High Incidence of Hiatal Hernia in Esophageal Atresia and Its Etiologic Factors (선천성 식도폐쇄 수술 후 열공탈장의 호발과 그 유발인자)

  • Hiatal hernia is a very rare disease in the pediatric population. However information from our esophageal atresia postoperative follow-up program has hypotheses; "Hiatal hernia may more frequently occur in postoperative esophageal atresia patients (EA group) than in the general pediatric population (GP group)" and "The tension on the esophagus after esophageal anastomosis may be an important etiologic factor of hiatal hernia in EA group". To prove the first hypotheses, we compared the incidence of hiatal hernia in the GP group with the incidence in the EA group. The Incidence in the GP group was obtained from national statistic data from Statistics Korea and Health Insurance Review and Assessment Service of Korea. The incidence in the EA group was obtained from the medical record and the imaging studies of our esophageal atresia postoperative follow-up program. To prove the second hypothesis, the presumptive risk factors for the development of hiatal hernia in EA group, such as the type of esophageal atresia, degree of esophageal gap, the stage operation and the redo-operation with resection and re-anastomosis of esophagus were analyzed statistically. The total number of patients in the EA group was ninety-nine and there were 5 hiatus hernias. The incidence of EA group (5 %) is significantly higher than incidence of GP group (0.024 %). (p=0.0001) The statistical analysis of the presumptive risk factors for hiatal hernia development in EA group failed to show any evidence of correlation between postoperative esophageal tension and the hiatal hernia. This study shows that the postoperative patients with esophageal atresia have high occurrence of hiatal hernia and should be followed up carefully to detect hiatal hernia.

Biological control of Lettuce Sclerotinia rot using Bacillus mojanvinensis Pro-EB 15 strain.

  • This studies were investigated the occurrence of Sclerotinia rot by Sclerotinia Sclerotiorum at the lettuce field in Uiryeong-Gun, Gyeongsangnam-Do and were isolated the most effective microorganism for the biological control to the pathogen, S. sclerotiorum YR-1 strain from diseased soil and lettuce leaves. For the pathogenicity test, the most suitable inoculmn density of YR-1 strain was selected as the mycelial suspension of 40m1 showing disease incidence of 80%, and the symptom showed as same as at the fields, the leaves and stem had rotten and developed white downy mycelial at the diseased lesion on the leaves and stems, and produced black and irregular sclerotinia. On the PDA dual test, about 300 isolates were examined the antifungal activity to the pathogen, YR-1 strain, and among them, A-2, A-7, and RH-4 strain were selected most effective antagonistic bacteria. At pots test, the control value of A-7 strain showed the highest value as 85% which was more effective than that of others in a growth chamber. For the promotion of control effect, the selected 3 isolates were spayed on the lettuce leaves as a sole and/or mixed treatments in a growth chamber, the mixed treatment of A-7 and RH-4 strain showing the control value of 90% was most effective than that of sole treatment with A-7 or RH-4 strain showing the control value of 80%, respectively and mixed treatment with A-2 and A-7 strain and A-2, A-7 and RH-4 strain. In addition, 3 bacteria re-isolated from diseased soils, and all of the selected 6 isolates investigated the control effect at pots in a growth chamber, According to the results, A-7 and Pro-EB 15 strain showed the control value of 91.0% and 90.1% respectively, and they were selected most effectual antagonistic bacteria to control lettuce sclerotinia rot and identified as the Bacillus mojanuinensis by 16s RNA analysis. This is the first report on the biological control using by B. mojanvinensis to the lettuce Sclerotinia rot.

Occurrence of Pome Fruit Viruses on Pear Trees (Pyrus pyrifolia) in Korea (국내에서 발생하는 배나무 바이러스병)

  • Three pome fruit viruses, Apple chlorotic leaf spot virus (ACLSV), Apple stem grooving virus (ASPV) and Apple stem pitting virus (ASGV) were detected in pear trees (Pyrus pyrifolia) using double antibody sandwich enzyme-linked immunosorbent assay (DAS-ELISA) in Ansung, Naju and Ulsan provinces of Korea. Infection rate of three viruses was 35.2% from 452 leaf samples of the three cultivars of pear trees. Also, each of three viruses was detected by reverse transcription polymerase chain reaction (RT-PCR) for a limited number of samples. Infection rate of three viruses was 86.3% from 233 leaf samples of the three pear cultivars. The virus infection rates by RT-PCR were much higher than ELISA. ASGV was prevailing on pear with 74.2%, whereas ASPV and ACLSV were found in 34.8% and 0.4% of tested samples, respectively. Symptoms caused by ASGV showed black spots of infected Niitaka cultivar leaves. The ACLSV, ASPV and ASGV isolates showed 83~94% sequence identity at a nucleotide level to other pome fruit virus isolates when analyzed by NCBI BLAST. Pome fruit viruses occurring in pear were ACLSV, ASPV and ASGV. This is the first report of pear trees infected ASPV in Korea.

