진료원 직무교육 교과과정 개선을 위한 일 연구)

  • This study was designed to develop a conceptual framework for the curriculum and develop the details of the learning content for the education of Community Health Practitioners (CHPs). Since education programs for CHPs started 10 years ago, concepts related to CHP services have changed because of changes in society. The objectives of the study were as follows : 1) to analyse the usefulness of the present education program for CHPs, 2) to analyse the Job performance and self -confidence of the CHPs, 3) to identify the health needs of the clients served by the CHPs and the community problems related to health. 4) to develop a conceptual framework for the curriculum, for the education of CHPs, 5) to develops details for the learning content of the education program for CHPs. Phase I of the study was conducted by questionnaires to 150 CHPs who have worked in remote rural areas for more than 2 years. Among them, 147 responded. Data was collected from August 16, to August 25, 1990. In order to identify the health needs of the community people, research within the last five years was reviewed and analyzed. The data on 1, 842 communities gathered by the WHO Nursing Collaborations Center of the College of Nursing, Yonsei University was utilized to identify community problems related to health and the self - confidence in job performance of the CHPs. Psase II of the study consisted of a workshop with 13 professionals including Community Health Practitioners to evaluate the existing education program and a conceptual framework of the curriculum for the job education of CHPs. The results of the study are Summariged below : 1. The only 26 among 45 content items of the education program related to job skills was used by 80% of the responding CHPs. The knowledge of $\ulcorner$Networking community organization$\lrcorner$ was used by only 53.7% of the respondents. Educational content about $\ulcorner$Mental disease$\lrcorner$ was used by less than 50% of CHPs because of a knowledge deficit. 2. The CHPs reported that their activities concentrated on clinical services during the last six months. The survey showed that they seemed to neglect the activities for health promotion and disease prevention. Thus, $\ulcorner$Education for community loaders$\lrcorner$(15.9%), $\ulcorner$Activity for eavironmental health$\lrcorner$(16.3%) and $\ulcorner$Social work for needey people$\lrcorner$(23.3%) were done by less than 30% of CHPs. 3. More than 90% of CHPs reported being self - confident for the activities of $\ulcorner$Health education and counselling$\lrcorner$, $\ulcorner$Medicine prescription$\lrcorner$ and $\ulcorner$Immunization$\lrcorner$. But 50% of CHPs reported that they were not have self - confident in $\ulcorner$Management of water and environmental health$\lrcorner$ and only 25.6% of CHPs could insert an IUD independently. 4. It was identified that respiratory diseases and the gastrointestinal diseases were most common problems for the community people, followed by musculoskeletal and skin problems. 5. The community problems were classified into eight categories : physical environmental problems, environmental hygiene, health problems, health behavior, social problem, lack of resources, financial problem and the problems of the cultural and value system. 6. The conceptual framework consisted of the target population and their health status, nursing process working site and primary health care services such as health promotion, disease prevention, treatment and rehabilitation. 7. The contents of curriculum of education program for CHPs were formulated from the results of this study.

Main Indicators of National Health Insurance during 40 Years (건강보험 40년의 주요 지표)

  • This year marks the 40th anniversary of the introduction of National Health Insurance (NHI) which has contributed to improving public health and accessibility. This article aims to show the trends of main indicators during the last 40 years. NHI has achieved rapid expansion of target population (1977-1989). The percentage of population covered increased from 8.8% in 1977 to 94% in 1990. The average number of visit days per person was 0.75 in 1977 but significantly increased to 31.11 in 2015. In 2015, NHI revenues were 52.4 trillion won and expenditures were 48.2 trillion won which is 9.5 times and 9.6 times higher than in 1995. NHI achieved universal coverage in short period of time and has contributed to improving the healthcare status. However, there still remain problems including low-benefit coverage and high out of pocket money. Therefore, the effort to reform these problems is needed.

Health Status of Preschoolers Depending on the Socioeconomic Status (일개 시 미취학 아동의 사회경제적 계층별 건강관리 현황)

  • Purpose: The current descriptive study examined the health and healthcare status of preschoolers in poverty group compared to those of comparison group. Method: Data were collected from 51 mothers in federal poverty group and 49 in low-income group and 200 mothers in normal income comparison group in one city. The instruments used for this study were Korean CBCL and health related questionnaire. Results: Public assistance group showed more low-birth-weight infants, developmental delay, and hospital admission experiences. Their compliance for vaccination and congenital metabolic disorder test were lower than those of comparison group. Preschoolers' behavior problems were not significantly different depending upon the income of the group. Conclusion: Preschoolers' health and healthcare status are affected by the socioeconomic characteristics of the family. It indicated an need for anticipatory guidance and early intervention that might help to prevent or ameliorate potential health problems in poverty.

