Beyond the clinical walls: registered dietitian nutritionists providing medical nutrition therapy in the home setting

  • BACKGROUD/OBJECTIVES: Registered dietitian nutritionists (RDN) are providers of medical nutrition therapy (MNT) to address health and chronic disease. Traditionally, RDNs have provided care in healthcare facilities including hospitals and private care facilities. The purpose of this study was to determine how RDN individualized MNT in the home impacted nutrition, physical activity, and food security. SUBJECTS/METHODS: This is a secondary data analysis. The mean age of the participants (n = 1,007) was 51.6 years old with a mean body mass index (BMI) of 34.1 kg/m2. Individualized MNT visits were delivered by an RDN in the home setting from January to December 2019. Participants were referred by healthcare professionals or self-referred. Participants had MNT benefits covered by their health insurance plan (43.3% Medicaid; 39.8% private insurance; 7.9% Medicare, 9% other). Health outcomes related to nutrition care were measured. Outcomes included self-reported consumption of nutrition factors and physical activity. Our secondary outcome focused on food security. The changes in weight, BMI, physical activity, and nutrition factors were analyzed by a linear regression model or linear mixed model, adjusting for age, sex, baseline value, and number of appointments. Food security was summarized in a 2 by 2 contingency table. RESULTS: Baseline values had significantly negative impacts for all changes and number of appointments was significant in the changes for weight and BMI. Increases in physical activity were significant for both female and male participants, 10.4 and 12.6 minutes per day, respectively, while the changes in weight and BMI were not. Regarding dietary factors, the consumption total servings per day of vegetables (0.13) and water (3.35) significantly increased, while the consumption of total servings of whole grain (-0.27), fruit (-0.32), dairy (-0.80) and fish (-0.81) significantly decreased. About 24% (of overall population) and 45% (of Medicaid population) reported improvements in food security. CONCLUSIONS: This study found that home visits were a useful setting for MNT delivered by RDNs. There is a strong need for individualized counseling to meet the participants' needs and personal goals.

  • This study is to analyze the relationship between self-esteem and happiness of parents and children in the 4th-grade elementary school and the 1st-grade of middle school using Korean Children and Youth Panel Survey(KCYPS) 2018 data by the National Youth Policy Institute. The context of analysis is to determine how parents' self-esteem and happiness affect parenting attitudes, and furthermore, how they affect their children's academic activities, self-esteem and happiness. This study is able to explore the level at which self-esteem and happiness, the mental health measure of children and adolescents, were linked to parental self-esteem and happiness. And it suggests that the importance of the role of a parent as a family background has been confirmed in promoting the healthy development of children and adolescents.

Conditions for Integrated Public Welfare Delivery System: Case Studies in Britain (통합적 공공 복지전달체계를 위한 조건: 영국 사례 연구)

  • There has been constant attempts for integration of public welfare delivery system which lead to integrated case management, then this become more significant issue regarding 'community care' recently in Korea. However, most of them was limited to fractional organization rearrangement or more use of private resources rather than realising user-centred approach. Therefore, in this research, we would like to do a couple of case studies in Britain which has rich experience of reforms for integrated approaches between health and social services and Troubled Family programme resembles the integrated case management in Korea by visiting a London Borough and a local council respectively. In conclusion, we found three conditions are required to establish successful integrated delivery system: elimination of institutional barriers relating to workforce, organization, and finance; shared objectives and partnership among the professional groups and agencies; and information sharing system with technical support regarding individual cases.

Analysis on the Factors Influencing Elderly's Successful Aging: Group Comparison by Household Types (노인의 성공적 노후 영향요인 분석 : 가구유형별 집단비교)

  • The purpose of this study are the following two: first, to identify success level of elderly's Aging by their household types and second, to analyze the factors affecting successful aging. The results of this study are expected to be used as basic data for policy and program development for the successful life of the elderly. For this purpose, 699 single household, 1,412 couple household and 238 couple/child household data in personal survey and supplementary survey of the 4th Korean Retirement and Income Study. According to the analysis, the level of successful later life of single household is lower than that of couple or couple/children household. Multiple regression analysis was conducted to identify the factors affecting successful aging in single household. The results showed that economic activity, economic independence, housework, neighborhood affairs, family relations, and parent-child relationship were statistically significant. Economic independence, physical health, friendship, and parent-child relationship were found to affect in couple household. Physical health and friendship were found to be significant factors for successful aging in couple/child household.

An Analytical Study on Research Trends of the Library Services for the Information Poor in Korea (정보불평등 계층을 위한 도서관서비스 관련 국내 연구동향 분석)

  • Libraries are recently emerging as the core institutions designed to resolve the information equality that is forecasted to further deepen with the arrival of the fourth industry revolution era. This study thus analyzed the trends of studies on the library services offered to people suffering information inequality in Korea, and proposed measures for the activation of the research and future studies so as to strengthen the domestic library services targeting the people suffering information equality and to promote the research. As a result, proposed as measures for the activation of research were (1) Policy support and project expansion at the national level, (2) Various approaches and research environment creation at the academic level, (3) Strengthening of services for the various levels of people suffering information inequality and deployment of professional librarians at the level of libraries, and (4) Encouraging librarians to enroll with graduate schools and to conduct research. Further, proposed were the future studies that can be approached according to the various levels of people suffering information inequality, such as disabled people, multicultural family members, elderly people, children, youth, agricultural and fishing areas, low-income people, and refugees(North Korea defectors).

