Deep Brain Stimulation of the Globus Pallidus in a 7-Year-Old Girl with DYT1 Generalized Dystonia

  • The experience of pediatric deep brain stimulation (DBS) of the globus pallidus internus (GPi) in the treatment of early-onset DYT1 generalized dystonia is still limited. Here, we report the surgical experience of bilateral GPi-DBS under general anesthesia by using microelectrode recording in a 7-year-old girl with early-onset DYT1 generalized dystonia. Excellent improvement of her dystonia without neurological complications was achieved. This case report demonstrates that GPi-DBS is an effective and safe method for the treatment of medically refractory early-onset DYT1 generalized dystonia in children.

Management of traumatic neuralgia in a patient with the extracted teeth and alveoloplasty: a case report

  • A majority of patients who sustain injuries to the peripheral sensory nerves of the face and jaws experience a slow but gradual return of sensation that is functional and tolerable, if not the same as before the injuries. However, long-term effects of such injuries are aggravating for many patients, and a few patients experience significant suffering. In some of these patients, posttraumatic symptoms become pathological and are painful. The predominant painful components are (1) numbing anesthesia dolorosa pain, (2) triggered neuralgiaform pain, (3) burning and aching causalgiaform pain, and (4) phantom pain. This is a case report of conservative management of traumatic neuralgia and neuritis as part of posttraumatic pain syndromes in geriatric patients who have undergone the teeth extraction and alveoloplasty.

  • Purpose: Cases of male external genitalia lymphedema are relatively uncommon in the clinical physical therapy field. Of more than two thousand clinical cases treated by the author in the lymphedema treatment center, only 4~5 cases of male external genitalia lymphedema were encountered. The present case report presents the experience and treatment results of male external genitalia lymphedema from urological cancer. Method: This case was a 47-year-old man, who was treated with complex decongestive physical therapy consisting of manual lymph drainage, compression bandage (including the external genitalia), remedial exercise, skin care. Result: The results were relatively good. The clinical experience would suggest that early treatment and education can reduce the external genitalia lymphedema. However, the treatment efficacy remains limited. Therefore, early treatment is vital for more successful outcomes. Conclusion: The relief of symptoms and a decrease in male external genitalia lymphedema may be the best result from long-term conservative management.

  • Construction projects have shown a dramatic increase in size and complexity during the recent years that precise management has been recognized as one of key factors leading the project to success. It has been investigated that management in construction sites is generally done based on the experience of site engineers. Such experience based decision making lacks reliability that could bring disastrous results such as delay and cost overruns. The purpose of this study is to suggest a process progress pattern analysis method that is expected to be used as an objective reference for site managers to aid the decision making done regarding process progress management.

  • Spinal muscular atrophies are inherited neurodegenerative disorders affecting anterior hem cells. There are various problems, especially weakness of respiratory muscle and abnormal reaction to muscle relaxants during the general anesthesia. And gingival hyperplasia can make the proper airway management difficult. Experience with anesthetic management in a patient with spinal muscular atrophy combined with gingival hyperplasia has been very rare. We report the anesthetic experience of a wheel-chair-bound child, who underwent gingivectomy under general anesthesia. The child was safely managed with fibroscopic nasotracheal intubation under sevoflurane without muscle relaxants. Also, there was no deterioration of her underlying neurologic conditions.

A Study on the Influence of Mobile Music Application Users' Social Experience on Purchasing Intention - The Mediating Effect of Brand Attachment

