Levels of Tobacco-specific Metabolites among Non-smoking Lung Cancer Cases at Diagnosis: Case-control Findings

  • Background: Environmental tobacco smoking (ETS) significantly contributes to morbidity and mortality and is a known risk factor for lung cancer development in lifelong nonsmokers. The metabolite 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanol (NNAL) and its glucuronides (NNAL-Glucs) have now emerged as leading biomarkers for the study of carcinogen exposure in non-smokers exposed to ETS. Materials and Methods: We carried out our study on NNAL in the urine of non-smokers exposed to ETS and the association between ETS and lung cancer. Subjects were enrolled from 2008-2010. NNAL was analyzed for 74 non-smoking lung cancer and 85 healthy controls. The main objective of this study was to provide an estimate of the risk of lung cancer from exposure to ETS in the Korean population. Results: The mean NNAL concentration in urine was significantly lower in non-smoking patient groups (n=74) than in control groups (n=85) ($4.7{\pm}15.0$ pg/mg, $6.5{\pm}17.9$ pg/mg, respectively, Mann-Whitney U test, p<0.001). Conclusions: The urine NNAL of non-smoking patients with lung cancer was not elevated with regard to the non-smoking control group. This may be due to life-style changes after diagnosis. A prospective study will be needed to evaluate the association of NNAL and non-smoking lung cancer.

  • Viet Nam is a country with the highest rate of adult male smokers in the world. In 2010, the percentage of adult male smokers was 47.4%. Each year in Viet Nam, there are 40,000 lives lost from tobacco-related diseases. The Global Tobacco Youth Survey is an international standardized survey that has been conducted in more than 140 countries. To provide evidence for tobacco control, Viet Nam already conducted two rounds of GYTS in 2003 and 2007. The GYTS in 2014 is the survey's third iteration. This paper aims to document key methodological details and socio-demographic characteristics of the respondents included in the 2014 GYTS in Viet Nam. This survey followed international protocol and was conducted in 13 provinces representative of 6 geographical regions. A total of 3,430 school children, aged 13 to 15 years used a standardized answer sheet to answer 76 questions about seven tobacco-related topics, including prevalence of tobacco use, environmental tobacco smoke, access and availability, media and advertisement, cessation, knowledge and attitudes, tobacco-related school curriculum. This GYTS provides valid and reliable data for monitoring tobacco use among youth in Vietnam and is recommended to be regularly repeated.

Measurement of Environmental Tobacco Smoke in the Air of Offices in Urban Areas - Focusing on the Impact of Smoking on the Concentrations of Suspended Particles - (도시지역 사무실내 공기 중 환경담배연기의 측정 - 흡연이 부유먼지 농도에 미치는 영향을 중심으로 -)

  • This study was carried out to evaluate non-smoker's exposure levels to environmental tobacco smoke (ETS) in the air of offices in urban areas. A total of 65 offices were selected from two large cities, i.e. Daegu and Daejeon. The field sampling was conducted repeatedly in summer (1999) and winter (1999~2000). The measured ETS markers included respirable suspended particles (RSP as PM$_{40}$ ), vapor and particulate phase ETS markers, including nicotine, 3-ethnyl pyridine (3-EP), ultraviolet absorbing particulate matter (UVPM), fluorescing particulate matter (FPM), and solanseol in ETS particles (SolPM). RSP was measured gravimetrically by a microbalance. The particle samples were then used for the determination of particulate ETS markers by HPLC, while vapor phase markers determined by GC/NPD. The analytical methods were validated for repeatability, linearity, detection limits, and duplication precision. The concentrations of RSP and other ETS markers were significantly higher in smoking offices than non-smoking offices. Despite the similar smoking strength in each office for different seasons, the concentration levels of ETS components appeared to be higher in winter than summer. The contributions of ETS to RSP concentrations based on SolPM, FPM, and UVPM methods were estimated to be in the range of 15.2 ~ 25.3% in smoking offices, whereas 2.4 ~ 15.9% in non-smoking offices. The cooling and heating types did not affect significantly the concentrations of RSP and other ETS markers. Finally, further research issues were suggested to obtain more scientific information on the non-smoker's exposure to ETS with respect to the frame of risk assessment..

