미취학 아동의 영양지수 평가 및 관련 요인 (Evaluation of Nutrition Quotient and Related Factors in Preschool Children)

  • Objectives: This study was conducted to evaluate the nutrition quotient (NQ) by gender and understand which factors influence NQ in preschool children. Methods: Subjects were 245 children (110 boys, 135 girls) aged 4-6 years and their parents. The questionnaire composed of demographic characteristics, eating behavior factors and the NQ questions. The NQ consisted of 19 food behavior checklist items and all items were grouped into 5 factors: balance, diversity, moderation, regularity, and practice. Inbody J05, a measurement device that measures individual's body composition was used to measure children's anthropometric data. All data were statistically analyzed by SPSS program (Ver. 20) and the statistical differences in variables were evaluated by Student t-test, ${\chi}^2-test$, One-way ANOVA and Duncan's multiple range test. Results: The total NQ score of the subjects was 65.3. The NQ score of girls (67.0) was significantly higher than that of the boys (63.2) (p<0.01). The girls' average scores of NQ factors including diversity (p<0.01) and practice (p<0.05) were higher than those of the boys. The analysis of related-factors influencing NQ scores showed that there was a significant difference among the groups according to feeding methods during infancy (p<0.05), breast feeding group being the highest. Furthermore, the NQ score showed a significant difference depending on how to correct children's unbalanced diet as well as parents' nutrition knowledge. The NQ score of obesity group tended to be lower than that of underweight group although there was no significant difference. Conclusions: Overall results indicated that the girls had better quality of diet and eating habits than the boys. Children and their parents need proper nutrition education and counseling to correct children's eating habits and to improve diet quality in kindergartens and in children care centers.

  • 목적 : 본 연구는 능동적 구강놀이 병행한 Ayres Sensory Integration®(ASI®) 적용이 아동의 구강 민감도와 섭식행동의 문제에 미치는 영향에 대해 알아보고자 하였다. 연구방법 : 발달지연으로 진단받은 만 3세 6개월의 남자 아동을 대상으로 2018년 8월부터 2018년 9월까지 능동적 구강놀이 병행한 Ayres Sensory Integration®(ASI®)를 실시하였다. 실험설계는 단일사례연구(single-subject design) 중 ABA'설계를 사용하였고 15회기의 중재를 시행하였다. 중재기간 동안 아동의 구강민감도 변화를 측정하기 위해 임상 구강민감도 측정(Measuring Oral Sensitivity in Practice)을 실시하였다. 구강감각처리 어려움으로 섭식활동에 영향을 주는 행동은 한국형 섭취 행동 검사(Korean Children's Eating Behavior Inventory; K-CEBI), 식품 섭취 빈도 체크리스트를 중재 전후로 실시하였다. 결과 : 능동적 구강놀이와 ASI 중재기간 동안 임상 구강민감도와 섭취행동의 문제는 감소하였고 편식행동에는 변화가 없었다. 결론 : 능동적 구강놀이 병행한 ASI 중재는 아동의 구강민감도와 섭취행동의 문제를 감소하는데 긍정적인 효과가 있었다.

학령기 아동 대상 영양지수 개발과 타당도 검증 (Development of nutrition quotient for elementary school children to evaluate dietary quality and eating behaviors)

