인천지역 중소규모 사업장들의 보건관리대행제도에 대한 인식 및 태도 (Perception and Attitude toward Group Health Management and Service System for Small and Medium Industries in Inchon)

  • To investigate perception and attitude toward Group Health Management and Service System for Small and Medium Industries, a survey using self-administered questionnaire was conducted to a part of industries in Inchon, at October 1992. Major dependent variables were perception, attitude, and practice related with Group Health Management and Service System; these variables were measured by 3-point Likert like scale consisted of 7, 5, and 5 items, respectively. Data were collected in 149 industries,72.7% out of 205. Perception nab slightly high, 1.25; attitude was some positive, 1.46; and practice was some passive,0.94. Major determinants of perception, attitude, and practice were sex and age of industry's health manager; perception influenced upon attitude, and perception and attitude had positive effect upon practice. To activate Group Health Management and Service System, it is necessary to develop education and promotion programs for industry's health managers of small and medium industries.

Occupational Health Management in the Lead Industry: The Korean Experience

  • In 1967, the problem of occupational lead exposure came to public attention in Korea. Since then, regular progress has been made in lowering workplace lead exposures, instituting new workplace controls, and implementing health examinations of exposed workers. Past serious lead poisoning episodes made it possible to introduce biological monitoring programs on a voluntary basis in high-lead-exposure facilities in Korea. Industry-specific occupational health services for lead workers in Korea during the last 22 years can be categorized into three phases. During the first phase (1988-1993), efforts were directed at increasing awareness among workers about the hazards of lead exposure, biological monitoring of blood zinc protoporphyrin began, and a respiratory protection program was introduced. During the second phase (1994-1997), a computerized health management system for lead workers was developed, blood-lead measurement was added to biologic monitoring, and engineering controls were introduced in the workplace to lower air-lead levels to comply with air-lead regulations. Finally, during the third phase (1998-present), a new biomarker, bone-lead measurement by X-ray fluorescence, was introduced. Bone-lead measurement proved to be useful for assessing body burden and to demonstrate past lead exposure in retired workers. Occupational health service practice for lead workers, including the industry-specific group occupational health system, has brought considerable success in the prevention of lead poisoning and in reducing the lead burden in Korean lead workers during the last several decades. The successful achievement of prevention of lead poisoning in Korea was a result of the combined efforts of lead workers, employers, relevant government agencies, and academic institutes.

경인지역 일부 근로자와 보건담당자의 보건관리에 대한 인식 및 태도 (Knowledge and Attitude of the Workers and the Health Personnel on the Health Management in Kyung-In Area)

  • 본 연구는 대행기관의 보건관리를 받고 있는 경인지역 51개 사업장 근로자들 중 무작위로 추출된 247명과 보건담당자 46명의 보건관리에 대한 인식과 태도를 조사한 것이다. 1992년 12월부터 1993년 2월까지 3개월간 설문조사를 실시하였으며, 조사결과는 다음과 같다. 1. 생산직근로자는 일반적 직업적 특징상 보건담당자보다 연령, 학력 및 직위 등이 유의하게 낮았다. 2. 생산직근로자는 담당자보다 직업병에 걸릴 것이라는 인식이 강하고 작업환경에 대해 더욱 열악하다고 인식하고 있었으며 작업환경 측정이 유해인자 평가에 유용하지 않다고 생각하는 경향을 보였다. 또한 두 군 모두에서 작업환경 측정이 유해인자를 평가하는데에 유용하다고 생각할수록 작업환경이 개선되고 있다고 생각하는 경향이 있었다. 3. 보건관리대행에 대한 인식도 두 군간에 유의한 차를 보여서 근로자의 72.6%가 보건관리대행을 받고 있는지를 알지 못하고 있는 반면, 보건담당자의 82.2%가 잘 알고 있다고 응답하였다. 그러나 보건관리대행의 필요성에 있어서는 두 군간에 유의한 차가 없었으며, 전체의 79.0%가 보건관리대행이 필요하다고 응답하였다. 4. 보건교육에 있어 근로자의 73.1%가 보건교육을 받은 적이 없는 반면, 전체의 93.0%가 보건교육의 필요성을 인식하고 있었다. 보건교육의 필요성은 보건교육에 대한 만족하는 정도와 관련이 있었고, 또한 보건교육의 만족 정도는 보건관리대행의 필요성 인식과 관련이 있었다. 5. 두 군 모두에서 현행의 보건관리가 너무 형식적이라고 인식하고 있었으며, 이러한 형식성의 탈피와 정기 건강진단에 대한 추후 관리를 희망하고 있었다. 이상으로 미루어 보아, 대행기관의 사업장에 대한 보건관리는 근로자들의 적극적인 자세에 비하여 효율적으로 이루어지지 않고 있다고 판단되었다. 그러므로 근로자들이 작업환경의 유해요인에 대해 올바르게 인식하므로써 적절한 태도를 취할 수 있도록 하기 위하여서는 보건담당자뿐 아니라 근로자에 대한 보건교육을 포함한 보건관리를 보다 적극적인 자세로 강화시키고 정착시키는 것이 필요할 것으로 제시되었다.

