The aim of this study was to prove that the hypothesis of half dose (HD) allergen-specific immunotherapy (ASIT) in the treatment of canine atopic dermatitis (CAD) would result in a similar success rate compared to the standard dose (SD) ASIT. Clinical signs were evaluated using a third version of the Canine Atopic Dermatitis Extent and Severity Index (CADESI-03) prior to ASIT (day 0), at the end of induction (day 43), and at three month afterwards (day 90). Of the 18 atopic dogs, 12 dogs (SD group: 6; HD group: 6) had a good - excellent response to the house dust mites-specific immunotherapy. The efficacies of ASIT were 66.6% in both groups. The grades of reduction rate CADESI-03 were not different between two groups. Therefore, half dose protocol of house dust mites-specific immunotherapy is an effective and efficient method to treat CAD.

Recent studies have revealed the efficacy of incorporating delayed channel state information at transmit side (CSIT) in transmission scheme design. This paper focuses on transmit precoder design to maximize the ergodic sum-rate in a two-user Multiple-Input Single-Output (MISO) system with delayed and statistical CSIT. A new transmit strategy which precodes signals in all transmit slots is proposed in this paper, denoted as all time-slots precoding Alternative MAT (AAMAT). There is a common procedure in conventional delayed-CSIT based schemes, which is retransmitting the overheard interferences. Since the retransmitting signal is intended to both users, all previous schemes tend to use only one antenna. We however figure out an improvement in spectral efficiency could be realized if all antennas can be utilized. In this paper, we detail the design of the procoder which enabling all antennas and also we compute a lower bound of the ergodic sum-rate in an ideal condition. In addition, simulation results demonstrate the superiority of our proposed scheme.

Background We report the efficacy of a dual-plane approach using a Dufourmentel skin flap with a purse-string suture of the de-epithelized dermis to manage pseudoaneurysm at the vascular access site for hemodialysis. Methods A retrospective analysis was conducted of 61 patients from 2013 to 2018 with pseudoaneurysms at the arteriovenous fistula or graft who were treated with rhomboid excision, vessel repair with a purse-string suture, and a full-thickness Dufourmentel skin flap. The success rate was defined as the probability of complete wound closure and intact vascular access patency without infection or other complications. Results The success rate was 93.4% at 6 months postoperatively. Complications included newly occurring pseudoaneurysms (n=2), wound dehiscence (n=1) and bleeding (n=1). There were no complications such as stenosis or thrombosis from the procedure. Conclusions A dual-plane approach using a Dufourmentel skin flap with a purse-string suture for vessel repair was shown to be a favorable option for managing stable, small (diameter <2 cm) pseudoaneurysms without infection, rapid expansion, or patency issues of the vascular access.
Purpose: The aim of this study was to evaluate the degree of achievement of anterior tooth movement using a thermoplastic clear aligner by superimposing pretreatment, setup, and post-treatment models and to analyze the factors that influence it. Materials and Methods: Forty patients who underwent clear aligner treatment at the Department of Orthodontics were included in the study. The pretreatment, setup, and post-treatment models were superimposed, and the achievement rates were evaluated. We analyzed the factors that caused changes in achievement rate on each condition. Result: The average achievement rate of total tooth movement was 57.65%. Mesiodistal and anteroposterior displacements were achieved better than vertical displacement and rotation. In each displacement, posterior, mesial, and extrusive movements were better achieved than the rotation. Spacing was better corrected than crowding, especially in the mesiodistal direction. Most of the tooth movements were achieved better when using 3-step thicknesses of aligners were used. Conclusion: The treatment results of clear aligners may differ depending on the type of tooth movement, age and other treatment conditions. Clinicians should consider unfavorable conditions based on the results of this study.