  • The study of the elderly and poor oral status interpersonal relationships and smooth social life limited to give is the social alienation and isolation, promoting to having problems with a sense of the elderly subjective oral health status and social efficacy affects whether analyzed. 1. Subjective oral health status authoring feel healthy food disorders, toothache, periodontal problems, tmj pain, dry mouth, bad breath symptoms such as 'sometimes' 'often' than a 'no' if you appear to be a highly subjective and social efficacy Efficacy of oral health status and social influence were more (p<0.01). 2. Subjective oral health status of the seven kinds of sub-variable that oral health status, food authoring disorders, toothache, gum disease, jaw joint or more, dry mouth, bad breath instantly and look at the relationship between social efficacy oral health status, ability of mastication, pain in oral, gum disease, tmj pain, dry mouth, presence of halitosis than positive (+) was correlated.

A Comparison between the Second KoreanWorking Conditions Survey (KWCS) and the First KWCS

  • Background: The study was designed to assess the changes in working conditions through a comparative analysis of the characteristics of working conditions in 2006 and 2010. Methods: We performed a comparative analysis of the data related to the first Korean Working Conditions Survey (KWCS) and the second KWCS in the categories of demographic characteristics, quality of labor, exposure to hazards, and health problems. Results: From our analysis of the demographic characteristics, we saw an increase in labor force participation rate of women and elderly people. As a result of the investigation with regards to working hours, the ratio of employees who worked for ${\geq}49$ hours per week was decreased and the ratio of employees who worked for ${\geq}40h/wk$ increased. As for exposure to hazards, exposure to tobacco smoke notably decreased in 2010 compared with 2006. With regards to health problems, there was a sharp increase in the number of people who complained of muscle pain in their arms and legs. Conclusion: KWCS data included many aspects of working conditions as a nationwide sample. In addition, because this is a periodic nationwide survey, the labor force, working hours, harmful factor exposure, and the change in health problems characteristics according to the flow of time could be investigated. The information comparing the main results of the first survey conducted in 2006 and the second survey conducted in 2010 obtained through this study can be used as an important base material for the establishment of the national policy.

The Current Status of Student Mental Health Programs and the Needs for Related Training: Focusing on School Nurses and School Counselors (학생정신건강사업 현황 및 관련 교육 요구도: 초중고 보건교사와 상담교사 대상)

  • Purpose: The leading cause of death among teenagers in Korea is suicide. Also, it is known that mental health of Korean adolescents identified based on the levels of perceived stress or depression is very poor. The purpose of this study was to examine the current status of student mental health programs in Korea and the associated problems and to explore ways to make the programs more effective. Methods: The subjects of the study were a total of 115 school nurses and school counselors in elementary, middle and high schools. The study examined the current status and the associated problems of school mental health programs as well as the needs for teacher training programs related to student mental health. Results: Few mental health programs other than counseling for at-risk students were being carried out. The problems associated with student mental health management were, in terms of management, 'accountability in case of incidents such as student suicide' and 'parents uncooperative in their kid's mental health management (visit to hospital, etc.)' and, in terms of manpower, 'unclear R&R between school nurses and school counselors' and, more fundamentally, 'the lack of human resources.' The needs for teacher training programs related to student mental health were very high and the content required most was 'ways to manage stress of students and teachers themselves,' followed by 'interpersonal conflicts management & conservational skills,' and 'ways to manage student mental illness.' Conclusion: In conclusion, to increase the effectiveness of student mental health programs, we should move on from the current system which focuses only on early discovery and intervention to a more comprehensive system that includes preventive approaches, while having clearer R&R and more workforce dedicated to student mental health programs and attracting more parental cooperation and support.

A Study on Efficiency of Case Management Practical Model for Industrial Injury Inpatients (산업재해 입원환자를 위한 사례관리실천모형의 효과성에 관한 연구)

  • This study is to observe the effectiveness of the applied model and to present the improvement plan and directions for development for the case management practical model suitable for the actual condition of Korea Labor Welfare Co. and needs of the industrial injury patients. The concrete purpose of this study is: First, observe the difference of stressor experience and experience degree between the experimental group and the comparative group. Second, observe the difference of stress of the experimental group and the comparative group. Third, find out how the stress affects the support degree and satisfaction degree. Fourth, present the improvement plan of case management model, which can promote the psychosocial rehabilitation of the industrial injury patient based on the research results. The outline of the main research results identified in this study is as follows. The stressors the industrial injury patients perceived are health problems, family matters, the problems concerning hospital recuperation (hospital staff and environmental problems), economical problems, problems of coming back to society, problems with companies, problems with Korea labour Welfare Co. and other problems. And the experience of stressor was prominently lower in experimental group than comparative group in the whole problem, health problem, problems with Korea Labour-Welfare Co. and other problems. The stressor experience degree was conspicuously lower in experimental group in the whole problem experience degree, health problem experience degree, problem with Korea Labour Co. experience degree and other problem experience degree. Besides whether or not the case management is applied is having a prominent affect on the primary factor affecting the stressor experience degree, therefore the patients applied with case management has less stressor than the patient who didn't. The difference of degree of tension experienced by the stressor in both groups, the degree of stress, was not conspicuous in statistics so it shows that the application of case management in this research has not affected the degree of tension. The field which had been the most help was emotional support in help level the experimental group perceived through applying case management about industrial accident patients and recuperation, compensation problem, medical treatment problem, family matters has been helpful in this order. The help level of the whole problem was in higher level than the middle value. The stress factor which affects the case management problem settlement is the whole body of stress. The satisfaction level of help through applying case management was highest in emotional support and family matters, recuperation problem, company problem, compensation problem, and medical treatment problem was the next highest. The satisfaction level of the whole problem was higher than the middle value. The stress factor affecting the satisfaction level of help is the whole body of stress. Therefore to reduce the stress level of industrial accident patients and for them to come back to local societies, we need to reinforce the continuance and responsibility of case management model, increase staff, reinforce the role of counsel and medical treatment, intervene in the patient's plan of leaving the hospital, develop social support system and the need to establish After Care Center.