Factors Affecting Re-smoking in Male Workers (남성 근로자의 재흡연에 관련된 요인)

  • Objectives: This study was performed to examine the factors affecting re-smoking in male workers. Methods: A self-administrated questionnaire survey was conducted during April 2003 to examine the smoking state of 1,154 employees of a company that launched a smoking cessation campaign in1998. Five hundred and eighty seven persons, who had stopped smoking for at least one week, were selected as the final study subjects. This study collected data on smoking cessation success or failure for 6 months, and looked at the factors having an effect on re-smoking within this period. This study employed the Health Belief Model as its theoretical basis. Results: The re-smoking rate of the 587 study subjects who had stopped smoking for at least one week was 44.8% within the 6 month period. In a simple analysis, the re-smoking rates were higher in workers with a low age, on day and night shifts, blue collar, of a low rank, where this was their second attempt at smoking cessation and for those with a shorter job duration (p<0.05). Of the cues to action variables in the Heath Belief Model, re-smoking was significantly related with the perceived susceptibility factor, economic advantages of smoking cessation among the perceived benefits factor, the degree of cessation trial's barrier of the perceived barriers factor, smoking symptom experience, recognition of the degree of harmfulness of environmental tobacco smoke and the existence of chronic disease due to smoking (p<0.05). In the multiple logistic regression analysis for re-smoking, the significant variables were age, perceived susceptibility for disease, economic advantages due to smoking cessation, the perceived barrier for smoking cessation, recognition on the degree of harmfulness of environmental tobacco smoke, the existence of chronic disease due to smoking and the number of attempts at smoking cessation (p<0.05). Conclusion: From the result of this study, for an effective smoking ban policy within the work place, health education that improves the knowledge of the adverse health effects of smoking and the harmfulness of environmental tobacco smoke will be required, as well as counter plans to reduce the barriers for smoking cessation.

Urinary cotinine concentration by passive smoking in the PC game room (PC방에서의 간접흡연에 따른 요중 코티닌의 농도)

  • Tobacco smoke was confirmed as a human carcinogen by many research results. Because many adolescents stay long time in the PC game room, they are exposed to much of tobacco smoke. To evaluate the effect of passive smoking in the PC game room, airborne nicotine concentrations in 2 PC game rooms in Sung-nam city and urinary cotinine concentrations were measured for 20 adolescents. And the subjects were interviewed for duration and time in PC game room and smoking pattern. Subjects are composed of each of 10 smokers(5 males and 5 females) and 10 nonsmokers(5 males and 5 females). They stayed for three hours in the PC game room without smoking. Concentrations of nicotine in smokers and nonsmokers were 129.72 $\mu$g/$^3$ and 99.99 $\mu$g/m$^3$, respectively. Urinary cotinine concentrations were increased as time goes on after exposure to nicotine and showed maximum value at 9.45 hours after nicotine exposure and were 32.21 and 110.66 $\mu$g/L for nonsmoker and smokers. The more using time and frequency in PC game room, the higher urinary cotinine maximum concentration and the longer using duration, also the more increase urinary cotinine concentration. Urinary cotinine has a tendency to increase by passive smoking. Therefore, it is recommended that the effective control for indoor air quality and extensive research be needed to reduce nicotine concentration by passive smoking in the PC game room.

Genetic Polymorphism of Glutathione S-transferases M1 and T1, Tobacco Habits and Risk of Stomach Cancer in Mizoram, India

  • Aim: The incidence of stomach cancer in Mizoram is highest in India. We have conducted a population based matched case-control study to identify environmental and genetic risk factors in this geographical area. Methods: A total of 102 histologically confirmed stomach cancer cases and 204 matched healthy population controls were recruited. GSTM1 and GSTT1 genotypes were determined by PCR and H. pylori infections were determined by ELISA. Results: Tobacco-smoking was found to be an important risk factor for high incidence of stomach cancer in Mizoram. Meiziol (local cigarette) smoking was a more important risk factor than other tobacco related habits. Cigarette, tuibur (tobacco smoke infused water) and betel nut consumption synergistically increased the risk of stomach cancer. Polymorphisms of GSTM1 and GSTT1 genes were not found to be directly associated with stomach cancer in Mizoram. However, they appeared to be effect modifiers. Persons habituated with tobacco smoking and/or tuibur habit had increased risk of stomach cancer if they carried the GSTM1 null genotype and GSTT1 non-null genotype. Conclusion: Tobacco smoking, especially meiziol is the important risk factor for stomach cancer in Mizoram. GSTM1 and GSTT1 genes modify the effect of tobacco habits. This study is a first step in understanding the epidemiology of stomach cancer in Mizoram, India.

