In FMEA, the risk priority number(RPN) is used for risk evaluation on each failure mode. It is obtained by multiplying three components, i.e., severity, occurrence, and detectability of the corresponding failure mode. Each of the three components are usually determined on the basis of the past experience and technical knowledge. But this approach is not strictly objective in evaluating risk of a given failure mode and thus provide somewhat less scientific measure of risk. Assuming a homogeneous Poisson process for occurrence of the failures and causes, we propose a more scientific approach to evaluation of risk in FMEA. To quantify severity of each failure mode, the mission period is taken into consideration for the system. If the system faces no failure during its mission period, there are no losses. If any failure occurs during its mission period, the losses corresponding to the failure mode incurs. A longer remaining mission period is assumed to incur a larger loss. Detectability of each failure mode is then incorporated into the model assuming an exponential probability law for detection time of each failure cause. Based on the proposed model, an illustrative example and numerical analyses are provided.

In this paper, a model pole transformer was designed to improve the reliability of pole transformers and its short circuit withstand strength was tested. The failure causes of the pole transformers which had been operated in the field were analyzed and several patterns of the causes were determined. A novel countermeasure was devised to prevent the failures of the pole transformers in advance. A model pole transformer was designed and made in accordance with the given specifications to strengthen the short circuit withstand capability and its characteristic was tested. As a result, the advanced performance of the model pole transformer was confirmed and novel techniques in manufacturing process was suggested. It is considered that all the methods employed in the developing process will be helpful to design and manufacture the pole transformers thus the reliability of pole transformers in distribution lines will be improved.
The electronics factory fire, that occurred at 15:40 on August 21, 2018, killed nine people and injured six. This study analyzes the causes of many of the casualties from the fire, and is based on fire investigation results and so on. The findings suggest that major causes included failure of the automatic alarm system to function, failure of the emergency broadcasting system to function, the fire suddenly spreading due to polyurethane foam in a ceiling on the 4thfloor, a power outage immediately after the fire started, a sprinkler system that was not working, a delay in reporting the fire, and improper management of fire facilities by a fire safety management company.

The extreme steam temperature deviation experienced in the superheater of a tangentially fired boiler can seriously affect its economic and safe operation. This temperature deviation is one of the main causes of boiler tube failures. The steam temperature deviation is mainly due to the thermal load deviation in the lateral direction of the superheater. The thermal load deviation consists of several causes. One of the causes is the non-uniform heat flow distribution of burnt gas on the superheater tube system. This distribution is very difficult to measure in situ using direct experimental techniques. So, we need thermal load model to estimate the tube temperature. In this paper, we propose a thermal load distribution model by using CFD analysis and plant data. We successfully predict the tube temperature and the steam flow rate in a final superheater system from the thermal load model and one dimensional heat-flow system analysis. The proposed model and analysis method would be valuable in preventing the frequent tube failure of the final superheater tubes.
We scrutinized the 64 cases of TGA and Taussig-Bing anomaly who underwent Senning and Mustard operations from Jan. 1981 to May 1988. The ratio of male to female was 45; 19, and the age at operation varied from 2 months to 18 years [mean 18.9*32.9 months]. The in-hospital mortality was in 24 cases [37.5%] and the major causes were myocardial failure and congestive heart failure associated with arrhythmias. The risk factors for hospital mortality were complex TGA, prolonged bypass time and high postoperative CVP. In addition, mortality increased during the first year the procedure was used. Late mortality occurred in 6 cases and the major causes was congestive heart failure, and there was not any significant risk factor noted in late mortality. Early arrhythmia developed in 37.5%, all of which were transient and self limited and 7 cases of early mortality were related to the arrhythmias. Late arrhythmias developed in 8 cases, but 7 cases were transient. One case died with junctional tachycardia. Of significance the one case that died late by arrhythmia had a similar junctional tachycardia in the early postoperative period. The survival rate in all cases disregarding initial in-hospital mortality 1YSR 89.8% and 5YSR 84.3%, but because of short duration of follow up this is not significant. We concluded that early hospital mortality could be decreased by operating at an earlier age and by adjusting the appropriate operation method.
The management aggravation of forests and the increase in the demand on resorts promote the urban use of forests, and the development of forests-using resorts is becoming an important political issue. Based on this understanding, this paper purports to examine the causes for the failure of the Shinrin Toshi policy (development of living space and resorts) which Japan promoted, and also examined the future direction of the development of forest resorts in Korea. As a result of the study, it was found that the Shinrin Toshi policy of Japan was promoted through the new changes of bubble economy and forest culture theory, and various strategies for the co-existence of human beings and nature were worked out. However, the policy took precedence without concrete plan production, which was found as the direct cause for the failure. The reasons for not having concrete plan production were: the target and the resort lifestyle (who will use it and what life will be developed) were not clear; the subject of the policy was restricted to state forests; and the dependence on civil capital in the promotion of the policy was too big. Therefore, in order to make the Shinrin Toshi policy successful, concrete lifestyle how the forest resort will be used and operated - needs to be set up and resources should be concretized. For this, the concrete demand on the forest resort life and the traditional community property of forest areas will have to be examined.
In an entrepreneurial ecosystem, the failure rate of startups is extremely high at 90%, and every startup that fails becomes an orphan. This phenomenon leads to higher costs of failure for the entrepreneurs in the ecosystem. Failed startups have many lessons to offer to the ecosystem and offer guidance to the potential entrepreneur, and this area is not fully explored compared to the literature on successful startups. We use a case based method distinguishing a failed startup and a successful startup, studying the entrepreneurial characteristics and firm level factors which cause the failures, in the technology startup ecosystem of Bangalore. We study one of the modes of exit adopted by failed startup entrepreneurs and draw key lessons on causes that culminate in failures. We have identified that factors such as the time to minimum viable product cycle, time for revenue realization, founders' complementary skillsets, age of founders with their domain expertise, personality type of founders, attitude towards financial independence and willingness to avail mentorship at critical stages, will decisively differentiate failed startups from the successful ones. Accordingly, implications have been derived for potential entrepreneurs for reducing the cost of failures in the entrepreneurial ecosystem.

