Tracheo-gastric fistula is a very rare late complication of total laryngopharyngoesophagectomy with gastric pull-up procedure. It usually occurs after transhiatal esophagectomy, but it has only rarely been reported after total laryngopharyngoesophagectomy with gastric pull-up procedure. Chronic irritation and gastric ulcer may be the cause of tracheogastric fistula. To prevent fistula formation, active management of gastric ulcer and avoidance of mucosal irritation are necessary. We report a case of a tracheogastric fistula which occurred two years after total laryngopharyngoesophagectomy with gastric pull-up procedure that maybe occurred by chronic irritation and gastric ulcer.
Prodrugs of flurbiprofen, 1,2-ethanediolester(FE) and 1,4-butanediolester(FB) were prepared and their biopharmaceutical studies were performed. The prodrugs showed high stability in simulated gastric fluid, simulated intestinal fluid and pancreatin-saturated solution. Pharmacokinetic parameters of the prodrugs were similar to those of their parent drug. However they showed less acute toxicity and gastric irritation and higher anti-inflammatory and analgesic effects.
Three 1,2-benzothiazine derivatives were synthesized, and their analgesic / anti-inflammatory efficacy and their effect s of gastric irritation were evaluated. Among the three compounds, 39 exhibited the most potent anlagesic action, but the effect was weaker than that of piroxicam. Nonetheless, the compound showed 4 times more potent analgesic action with less gastric damage than did ibuprofen. These compounds did not show anti-inflammatory effect at an oral dose of 5 mg/kg.
After stroke, there are many cases of gastro-intestinal bleeding in patients with dysphagia who are fed by Levin tubes. Ulcers in the stomach and duodenum are occasionally produced by psychiatric stress, irritation with the Levin tube and central nerve rusturbances. Because Dansambohyul-tang has been used to treat deficiency of blood, alleviate pain, tranquilize and to treat patients differentiated with an insufficiency of the heart and the spleen in oriental medicine, we treated a 62 year-old male patient who suffered from gastric ulcers with gastro-intestinal bleeding after stroke, and who was differentiated with an insufficiency of the heart and the spleen with Dansambohyul-tang, herb complex. After 20 days of treatment with Dansambohyul-tang, we observed improvement in the gastric ulcer through gastroscopy; serum hemoglobin increased by 5.1g/dl after 2 months; and symptoms (pale complexion.dysphagia.general weekness) were alleviated. So Dansambohyul-tang shows therapeutic effects on gastric ulcers with gastro-intestinal bleeding after stroke.
In order to reduce gastric irritation of iron preparations, one prodrug, glycine complex of ferrous fumarate, was synthesized, identified, examined for physico-chemical properties and compared with ferrous fumarate, ferrous sulfate and ferroglycine sulfate. That novel ferroglycine fumarate (FGF) resulted in higher dissolution rate in water, artificial gastric and intestinal juice. The absorption rate constants $(K_a)$ in rat of FGF was greater and ulcerogenic dose on stomach was increased remarkably than those of iron parent materials and ferroglycine sulfate.
Gastrointestinal irritation is the most frequent adverse effect in patients chronically taking nonsteroidal antiinflammatory drugs (NSAIDs) for the treatment of arthritic conditions. Gastroprotective effect of DA-9601, a new antiulcer agent from Artemisiae Herba extract, against NSAID was evaluated in a rat model of arthritis that is similar in many aspects to human rheumatoid arthritis. Daily oral dosing of naproxen (30 mg/kg), one of the most commonly used NSAID, induced apparent gastric lesions as well as a significant decrease in mucosal prostagiandin $E_2;(PGE_2)$ and prostagiandin F_${1{\alpha}}$$(PGF_{1{\alpha}})$ levels. Coadministration of DA-9601 prevents naproxen-induced mucosal injury and depletion of prostaglandins, in a dose-related manner. DA-9601 did not alter the antiinflammatory or analgesic effect of naproxen. The present results suggest that DA-9601 may be useful as a mucoprotectant against NSAIDs in clinical practice.
