사회적 관계망의 구조적, 기능적 측면과 건강증진행동과의 관계: 지역사회 거주 노인을 중심으로 (Association Between Structural and Functional Aspects of Social Networks and Health Promoting Behaviors: Focusing on Community-Dwelling Older Adults)

  • 목적 : 본 연구는 지역사회 노인의 사회적 관계망과 건강증진행동을 파악하고 사회적 관계망의 구조적, 기능적 측면이 건강증진행동에 어떠한 영향을 미치는지 알아보고자 하였다. 연구방법 : 본 연구는 지역사회에 거주하는 65세 이상 노인 226명을 대상으로 사회적 관계망의 구조적, 기능적 측면과 라이프스타일 영역별 건강증진행동을 조사하였다. 사회적 관계망은 비동거 가족, 친인척, 친구, 이웃의 4가지 관계망을 대상으로, 구조적 특성인 관계망 크기와 접촉빈도, 기능적 특성인 사회적 지지를 조사하였다. 일반적 특성에 따른 건강증진행동 정도를 비교하기 위해 독립표본 T검정과 일원배치 분산분석을 실시하였다. 사회적 관계망과 건강증진행동과의 관계를 확인하기 위해 피어슨 상관분석을 실시하였고, 사회적 관계망이 건강증진행동에 미치는 영향을 확인하기 위해 위계적 회귀분석을 실시하였다. 결과 : 사회적 관계망 크기는 친구 관계망에서 가장 높았고 이웃 관계망에서 가장 낮았으며, 접촉빈도와 사회적 지지는 비동거 가족 관계망에서 가장 높았고 이웃 관계망에서 가장 낮았다. 건강증진행동은 일상생활활동 영역에서 가장 높았으며, 생산적·사회적 활동 영역에서 가장 낮았다. 일반적 특성 중 성별, 주관적 경제상태, 주관적 건강상태, 우울수준에 따른 건강증진행동은 유의한 차이를 보였다. 사회적 관계망의 구조적, 기능적 측면의 모든 하위요인들은 건강증진행동과 유의한 정적 상관관계를 보였다. 위계적 회귀분석 결과 이웃의 사회적 지지가 노인의 건강증진행동에 유의한 영향을 미치는 변인으로 나타났고, 일반적 특성 중 성별, 우울도 영향요인으로 나타났으며 이 모델은 37%의 설명력을 보였다. 결론 : 지역사회 노인의 건강증진행동 촉진을 위해서는 이웃의 사회적 지지를 중심으로 사회적 관계망을 고려한 건강증진 프로그램의 개발 및 관련 정책이 요구된다.

영양염류, 투명도 및 엽록소를 이용한 인공호 영양상태, 경험적 모델 분석 및 변수들 간의 상호관계 (Reservoir Trophic State and Empirical Model Analysis, Based on Nutrients, Transparency, and Chlorophyll-${\alpha}$ Along with Their Relations Among the Parameters)

  • 본 연구는 연구의 목적은 총질소(TN), 총인(TP), 엽록소(Chl), 투명도(SD)의 변수를 이용하여 호수의 영양상태(Trophic state)를 평가하였고, 전기전도도에 따르면 부유물질(SS)의 역동성을 비교 평가하여 총질소-엽록소(TN-Chl), 총인-엽록소(TP-Chl), 엽록소-투명도(Chl-SD)의 경험적 모델을 분석하였다. 호소의 영양상태 분석에 따르면, 36개 인공호 중 절반이상이 부영양-과영양화 상태 (Eutrophy-Hypertrophy)로 나타났다. 총인의 월 변이(% Variation)가 최고 500%까지 상회하였으며 특히 8월에는 연중 최고치를 보였다. 한편 총질소의 월 변이는 90% 이하로 나타났으며, 모든 호수에서 평균 총질소 농도는 1.2 mg L$^{-1}$ 이상을 상회하여, 배경 농도가 높은 것으로 나타났다(부영양-과명양화상태). 경험적 모델 분석에 따르면 투명도의 변이는 총인(R$^2$=0.15, p<0.001) 및 총질소 (R$^2$=0.20, p<0.001)보다 주로 엽록소 (R$^2$=0.31, p<0.001)에 의해 설명되는 것으로 나타났다 총인, 총 질소의 비 (TN : TP ratio)의 분석에 따르면, 대부부의 인공호는 조류 생장에 있어 잠재적인 인(P)의 영향을 시사하였다. 따라서 식물성 플랑크톤 성장은 질소보다 인에 의하여 조절 되는 것으로 나타났다. 수질 변수의 연 평균값에 로그-전환(Log$_{10}$ transformation)한 후 실시한 선형 회귀분석에 따르면 엽록소는 총인 및 총질소에 의해 각각 30%, 15% 설명되어, 연관성이 극히 낮은 것으로 나타났다. 그러나 개별 호소에 대한 선형 회귀분석 일부 총인-엽록소가 강한 정 상관관계 (R$^2$=0.62, p=0.002, n=12)를 총질소-엽록소에서는 유의성이 없는 것으로 나타났다(p=0.892, n=12). 상기 연구를 종합해보면 경험적 모델 분석 시 자료의 평균효과(Averaging effect)는 모델의 변이성을 설명하는 데 중요한 것으로 나타났다.