Satisfaction of industrial health care managers regarding the work of industrial hygiene engineers: a cross-sectional study

  • Background: A group health service is a system that delegates workplace health management to an entrusted institution. There have been various studies on group health services to date, but recent changes, such as an increase in foreign workers, are rapidly changing industry characteristics. Methods: Satisfaction was assessed using a 27-question survey distributed among 203 workplaces employing health professionals. The survey items consisted of general characteristics, comprehensive satisfaction, requirements for health professionals' work, and satisfaction with work environment management, ergonomic management, and healthcare management. Multiple regression and frequency analyses were performed. Results: The comprehensive satisfaction was 4.08 points on average, out of 5. The comprehensive satisfaction of health professionals in the industry was positively correlated with each factor. Hazardous materials and chemical management (material safety data sheets, MSDSs) were the most common requirements. Conclusion: A low level of satisfaction with work environment management indicates high demand for healthcare management. The working environment should be improved by identifying characteristics of the workplace, examining harmful substances, inspecting equipment, and enhancing worker methods. The shorter the work experience of health professionals, the more dependent they are on group health services. The variables affecting comprehensive satisfaction were the period of work, healthcare management satisfaction, and work environment management satisfaction. Most of the requirements of health professionals in the workplace were practical improvement case presentations, MSDSs, and legal document management.

산재장해인의 장기요양서비스 요구 분석 (Long-Term Care Needs Assessment of the Disabled Workers After an Industrial Injury)

  • Purpose: The objective of this study was to evaluate long-term care needs using RAI MDS-HC and MI-CHOICE among the disabled workers. Methods: Data were obtained from 45 personal care recipients with the disability of mental and nervous system, and analyzed using SAS 9.1 by applying t-test, ${\chi}^2$ test, or fisher's exact test. Results: Only 'bed mobility' and 'indoor ambulation' items of ADL and problem activity were statistically significant factors by the level of personal care benefit. By MICHOICE grouping, 20.0 percent of subjects belonged to nursing home group, 51.5 percent were home care service, 28.9 percent were intermittent personal care. Conclusion: Personal care services in industrial accident compensation insurance have been categorized with two groups according to level of disability. But our results could contribute to provide personal care service according to the long term care needs.

근로자의 피로수준에 따른 직무 스트레스 및 삶의 질 (A Study on the Relationship between Fatigue Level, Job Stress and Quality of Life for Workers)

  • Purpose: The purpose of this study was to seek ways to improve the quality of life of workers by separating workers into the low fatigue group and the severe fatigue group depending on the level of fatigue level, and analyzing and understanding the difference between quality of life and job stress according to the fatigue level. Methods: For the 'Development of Mibyeong management system which was a consumer-directed health care service', the data of job stress, fatigue and quality of life was collected from 115 workers who visited the oriental hospitals located in J city and C city from Jul, 2012 to Dec, 2012. Results: The severe fatigue group showed significantly higher job stress than the low fatigue group in physical environment (p=.004), job demand (p<.001), interpersonal conflict (p=.012), lack of reward (p=.062). The severe fatigue group showed lower degrees both in physical component summary (PCS) (p<.001) and mental component summary (MCS) (p<.001) measured by SF-12 and the quality of life (p<.001) measured by EQ5D than the lower fatigue group. A relatively strong negative correlation was found in the item of mental component summary (MCS) measured by SF-12. Conclusion: Considering our results that high fatigue level show negative impact on quality of life and job stress, we suggest to provide a variety of health promotion strategies to improve the quality of life through the prevention and relief of fatigue for workers.