Purpose: Smoking is a way of coping with anxiety and stress. This study aimed to identify the effects of forest-thermal combined therapy on anxiety and depression in smokers who desire to quit smoking. Methods: Thirty participants were included in the study, 15 in the experimental group and 15 in the control group. Those in the experimental group participated in a three-day forest-thermal combined therapy program. The program includes forest walks, meditation and thermal therapy in the charcoal kiln. Results: Before and after the program, physiological indicators such as cortisol, heart rate variability, and serotonin anxiety level using the state-trait anxiety inventory (STAI), and stress level using the psychosocial well-being index (PWI) were measured in both groups. The differences in STAI (p=.012) and PWI (p=.006) scores between the experimental and control groups were statistically significant. However, cortisol, heart rate variability, and serotonin were not significantly different between the two groups after the program. Conclusion: These results show that forest-thermal combination therapy effectively reduces anxiety and stress in smokers. It suggests that forest-thermal therapy can potentially increase smoking cessation rates.
The Multi-access Edge Computing (MEC) paradigm equips network edge telecommunication infrastructure with cloud computing resources. It seeks to transform the edge into an IT services platform for hosting resource-intensive and delay-stringent services for mobile users, thereby significantly enhancing perceived service quality of experience. However, erratic user mobility impedes seamless service continuity as well as satisfying delay-stringent service requirements, especially as users roam farther away from the serving MEC resource, which deteriorates quality of experience. This work proposes a deep reinforcement learning based service mobility management approach for ensuring seamless migration of service instances along user mobility. The proposed approach focuses on the problem of selecting the optimal MEC resource to host services for high mobility users, thereby reducing service migration rejection rate and enhancing service availability. Efficacy of the proposed approach is confirmed through simulation experiments, where results show that on average, the proposed scheme reduces service delay by 8%, task computing time by 36%, and migration rejection rate by more than 90%, when comparing to a baseline scheme.
Immune checkpoint inhibition has been established as a new treatment option for various types of carcinoma, and many clinical trials are being actively conducted as a treatment for advanced or metastatic gastric cancer, either as a monotherapy with an immune checkpoint inhibitor or as a combination therapy with standard chemotherapy. In the CheckMate-649 clinical trial to confirm the efficacy of the combination of nivolumab and chemotherapy (FP) in advanced gastric cancer and gastroesophageal junction cancer, nivolumab group showed improvement in overall survival in programmed death ligand 1-positive cancer patients compared with placebo group. Also, the combination therapy of pembrolizumab, trastuzumab and chemotherapy (FP) in first-line treatment was tested through the KEYNOTE-811 trial. The pembrolizumab group showed 22.7% of improvement in objective response rate compared with placebo group. Accordingly, the combination of nivolumab/pembrolizumab with standard chemotherapy was approved for the first-line treatment. In KEYNOTE-059 trials for patients with progressive disease after at least two lines of chemotherapy, pembrolizumab monotherapy showed improvement in objective response rate and overall survival, and the use of pembrolizumab was approved for the third-line or more treatment. In this article, we review the result of clinical trials related to immune checkpoint inhibitors that have been recently introduced in the treatment of gastric cancer.
Perioperative treatment with conventional cytotoxic chemotherapy for resectable non-small cell lung cancer (NSCLC) has proven clinical benefits in terms of achieving a higher overall survival (OS) rate. With its success in the palliative treatment of NSCLC, immune checkpoint blockade (ICB) has now become an essential component of treatment, even as neoadjuvant or adjuvant therapy in patients with operable NSCLC. Both pre- and post-surgery ICB applications have proven clinical efficacy in preventing disease recurrence. In addition, neoadjuvant ICB combined with cytotoxic chemotherapy has shown a significantly higher rate of pathologic regression of viable tumors compared with cytotoxic chemotherapy alone. To confirm this, an early signal of OS benefit has been shown in a selected population, with programmed death ligand 1 expression ≥50%. Furthermore, applying ICB both pre- and post-surgery enhances its clinical benefits, as is currently under evaluation in ongoing phase III trials. Simultaneously, as the number of available perioperative treatment options increases, the variables to be considered for making treatment decisions become more complex. Thus, the role of a multidisciplinary team-based treatment approach has not been fully emphasized. This review presents up-to-date pivotal data that lead to practical changes in managing resectable NSCLC. From the medical oncologist's perspective, it is time to dance with surgeons to decide on the sequence of systemic treatment, particularly the ICB-based approach, accompanying surgery for operable NSCLC.
