Objective: When an alveolar cleft is too large to close with adjacent mucobuccal flaps or large secondary fistula following a primary bilateral palatoplasty exists, a one-stage procedure for bone grafting becomes challenging. In such a case, we used the tongue flap to repair the fistula and cleft alveolus in the first stage, and bone grafting to the cleft defect was performed in the second stage several months later. The purpose of this paper is to report our experiences with the use of an anteriorly-based Y-shaped tongue flap to fit the palatal and labial alveolar defects and the ultimate result of the bone graft. Patients: A series of 14 patients underwent surgery of this type from January 1994 to December 1998.The average age of the patients was 15.8 years old (range: 5 to 28 years old). The mean period of follow-up following the 2nd stage bone raft operation was 45.9 months (range: 9 to 68 months). In nine of the 14 cases, the long-fork type of a Yshaped tongue flap was used for extended coverage of the labial side alveolar defects with the palatal fistula in the remaining cases the short-forked design was used. Results: All cases demonstrated a good clinical result after the initial repair of cleft alveolus and palatal fistula. There was no fistula recurrence, although Partial necrosis of distal margin in long-forked tongue flap was occurred in one case. Furthermore, the bone graft, which was performed an average of 8 months after the tongue flap repair, was always successful. Occasionally, the transferred tongue tissue was bulging and interfering with the hygienic care of nearby teeth; however, these problems were able to be solved with proper contour-pasty performed afterwards. No donor site complications such as sensory disturbance, change in taste, limitations in tongue movement, normal speech impairments or tongue disfigurement were encountered. Conclusion: This two-stage reconstruction of a bilateral cleft alveolus using a Y-shaped tongue flap and iliac bone graft was very successful. It may be indicated for a bilateral cleft alveolus patient where the direct closure of the cleft defect with adjacent tissue or the buccal flap is not easy due to scarred fibrotic mucosa and/or accompanied residual palatal fistula.

The objective of the present investigation is to explore the possibility of improving the performance of a centrifugal fan at low Reynolds numbers using a simple passive means, namely Gurney flap (GF). GFs of 1/$8^{th}$ inch brass angle (3.175 mm) corresponding to 15.9% of blade exit height or 5.1% of blade spacing at the impeller tip are attached to the impeller blade tip on the pressure surface. Performance tests are carried out on the centrifugal fan with vaneless diffuser at five Reynolds numbers (viz., 0.30, 0.41, 0.55, 0.69, $0.82{\times}10^5$, i.e., at five speeds respectively at 1,100, 1,500, 2,000, 2,500 and 3,000 rpm) without and with GF. Static pressures on the vaneless diffuser hub and shroud are also measured for each speed at four flow coefficients [${\phi}$=0.23 (below design flow coefficient), ${\phi}$=0.34 (design flow coefficient), ${\phi}$=0.45 (above design flow coefficient) and ${\phi}$=0.60 (above design flow coefficient)] with and without GF. From the performance curves it is found that the performance of the fan improves considerably with GFs at lower Reynolds numbers and improves marginally at higher Reynolds number. Similar improvements are observed for the static pressures on the diffuser hub and shroud. The effect of Reynolds number on the performance and static pressures is considerable. However the effect is reduced with GFs.

Background The advent of skin-sparing mastectomy has allowed for the reconstruction of the breast and nipple with improved cosmesis. However, the nipple-areolar complex (NAC) in Asian patients is more pigmented and scars easily. Therefore, commonly described incisions tend to result in poor aesthetic outcomes in Asian patients with breast cancer. Methods We describe an algorithmic approach to skin-sparing mastectomy incisions in Asian patients on the basis of the location of the biopsy scar and the tumor site and size. Four incision types are described: peri-areolar, a peri-areolar incision with a second distant skin paddle, "racquet handle," and peri-areolar with adjacent skin excision. Results 281 immediate breast reconstructions were performed between May 2001 and February 2012 after skin-sparing mastectomy. The mastectomy incisions used included the peri-areolar design (n=124, 44%), peri-areolar design with a second distant skin paddle (n=39, 14%), "racquet handle" (n=21, 7.5%), and peri-areolar design with adjacent skin excision (n=42, 14%). The traditional elliptical incision and other variants where the NAC outline was not preserved were performed in the remaining 55 patients. The average follow-up was 44.7 months during which there was 1 case of total flap loss and 7 cases of partial flap necrosis; all remaining flaps survived. 24% of the patients (68/281) underwent subsequent nipple reconstruction. Conclusions Our algorithm avoids breast incisions that are randomly placed or excessively long and prevents the unnecessary sacrifice of normal breast skin. This allows skin-sparing mastectomy and immediate breast reconstruction to be performed with a consistently achievable aesthetic result in Asian women without neglecting oncological safety.

A segment of the wave board has been expressed as a submerged line segment in the two dimensional wave flume. The lower end of the line segment could be extended to the bottom of the wave flume and the other opposite upper end of the board could be extended to the free surface. It is assumed that the motion of the wave board could be defined by the sinusoidal motion in horizontal direction on either end of the wave board. When the amplitude of sinusoidal motion of the wave board on lower and upper end are equal, the wave board motion could express the horizontally oscillating submerged segment of piston type wave generator. The submerged segment of flap type wave generator also could be expressed by taking the motion amplitude differently for the either end of the board. The pivot point of the segment motion could play a role of hinge point of the flap type wave generator. Simplified analytic solution of oscillating submerged wave board segment in water of finite depth has been derived through the first order perturbation method at two dimensional domain. The case study of the analytic solution has been carried out and it is found out that the solution could be utilized for the design of wave generator with arbitrary shape by linear superposition.

