  • Facial nerve paralysis following the administration of a local anaesthetic can be alarming. By reading reports of such incidents, dentists who find themselves in similar situations will be able to reassure their patients and act accordingly. This article reviews the classifications of anesthetic complication, local complications, etiology, prevention, treatment of facial nerve paralysis fellowing the administration of a local anaesthetic. A thorough knowledge of the relevant anatomy pertinent to the various injections used in dental surgery is essential.

사지위약감과 안면마비를 주소로 하는 길랑바레 증후군 환자의 한방치료 증례보고 1례 (A Case Report of a Patient with Guillain-Barre Syndrome Complaining of Limb Weakness and Facial Paralysis That Improved After Korean Medicine Treatment)

  • Objectives: This study reports a case of Guillain-Barre Syndrome (GBS) in which the patient experienced improved limb weakness, facial paralysis, paresthesia, and systemic pain after Korean medicine treatment. Methods: A 25-year-old female patient diagnosed with GBS received the herbal medicine Banhasasim-tang Soft Ext., acupuncture, electroacupuncture, moxibustion, cupping, and rehabilitation treatment. To confirm the change in symptoms, the manual muscle test (MMT), Korean Version of Modified Barthel Index (K-MBI), Yanagihara grading system (Y-score) and Numeric Rating Scale (NRS) were performed. Results: After Korean medicine treatment, there was a significant improvement in GBS-related clinical symptoms. Conclusions: Korean medicine treatment could be effective in improving symptoms of limb weakness, facial paralysis, paresthesia, and systemic pain related to GBS. However, this study has limitations as a case report, and more studies are needed.

말초성 안면신경마비에 대한 황련해독탕약침과 자하거약침의 효능 : 후향적 비교 연구 (Efficacy between Hwangryunhaedok-tang Pharmacopuncture Therapy and Hominis Placenta Pharmacopuncture Therapy on Peripheral Facial Paralysis : Retrospective Comparision Study)

  • Objectives : The purpose of this study is to compare the effects of Hwangryunhaedok-tang pharmacopuncture therapy with Hominis Placenta pharmacopncture therapy in hospitalized patients with peripheral facial paralysis. Methods : We investigated 34 cases of patients with peripheral facial paralysis who were admitted into the Dept. of Acupuncture & Moxibustion of Dongshin University Suncheon Oriental Hospital from February 1, 2014 to June 31, 2015. Subjects were divided into two groups, Hwangryunhaedok-tang pharmacopuncture group(HR group), and Hominis Placenta Pharmacopuncture group(JH group). HR group was treated by Hwangryunhaedok-tang pharmacopuncture five times a week and JH group was treated by Hominis Placenta Pharmacopuncture five times a week. And both groups were treated by acupuncture, electroacupuncture, herbal medicine therapy, western drug therapy equally. To investigate the effectiveness of treatment, we used House-Brackmann Grading System, Yanagihara's unweighted grading system and Sunnybrook facial grading scale at before admission and after admission. Results : Each scores by 3 evaluation methods improved both in two groups. However, there were no significant differences in improvement between two groups. Conclusions : These results suggest that the Hwangryunhaedok-tang pharmacopuncture therapy is as effective as Hominis Placenta pharmacopuncture therapy to improve symptoms of peripheral facial paralysis.

구안와사(口眼喎斜)에 대한 한의(韓醫) 및 한(韓)·서의(西醫) 협진(協診) 치료(治療)의 임상(臨床) 관찰(觀察) (Comparative Clinical Study between Oriental Medicine and Oriental-western Medicine Treatment on Facial Nerve Paralysis)

