A Study of Factors Affecting Measurement of Kidney Size in Ultrasonography (초음파로 신장의 크기 측정 시 미치는 영향에 관한 연구)

  • Since measuring the size of kidney with sonography becomes an important index for diagnosis, treatment, and prognostic prediction in kidney disease, the accurate measurement and evaluation on this are clinically very important. Accordingly, the purpose of this study was to increase reproducibility and objectivity in measuring the size of kidney by enumerating factors that have an impact for measurement. It targeted 44 adults in Korea at the age of 21-27. It measured in order for both kidneys to be seen most largely while changing a subject-examiner's position in a state of fasting for 8 hours and a transducer's approaching direction. It compared a size of kidney by measuring, respectively, with the same method in 30 minutes and in 1 hour after drinking water in 700-1,000cc. In case of the lateral approach scan in decubitus position, the average length of the kidney both to the right and the left and the deviation of measurement to be the largest. In NPO(None Per Oral) state, the average length in the right kidney was 10.19cm, and the average length in the left kidney was 10.33cm. In 60 minutes after taking moisture, the average length in the right kidney was 10.94cm, and the average length in the left kidney was 11.13cm. In comparing the average length of the kidney in NPO state and its average length in 60 minutes after taking moisture, the size swelled by 7.3% for the length in the right kidney and by 7.7% in the left, thereby having been indicated to be statistically significant(P<0.003). The measurement in a size of kidney by using ultrasound may be measured differently depending on a patient's state of taking moisture and a transducer's approaching direction. It is thought that when the measurement in a size of kidney is especially important clinically, the intake and intake time in moisture need to be considered and that measuring with the posterior approach in prone position is a good method aiming to increase reproducibility in measuring length of the kidney.

Evaluation of Beam Modeling Using Collapsed Cone Convolution Algorithm for Dose Calculation in Radiation Treatment Planning System (방사선치료계획시스템의 Collapsed Cone Convolution 선량계산 알고리듬을 이용한 빔 모델링의 정확성 평가)

  • This study aims to evaluate the accuracy of the collapsed cone convolution (CCC) algorithm for dose calculation in a treatment planning system (TPS), CorePLAN$^{TM}$. We implemented beam models for various setup conditions in TPS and calculated radiation dose using CCC algorithm for 6 MV and 15 MV photon beam in $50{\times}50{\times}50cm^3$ water phantom. Field sizes were $4{\times}4cm^2$, $6{\times}6cm^2$, $10{\times}10cm^2$, $20{\times}20cm^2$, $30{\times}30cm^2$ and $40{\times}40cm^2$ and each case was classified as open beam cases and wedged beam cases, respectively. Generated beam models were evaluated by comparing calculated data and measured data of percent depth dose (PDD) and lateral profile. As a result, PDD showed good agreement within approximately 2% in open beam cases and 3% in wedged beam cases except for build-up region and lateral profile also correspond within approximately 1% in field and 4% in penumbra region. On the other hand, the discrepancies were found approximately 4% in wedged beam cases. This study has demonstrated the accuracy of beam model-based CCC algorithm in CorePLAN$^{TM}$ and the most of results from this study were acceptable according to international standards. Although, the area with large dose difference shown in this study was not significant region in clinical field, the result of our study would open the possibility to apply CorePLAN$^{TM}$ into clinical field.