건설업 근로자의 유해 작업 환경 노출 실태에 관한 연구 (Health Hazardous Substances in Construction Work in Korea)

  • This study was performed to evaluate the working conditions In construction workers through measurements of working environments and to improve the working conditions and to design the appropriate environmental and health management system. The results of measurement for working environments in construction work are as following: 1. The excess rate of noise and dust concentration in express highway construction, road construction, building construction and interior painting was 28.6% and 24.5% respectively. 2. The excess rate of mixed organic solvents concentration in interior painting was 39.3%. 3. The chain type bulldozer has the highest level of vibration; the excess rate was 85.7%, and among the vibration instruments, the level of left hand's vibration of all vibrators exceeded the criterion and in the case of drilling all the level of vibration exceeded the criterion in both hands. On the basis of the results in this research the environmental and health management system for construction workers must be considered. Therefore, it is necessary to be introduced the special medical examination and measurement of working environments in construction industry. In defining "the working place" of Article 39 of Enforcement regulations of industrial safety and health act, the word "indoors" should be deleted. Then the eligible industry for measurement of working environment must be extended. And it is also necessary for construction industry to be performed group health management service by agencies such as other manufacturing industry.

Perchlotoethylene을 사용하는 세탁소 작업자들의 직업성 폭로와 건강에 관한 연구 (Evaluation of Worker's Health and Occupational Exposure to Perchloroethylene in Laundries)

  • 본 연구는 1993년 10월부터 1994년 3월 사이에 PCE를 세정제로 사용하는 세탁소중 충청권에 있는 23개소의 세탁소 작업자 30명을 폭로군으로, 모대학교 사무직원 42명을 비폭로군으로 하여 자각증상을 조사하였고, 세탁소 작업자의 PCE 폭로농도 및 작업장 기중농도를 측정하였다. 혈액과 뇨를 채취하여 혈액학적, 임상화학적 검사 및 뇨단백, 뇨당을 검사함으로써 PCE 폭로정도에 따른 관련성을 분석하여 다음과 같이 결과를 요약하고, 세탁소의 작업장 환경을 양호하게 관리하기 위한 몇가지 방안을 제안하고자 한다. 1. 폭로군이 비폭로군에 비해 조사항목 대부분에서 높은 자각증상 호소율을 나타냈으며, 특히 호흡기계, 심혈관계, 소화기계, 신경정신계에서 높은 자각증상을 호소했다. 개인폭로농도와 자각증상 호소율은 상호 관련성이 없었다. 2. 1일 8시간 작업시 PCE의 허용농도는 우리나라의 경우 50.0ppm, ACGIH에서는 25.0ppm으로 권장하고 있다. 조사대상 세탁소 형태별로 작업자의 PCE폭로농도는 체인본부가 16.85ppm, 일반 세탁소가 8.83ppm, 셀프-서비스점이 3.07ppm으로 나타났다. 전체적으로 세탁소 작업자의 PCE 폭로농도는 25.0ppm 이하가 19개소(82.6%)였으며, 25.0ppm 이상 측정된 세탁소는 4개소(17.4%)였고, 50.0ppm을 넘는 세탁소는 한 곳도 없었다. 이상의 결과에서 측정대상 세탁소의 작업환경 관리가 비교적 양호함을 알 수 있다. 3. PCE 기기의 가동횟수가 증가할수록 또한 작업량(세탁량)이 많아질수록 PCE 폭로정도는 증가하였다. 또 개방형 PCE 기기를 사용하는 작업자의 폭로농도는 6.18ppm, 밀폐형은 11.48ppm으로 나타났으나, 밀폐형을 사용하는 세탁소의 경우 개방형보다 작업량이 2-3배 정도 많았으며, 이 결과는 기기의 형태보다 작업량과 관계가 있는 것으로 나타났다. 4. 혈액학적 검사에서는 WBC, RBC, Hgb, Hct, MCV, MCH, MCHC, PLT 모두 정상으로 나타났으며, 혈액의 임상화학적 검사에서도 간기능, 신장기능 진단에 이용되는 GOT, GPT, ALP, U-A 및 LDH가 정상으로 나타났다. 5. 뇨단백 및 뇨당 검사에서는 대부분 정상이었고, 3명의 뇨단백이 30mg/dl 이하로 약간 배출되었으며, 뇨당은 3명이 100mg/dl 이하, 2명이 100-250mg/dl로 약간 높게 배출하였다. 이상의 결과에서 PCE 폭로군은 비폭로군보다 신체의 여러부분에 대해 높은 자각증상 호소율을 나타냈다. 조사대상 세탁소의 작업환경은 비교적 양호하게 관리되고 있었으며, 혈액학적 및 임상화학적 검사는 직업성으로 인해 이상이 유발되었다고 평가할만한 사실은 관찰되지 않았다. 그러나 세탁소에서는 PCE뿐만 아니라 얼룩을 제거하기 위하여 여러 종류의 유기용제를 사용하기 때문에 작업자들은 항상 위해한 환경 인자에 노출되고 있다고 사료되므로, 작업장 환경을 보다 쾌적하게 관리하기 위해서는 PCE의 기계적 누출을 조사하고, 의류의 충분한 건조, 적절하고 합리적인 환풍기의 가동 및 개인보호구 착용 등에 관심을 기울여야 할 것이다.