  • The following results have been obtaind after examing 72patients with facial paralysis who were hospitalzed and treated through the time period of December 1st, 1996 to November 30th, 1999 at the Department of Acupuncture and Moxibustion of Seoul Oriental Medicine, Kyungwon University. During the examination, those 72 patients were divided into two groups, and One group was treated by oriental-western treatment, the other group was treated by oriental treatment. Oriental treatments were acupuncture and moxibustion, herb medicine, physical treatment and hygienic treatment. The one of main western treatments is steroid therapy. The results were obtained as follows : 1. In regard to signs at the first medical examination, lacrimation was showed highest number and facial paralysis, dysgeusia, hyperacusis were showed in numerical order. 2. In regard to prescription of oriental herb medicine, Kamissangbotang(加味雙補湯) was prescribed in greatest numbers and Boyangwhanotang(補陽還五湯), Kamiboiktang(加味補益湯), Ligigepungtang(理氣祛風湯) were prescribed in numerical order. 3. In regard to treatment number, 10~19 times for treatment was showed highest number and 1~9 times, 20~29 times, 40~49 times were showed in numerical order . 4. In regard to mean treatment times about injury region and main sign, the effect of oriental-western treatment was showed as follows: lacrimal gland disorder, hyperacusis, dysgeusia, facial paralysis were treated for 15.1, 27, 13.2 and 21.4 times, repectively. The effect of oriental treatment was showed as follows: lacrimal gland disorder, hyperacusis, dysgeusia, facial paralysis were treated for 34.8, 22.1, 33.8 and 16.3 times, respectively. 5. In regard ta the effect of treatment about injury region and main sign, oriental-western treatment was showed as follows: cases of lacrimal gland disorder were showed 1 of excellent case, 1 of fair case, 5 of good cases. In hyperacusis patients, there was showed 1 of fair case. In dysgeusia patients, there was showed 1 of excellent case, 3 of fair cases, 1 of good case. In facial paralysis, there were showed 5 of fair cases. Oriental treatment was showed as follows: In lacrimal gland disorder, the excellent were 4 cases, the fair were 10 cases, the good were 3 cases and the poor were 4 cases, In hyperacusis, the fair were 5 cases, the poor 2 cases. In dysgeusia, the excellent were 4 cases, the fair were 1 case, the good were 1 case and the poor was 1 case, In facial paralysis, the excellent were 9 cases, the fair were 4 cases, the good 3 cases and the poor were 3 cases. 6. The effect of total treatment was as follows: 30 cases were showed fair effect, 19 cases were showed excellent effect, 13 cases were showed good effect and 10 cases were showed poor effect. 7. In regard to attack factor, overlabour was showed highest number and wind-cold, mental stress, trauma, ear disease, common cold, dental diseae, reason unknwon were showed in numerical order. 8. In regard to premonitory symptoms, non significant symptoms were in 38 cases, the pain of peri-stylomastoid region were in 38 cases and headache, dysaesthesia of periorbit, dysgeusia, stomatitis, eyelid tic were showed in numerical order. 9. In regard to sex, male were 33 cases and female were 39 cases. The distribution of age was disclosed that thirty, forty, fifty, seventy, sixty, twenty and below twenty years were revealed in turn. Sex and paralytic side were showed as follows: male-left were 15 cases, male-right were 18 cases, female-left were 19 cases and female-right were 19 cases. In regard to attack frequence in month, March was showed highest number and January, April, May, August, October, etc were showed in numerical order. In regard to attack frequence in season, spring was showed highest number and winter, summer, fall were showed in numerical order, but attack frequence between four seasons wasn't showed significant difference.

급성 벨마비에서 Acyclovir의 효과 (The Effect of Acyclovir in Acute Stage of Bell's Palsy)

  • Background : Bell's palsy(BP) is defined as an idiopathic peripheral facial paralysis of sudden onset and account more than 50% of facial paralysis. It's etiology is unclear, but herpes simplex virus type-1(HSV-1) has been the most suspicious causative agent of BP that ever been studied. We evaluated the effect of add-on acyclovir in acute stage of BP. Methods : Subject consisted of 35 patients who developed acute idiopathic unilateral facial nerve palsy(16 men and 19 women with age 9-78 years old). The treatments were started within 10 days after onset of BP. Facial nerve function was assessed by the House-Brackman facial nerve grading scale and facial nerve conduction study including blink reflex. Follow-up evaluation were made 2 month after onset. Twenty of 35 patients were treated with combined therapy of acyclovir and prednisone. As a control group, 15 patients were treated with prednisone only. We compared the improvement of neurologic defects at recovery phase. Results : Compared with two groups, difference in grading scale at recovery phase is statistically significant(p<0.01). So, acyclovir-prednisone group showed a significant improvement in grading scale at recovery phase compared with prednisone group. Conclusion : We identified the benefits of add-on acyclovir in the acute stage of BP.

안면신경마비 후유증 정안침 증례보고 (Case Study of a Patient with Sequelae of Facial Palsy)

  • This study examines a clinical progress of treatment for the sequelae of facial palsy through Jung-ahn acupuncture. The patient in this case was diagnosed with facial paralysis a few years ago. The patient was treated with Korean medicine and Western medicine, but was given up without improvement. The paretic symptom was found out in left side of the face. Also facial spasm and epiphora caused by blepharoptosis were existed. The patient got 8 times Jung-ahn acupuncture treatment from September 18th, 2014 to September 26th, 2014. House-Brackmann facial nerve grading system(H-B scale) was implemented. On the first time of the treatment, H-B scale was Grade Ⅴ and facial nerve grading was 2/8. Facial spasm and epiphora caused by blepharoptosis in lower eyelid were appeared on facial expressions and conversation. After total 8 treatments(therapies), H-B scale was Grade Ⅲ and facial nerve grading was 5/8. The symptoms of facial paralysis and blepharoptosis were improved. Jung-ahn acupuncture is estimated to be effective in facial palsy sequela. More cases are required to develop treatment of facial palsy sequela.