Mixed urinary incontinence (UI) is common in women. This study aimed to assess the efficacy of anti-incontinence surgery in female patients with equally severe stress UI (SUI) and urge UI (UUI). All patients had equal severity of SUI and UUI. The postoperative cure rate was categorized into the cure group (CG) and failure group (FG). Postoperative satisfaction was categorized into the satisfaction group (SG) and the dissatisfaction group (DG). Statistical significance was set at P<0.05. Ninety patients (SG, 73.3%; DG, 26.7%; CG, 93.3%; FG, 6.7%) were included in the study. In the univariate analysis, body mass index (BMI), total bladder capacity, and overactive bladder symptom score (OABSS) were significantly different between the SG and DG groups. Peak urinary flow, Valsalva leak point pressure (VLPP), and OABSS were significantly different between the CG and FG groups. In the multivariate analysis, OABSS (P=0.001) and BMI (P=0.032) were independent predictors of postoperative satisfaction. VLPP (P=0.023) was the only independent factor associated with the postoperative cure rate. In equal severity of SUI and UUI, VLPP was found to be the only independent factor associated with postoperative cure rates. Higher VLPP values were associated with higher cure rates. BMI and OABSS were identified as independent predictors of postoperative satisfaction, with lower BMI and OABSS associated with higher postoperative satisfaction.
연구배경: 표피성장인자수용체(epidermal growth factor receptor, EGFR) 티로신 활성효소 억제제(tyrosine kinase inhibitor, TKI)는 진행성 비소세포폐암의 새로운 치료제이다. 몇몇 연구 결과 gefitinib와 erlotinib에 대한 반응률과 반응 예측인자에 차이가 있을 가능성을 제시하였다. 저자들은 한국인 진행성 비소세포폐암에서 gefitinib와 erlotinib의 효과 및 독성을 비교하고 각 약제에 대해 서로 다른 반응 예측인자가 있는지 평가하였다. 방 법: 2003년 7월부터 2009년 2월까지 이화여자대학 교부속병원에서 진행성 비소세포폐암으로 gefitinib 또는 erlotinib로 치료 받은 환자들의 임상정보를 수집하였다. 중앙 생존기간은 Kaplan-Meier법으로 계산하였다. 결 과: 대상 환자는 총 86명이었다(gefitinib군 52명 대 erlotinib군 34명). 나이의 중앙값은 64세였고 53명(62%)이 남자였다. 86명 중 83명에서 반응평가가 가능했으며, 83명 중 35명이 반응을 보였고 12명이 안정성 질환이었으며 36명이 진행성 질환으로, 치료 반응률이 42%였고 질병 조절률이 57%였다. 중앙 추적관찰기간 502일 동안, 진행까지의 중앙기간은 129일이었으며 중앙 생존기간은 259일이었다. 치료 반응률(gefitinib 44% 대 erlotinib 39%, p=0.678), 중앙 생존기간(gefitinib 301일 대 erlotinib 202일, p=0.151) 및 병의 진행까지 기간의 중앙값(gefitinib 136일 대 erlotinib 92일, p=0.672)은 두 군 사이에 차이가 없었다. 두 약제는 비슷한 독성을 보였다. Cox 회귀모형을 이용한 다변수분석에서 선암이 생존과 관련된 독립적인 예후인자였다(상대위험도: 0.487, 95% 신뢰구간: 0.292~0.811, p=0.006). 아집단 분석 결과 두 약제에 대한 서로 다른 반응 예측인자는 없었다. 결 론: Gefitinib와 erlotinib 사이에 반응률, 생존기간, 진행까지의 기간 및 독성에 차이는 없었다. 각 약제에 대한 특이적인 반응 예측인자도 없었다.