  • 본 연구는 생애주기 중 학령기 아동을 위한 영양지수 (nutrition quotient for elementary school children, NQ-C) 모형을 개발하고자 하였다. 학령기 아동 영양지수 평가항목 선정을 위해 학령기 아동 대상의 영양섭취 및 식생활 관련 문헌, 국민건강영양조사 자료, 식생활지침 및 국가 영양정책, 전문가 집단의 포커스 심층면접을 통해 파일럿 조사용 체크리스트 41문항 (식품섭취 영역: 20문항, 식행동·식습관 영역: 21문항)을 개발하였다. 학령기 아동 260명을 대상으로 체크리스트 문항과 1일 식사섭취조사를 포함한 파일럿 조사를 수행하고, 체크리스트 평가항목별 점수와 식사섭취 조사 결과인 영양섭취 수준 간의 상관성이 높은 24개 평가항목으로 구성된 전국단위 조사용 체크리스트를 개발하였다. 전국단위 조사는 시도별, 학교 유형별로 층화추출된 초등학생 1,144명을 대상으로 면대면 조사를 수행하였다. 탐색적 요인분석을 실시하여 학령기 아동 영양지수 평가항목을 20개 항목, 5개 요인으로 분류하고, 구조방정식 모형을 적용하여 평가항목의 구성타당도를 검증하고 추정된 경로 계수를 활용하여 5개 요인별 가중치와 평가항목별 가중치를 산출하였다. 학령기 아동용 영양지수를 위한 5개 요인과 가중치는 각각 균형 0.25, 다양 0.15, 절제 0.25, 환경 0.15, 실천 0.20 이었다. '균형' 요인에는 과일, 흰 우유, 콩이나 콩제품, 생선류, 고기류 섭취 빈도 5개 평가항목이 포함되었고, '다양' 요인에는 끼니별 채소 반찬 섭취, 반찬 골고루 먹기, 편식의 3개 평가항목이, '절제' 요인에는 달거나 기름진 빵, 가공 음료, 가공 육류, 라면, 카페인 음료, 길거리 음식 (학교 앞 음식) 섭취 빈도의 6개 평가항목이, '실천' 요인에는 영양성분표시 확인, 식사 전 손 씻는 빈도, 숨이 찰 정도의 운동 빈도 3개의 평가항목이, '환경' 요인에는 아침식사 빈도, 가족과 함께 식사하는 빈도, 정해진 장소에서 식사하기의 3개 평가항목이 포함되었다. 전국단위 조사대상자의 영양지수 (NQ-C) 평균 점수는 65.7점이었고, 요인별 점수는 균형 66.4점, 다양 67.0점, 절제 55.4점, 환경 75.3점, 절제 69.2점이었다. 영양지수와 각 요인별 등급은 전국단위 조사 결과의 백분위수 분포를 기준으로 75 to 100백분위의 경우 '상' 등급, 50 to < 75 백분위의 경우 '중상' 등급, 25 to < 50백분위의 경우 '중하' 등급, 0 to < 25 백분위의 경우 '하' 등급으로 분류하였다. 학령기 아동 영양지수 (NQ-C) 점수가 72.4-100점의 경우 '상' 등급, 65.8-72.3점의 경우 '중상' 등급, 59.2-65.7점의 경우 '중하' 등급, 65.7점 미만의 경우 '하' 등급으로 판정되었다. 본 연구에서 개발된 학령기 아동 영양지수 (NQ-C)는 20개 평가항목으로 구성된 체크리스트를 통해 영양지수 점수와 균형, 다양, 절제, 실천, 환경 요인의 점수 산출이 가능하고, 상대적인 영양지수 등급 부여를 통해 식사의 질과 영양상태가 양호한 지를 평가할 수 있다. 따라서, 학령기 아동 영양지수는 초등학생들의 식사의 질과 식행동 평가, 영양교육의 효과 평가 뿐만 아니라 정부의 영양교육사업, 영양정책의 효과 평가 등에도 활용될 수 있을 것으로 사료된다.

The effect of a nutritional education program on the nutritional status of elderly patients in a long-term care hospital in Jeollanamdo province: health behavior, dietary behavior, nutrition risk level and nutrient intake

  • This study was conducted to assess improvements in nutritional status following the application of nutrition education to elderly patients in a long-term care hospital. The study was carried out from January to May 2009, during which a preliminary survey, a pretest, the application of nutrition education, and a post-test were applied in stages. The number of subjects at pretest was 81, and the number of participants included in the final analysis was 61 (18 men, 43 women), all of whom participated in both the nutrition education program and the post-test. The survey consisted of general demographic items, health behaviors, dietary behaviors, the Nutrition Screening Initiative checklist, and nutrient intake assessment (24 hour recall method). The nutrition education program lasted for four weeks. It included a basic education program, provided once a week, and mini-education program, which was offered daily during lunch times. The survey was conducted before and after the education program using the same assessment method, although some items were included only at pretest. When analyzing the changes in elderly patients after the nutritional education program, we found that, among subjective dietary behaviors, self-rated perceptions of health (P<0.001) and of depression (P<0.001) improved significantly and that dietary behavior scores also improved significantly (P<0.001), while nutritional risk levels decreased. In terms of nutrient intake, subjects' intake of energy, protein, fat, carbohydrate, calcium, phosphorus, iron, vitamin A, thiamin, riboflavin, niacin, and vitamin C all increased significantly (P<0.001). These results indicated that nutritional education is effective in improving the nutritional status of elderly patients. We hope that the results of this study can be used as preliminary data for establishing guidelines for nutrition management tailored to elderly patients in long-term care hospitals.