Analysis on the Decrease of Planning Target Volume in the Case of Lung Radiation Surgery with the Application of Respiratory Gated Radiotherpy Method (폐암 환자의 방사선수술 시 호흡연동 방사선치료 방법의 적용을 통한 계획용표적체적 감소 효과 분석)

  • The application of a respiratory gated radiotherpy method to the lung radiation surgery was evaluated compared with the conventional method in which the whole tumor motion range is considered in the delineation of PTV (Planning target volume). The four dimensional CT simulation images were acquired for the five NSCLC (Non-small cell lung cancer) patients for radiation surgery. The respiratory gated plan was prepared with the 50% phase CT images and the conventional method was planned based on the ITV (Internal target volume) which include all the target volumes created in each phase CT images within a whole respiratory period. The DVH (Dose volume histogram) of OAR (Organ at risk) which calculated in each method was compared for the evaluation of the plan properness. The relative decrease of OARs' DVH were verified in the application of respiratory gated method. The average decrease rate were $16.88{\pm}9.97%$ in the bronchus, $34.13{\pm}19.15%$ in the spinal cord, $28.42{\pm}18.49%$ in the chest wall and $32.48{\pm}16.66%$ in the lung. Based on these results, we can verified the applicability and the effectiveness of the respiratory gated method in the lung radiation surgery.

Effect of mixed Cropping with Legume and using fermented Cattle Manure on Productivity of Whole Crop Barley (Hordeum vulgare L.) and Organic Hanwoo feeding Capacity in Gyeongbuk Region of Korea (경북지역에서 콩과 사료작물의 혼파 재배와 발효 우분의 시용이 청보리의 생산성과 유기 한우 사육능력에 미치는 영향)

  • This study was aimed to grow organic forage as a prime requirement for organic Hanwoo cattle production over a period of 3 years (2012~2015) in Gyeongbuk region, Korea through mixed cropping of winter fodder barley with legume and application of fermented cattle manure. The effect of barley-legume mixed cropping and application of different levels of fermented cattle manure was studied on forage productivity of whole-crop barley (WCB) and evaluation of feeding capacity of Korean native cattle (Hanwoo) per unit area. The collected data was analyzed using SAS 9.3 software. Barley-legume mixed cropping increased (P<0.05) yields of dry matter (DM), crude protein (CP) and total digestible nutrients (TDN) as compared to those of sole WCB as monoculture. Similarly, acid detergent fiber (ADF) contents and relative feed value (RFV) were also higher (P<0.05) in barley-legume mixed sowing forage compared with those of sole WCB as monoculture. In case of manure application, yields of DM, CP and TDN were also found (P<0.05) highest in 100~150 kg N/ha. However, varying cattle manure levels did not influence (P>0.05) CP and neutral detergent fiber (NDF) contents. In response to increasing nitrogen application level, ADF content increased (P<0.05) but TDN content and RFV decreased (P<0.05). The Barley-legume mixed sowing and increasing application levels of fermented cattle manure also significantly enhanced (P<0.05) feeding capacity of organic Hanwoo (head/ha) having 450 kg body weight with 400 g of daily gain. Findings of present study indicated that barley-legume biculture not only cut nitrogen application level of fermented cattle manure by over 50 kg per ha but also increased their feed value and productivity of forage as compared to WCB monoculture. This would be contribution to the diversity of forage resources as well as production of organic animal product by creating low cost and high quality organic forage.

Clinical Evaluation of Instrumental Esophageal Perforation (기구에 의한 식도천공에 대한 임상적 고찰)

  • Background: Esophageal perforation is an uncommon problem, but it is associated with high mortality. We performed a retrospective review of patients with instrumental esophageal perforation to assess the outcome of current management techniques. Material and Method: We retrospectively analyzed all cases of instrumental esophageal perforation diagnosed at our hospital from January 1999 through to March 2005. The study group consisted of 12 patients (8 women and 4 men) with a mean age of 48.8 years (range, $21{\sim}83$ years). We reviewed the effects of the surgical or medical treatments in various conditions of patients, such as of various sites of perforation and time delayed after injury. Result: Perforations were due to diagnostic endoscopy (50.0%, 6/12), esophageal bougination for benign stricture (33.3%, 4/12), endoscopic port insertion (8.3%, 1/12), and tracheal intubation (8.3%, 1/12). The perforated sites were thoracic in 7 patients and cervical in 5. The treatment included resection and reconstruction (5 cases), incision and drainage (4 cases), medical treatment (2 cases), and closed thoracostomy drainage only (1 case). Post-operative complications of transient pneumonia and wound infection were developed in 1 patient respectively. Both occurred in two patients with diffuse mediastinal abscess formation. The overall mortality was 8.3% (1/12) in one old patient who was managed medically for cervical esophageal perforation. Conclusion: We concluded that surgical treatment for esophageal perforations was safe and effective whether diagnosed early or lately.