In this paper, a flap valve micropump with an ionic polymer-metal composite (IPMC) actuator was designed, fabricated, and experimentally characterized. A multilayered IPMC based on Nafion/layered silicate and Nafion/silica nanocomposites was fabricated for the actuation section of the micropump. The IPMC diaphragm, a key element of the mircopump, was designed so that the IPMC actuator was supported by a flexible polydimethylsiloxane (PDMS) structure at its perimeter. This design feature enabled a significantly high displacement of the IPMC diaphragm. The overall size of the micropump is $20{\times}20{\times}5$${mm}^3$. Water flow rates of up to 760 ${\mu}l$/min and a maximum backpressure of 1.5 kPa were recorded. A significant advantage of the proposed micropump is its low driven voltage from only 1-3 V. In addition, a simple and effective design, and an ease of manufacturing are other advantages of the present micropump.
Background: Tongue reconstruction is challenging with the unique function and anatomy. Goals for reconstruction differ depending on the extent of reconstruction. Thin and pliable flaps are useful for tongue tip reconstruction, for appearance and mobility. This study reports lateral arm free flap (LAFF) as a safe and optimal option for hemi-tongue reconstruction, especially for tongue tip after hemiglossectomy. Methods: Thirteen LAFFs were performed for hemi-tongue reconstruction after hemiglossectomy from 1995 to 2018. Of the 13 patients, seven were male and six were female, age varying from 24 to 64 years. Results: All flaps healed uneventfully without complications. Donor sites were closed primarily. The recipient vessels for microvascular anastomosis were mainly superior thyroidal artery, external jugular vein. All patients returned to normal diet, with no complaints regarding reconstructed tongue and donor site. Conclusion: The LAFF is hairless, thin (especially with lateral epicondyle approach), and potentially sensate. They are advantageous features for tongue tip and hemi-tongue reconstruction. Donor site sacrifices the inessential posterior radial collateral artery, and the scar is hidden under short sleeve shirts. We believe that LAFF can be considered as the first choice flap for hemitongue reconstruction, over radial forearm free flaps.

The current status of Canadian research on rotor-based actively controlled technologies for helicopters is reviewed in this paper. First, worldwide research in this field is overviewed to put Canadian research into context. Then, the unique hybrid control concept of Carleton University is described, along with its key element, the "stiffness control" concept. Next, the smart hybrid active rotor control system (SHARCS) projected's history and organization is presented, which aims to demonstrate the hybrid control concept in a wind tunnel test campaign. To support the activities of SHARCS, unique computational tools, novel experimental facilities and new know-how had to be developed in Canada, among them the state-of-the-art Carleton Whirl Tower facility or the ability to design and manufacture aeroelastically scaled helicopter rotors for wind tunnel testing. In the second half of the paper, details are provided on the current status of development on the three subsystems of SHARCS, i.e. that of the actively controlled tip, the actively controlled flap and the unique stiffness-control device, the active pitch link.

Korean coastal fishery suffers from profitability degradation due to a decrease in fisheries resources, pollution in coastal waters, fuel coast increase, and market opening for aquaculture products. The next generation Korean fishing vessel aims at the improvement of energy efficiency, enhancement of crew welfare, and safety. These purposes can be accomplished by adopting a new standard hull form with improved resistance performance and a modernized residence facility on the deck. In order to improve resistance performance, this study attempts to optimize design variables for stern flaps for three kinds of fishing vessels - coastal multi-purpose, coastal trap, and dredged nets. A series of model tests for these fishing vessels was carried out in the towing tank of Pusan National University. The results indicate that for some cases, the stern flap caused the stern trim of the vessel to decrease, leading to the resistance reduction.

This paper addresses a method for structural optimum design of composite rotor blade. The basic model of a composite helicopter main rotor blade is designed and its parameters determining the structural/dynamic properties are studied. Through the investigation of flap/lag/torsional stiffness, the structural properties of the model are analyzed. In this study, helicopter rotor blades are analyzed by using VABS. The computer program VABS (Variational Asymptotic Beam Section Analysis) uses the variational asymptotic method to split a three-dimensional nonlinear elasticity problem into a two dimensional cross-sectional analysis and a one-dimensional nonlinear beam problem. This is accomplished by taking advantage of certain small parameters inherent to beam-like structures. In addition, the rotational stability of the blade is estimated by the frequency diagram from FE analysis(MSC.Patran/Nastran) to understand its vibrational property. From the result, design parameters to determine and optimize the properties of the model are presented.
To reduce the drag rise on the advancing helicopter rotor blade tips, the tip of the blade is modified to have sweep, anhedral and pretwist. The equations of motion of rotor blade with these tip angles were derived using Hamilton principle, programmed using FORTRAN and named as ARMDAS(Advanced Rotorcraft Multidisplinary Design and Analysis System). Rotating frequency analysis of rotor blades with swept tipe was performed that is necessary in conceptual and preliminary design phases of the helicopter design. Vibration analysis of non-rotating blades was also accomplished and compared with MSC/NASTRAN resutls for the basis of comparison with the vibration test data. The rotating frequency analysis of blades with an actual rotor blade data was also performed to verify coded program and to check the possibility of a resonance of an actual rotor blade at the specific rotating speed.