담석 형성에 영향을 주는 식생활 관련 요인 연구 (Study on Dietary Risk Factors in Gallstone Formation)

  • It has been reported frequently that clinical features of gallstone disease in Korean were similarly changing to those of Occidentals. This changing was thought to be due to Westernized lifestyle and dietary patterns in Korean. The purpose of this study was to investigate the nutritional risk factor among patients with gallstone. The subjects of this study were 90 gallstone group with biliary stone as confirmed by cholecystectomy and 111 control group with no biliary stone as confirmed by ultrasonography adjusted according to age and gender. Anthropometric indices and biomarkers were measured and dietary habit as well as nutrient consumption pattern were investigated using a structured checklist of health-related eating behavior and a semi-quantitative food frequency questionnaire. The mean age was higher in GG (gallbladder stone patient group) than CG (control group). The average BMI and WHR (waisthip ratio) tended to be higher than those in CG. WC (waist circumference) and WHR of women in GG were significantly higher than in CG. GG had lower levels in TC (p < 0.01), LDL-C (p < 0.05), and HDL-C (p < 0.05), but higher FBG levels (p < 0.001). GG tended to be associated with lower physical activity and more frequent consumption of meat, meats with high fat and high sugar content food. The consumption levels of fiber (p < 0.05), vitamin C (p < 0.05), calcium (p < 0.01) in GG were significantly lower than in CG. These findings showed that the association with incidence of gallbladder stone and anthrophometric indices and dietary consumption patterns. Further study may be necessary to elucidate the dietary risk factors in the changing patterns of gallstone disease.

전반적 발달장애아동의 감각처리능력과 섭식 특성의 상관관계 (Correlation Between Sensory Processing Ability and Characteristics of Eating for Children With Pervasive Developmental Disorders)

  • 목적 : 전반적발달장애아동과 일반아동의 감각처리능력과 섭식특성에 어떤 차이가 있는지 알아보고 전반적발달장애아동의 감각처리능력과 섭식특성의 상관관계를 알아보기 위한 것이다. 연구방법 : 본 연구의 대상은 만 4세에서 6세의 전반적발달장애로 진단받은 아동과 만 4세에서 6세 사이의 일반적 아동이다. 연구도구는 자폐아동의 섭식행동 체크리스트(Brief Autism Mealtime Behavior Inventory: BAMBI), The Sensory Checklist의 Food 영역, 감각에 대한 아동의 반응 질문지(Short Sensory Profile: SSP)이다. 전반적 발달장애아동과 일반아동의 부모를 대상으로 하는 설문조사를 기관을 통해 실시하였다. 전반적발달장애아동의 설문지 45부와 일반아동의 설문지 109부를 분석하여 감각처리, 섭식행동, 구강섭식의 차이와 상관관계를 알아보았다. 결과 : 전반적발달장애아동과 일반아동의 감각처리능력은 차이가 있었다. 감각처리능력의 모든 하위 영역에서도 차이가 있었다. 전반적발달장애아동과 일반아동의 섭식특성인 섭식행동과 구강섭식에서는 차이가 없었다. 전반적발달장애아동은 감각처리능력에 어려움이 많을수록 섭식행동의 문제가 많이 나타났다. 전반적발달장애아동의 감각처리능력의 하위 영역 중 맛/냄새민감성, 청각여과하기에 어려움이 많을수록 섭식행동의 문제가 많이 나타났다. 그리고 감각처리능력의 하위 영역 중 과소반응/자극추구행동, 청각여과하기와 구강섭식은 상관관계가 있었다. 결론 :본 연구를 통해 전반적발달장애아동과 일반아동의 감각처리능력과 섭식특성을 비교하고 감각처리능력과 섭식 특성의 상관관계를 파악할 수 있었다. 본 연구의 결과를 전반적발달장애아동의 섭식행동 중재를 위한 프로그램 제공시에 활용할 수 있을 것이다.