Bridging Research and Extension Gaps of Paddy Yield in Andhra Pradesh, India

  • Many paddy cultivating farmers in the country are forced to use their limited resources to produce adequate food for their family, leading to the degradation and reduction in potential of these resources. The yield levels of paddy at the farmers' level and in the Front Line Demonstrations (FLDs) conducted in the farmers' fields is not at par with potential yield of the paddy variety. The gap between potential yield of crop variety and yield realized in FLDs refers to Research gap and the yield gap between FLDs and due to farmers' practice refers to Extension gap. The earlier studies conducted in India in general and in Andhra Pradesh in particular highlighted the existence of both research and extension gaps with reference to paddy. It is essential that, the narrowing of both research and extension gaps is not static, but dynamic considering the influence of technological interventions in boosting paddy yields at FLDs level and at farmers' level and also with the improvement of the yield potential of paddy varieties. This calls for integrated and holistic approaches to address these two gaps and with this background, the researcher aimed at this in depth study. The findings revealed that, research gaps are high with reference to weed management and pest management and extension gaps are high with reference to farm mechanization followed by fertilizer management. Reliable source of seed, capital use and frequency of meetings with Scientists or Agricultural Officers significantly influence the extension gaps in paddy. Farmers also prioritized socio-economic and technical constraints and the analysis infers that, it is high time now for the farmers to adopt the planned technological interventions on scientific scale to minimize the extension gaps to the extent possible. As the enabling environment in the State of Andhra Pradesh is highly encouraging for the farmers with relevant policy instruments in the form of subsidized inputs, free power, credit at concessional rates of interest, constructing irrigation projects etc., the adoption of the proposed technological interventions significantly contribute to minimizing both research and extension gaps in paddy cultivation in Kurnool district of Andhra Pradesh.

An Empirical Effect of the Belief Variables on Recommendation Intention for Using Kiosk Service (키오스크 서비스의 추천의도에 영향을 미치는 신념변수에 관한 실증적 분석)

  • The purpose of this study is to identify belief variables of kiosk that affect satisfaction and recommendation intention by applying vertically extended technology acceptance model (TAM). As results of this study are as follows. Firstly, the perceived usefulness and perceived ease of use have an impact on satisfaction statistically. Second, the perceived usefulness and perceived enjoyment of the kiosk showed a positive(+) influence on the recommendation intention. Third, users who satisfied with kiosk service have strong recommendation intention to their friends, family and colleagues so on. These findings support that the usefulness and usability of the new information technology identified in the existing technology acceptance model is a key variable affecting user reactions (satisfaction and recommendation intention). Additionally, the perceived enjoyment is an important factor as new belief variable for explaining the formation of recommendation intention through satisfaction of kiosk service. The results of this study contribute to verifying the empirical model between the belief variables(perceived usefulness, perceived ease of use, perceived enjoyment) and the recommendation intention.

A study on Anti-diabetic Mechanism of Ethanol Extract of Dendrobii Herba (석곡 에탄올 추출물의 항당뇨 약리기전에 관한 연구)

  • Antidolary active and anti-sugar mechanisms of the ova family (石斛; Dendrobii herba) ethanol extract (EED) were investigated. The EED was administered orally four times a day in a diabetic mouse induced by strepto Joe Toshin to reveal and reveal its pharmacological miracle through experimental studies that reduce the liver function of empty blood sugar, glythamic oxal acetate levels, insulin levels and glutamic acid trans aminaase and glutamic acid pyruvic acid trans amine. EED increased insulin secretion by glucose in RINm5F beta cells as well as intraperitoneal glucose intakes in L6 muscle cells. Thus, EED has shown great promise in displaying anti-diabetes activity not only by increasing insulin secretion but also by increasing intakes per cell, and hopes that future research on pharmacological mechanisms for quartz (Dendrobii herba) ethanol extract will be more active and contribute greatly to the treatment of diabetes.

  • Geron-technology contributing to successful "Aging in place" increases the quality of life for the elderly and decreases the social caring cost. This study aims to identify influencing factors on pre-implementation acceptance of geron-technology for the elderly residing in a community. Data for 129 participants were analyzed. Results indicated that location, physical function, lack of technical support by family members, and the high financial cost as a 28% power were identified as the influencing factors on pre-implementation acceptance of geron-technology. Therefore, in the development of health and welfare strategy, these influencing factors should be considered in order to archive 'aging in place' for the elderly.

Factors to Disturb Adult Learner's e-Learning Persistence: A Case Study of H-Cyber University in Seoul Korea (성인학습자의 e-러닝 학업지속 장애요인에 대한 사례 연구:서울소재 H사이버대학을 중심으로)

  • This study investigates factors to disturb the e-Learning persistence of adult learners based on a case study of Cyber University students located in Seoul Korea. Main findings of our research show that economic burden, shortage of studying hours, digital literacy problem, perceived isolation, inefficient interaction between lecturer and student, and support of family are primary factors to influence the persistence of online education in a Cyber university. From our case study, we recommend to improve the scholarship system, and supplement the offline special lectures at weekends in order to alleviate perceived isolation. A novelty of this paper is that economic burden is highlighted to influence the persistence of adult learners.