  • With the gradual in-depth use of social elements, music social networking has attracted more and more attention and discussion in the industry. Therefore, starting from the user experience theory and purchase intention theory, this paper explores the mechanism of user social experience and purchase intention in mobile music applications, and adds the mediating role of brand attachment on the basis of literature research. Based on previous studies on user experience measurement of social network and evaluation report of mobile music applications in the industry, and combined with in-depth interviews of users, whether the user experience of mobile music applications, which pays more and more attention to social elements, has an impact on purchase intention or not? What specific social experience can help form purchase intention? Are mediating variables at work? In practice, a total of 398 formal questionnaires were collected to obtain first-hand data. Later, reliability analysis, factor analysis and structural equation model test were carried out successively to verify the hypothesis. According to the results of empirical research, firstly, content experience, atmosphere experience and interactive experience in users' social experience of mobile music applications have a significant impact on brand attachment, secondly, atmosphere experience has a significant impact on purchase intention, and thirdly, brand attachment has a significant impact on users' purchase intention. Fourthly, brand attachment plays an intermediary role in the relationship between users' atmosphere experience and content experience and purchase intention. Based on the above research conclusions, the author puts forward corresponding marketing strategy Suggestions, which have certain guiding significance for mobile music applications to improve users' purchase intention, and have certain reference significance for the development of mobile music applications and the expansion of business model.

Carpal Tunnel Syndrome by Ganglion: A Case Report (수근관에 발생한 결절종으로 인한 수근관 증후군의 치험1례)

  • Purpose: Carpal tunnel syndrome is the most common peripheral compressive neuropathy. Most cases are idiopathic, but rarely carpal tunnel syndrome can be associated with a ganglionic mass. We report our recently encountered experience of surgical treatment of carpal tunnel syndrome caused by a simple ganglionic mass. Methods: A 53-year-old man presented with chief complaints of numbness and hypoesthesia of his left palm, thumb, index finger, long finger, and ring finger of one and half month duration. Physical examination revealed positive Tinnel's sign without previous trauma, infection or any other events. Electromyography showed entrapment neuropathy of the median nerve. Magnetic resonance imaging (MRI) showed an approximately 2.0 cm-sized mass below the transverse carpal ligament. Upon surgical excision, a $1{\times}1.5cm^2$ mass attached to the perineurium of the median nerve and synovial sheath of the flexor digitorum superficialis and redness and hypertrophy of the median nerve were discovered. With surgical intervention, we completely removed the ganglionic mass and performed surgical release of the transverse carpal ligament. Results: The pathology report confirmed the mass to be a ganglion. The patient exhibited post-operative improvement of his symptoms and did not show any complications. Conclusion: We present a review of our experience with this rare case of carpal tunnel syndrome caused by a ganglionic mass and give a detailed follow-up on the patient treated by surgical exploration with carpal tunnel release.

  • This paper discusses about an empirical study to design a suitable experience monitoring and sampling system for mobile phone applications. We designed a new experience sampling method (ESM) research system iteratively. The research system is characterized by context-aware experience sampling, real-time data management, supporting data analyzing tools, and integrating ESM with ex-situ diary. From literature review, we derived initial design of the ESM research system. With the first system, we held the first experiment that evaluates three applications. From the experiment, we found that more functions are required, thus we improved the ESM system. Finally, we propose an ESM system design goal for the third iteration with experience of two experiments.

Nursing Students' Lived Experience of Blind Person (간호학생들의 시각장애인 봉사체험 연구)

  • This is a Phenomenological study to describe the experience of nursing students on blind persons. Report and individual interviews were conducted on subjective experience of 5 nursing students. Data were analyzed through Giorgi's method in which meaningful statements. The results were from significant statements on reports and interview. These were clustered into 17 theme and 4 central meaning. Nursing Students started practice with prejudice, fear, and expectation at the same time. They also experienced complicated feelings (expansion of consciousness, growth of self, expansion of emotion, true presence)for the blind person. Make a situational structural description from theme and central meaning. Make a general structural description from situational structural description.

Clinical Experience with Intraaortic Balloon pump - Report of 31 cases - (IABP 치험 [31례 보고)

  • From December 1981 to June 1987, thirty one patients suffering from low output syndrome after cardiac operation received cardiac assist with intraaortic balloon pump. Fifteen patients survived [survival rate 48.4%]. After receiving assist with IABP, urine output increased, heart rate and central venous pressure decreased, reflecting improved myocardial performance. Poor preoperative hemodynamic and functional status resulted in poor survival despite of assist with IABP. Poor pre-IABP hemodynamic status also resulted in high mortality after assist with IABP. Our experience suggests that IABP should be introduced early in the course of worsening to get good result.