어린이 영양교육 실태 및 영양상태 분석 (Analysis of the Nutrition Education Realities and Nutritional Status in Children)

  • This study investigated the nutrition education realities and nutrition status of children in community child centers, by analyzing the status of nutrition education, nutrition quotient (NQ), and the level of maintaining dietary guidelines. The subjects were 173 children from grades 1 to 6, enrolled in community child centers, Jinju, Kyungnam. The NQ was examined by a questionnaire, which was a checklist of 19 food behavior items. The distribution of scores (out of 100) in the nutrition quotient were as follows: total score of NQ was 59.4, balance 56.6, diversity 60.6, moderation 65.6, regularity 60.9, and practice 56.7. Nutrition quotient was higher in the higher graders due to significant differences in the area of variety. The level of maintaining dietary guidelines was higher in girls, especially in the area 'eat politely with family', and higher amongst the upper graders in the area 'have safe snack wisely'. To improve the eating habits and nutritional status of the children in community child centers, their nutritional state should be checked with regular and systematic education, and their nutritional management should be pursued continuously. Since the assessment of the eating behaviors and the nutritional state of children is important at home as well as in schools and community child centers, nutritional education should be further extended to the parents and their care givers. This study can be implemented as basic material for the nutritional education of children, to minimize the dangers of malnutrition and to help build up the right eating habits amongst children in community child centers.

노인의 인지기능상태에 따른 식품과 영양소 섭취실태 (Food and nutrient intake status of Korean elderly by degree of cognitive function)

  • 본 연구는 용인시에 거주하는 노인들을 대상으로 간이인지기능검사 (MMSE-DS)와 영양건강위험도 조사 (NSI DETERMINE checklist) 및 식품섭취실태를 통하여 인지기능 저하와 영양 식생활 실태와의 관련성을 살펴보고자 하였다. 1) 전체 조사대상 노인 (n = 409)의 평균 나이는 73.3세였고, 남자가 32.3%, 여자가 67.7%이었다. 인지기능 저하로 판정된 사람은 105명으로 대상자의 25.7%가 인지기능 저하로 판정되었으며, 성별에 따른 인지기능 정도의 차이는 없었다. 2) 조사대상 노인들의 운동 빈도는 유의차는 없었으나, 인지기능저하군에서 정상군보다 '운동을 거의 하지 않는다'는 비율은 더 높고, '거의 매일 운동을 한다'는 비율은 낮은 편이었다. 1일 평균 운동 시간은 인지기능저하군이 정상군보다 하루에 1~2시간씩 운동을 하는 사람들은 더 적고, 30분~1시간씩 운동을 하는 사람들은 더 많아서 총 운동시간이 적은 것으로 나타났다. 3) NSI DETERMINE 검사를 이용한 영양건강 위험도는 저위험군 270명 (66%), 중위험군 90명 (22%), 고위험군 49명 (12%)이었으며, 인지기능저하군에서 정상군보다 영양건강의 중위험군과 고위험군의 비율이 유의적으로 높았다. 4) 조사대상자의 평균 에너지 섭취량은 1,528.6 kcal로 에너지 필요추정량 (EER)의 84.4%를 섭취하였고, 인지기능저하군에서 정상군보다 다불포화지방산을 유의하게 적게 섭취하였다. 또한, 인지기능저하군은 티아민, 리보플라빈, 철을 평균필요량 미만으로 섭취하는 사람의 비율이 정상군보다 유의적으로 많았다. 5) 식품섭취빈도조사에서 조사한 63개 식품 중 조사대상자의 인지기능에 따라 유의차를 보인 식품은 6가지로 쌀밥과 과자류의 섭취빈도는 인지기능저하군에서 정상군보다 유의하게 높았고, 고등어, 고추, 귤과 수박의 섭취빈도는 인지기능저하군에서 유의하게 낮았다. 본 연구를 통하여 인지기능이 저하된 노인의 경우 정상노인보다 운동 빈도와 운동 시간이 적고, 영양건강위험도는 높으며, 불포화지방산의 섭취가 적고, 티아민, 리보플라빈, 철 등이 정상군보다 더 부족할 가능성이 높음을 확인하였다. 또한, 인지기능저하군에서 쌀밥과 과자류의 섭취는 더 많으나, 고등어, 고추, 귤, 수박의 섭취 빈도는 더 적은 것으로 나타났다. 앞으로 인지기능 손상을 지연시켜 개인의 건강 장수와 사회적 의료비용 절감을 가져오기 위해서는 인지기능 관련 식품들이 어떤 기전으로 인지기능에 영향을 미치는 가에 대한 후속 연구가 필요하다고 하겠다.

