Service Quality beyond Access: A Multilevel Analysis of Neonatal, Infant, and Under-Five Child Mortality Using the Indian Demographic and Health Survey 2015~2016

  • Purpose: The purpose of this study was to derive contextual indicators of medical provider quality and assess their relative importance along with the individual utilization of antenatal care (ANC) and institutional births with a skilled birth attendant (SBA) in India using a multilevel framework. Methods: The 2015~2016 Demographic and Health Survey (DHS) from India was used to assess the outcomes of neonatal, infant, and under-five child mortality. The final analytic sample included 182,980 children across 28,283 communities, 640 districts, and 36 states and union territories. The contextual indicators of medical provider quality for districts and states were derived from the individual-level number of ANC visits (<4 or ${\geq}4$) and institutional delivery with SBA. A series of random effects logistic regression models were estimated with a stepwise addition of predictor variables. Results: About half of the mothers (47.3%) had attended ${\geq}4$ ANC visits and 75.8% delivered in institutional settings with SBAs. Based on ANC visits, 276~281 districts (43.1~43.9%) and 13~16 states (36.5~44.4%) were classified as "low" quality areas, whereas 268~285 districts (41.9~44.5%) and 8~9 states (22.2~25.0%) were classified as "low" quality areas based on institutional delivery with SBAs. Conditional on a comprehensive set of covariates, the individual use of both ANC and SBA were significantly associated with all mortality outcomes (OR: 1.17, 95% CI: 1.08, 1.26, and OR: 1.10, 95% CI: 1.02, 1.19, respectively, for under-five child mortality) and remained robust even after adjusting for contextual indicators of medical provider quality. Districts and states with low quality were associated with 57~61% and 27~43% higher odds of under-five child mortality, respectively. Conclusion: When simultaneously considered, district- and state-level provider quality mattered more than individual access to care for all mortality outcomes in India. Further investigations are needed to assess the importance of improving the quality of health service delivery at higher levels to prevent unnecessary child deaths in developing countries.

Adolescents' Self-control and Big Five Personality Types Affecting Maladaptive and Adaptive Computer Game Use State (청소년의 Big Five 성격 유형과 자기 조절 성향이 게임 과용, 선용 행태에 미치는 영향)

  • Adolescents reach the game-use states of adaptive and maladaptive by the absorption to computer game. Authors claimed that the two states are commonly related with the time of game-use, and the degree of them are distinctive according to adolescent individuals, specifically their self-control propensity. Authors proposed a conceptual research model that Big Five personality types predict their self-control which moderates the relationships from game use-time to the maladaptive and adaptive states. The data to test its validity and reliability had been sampled 999 Korean students in elementary school, middle school, and high school. Resultingly, the openness and conscientiousness of the adolescents affected positively on the self-control, which moderated negatively the relationship from the game use time to the maladaptive use state, but the positive moderation on the relationships from game use time to adpative state was not significant. These results mean that we could apply teenager's Big Five personality type and their self-control traits as a tool for preventing teens from the overuse state like addiction.

Meaning of Marriage & Childbirth among Korean Female University Students Living in the United States: A Phenomenological Approach (미주 한인 여대생이 생각하는 결혼과 출산의 의미: 현상학적 접근)

  • Purpose: This study aimed to describe the meaning of marriage and childbirth among Korean female university students living in the United States, who are influenced both by Korean culture at home and American culture in social contexts. Methods: This qualitative study used phenomenological approaches. Data were collected through individual in-depth interviews with five Korean female university students living in the United States. Data were analyzed using Colaizzi's method to derive the meaning of marriage and childbirth. Results: From the data, 239 meaningful statements, 49 meaning formations, 22 themes, and 12 theme clusters were extracted. Additionally, five major categories emerged. These categories were 1) marriage as an opportunity for self-growth, 2) the mirror of life: parents, 3) oneself who cannot give up, 4) a desire to live a stable life, and 5) ambivalence on the culture of two countries. Conclusion: To improve the positive meaning of marriage and childbirth, a positive role model such as a happy married life of parents at home must be ensured. Finally, institutional strategies should be established to support the balance between home and work, as well as various policies which support women's pregnancy and childbirth.

  • The clad samples of five plies of sheets comprising ferritic stainless steel (STS) and aluminum (Al) were prepared by roll-cladding at $350^{\circ}C$. The evolution of strain states and textures during roll-cladding of STS430/AA3003/AA3003/AA3 003/STS430 and STS430/AA3003/STS430/AA3003/STS430 was investigated by measurements of crystallographic textures and by simulations with the finite element method (FEM). Because the deformation mainly occurs in the Al layer during roll-cladding, the present investigation was focused on the Al layers located. The stacking sequence of sheet materials in the clad samples played an important role in the evolution of strain states during roll-cladding.

Manifestation of Affect States Across Daily Interpersonal Contexts Depending on the levels of Adolescent Depression (일상생활에서 나타나는 청소년기 우울증의 정서적 증상 - 대인맥락 변화를 중심으로 -)

  • The goal of this study was to investigate the daily manifestation of positive and negative affect states of early adolescent depression across daily interpersonal contexts. Forty-five boys and 44 girls in the second `year of middle schools participated and were classified into 3 groups according to CDI scores. Among 89 students 59 students in the highest or the lowest CDI groups continued to participate in the next step of this study. Using the Experience Sampling Method(ESM), the 59 students reported their affect states 6 times per day across four daily interpersonal contexts: the contexts alone, with all classmates, with a few friends, and with family members. The major findings were as follows. frst, the high CDI group felt more worried, angrier, more hurried, less happy, less interested, less control, and less relaxed than the low CDI adolescent group. Second, the negative affect states of the high CDI adolescent group did not vary across the interpersonal contexts, whereas their positive affect states were the highest in the context with a few friends. Implications of the findings were discussed in illuminating the phenomenology of adolescent depression.