Effects of Preoperative Combined Antiplatelet Agents on the Postoperative Bleeding that Occurs during Off-pump Coronary Artery Bypass Surgery (무심폐기하 관상동맥우회수술에서 아스피린과 Clopidogrel의 수술 전 병합투여가 수술 후 출혈에 미치는 영향)

  • Background: It is well known that preoperative administration of combined antiplatelet agents can have an impact on the postoperative bleeding, the requirement for transfusion and the need for reexploration during on-pump coronary artery bypass surgery. Yet its effects have not been well evaluated in the case of off-pump coronary artery bypass surgery. Material and Method: We performed a retrospective study of nineteen patients who underwent OPCAB from March 2003 to December 2004. All the patients had taken antiplatelet agents until 12 hours before operation. The patients were divided into bo groups as an aspirin group and a combined (aspirin+clopidogrel) group. The perioperative platelet count, the hemoglobin level, the hematocrit, the prothrombin time and the aPTT were compared between both groups. The amount of postoperative bleeding, the transfusion requirement and the need for re-exploration to control bleeding were also compared between both groups. Result: There was no difference of operation time and the intraoperative ACT between the aspirin group and the combined group. The amount of blood loss through the chest tube for 24 hours was not different between the aspirin group $(697{\pm}271mL)$ and the combined group $(944{\pm}432mL)$. The number of patients who received blood transfusion was also not different between both groups. There was no patient who required reexploration for bleeding control in both groups. The perioperative hemoglobin level and hematocrit were also not different between both groups, but the postoperative hemoglobin level and hematocrit were decreased significantly in the group. Conclusion: The Preoperative combined antiplatelet (aspirin+clopidogrel) therapy group was not different from the aspirin group for the amount of postoperative bleeding, the amount of blood transfusion and the need for reexploration during off-pump coronary artery bypass grafting. This subject needs further evaluation because of small population in our study.

Evaluation of Retro recon for SRS planning correction according to the error of recognize to coordinate (SRS의 좌표 인식 오류 시 Retro recon을 이용한 수정 방법에 관한 평가)

  • Purpose : The purpose of this study was to evaluate the Retro recon in SRS planning using BranLAB when stereotactic location error occurs by metal artifact. Materials and Methods : By CT simulator, image were acquired from head phantom(CIRS, PTW, USA). To observe stereotactic location recognizing and beam hardening, CT image were approved by SRS planning system(BrainLAB, Feldkirchen, Germany). In addition, we compared acquisition image(1.25mm slice thickness) and Retro recon image(using for 2.5 mm, 5mm slice thickness). To evaluate these three images quality, the test were performed by AAPM phantom study. In patient, it was verified stereotactic location error. Results : All the location recognizing error did not occur in scanned image of phantom. AAPM phantom scan images all showed the same trend. Contrast resolution and Spatial resolution are under 6.4 mm, 1.0 mm. In case of noise and uniformity, under 11, 5 of HU were measured. In patient, the stereotactic location error was not occurred at reconstructive image. Conclusion : For BrainLAB planning, using Retro recon were corrected stereotactic error at beam hardening. Retro recon may be the preferred modality for radiation treatment planning and approving image quality.