담석증 환자의 식생활양식 및 영양 실태 조사 (A Descriptive Study of Gall Stone Patient's Dietary Habits and Nutritional Status)

  • Gallstone composition has changed over the past decades in the Korean with a prominent increase in the prevalence of cholesterol gallstone. This trend is possibly due to the westernization of dietary habits. The purpose of this study was descriptive of GB patient's health related eating behaviors and nutrient consumption patterns. One hundred and six gallstone patients who have had cholecystitis surgery enrolled in this study. Anthropomertic indices, such as height, weight, waist circumference, and hip circumference were measured and calculated BMI and WHR. As the biomarker, TC, TG, LDL-C, HDL-C, FBG level and SBP/DBP were measured and analyzed the relationship with GB stone formation. The structured checklist of health related eating behavior and the semiquantitative food frequency questionnaire after pre-test was used in the face to face interview study. The mean age was $54.9{\pm}13.3$ and gallstone disease was more frequent in the fifties and sixties. Mean BMI was $24.3{\pm}2.8$ in males, and $23.4{\pm}3.9$ in females, the average of waist circumference was $91.1{\pm}7.0cm$ in males and females were $85.4{\pm}9.6cm$. The WHR of men and women was $0.93{\pm}0.0,\;0.90{\pm}0.1$, respectively. The obesity and overweight trend was observed in gallstone patients. The mean blood sugar was researched at $114.5{\pm}35.0mg/dL$. And the ratio of both symptoms DM and gallstone was 26.4%. The rate of dramatic eating repast was significantly higher in the females (p<0.01). Males tended to more frequently consume meat, of high fat content meats and greasy food consumption when eating out. The CPF ratio was 57:16.5:26.5. There was a significant positive correlation between WC and energy (r=0.252, p<0.05), carbohydrate (r=0.255, p<0.05) and niacin (r=0.227, p<0.05). In addition, carbohydrate were significantly correlatied with TC (r=0.230, p<0.05). BMI appeared positive in correlation of protein (r=0.201, p<0.05) and fat (r=0.205, p<0.05). These findings provide a little association that dietary habits are related with cholesterol gallstone formation.

문제 행동을 가진 초등학생의 영양 상태와 모발 미네랄 함유량 (Nutritional Status and Hair Mineral Content of Elementary School Children with Behavioral Problems)

  • This study was performed to assess the nutritional status and hair mineral content of children with behavioral problems and compare the values with a gender, age-matched control group. The subjects were recruited from S elementary school children in Seoul, Korea. Students scored higher than 60 points were diagnosed with behavioral problems according to the Korean-Child Behavior Checklist. Nutritional assessment of the two groups (behavior problem group: male n=15, female n=24; $9.6{\pm}1.6$ years, control group: male n=16, female n=18; $9.5{\pm}1.9$ years) was performed using a nutritional survey and by measuring hair mineral contents. In the results, food frequency questionnaire analysis showed that the intakes of anchovy (P<0.05), soybean curb (P<0.01), radish (P<0.05), bean sprouts (P<0.05), spinach (P<0.05), carrot (P<0.05), pumpkin (P<0.05), lettuce (P<0.05), cabbage (P<0.01), apple (P<0.05), and milk (P<0.01) were higher in the control group than the behavior problem group, whereas intakes of ramyeon (P<0.05), cookies (P<0.05), and coke (P<0.01) were higher in the behavior problem group than the control group. Intakes of most nutrients such as plant-derived protein (P<0.05), fiber (P<0.05), plant-derived calcium (P<0.05), phosphorus (P<0.05), plant-derived iron (P<0.05), vitamin B2 (P<0.05), vitamin B6 (P<0.05), vitamin C (P<0.01), vitamin E (P<0.05), and folate (P<0.05) were significantly higher in the control group than the behavior problem group. Hair analysis showed that the levels of arsenic (P<0.05), mercury (P<0.001), uranium (P<0.05), iron (P<0.001), boron (P<0.01), and germanium (P<0.001) were lower, but the levels of phosphate (P<0.05), chromium (P<0.001), sodium (P<0.05), and sulfur (P<0.001) were higher in the behavior problem group than the control group. Conclusively, behavioral problems constitute a complicated condition in which nutritional factors may play major roles. However, it is still under investigation as to whether or not modification of dietary habits or nutritional supplementation can improve children's behavior, since symptoms require a broad understanding of the environmental and genetic interactions.