Discriminative validity of the timed up and go test for community ambulation in persons with chronic stroke

  • Objective: The timed up and go (TUG) test is method used to determine the functional mobility of persons with stroke. Its reliability, validity, reaction rate, fall prediction, and psychological characteristics concerning ambulation ability have been validated. However, the relationship between TUG performance and community ambulation ability is unclear. The purpose of this study was to investigate whether the TUG performance time could indicate community ambulation levels (CAL) differentially in persons with chronic stroke. Design: Cross-sectional study. Methods: Eighty-seven stroke patients had participated in this study. Based on the self-reporting survey results on the difficulties experienced when walking outdoors, the subjects were divided into the independent community ambulation (ICA) group (n=35) and the dependent community ambulation group (n=52). Based on the area under the curve (AUC), the discrimination validity of the TUG performance time was calculated for classifying CAL. The Binomial Logistic Regression Model was utilized to produce the likelihood ratio of selected TUG cut-off values for the distinguishing of community ambulation ability. Results: The selected TUG cut-off values and the area under the curve were <14.87 seconds (AUC=0.871, 95% confidence interval=0.797-0.945), representing a mid-level accuracy. Concerning the likelihood ratio of the selected TUG cut-off value, it was found that the group with TUG performance times shorter than 14.87 seconds showed a 2.889 times higher probability of ICA than those with a TUG score of 14.87 seconds or longer (p<0.05). Conclusions: The TUG can be viewed as an assessment tool that is capable of classifying CAL.

스마트 워치 소비자 저항에 영향을 미치는 요인: 수용 보류 집단의 성별, 연령별 집단 차이 비교 (Consumer Resistance to Smartwatches: Gender and Age Differences)

  • 본 연구는 스마트 워치 수용을 보류하고 있는 소비자 집단을 중심으로 스마트 워치 소비자 저항에 영향을 미치는 요인들을 살펴보았다. 본 연구는 20-40대 소비자 407명의 설문 자료를 기반으로 분석을 실시하였으며, SPSS 19.0을 활용하여 빈도분석, 기술통계, 요인분석, 신뢰도 검증, 상관관계 분석, t-test, 그리고 다중회귀 분석을 실시하였다. 본 연구의 결과는 다음과 같다. 첫째, 상대적 이점은 모든 성별과 연령 집단에서 스마트 워치 소비자 저항을 감소시키는 요인으로 확인되었다. 둘째, 복잡성은 여성 소비자 집단, 20대, 그리고 40대 소비자 집단에서 스마트 워치 소비자 저항을 증가시키는 요인으로 확인되었다. 셋째, 디자인 심미성은 남녀 소비자 집단, 20대 소비자 집단에서 스마트 워치 소비자 저항을 감소시키는 요인으로 확인되었다. 넷째, 주관적 규범은 여성 소비자 집단, 20대, 그리고 30대 소비자 집단에서 스마트 워치 소비자 저항을 감소시키는 요인으로 확인되었다. 다섯째, 프라이버시 위험은 남성 소비자 집단과 40대 소비자 집단에서 스마트 워치 소비자 저항을 증가시키는 요인으로 확인되었다. 본 연구의 결과들은 스마트 워치 수용보류 집단의 소비자 저항을 이해하는데 기초 자료를 제공할 수 있을 것이다.