구안와사 입원환자 25명에 대한 황련해독탕 약침을 병행한 한방치료 증례 보고 (25 Cases of Facial Paralysis Inpatients by Korean Medical Treatment with Hwangryunhaedok-tang Pharmacopuncture)

  • Objectives : The purpose of this report is to show the effectiveness of traditional korean medical treatment with Hwangryunhaedok-tang pharmacopuncture. Methods : Twenty five patients were treated by korean medical treatment including Hwangryunhaedok-tang pharmacopuncture. We measured valuation standards such as House-Brackmann Grade(H-B Grade) and Numeric Rating Scale(NRS) to assess the effectiveness of treatments. These valuation standards were measured at admisson and discharge. Results : The results were obtained as below. 1. In House-Brackmann Grade, 60.0% of patients were improved and 36.0% were maintained at the same level. 4.0% of patients were worsen. 2. Average of Numeric Rating Scale was changed from 8.5 to 3.0 after whole treatment. Conclusions : Korean medical treatments including Hwangryunhaedok-tang pharmacopunture can be effective for improving symptoms of facial paralysis.

안면신경마비의 발생과 사회경제적인 요인과의 관계 (A Study on the Relationship of Incidence of Facial Palsy with Socioeconomic Factors)

  • Objectives : Bell's palsy is the most common acute facial paralysis but its causes still unclear. At present, one of the most widely accepted cause is viral infections, and generally socioeconomic factors influence the viral infections. The purpose of this study is to investigate the relationship of incidence of facial palsy with socioeconomic factors. Methods : Seventeen patients with a acute facial palsy, who volunteered and completed all of the assessment measures participated. Using data on 17 participants, we examined the independent contribution of education, income, and occupation to a risk factor of severe facial palsy. Severity of global facial impairment was assessed by the facial disability index (FDI), the house-brackmann facial nerve grading system, WHO quality of life - bref (WHOQOL-BREF) and visual analogue scale (VAS) about discomfort of life. Results : There was no correlation between severity of facial palsy and gender, marriage, education, or occupation. Age greater than 60 years (p<0.05), and low monthly income(p<0.05), poor self-rated health was associated with greater severity of idiopathic facial paralysis. Conclusions : The number of subjects with facial palsy in our study (n-17) was small, and therefore generalization to larger patient populations might be unwarranted. But according to the outcome, we suggest that socioeconomic factors, especially low monthly income influence severity of bell's palsy.

Oral findings and dental management of a patient with Moebius syndrome: a case report

  • Moebius syndrome is a rare, congenital neurological disease involving facial paralysis and limitation of eye movements. It results from maldevelopment of the sixth and seventh cranial nerves. Dental features of this syndrome include micrognathia, microstomia, tongue deformity, cleft palate, hypoplasia of the teeth, and congenital missing teeth. A 7-year-old female with Moebius syndrome was referred from a local dental clinic for caries treatment. She presented with facial paralysis and microstomia. Oral findings included multiple caries with enamel hypoplasia, congenital missing teeth, and tongue deformity. Dental treatments including restorative and preventive procedures were performed. Oral findings and management aspects of Moebius syndrome for this case are discussed. Early evaluation and multidisciplinary care are needed for children with Moebius syndrome.

이성대상포진의 치험1례 (A Case Report of Herpes Zoster Oticus)

  • Herpes zoster oticus which is called Ramsay Hunt syndrome, geniculate ganglion herpes and otic herpes, is characterized by a viral prodrome with otalgia, vesicular eruptions, facial paralysis, hearing disturbance, tinnitus, vertigo and other symtoms. Among cranial nerves, trigeminal and facial nerves are the most commonly affected in patients with herpes zoster oticus, but on rare occasions 8th, 9th, 10th, 11th, 12th cranial nerves and even the upper cervical nerve are influenced as well.. In general, it has poor prognosis compared with Bell's palsy, leaving permanent facial nerve dysfunction. We report a case of Herpes zoster oticus, which was fully recovered by acupuncture and herbal medication within 12 days. We used $FEMA^{11)}$ and House-Brackmann's grading system to assess the degree of paralysis in each part of face.