  • This study analyzed the curriculum and clinical practicum of the department of clinical laboratory science in Korea. The 2017 educational curricula of all 3-year and 4-year universities were analyzed. Clinical practice was conducted by a questionnaire. As a result of curriculum analysis, the 3-year curriculum was able to grasp the curriculum that focused on the national examinations of medical technologists, and the 4-year curriculum was open to a variety of subjects reflecting the changes in the future, but only at a few universities. In addition, the autonomous enrollment application made it possible to take a national examination without enrolling in courses that students find difficult in major courses. In the case of clinical practice, it was difficult to standardize in various practical institutes, practice periods, and practice credits. Therefore, it will be necessary to standardize the composition of the credits according to the duration of clinical practice and the duration of education. Moreover, is necessary to revise the curriculum in consideration of medical technologist job analysis and clinical field, and it will be necessary to standardize the curriculum and clinical practice model through the Medical Technologist Evaluation Center.

Evaluation of Solidified Fuel Value of Dairy Cattle Manure Digested by Semi-Dry Anaerobic Digestion Method (젖소분뇨 반 건식 혐기소화 잔재물의 고체연료화 가능성 평가)

  • The objective of this study was to investigate feasibility of semi-dry anaerobic digestion using dairy cattle manure and to evaluate solidified fuel value of semi-dry anaerobic digestate. To evaluate semi-dry anaerobic digestion using dairy cattle manure, 950 mL bottle type anaerobic reactor was set in the constant temperature room maintained at $35^{\circ}C$. To produce anaerobic digestate for making solidified fuel, acrylic cylindrical anaerobic digester(1,000 mm width ${\times}$ 450 mm height) was set in the constant room temperature to carry out batch test of semi-dry anaerobic digestion using same dairy cattle manure. Moisture content of dairy cattle manure and inoculum solution for anaerobic digestion were 80.64% and 96.83%, respectively. The dairy cattle manure and the inoculum solution was mixed by 1:1 ratio(v/v) for anaerobic digestion. Water content and VS/TS(Volatile Solids/Total Solids) of mixture of substrate and inoculum were 89.74% and 83.35%, respectively. In case of non-inoculated anaerobic digester, the biogas was not produced. By the semi-dry anaerobic digestion, the calorific value of the digestate was reduced by 20% compare to fresh dairy cattle manure. In other hand, ash content increased from 15% to 18.4%. The contents of Cr, Pb, Cd and S of pellet produced from anaerobically digested dairy cattle manure were not against the standard regulation for livestock manure solidified fuel. Therefore, it can be used as fuel that anaerobic digestate produced after semi-dry anaerobic digestion using dairy cattle manure.

In vitro Fruit Assay for the Evaluation of Fungicide Activity Against Pepper Anthracnose (살균제 효과 검정을 위한 고추 탄저병의 실내 열매 검정법)

  • This study was performed to investigate the effect of conidial density, wetness period and temperature on conidial germination, appressoria formation and disease incidence. While there was not significantly correlated between conidial density and temperature, and conidial germination and appressoria formation, there was a significant correlation between those factors and disease incidence. The longer wetness period was, the higher the ratio of conidial germination, appressoria formation and the disease incidence was. The optimum conidial density, temperature and wetness period was $1{\times}10^6$ conidia $mL^{-1}$, $30^{\circ}C$ and 5 days, respectively. In case the wetness period was more than 5 days, the typical symptom was not found on pepper fruits because of the overgrowth of mycelia. Using this fruit assay method, which the pepper anthracnose pathogens were inoculated by spraying spore suspension on non-wounded or wounded pepper fruits, control effect of three fungicides were evaluated against pepper anthracnose by the protective and/or the curative application. Propineb showed high protective control activity, while it showed curative control activity on unwounded fruits, but did not showed curative control activity on wounded fruits. Tebuconazole, one of curative fungicide, showed higher control activity in non-wound inoculation than wound inoculation. Trifloxystrobin, one of strobilurin group, showed high both protective and control activity against anthracnose. In conclusion, we supposed that the newly developed in vitro pepper fruit assay can be used to evaluate antifungal activity of control agents against pepper anthracnose.