Spatio-temporal variabilities of nutrients and chlorophyll, and the trophic state index deviations on the relation of nutrients-chlorophyll-light availability

  • The object of this study was to determine long-term temporal and spatial patterns of nutrients (nitrogen and phosphorus), suspended solids, and chlorophyll (Chl) in Chungju Reservoir, based on the dataset of 1992 - 2013, and then to develop the empirical models of nutrient-Chl for predicting the eutrophication of the reservoir. Concentrations of total nitrogen (TN) and total phosphorus (TP) were largely affected by an intensity of Asian monsoon and the longitudinal structure of riverine (Rz), transition (Tz), and lacustrine zone (Lz). This system was nitrogen-rich system and phosphorus contents in the water were relatively low, implying a P-limiting system. Regression analysis for empirical model, however, showed that Chl had a weak linear relation with TP or TN, and this was mainly associated with turbid, and nutrient-rich inflows in the system. The weak relation was associated with non-algal light attenuation coefficients (Kna), which is inversely related water residence time. Thus, values of Chl had negative functional relation (R2 = 0.25, p < 0.001) with nonalgal light attenuation. Thus, the low chlorophyll at a given TP indicated a light-limiting for phytoplankton growth and total suspended solids (TSS) was highly correlated (R2 = 0.94, p < 0.001) with non-algal light attenuation. The relations of Trophic State Index (TSI) indicated that phosphorus limitation was weak [TSI (Chl) - TSI (TP) < 0; TSI (SD) - TSI (Chl) > 0] and the effects of zooplankton grazing were also minor [TSI (Chl) - TSI (TP) > 0; TSI (SD) - TSI (Chl) > 0].

외과계 중환자실의 섬망 발생 위험요인 조사연구 (Risk Factors related to Delirium Development in Patients in Surgical Intensive Care Unit)

  • Purpose: The purpose of this study was to examine the frequency and the course of delirium, and identify risk factors for the development of delirium in surgical intensive care unit (SICU). Methods: Subjects older than 19 years admitted to the SICU were recruited. After informed written consent, enrolled subjects had baseline cognitive and functional assessments. Subjects were assessed daily for delirium using the Confusion Assessment Method-ICU (CAM-ICU). Results: During the study period, 110 patients were enrolled. The overall incidence of delirium was 20% (22/110). The average time to onset of delirium was 3.04(${\pm}1.25$) days. Several variables were associated with an increased risk of delirium including older age (p<.05), higher admission APACHE II score (p<.001), use of opioid and analgesics (p<.01), using physical restraints (p<.001), and intraoperative hypotension (p<.05). In a multivariate logistic regression model, using physical restraints (p<.001), intraoperative hypotension (p<.05), and older age (p<.05) remained significant predictors of the delirium development. Conclusion: Using physical restraints, intraoperative hypotension and older age was strongly associated with development of delirium in the SICU. Prevention measures need to focus on identifying patients at higher risk for delirium development.

마우스 모델에서 항백탕 투여에 의한 종양 증식의 억제 및 Apoptosis의 유도 (Proapoptotic and antitumor effect of Hangbaek-Tang(HBT) in a tumor transplanted mouse model)

  • Objective : In vitro proapoptotic effect of Hangbaek-Tang (HBT) has been documented by one of us. In the present study, we aimed to demonstrate in vivo effect of HBT on tumor growth. Methods : In vitro selective cytotoxicity of HBT was examined by enumeration of viable cell numbers using BC3A mouse leukemic cells and normal spleen cells. In vivo effect of HBT (25 and 50 mg/mouse) on tumor growth was assayed using BC3A cells innoculated subcutaneously in the flank. Annexin-V apoptosis assay and PI staining was performed to determine the effective serum factor in HBT-treated mice. Leukocyte recruitment into peritoneum were analyzed by microscopy with a stained cytosmear of peritoneal lavage fluid. Results : HBT exhibited in vitro selective cytotoxicity to leukemic cells and did not show any toxicity on immune organs. In vivo i.p. administration of HBT induced significant reduction in tumor growth but not complete regression. Sera obtained from HBT-treated mice strongly inhibited BC3A cell growth in vitro and were revealed to markedly enhance apoptosis and accompanying cell death, when compared to those from PBS-treated mice. Abundant extravasation of leukocytes, especially neutrophils, into peritoneum was observed in HBT-treated mice. Conclusions : HBT causes leukemic, BC3A cell death in vivo via apoptosis as well as in vitro, for which functional involvement of leukocytes is suggested.