Ascorbic acid is one of the most well-known nutritional supplement and antioxidant found in fruits and vegetables. Calcium ascorbate has been developed to mitigate the gastric irritation caused by the acidity of ascorbic acid. The aim of this study was to compare calcium ascorbate and ascorbic acid, focusing on their antioxidant activity and effects on gastric juice pH, total acid output, and pepsin secretion in an in vivo rat model, as well as pharmacokinetic parameters. Calcium ascorbate and ascorbic acid had similar antioxidant activity. However, the gastric fluid pH was increased by calcium ascorbate, whereas total acid output was increased by ascorbic acid. In the rat pylorus ligation-induced ulcer model, calcium ascorbate increased the gastric fluid pH without changing the total acid output. Administration of calcium ascorbate to rats given a single oral dose of 100 mg/kg as ascorbic acid resulted in higher plasma concentrations than that from ascorbic acid alone. The area under the curve (AUC) values of calcium ascorbate were 1.5-fold higher than those of ascorbic acid, and the $C_{max}$ value of calcium ascorbate (91.0 ng/ml) was higher than that of ascorbic acid (74.8 ng/ml). However, their $T_{max}$ values were similar. Thus, although calcium ascorbate showed equivalent antioxidant activity to ascorbic acid, it could attenuate the gastric high acidity caused by ascorbic acid, making it suitable for consideration of use to improve the side effects of ascorbic acid. Furthermore, calcium ascorbate could be an appropriate antioxidant substrate, with increased oral bioavailability, for patients with gastrointestinal disorders.
In order to reduce gastric irritation in the stomach of iron preparations, ferroglycine fumarate (FGF) granules coated with hydroxyethylcellulose was made by matrix granulator, and the constrained optimization method, employing the Lagrange equation, was successfully applied to the manufacturing process design of controlled release tablets. The effects of stearic acid and dried corn starch on tablet hardness, friability, dissolution rate $t_{50%}$ and tablet volume were found to be very significant. In rabbit test, pharmacokinetic parameters $(K_a,\;C_{max}\;and\;AUC^{0-12})$ and urinary excretion rate $(K_e)$ of the controlled release FGF tablets were higher than those of controlled release ferroglycine sulfate tablets which were manufactured in the same optimal conditions. Controlled release FGF tablets were more stable than controlled release ferroglycine sulfate tablets in accelerated storage conditions.
Indomethacin, a non-steroidal antiinflammatory drug inducing gastric irritation, was microencapsulated using pectin-gelatin complex coacervation method. Optimum conditions for microencapsulation and dissolution characteristics of the microcapsules were studied. The optimum pH and pectin-gelatin ratio for microencapsulation were 3.8 and 1:2 respectively. As concentration of colloid solution increased, wall thickness of microcapsules were increased. The dissolution rate of Indomethacin-pectin-gelatin microcapsules prepared by 1.5% and 2% colloid solution were similar but slower than that of Indomethacin-pectin-gelatin microcapsules prepared by 1% colloid solution. The 50% release time ($T_{50%}$) of Indomethacin-pectin-gelatin microcapsules prepared by 1%, 1.5% and 2% colloid solutions were 3 min, 5 min, and 6 min respectively while that of Indomethacin powder was 50 min.
The evaluation of the anti-inflammatory analgesic and antipyretic activities of loxoprofen sodium given in intramuscular route was investigated as compared to oral application in rats and mice. The intramuscular $ED_{50}$ values of loxoprofen sodium in carrageenan edema and vascular permeability tests are 1.15 and 7.8 mg/kg, respectively, which represent more potent than in case of oral application. Its therapeutic effects in adjuvant arthritis were shown at 6 mg/kg l.m. and 3mg/kg p.o. Analgesic effect was shown to be more potent as given intramuscularly. Similar potency of antipyretic effects was shown in both administration routes. Considerably weak gastric damages were observed in intramuscular application.