A Statistical Study on Presymptoms of Stroke by Case-Control Study (환자 - 대조군 연구를 통한 중풍 전조증상에 대한 통계적 고찰)

  • Objectives : This study investigated warning signs and stroke presymptoms and their relationship with acute stroke patients in each stroke type. Methods : 217 patients with first-ever acute stroke within 2 weeks as the case group, 146 people without four major risk factors (hypertension, diabetes mellitus, hyperlipidemia and ischemic heart disease) as the healthy control group (Normals), and 160 people as the general control group (Controls) were recruited at the Stroke Medical Center in Daejeon University Oriental Medicine Hospital from July 2005 to March 2007 for this case-control study. We analyzed the odds ratio of each warning sign or presymptom by multivariate logistic analysis and evaluated each stroke type, as well as general characteristics such as age, sex, etc. Results : Paralysis or weakness of the face and limbs, visual field disorder or loss of vision or blurred and double vision in eyes or both eyes, and sudden difficulty in speaking or dysarthria showed significant increase respectively in each stroke type. In contrast, numbness or tingling sensation or lowering sensation of the face and limbs, numbness or dead sensation of 1st and 2nd fingers, tension at cervical lesion, blepharospasm, facial spasm, etc. had no significant relation with stroke occurrence. Conclusions : In this study we demonstrated that each presymptom have an influence on each stroke type and have different odds ratio for stroke. We believe this may contribute to interpreting the importance of warning signs or presymptoms for each type of stroke, and more prospective studies are needed.

  • Soybean yellow mottle mosaic virus (SYMMV) and Soybean yellow common mosaic virus (SYCMV) were recently isolated in Korea, and it has not been reported how two viruses were dispersed in Korea. In 2012, we performed nationwide survey in subsistence soybean farming. Suspicious virus-infected infected leave were collected from the field and a total of 682 soybean tissue samples were assayed by RT-PCR using triplex primers detecting SYMMV, SYCMV, and Soybean mosaic virus (SMV). On hundred two samples showed SMV positive, and SYMMV and SYCMV were detected in 116 and 17 tissue samples, respectively. No sample showed double infection of SYMMV and SYCMV, but there were double infection tissues indicating two viruses positive of SMV plus SYMMV (5 tissue samples) and SMV plus SYCMV (1 tissue sample). Through this first subsistence soybean farming field survey, we assumed soybean viruses were originated from home seed production managed by farmer. Thus, in order to prevent possible seed transmission and further damage caused by virus transmission, virus-free commercial soybean seeds are recommended to be planted.

A Study on Effect of Individual Education about Chemical Therapy for Preventing Cancer on Quality of Life of Patient Who Suffers From Cancer (항암화학요법에 관한 개별교육이 암 환자의 삶의 질에 미치는 효과)

  • The disease occurrence rate of the cancer is rapidly increased and It is becoming the main factor for the death The chemical therapy for preventing the cancer is recently used for many patients and thus extended the life of the patients. However, the side effect caused by the medical substances when performing the chemical therapy for preventing the cancer and the consequent mental and social problem are incurred to deteriorate the quality of the life. Therefore, it is needed to help the patient In order to reduce the above problems, and so this study was executed in order to examine the effect of the individual education on the quality of the life of the patients who are treated by the chemical therapy for preventing the cancer. The study was performed from Jan. 19. 2004 to Apr. 18. 2004 for the patients who were firstly treated by the chemical therapy for preventing the cancer in a university hospital located in the downtown in Seoul. The individual education on the chemical therapy for preventing the cancer was provided to 40 patients of the experimental group. The study plan is the experimental plan before and after the sole group, and it is the beginning experimental plan. The title of the booklet on the chemical therapy for preventing the cancer is "Cancer, The more you know, the more you can be cured", and the book was composed up of the side effect of the chemical therapy for preventing the cancer, treatment way for the side effect, and guide of the daily life. The survey with the questionnaire sheet was distributed to the experi- mental group before the education, the survey sheet was made out when they were hospitalized for 3-4 weeks after the education, and the data were examined by using SPSS statistical program with making our the survey questionnaires and the change of the quality of the life before and after the education of the patients who are treated by the chemical therapy for preventing the cancer was analyzed by using the parred t-test. The research result was verified that it has the meaningful that the quality of the life for the physical field, mental field, social field, and spiritual field after the individual education. The suggestion concluded by the above research result is as follows. First, there is the limitation to interpret the result since it was the beginning experimental plan for the sole group. Thus, it is suggested that the similar experimental plan should be executed with the expansion of the research subject and also with the contrast group. Second, it is suggested that the study on the change of the quality of the lifeaccording to the support of the individual education and family of the patients who are treated by the anti-cancer therapy.