윤리적 패션 소비행동과 영향 요인에 대한 비교문화 연구: 한국과 독일의 소비자를 중심으로 (Cross-Cultural Comparison on Ethical Fashion Consumption Behavior and Related Factors: Focusing on Consumers in South Korea and Germany)

  • This study analyzes the relation of the consumption values and ethical fashion consumption behavior as well as other influential factors between Korean and German ethical consumers based on the 'Modified Theory of Planned Behavior.' A survey was conducted on female consumers in their 20s and 30s, who experienced ethical fashion consumption in Seoul and Berlin. A factor analysis, reliability analysis, F -test and multiple regression analysis were performed for model verification. The research results indicated that emotional value (in case of Korea) and functional value out of consumption values (in case of Germany) have significant effects in regards to the influence of consumption values on ethical self-identity. The effects of ethical self-identity on ethical fashion consumption behavior indicated that Korean ethical self-identity influenced the ethical fashion consumption behavior of moderating and simplicity and eco-friendly certification versus local consumption as well as above factors (in case of Germany) explained by ethical self-identity. Both variables had a negative moderating effect in Korea in regards to the moderating effects of social comparison and materialism in the relations of ethical self-identity and ethical fashion consumption behavior; however, only materialism was an influential factor in Germany. The results of the research variables by individualism/collectivism indicated that the horizontal-collectivism group showed the highest ethical self-identity along with the lowest materialism tendency that had a high point of consumption behavior towards local consumption. However, the vertical-individualism group was outstanding. The consumption behavior aimed at moderating and simplicity.

실험계획법을 이용한 아크릴 점착제의 점착력 최적화 (Optimization of Peel Adhesion of Acrylic Pressure Sensitive Adhesive using Design of Experiments)

  • 아크릴 점착제의 점착력을 목표 수준으로 향상시키기 위해 실험계획법을 이용하여, 2-ethyl hexyl acrylate (2-EHA), ethyl acrylate (EA), metha acrylic acid (MAA), acryl amide (AAm)를 라디칼 용액 중합하여 점착력을 측정하였다. 실험계획법은 반응표면분석법의 하나인 혼합물 설계를 이용하여 특수 3차 모형으로 분석하였으며, 그 적용은 상용프로그램인 MINITAB을 사용하였다. 수립된 실험계획에 의해 실험하고 분석한 결과, 목표하는 점착력을 얻기 위한 단량체 조성은 2-EHA (0.8861), EA (0.0639), MAA (0.03), AAm (0.02)로 나타났으며, 그 회귀분석 식은 다음과 같았다. $$y=54.8816x_1+80.7067x_2-44.4700x_3-99.0288x_1x_2+60.7706x_1x_3-441.030x_2x_3+974.341x_1x_2x_3$$.

보육시설 아동의 감염성 질환 예방 관리를 위한 한의사 주치의 프로그램 개발 및 평가 (Development and Evaluation of Comprehensive Health Care Program for Infectious Disease Management in Child Care Centers by Doctor of Korean Medicine)

  • Objectives: The present study was carried out to develop and evaluate comprehensive health care program to prevent infectious disease and promote health in child-care centers by Doctor of Korean medicine. Methods: A nonequivalent control group pretest-posttest design study was conducted on 568 children and 85 child care teacher at 12 child care facilities for 12 weeks from July to October 2012. The program was consist of management, education, screening under concepts of traditional preventive medicine, Yangsaeng and Chimibyeong. Children's medical utilization due to infectious disease and attendance means functional status were measured by reports from parents. The Difference in difference(DID) estimator was applied data analysis, and added Zero-inflated negative binomial regression model. Also, attitudes on the infection of teacher was measured and analyzed through t-test. Results: After the intervention, the total medical utilization due to infectious disease decreased, but not significantly. Total absence, early leave and lateness decreased significantly. But, Attitude on the infection of child care teacher was not changed. The parent's satisfaction showed positive overall. Conclusions: The intervention program may be effective in preventing infectious disease and managing health in child-care center partially. To measure long-term effect, long-term study improved is requested.