수도권 지역 보건소 방문간호사의 계약직 취업경험: 고용형태 및 처우를 중심으로 (Contract Employment Experiences of Visiting Nurses at Public Health Centers in the Metropolitan Area: Focused on Employment Type and Treatment)

  • Purpose: This study was conducted with a focus group interview that drew out experiences, limitations, and difficulties in the workplace according to the employment conditions of visiting nurses in the public health centers. Methods: A total of 12 visiting nurses are those working in the public sector in Seoul and Gyeonggi province who were willing to participate in the interview. Analysis categories and coding were divided into three categories: compensation system, occupational status, and opinions to improve their treatment. Using the content analysis method, the current working status and compensation system of visiting nurses were described. Results: The main themes derived from the significant statements of visiting nurses were 'Ten years of frozen salary system', 'Full-time workers of their own league', 'Excluded from performance benefits', 'Every visiting nurses are virtually precarious', 'Experience of exclusion and discrimination', and 'Reasons and barriers to be a full-time worker'. All of the visiting nurses working in the community insisted on having equal treatment for work of equal value. Visiting nurses in the public health sector wanted to be set to the same payment system and the fair allowance system as well. It is necessary to continuously seek solutions to the problems left in insisting on the civil service of visiting nurses. Conclusion: Visiting nurses who were working in a precarious job position felt job insecurity, and experienced discrimination, alienation, and exclusion. Legal and institutional reform is needed to improve the treatment of visiting nurses.

가습기살균제 피해자의 아픔을 줄일 수 있었다 (It Was Possible to Reduce the Pain of the Victims of Humidifier Disinfectant)

  • Objectives: The purpose of this study is to reveal the circumstances under which the cases of harm to health caused by humidifier disinfectant were neglected and show the points where the number of victims and the degree of damage could have been reduced. In addition, it attempts to describe how damage management proceeded immediately after the incident and actually exacerbated the damage. Finally, it explores the unfortunate aspects of the recent trial. By doing so, it attempts to take this as an opportunity to consider whether a tragic event such as the humidifier disinfectant incident could occur in the future. Methods: This study collected and analyzed data on chemical material characteristics related to humidifier disinfectants, data on health effect characteristics, data on related laws and regulations from the Ministry of Environment, data related to the damage investigation by the Korea Environmental Industry and Technology Institute, and current contents. Results: The lack of related systems and laws is the area where the greatest responsibility for the cause of the humidifier disinfectant disaster falls, so it is difficult for the government to escape this responsibility. Establishing a dedicated department to identify the prevalence of certain diseases within the functions of the Health Insurance Review and Assessment Service to monitor health can greatly contribute to the prevention and management of diseases through early detection and management of group outbreaks caused by harmful factors. Humidifier disinfectant damage relief should have been expanded earlier beyond HDLI (humidifier disinfectant lung injury) to include non-specific diseases such as asthma, pneumonia, and interstitial pneumonia. The scope of relief benefits should have also been expanded earlier to include the payment of disability benefits. Fortunately, with the 2020 revision of the Special Act, the conditions for estimating causal relations were eased and individual screening systems such as health impact assessment were reorganized along with the introduction of a rapid screening system. Conclusions: The management system for chemical substances in a country is clearly of paramount importance, and the ministry in charge must have a response system in case of damage to health effects. Administration that looks at the victims' situation from their point of view is needed, and technical countermeasures are required to quickly recognize the prevalence of certain diseases.