A Study on Characteristics Related to the Current Use of Heated Tobacco Products among Adolescents (청소년의 궐련형 전자담배 현재 사용에 따른 관련 요인 분석)

  • Background: The use of heated tobacco products (HTP) causes many adverse health effects. Although the use of HTP by adolescents is prevalent worldwide, research related to it is very limited. There is a considerable lack of research related to the current HTP use rather than ever-HTP use. In particular, research related to predictive factors for current HTP use in adolescents is scarce. Objectives: The purpose of this study was to analyze related characteristics according to the current use of HTP among South Korean adolescents. Methods: This was a cross-sectional study that used data from the Seventeenth Korean Youth Risk Behavior Web-based Survey (KYRBWS). A total of 54,848 students in 2021 were included in this study. Chi-square-test, multiple logistic regression analysis, and chi-square test for trend were used for analyzing related characteristics according to use of HTP. Results: Overall, 715 (1.3%) students responded as having used HTP during the last 30 days among the 54,848 students. It was found that residence type, subjective body type recognition, subjective health recognition, alcohol use, habitual drug experience, close friend current smoking, and conventional cigarette smoking were significantly associated characteristics with the current use of heated tobacco products. Comparing 'very thin recognition' with 'very fat recognition', the adjusted odds ratio (OR) was 1.93 (95% confidence interval [CI]: 1.29~2.87) for current use of HTP. Additionally, comparing 'very unhealthy recognition' with 'very healthy recognition', the aOR was 3.82 (95% CI: 2.40~6.07) for current use of HTP. Conclusions: Based on these results, residence type, subjective body type recognition, subjective health recognition, alcohol use, habitual drug experience, close friend current smoking, and conventional cigarette smoking were associated with significantly increased odds of current HTP use. Therefore, the results of this study can provide useful evidence about adolescent behaviors in predicting current HTP use.

Improving the smoking quality of papermaking tobacco sheet extract by using electrodialysis

  • Papermaking tobacco sheet is an important reclaimed process for cigarette making. Traditionally, the pressure driven membrane was often used to isolate the effective compounds from the tobacco sheet extract. However, this method is difficult to remove small ionic compounds. Besides, membrane fouling is a major problem for effective use of these pressure driven membrane technologies. In this study, the electrodialysis process is used to removal the chloride ions and nitrate ions, thus the smoking quality of papermaking tobacco sheet extract can get improved. Three types of electrolytes ($Na_2SO_4$, NaCl and HCl) are chosen to prevent the generation of precipitation. The results indicate that 0.1mol/L HCl at current density of $30mA/cm^2$ is the optimal condition for the electrodialysis process. The removal rates of the Cland $NO{_3}^-$ in tobacco sheet extract are 97% and 98.4%, respectively. The electrodialysis process cost was estimated to be 0.11$/L. Naturally, electrodialysis is not only technological feasible, environmental-friendly and economical-attractive for tobacco extract treatment.

Removal Efficiency of Harmful Substances in Side-stream Tobacco Smoke by the Doping Components of Commercial TiO2 Photocatalysts (시판용 TiO2 광촉매의 doping 성분에 따른 비주류 담배연기의 유해물질 제거효율)

  • Tobacco smoke emitted during smoking is divided into a main-stream and side-stream smoke. Most of the tobacco smoke that spreads to a room while smoking is a side-stream one. The side-stream tobacco smoke is two to three times more harmful than that of the main-stream tobacco smoke. In this study, the removal efficiency of CO, $H_2S$, $NH_3$ and HCHO in a side-stream tobacco smoke using the doping component of $TiO_2$ photocatalysts was confirmed. As a result, CO was removed up to 78.37%, which indicated that the $TiO_2$ photocatalytic process is effective for CO removal. Also, the removal efficiencies of CO, $H_2S$ and HCHO were greatly affected by the amount of doped O and Si components of the $TiO_2$ photocatalyst. In conclusion, the more doped O and Si components had, the higher removal efficiencies of harmful substances were achieved.

Generation and Decay Phenomena of Environmental Tobacco Smoke in Controlled Experimental Atmosphere Chamber (환경이 조절되는 Chamber 내에서 Environmental Tobacco Smoke의 생성과 감소 현상)

  • This paper describes the generation and decay phenomena of gas, vapor and particulate phase components of environmental tobacco smoke in 18 m3 controlled experimental atmosphere chamber. Real time-weighted average concentration ratios of markers were determinated at no ventilation rates and sampling durations of starting to smoking 45 min. Average concentration of major ETS markers was no significant on the mainstream smoke contents of commercial cigarette and decay ratios were dependent on first order kinetic. RSP/nicotine, solanesol and 3-EP were good predictors of ETS concentration in the public indoor field. The concentration ratio of vapor phase and particulate phase components is highly variable to assessment of indoor air quality with ETS. Key words : ETS, chamber study, ETS markers.