A Review of Factors in Elderly Health-Promotion Behaviors (노인의 건강증진 행위와 관련된 변수분석)

  • Recently the number of elderly have been increasing at a rapid pace in Korea. Accompanying this aged society are numerous health problems. Whit this in mind health-promotion behaviors such as physical activity. good nutrition and stress management are vitally important to the elderly in reducing the risk of Health problems, maintaining health and improving the overall quality of life. Health promotion programs for the elderly must be developed. The purpose of th is study was to determine influential cognitive factors on health-promotion behaviors and the effects of previously implemented Health promotion programs. For this, previous studies were reviewed and analyzed. The results were as follows. 1. Cognitive factors on health-promotion behaviors were internal locus of control, perceived health status, self-efficacy, concerns about health, social support, attending social activities. 2. Components of health promotion programs were exercise and health education. Exercise was performed in most programs. The effects of exercise programs were improved flexibility, muscle strength. balance, cardiopulmonary function and elevated ability of daily living, perceived health status, quality of lift and a decrease depression. The results strongly suggest that complex health promotion programs should be developed. Health promotion programs need to include exercise, health education, health counseling and social activities. We have to consider cognitive factors on health-promotion behaviors.

Health Care Reform for Sustainability of Health Insurance (건강보험의 지속을 위한 개혁과제)

  • We achieved both industrialization and democratization during the shortest period in the world. We also achieved good performance in national health insurance: universal coverage, solidarity in financing, equitable access of health care. However, national health insurance system has faced the problem of sustainability: various expenditure and financing problems. The problem of sustainablity has two facets of economic sustainability and fiscal sustainability. Economic sustainability refers to growth in health spending as a proportion of gross domestic product(GDP). Rapid increasing rate of health spending exceeds the growth rate of domestic product. Growth in health spending is more likely to threaten other areas of economic activity. Concern on fiscal sustainability relates to revenue and expenditure on health care. Health care financing face demographic and technical obstacles. Democratic obstacle is aging problem. Technical obstacle is collection of contribution. Expenditure of health care has various problems in benefit structure and efficiency of health care system. In this article, I suggest several policy reforms to enhance sustainability: generating additional revenue from value added tax, changing method of levying contribution, increasing efficiency of health care system by introducing the competition principle. restructuring of benefit scheme of health insurance. contracting with health care institutions to provide health care services.

TEACHERS' PERCEPTIONS OF MENTAL HEALTH PROBLEMS IN STUDENTS AND PSYCHIATRIC CONSULTATIONS (학생들의 정신건강문제와 정신과 의뢰에 대한 교사의 인식도 조사)

  • This study is attempted to examine Korean teachers’ perceptions of mental health problems in students and their attitudes towards psychiatric consultations through a questionnaire survey. The results of this study are as follows. 1) Teachers thought that 5.3% of their students had mental health problems, 1.9% of students required psychiatric evaluation, and 1.2% of students were referred or recommended psychiatric evaluation by their teachers. 2) The most frequent mental health problems in students discovered by teachers were distractibility and inattention, lack of academic skills, and language difficulties in elementary schools;distractibility and inattention, conduct problems, and lack of academic skills in middle schools;and conduct problems, distractibility and inattention, physical symptoms, and substance abuse in high schools. 3) Teachers thought the mental health problems in students were caused by the family environment, psychological factors, the educational system, and a lack of mental health services. 4) Teachers desired smaller classes, improvement of the school environment, more time, regular mental health education, a special program for students with mental health problems, and the counseling staffs or consultants for the school mental health. 5) Teachers consulted with other teachers, the parents of the students, the counseling teachers, the health care teachers, the counseling institutes, the psychiatric clinics, and the principals in descending order to handle the hard case problems. The frequent reasons for failing in psychiatric consultations were the prejudice of parents against psychiatric services, the teachers’ sense of superiority in dealing with the problems of students, the prejudice of teachers themselves against psychiatric practice, and inaccessible professional consultation. 6) About 20.4% of teachers reported they had proposed psychiatric consultations or had recommended their students to receive psychiatric evaluations.