Mycobacterium Kansasii Disease Presenting As a Lung Mass and Bronchial Anthracofibrosis (폐종괴와 기관지 탄분섬유화로 발현한 Mycobacterium kansasii 감염 1 예)

  • The incidence of Mycobacterium kansasii pulmonary diseases are on the increase in Korea with the higher probability of occurrence in middle-aged and older men with underlying lung diseases Among nontuberculosus mycobacterial (NTM) infections, the clinical features of M. kansasii pulmonary infection are most similar to those of tuberculosis (TB). The chest radiographic findings of M. kansasii infection are almost indistinguishable from those of M. tuberculosis (predominance of an upper lobe infiltration and cavitary lesions), even though some suggest that cavities are more commonly thin-walled and have less surrounding infiltration than those of typical TB lesions. Although there are reports on the rare manifestations of M. kansasii infections, such as endobronchial ulcer, arthritis, empyema, cutaneous and mediastinal lymphadenitis, cellulites and osteomyelitis, the association with bronchial anthracofibrosis has not yet been reported. This report describes the first case of M. kansasii infection presenting as a lung mass in the right lower lobe with accompanying bronchial anthracofibrosis.

Occurrence of Anthracnose Caused by Glomerella cingulata on Eucaly trees in Korea (Glomerella cingulata에 의한 유카리나무 탄저병 발생)

  • Since 2003, anthracnose symptoms on Eucalyptus globulus were observed in farmer's field at Jisepo, Ilwoon-myon, Geoje city, Gyeongnam province, Korea. Typical symptoms of dark brown to black spot appeared on the leaves, twigs, and stems. Infected young trees were wilted, blighted and died eventually. The pathogen isolated from the typical symptom formed gray to dark gray colony on potato dextrose agar and showed optimum growth at $30^{\circ}C$. Conidia were single celled, colorless, cylindrical with obtuse ends, and $9{\sim}22{\times}3{\sim}6{\mu}m$ in size. Appressoria were dark brown, ovate to obovate, and $6{\sim}18{\times}4{\sim}10{\mu}m$ in size. Perithecia were black and globose in shape and $76{\times}274{\mu}m$ in size. Asci were clavate to cylindrical in shape and $42{\sim}76{\times}8{\sim}12{\mu}m$ in size. Ascospores were cylindrical, fusiform, slightly curved at the center, and $10{\sim}23{\times}4{\sim}6{\mu}m$ in size. On the basis of mycological characteristics and pathogenicity test on E. globulus, the pathogen was identified as Glomerella cingulata. This is the first report of the anthracnose on E, globulus caused by G. cingulata in Korea.

Diagnosis and Sequence Analysis of Japanese yam mosaic virus from Yam (Dioscorea opposita) (마(Dioscorea opposita)에 발생한 Japanese yam mosaic virus 진단 및 염기서열 분석)

  • We surveyed the occurrence of Japanese yam mosaic virus (JYMV) on Yam in Gyeongsangbukdo pronvince from 2013 to 2015. The symptoms of JYMV were yellow stripes and chlorosis in yam leaves and the infection rate was ranged from 33.6% to 40.8%. We determined nucleotide sequence encoding the polyprotein of JYMV isolate BRI from yam leaves using next-generation sequencing (NGS) method. The partial nucleotide portion (7,736 nucleotides) of the genomic RNA of the JYMV isolate BRI has been sequenced (accession No. KU309315). The region sequenced includes a single open reading frame (ORF) encoding a polyprotein composed of 2,497 amino acids containing the coat protein (CP) and 3' untranslated region (UTR). The genomic organization of this isolate shows almost the same to that of other members of JYMV. The JYMV isolate BRI showed 77% to 79% nucleotide identity with the Japanese and Chinese strains and isolates. This is the first report of the genome nucleotide sequence of JYMV from Dioscorea opposita in Korea.

Patterns of Esophageal Cancer in the National Cancer Institute at the University of Gezira, in Gezira State, Sudan, in 1999-2012

  • Background: Esophageal cancer (EC) is among the most common malignancies in Eastern Africa, but the occurrence of EC in Sudan has rarely been described in the scientific literature. This paper reports the results of a consecutive case series of all EC patients who visited one of the two public cancer treatment centers in the country in 1999-2012, providing a first description of this disease in a treatment center located in central Sudan. Materials and Methods: Clinical and demographic data for all EC patients who visited the Department of Oncology of the National Cancer Institute at the University of Gezira (NCI-UG) from 1999 to the end of 2012 were abstracted and tabulated by sex, tumor type and other characteristics. Results: A total of 448 EC patients visited NCI-UG in 1999-2012, and the annual number of EC cases increased steadily from 1999. Squamous cell carcinoma (SCC) was the predominant EC tumor type (90%), and adenocarcinoma (ADC) was reported in 9.4% of the EC cases. The overall male-to-female ratio for EC was 1:1.8, but the ratio was tumor type-dependent, being 1:2 for SCC and 2:1 for ADC. Only 20% of EC patients reported having ever used tobacco and/or alcohol, and the vast majority of these patients were male. At the time of EC diagnosis, 47.3% of the patients resided in Gezira State. Some EC patients from Gezira State seek out-of-state treatment in the national capital of Khartoum instead of visiting NCI-UG. Conclusions: The annual number of EC patients visiting NCI-UG has increased in recent years, approximately half of these patients being from Gezira State. Although this consecutive series of EC patients who visited NCI-UG was complete, it did not capture all EC patients from the state. A populationbased cancer registry would provide more complete data required to better understand EC patterns and risk factors.