High voltage ceramic capacitors are widely applied in power electronic circuits, such as filters, snubbers, and resonant circuits, due to their excellent features of high voltage endurance and low aging. This paper presents a result of failure analysis and reliability evaluation for high voltage ceramic capacitors. The failure nodes and failure mechanisms were identified in order to understand the failure physics in a component. The causes of failure mechanisms for zero resistance phenomena under withstanding voltage test in high voltage ceramic capacitors molded by epoxy resin were studied by establishing an effective closed-loop failure analysis. Also, the condition for dielectric breakdown was investigated. Particular emphasis was placed on breakdown phenomena at the ceramic-epoxy interface. The validity of the results in this study was confirmed by the results of accelerated testing. Thermal shock test as well as pressure cooker test for high voltage ceramic capacitor mounted on a magnetron were implemented. Delamination between ceramic and epoxy, which, might cause electrical short in underlying circuitry, can occur during curing or thermal cycling. The results can be conveniently used to quickly identify defective lots, determine mean time to failure (MTTF) of each lot at the level of Inspection, and detect major changes in the vendors processes.
Purpose: The purpose of this study is to point out that the Kaplan-Meier method is not valid to calculate the survival probability or failure probability (risk) in the presence of competing risks and to introduce more valid method of cumulative incidence function. Methods: Survival analysis methods have been widely used in biostatistics division. However the same methods have not been utilized in reliability division. Especially competing risks cases, where several causes of failure occur and the occurrence of one event precludes the occurrence of the other events, are scattered in reliability field. But they are not noticed in the realm of reliability expertism or they are analysed in the wrong way. Specifically Kaplan-Meier method which assumes that the censoring times and failure times are independent is used to calculate the probability of failure in the presence of competing risks, thereby overestimating the real probability of failure. Hence, cumulative incidence function is introduced and sample competing risks data are analysed using cumulative incidence function and some graphs. Finally comparison of cumulative incidence functions and regression type analysis are mentioned briefly. Results: Cumulative incidence function is used to calculate the survival probability or failure probability (risk) in the presence of competing risks and some useful graphs depicting the failure trend over the lifetime are introduced. Conclusion: This paper shows that Kaplan-Meier method is not appropriate for the evaluation of survival or failure over the course of lifetime. In stead, cumulative incidence function is shown to be useful. Some graphs using the cumulative incidence functions are also shown to be informative.
The failure of the reefer container causes a great loss of cost, but the current reefer container alarm system is inefficient. Existing studies using simulation data of refrigeration systems exist, but studies using actual operation data of refrigeration containers are lacking. Therefore, this study classified the causes of failure using actual refrigerated container operation data. Data imbalance occurred in the actual data, and the data imbalance problem was solved by comparing the logistic regression analysis with ENN-SMOTE and class weight with the 2-stage algorithm developed in this study. The 2-stage algorithm uses XGboost, LGBoost, and DNN to classify faults and normalities in the first step, and to classify the causes of faults in the second step. The model using LGBoost in the 2-stage algorithm was the best with 99.16% accuracy. This study proposes a final model using a two-stage algorithm to solve data imbalance, which is thought to be applicable to other industries.