  • This study represents results of performance-based fragility analysis of reinforced concrete (RC) bridge. Monte carlo simulation is performed to study nonlinear dynamic responses of RC bridge. Two-parameter log-normal distribution function is used to represent the fragility curves. These two-parameters, referred to as fragility parameters, are estimated by the traditional maximum likelihood procedure, which. is treated each event of RC bridge pier damage as a realization of Bernoulli experiment. In order to formulate the fragility curves, five different damage states are described by two practical factors: the displacement and curvature ductility, which are mostly influencing on the seismic behavior of RC bridge piers. Five damage states are quantitatively assessed in terms of these seismic ductilities on the basis of numerous experimental results of RC bridge piers. Thereby, the performance-based fragility curves of RC bridge pier are provided in this paper.

  • An inspection-maintenance policy is investigated for a system having various states. A policy is characterized by the type of maintenance and the next inspection time. Maintenance actions are classified into various types according to the depth of maintenance. Policy evaluation criterion is the expected cost accumulated up to the failure of the system. The problem is formulated as a Markov decision process and an optimal policy is found by using a policy improvement procedure. A numerical example illustrates the policy for a system having five states.

A Study on Verification of the Profile of Mood States(POMS) for Korean Elders (노인의 기분측정을 위한 도구(Profile of Mood States, POMS)의 타당화에 관한 연구)

  • This paper was done to verify the use of the Profile of Mood States (POMS) which was developed by McNair, Lorr, and Droppleman(1992) with modifications so that it is appropriate for Korean elders. Through the modified tool, it is possible to examine the mood of Korean elders and to contribute to the welfare of elders. The subjects were 370 elderly persons over 60 years old and the data for 319 persons(86%) were analyzed. The research tool was the POMS translated by Yun(1993) and corrected by the researcher. Data were collected between February 12 and April 9 in 1996 and analyzed using the SAS package. The result are as follows 1. Items with low Cronbach coefficient alpha which means low correlation with total items were removed. The items were removed : friendly, tense, considerate, relaxed, sympathetic, resentful, good-natured, rebellious, trusting, carefree. 2. Overlapped or ambiguous items were discussed by colleagues and elders through verification of con-tent-validity and were removed. Five items were removed in tense-anxiety, seven, in depression-dejection, three, in vigor-activity, three, in fatigue-inertia, two, in confusion-bewilder-ment, five, in friendliness, and six, in anger-hostility. Thirty four items remainined : angry, clear-headed, lively, confused, sorry, shaky, list-less, peeved, sad, active, blue, hopeless, spiteful, uneasy, unable to concentrate, fatigued, helpful, nervous, lonely, cheerful, bitter, exhausted, anxious, ready to fight, sluggish, helpless, alert, deceived, efficient, worthless, forgetful, terrified, vigorous, and uncertain about things. 3. Factor analysis was done in order to confirm construct validity and three factors were obtained from the result. The first factor, 'anxiety-depression' included 21 items, the second factor, 'vigor' included eight items, and the third factor, 'anger' included five items. Cronbach coefficient alpha for the 34 items was. 95. Based on the result, the following is suggested : 1. a contribute to elder's welfare can be made by examining Korean elder's mood in life. 2. there is a need to develop tools appropriate to Korean culture which can be used to examine elders' mood. 3. The modified POMS tool needs to be reverified with appropriate age groups and settings.

Seismic Performance based Fragility Analysis of Bridge Structure in terms of Soil Conditions (지반조건을 고려한 교량의 내진성능기반 취약도 해석)

  • The damage of earthquakes have to achieve by probabilistic evaluation because of uncertainty of earthquake. Fragility analysis is a useful tool for predicting the probability of damage induced by the probable earthquake. This paper presents the probability of damage as a function of peak ground acceleration and estimates the probability of five damage levels for the pier of prestressed concrete (PSC) bridge subjected to given ground acceleration. At each 100 artificial earthquake motions were generated in terms of soil conditions, and nonlinear time domain analyses were performed for the damage states of the pier of PSC bridge structures. These damage states are described by displacement ductility result from seismic performance based on existing research results. Using the damage states and ground motion parameters, five fragility curves for the pier of PSC bridges with five types of dominant frequencies were constructed assuming a log-nomal distribution. It was found that there was a significant effect on the fragility curves due to the dominant frequencies.

Six-Connected Contour Coding Using Contour States (윤곽 상태를 이용한 여섯 방향 윤곽부호화)

  • In this paper, we present efficient six-connected contour coding algorithms which can uniquely reconstruct any contour image and efficiently compress the contour data. We first design chain difference codes using two onward direction states, based on the fact that the probability distribution of the direction vectors of horiwntal/vertical direction state is different from that of the direction vectors of diagonal direction state. In order to increase coding efficiency, we also design chain difference codes using five states which are classified according to current and previous onward direction vectors. In addition, we also remove the END codeword to reduce total codeword occurrency. Experimental results show that when using 2 states and 5 states without END codeword total entropy decreases by about 12% and 14% for real images and by about 10% and 26% for a synthetic image, respectively.
