Comparison of Therapeutic Efficacy of Gefitinib and Erlotinib in Patients with Squamous Cell Lung Cancer

  • Background: Gefitinib and erlotinib are useful, molecular targeted agents in patients with non-small-cell lung cancer (NSCLC) who failed previous chemotherapy. We compared the efficacy and toxicity of two drugs in patients with squamous cell lung cancer, most of whom are male smokers. Methods: We retrospectively reviewed the clinical information on patients with NSCLC who were treated with gefitinib or erlotinib treatment at Chonnam National University Hwasun Hospital between July 2002 and November 2009. The overall response rate (ORR), overall survival (OS) and progression-free survival (PFS) were compared between the two drugs. Results: A total of 182 (100 gefitinib vs. 82 erlotinib) of 584 patients treated by targeted agents had squamous histology. Of the 182 patients, 167 (91.7%) were male and 159 (87.4%) were smokers. The ORR and disease control rate (DCR) were 4.9% and 40.6%, and there was no significant difference between gefitinib and erlotinib (ORR, 5.0% vs 4.8%; p=0.970; DCR, 40.0% vs 41.4%; p=0.439). The median OS in the gefitinib group was 12.1 months, and that in the erlotinib was 12.7 months (hazard ratio [HR], 1.282; 95% confidence interval [CI], 0.771~2.134; p=0.339). The median PFS for the gefitinib group was 1.40 months, compared with 1.37 months for the erlotinib group (HR, 1.092; 95% CI, 0.809~1.474; p=0.564). Skin rash ${\geq}$grade 3 was more common in erlotinib (12.2%) than gefitinib (1.0%, p=0.003) groups. Conclusion: This retrospective study showed that the two drugs appear to have similar antitumor efficacy and toxicity except for skin rash.

Choice of Capecitabine or S1 in Combination with Oxaliplatin based on Thymidine Phosphorylase and Dihydropyrimidine Dehydrogenase Expression Status in Patients with Advanced Gastric Cancer

  • Purpose: To study the efficacy of capecitabine or S-1 plus oxaliplatin (CAPOX or SOX) for treating thymidine phosphorylase (TP)- or dihydropyrimidine dehydrogenase (DPD)-positive advanced gastric cancer. Materials and Methods: Eighty-six patients with stage IIIC to IV gastric cancer were assessed for TP and DPD expression by immunohistochemistry. The association between CAPOX or SOX efficacy and TP/DPD expression was retrospectively analyzed. Results: There were no significant differences in the objective remission rate (ORR, 52.27% vs. 47.62%; P>0.05), disease control rate (72.73% vs. 73.81%, P>0.05), progression-free survival (hazard ratio [HR], 1.119; 95% confidence interval [CI], 0.739-1.741; P=0.586), and overall survival (OS; HR, 0.855; 95% CI, 0.481-1.511; P=0.588) between CAPOX and SOX. A higher number of stage IV patients showed TP positivity, while DPD-positive patients predominantly showed intestinal type of gastric cancer. In TP-positive patients, the ORRs associated with CAPOX and SOX treatments were 57.14% and 38.10%, respectively; OS was better with CAPOX than with SOX (HR, 0.447; 95% CI, 0.179-0.978; P=0.046). Among DPD-positive patients, the SOX treatment-associated ORR (60.87%) was significantly higher than the CAPOX treatment-associated ORR (43.48%). Furthermore, SOX treatment resulted in better OS than did CAPOX treatment (HR, 2.020; 95% CI, 1.019-4.837; P=0.049). Conclusions: No significant difference in clinical efficacy was found between CAPOX and SOX. TP-positive patients might respond better to CAPOX while DPD-positive patients may respond better to SOX. Our findings might serve as a guide for personalized chemotherapy for gastric cancer.

Antagonistic Effects of Doxapram and Yohimbine on Tiletamine-Zolazepam Anesthesia in Dogs (개에서 Tiletamine-Zolazepam 마취에 대한 Doxapram과 Yohimbine의 길항효과)

  • This study was performed to examine the general anesthetic efficacy of tiletamine-zolazepam, a mixture of phencyclidine-derived tiletamine and benzodiazepine-related zolazepam. The antagonistic activities of doxapram and yohimbine to the anesthetic effects of tiletamine-zolazepam were also studied. Thirty healthy mongrel dogs were divided into three groups (each of 10) twenty minutes after being anesthetized with tiletamine-zolazepam : T-Z-S group(tiletamine-zolazepam-saline), T-Z-D group (tiletamine -zolazepam-doxapram), T-Z-Y group (tiletamine-zolaz.pam-yohim bine). Various parameters wert evaluated in terms of the onset and recovery time of analgesia, respiration rates, hear rates, body temperature, electrocardiogram, blood chemistry, and lymphocyte blastogenesis. The results obtained through these experiment could be summarized as follows: 1. he anesthetic efficacy of tiletamine-zolazepam was considered desirable, with the onset time of anesthesia being as short as 0.23-0.24 minutes. 2. Both of the antagonistic effects of yohimbine and doxapram on the anesthesia induced by liletamine-zolazepan were evaluated statistically significant(p<0.05) as the recovery time was shortened from 39.3$\pm$4.9 min(T-Z-S group) to 25.3$\pm$2.9 nin(T-Z-Y group) and 29.9$\pm$8.8min(T-Z-D group), respectively. 3. Respiration rates were not changed by the treatments of both doxapram and yohimbine, with the only transient increase in the T-Z-D group. The changes in the respiration rate were not observed during the whole time course of the experiment. 4. Yohimbine(T-Z-Y group) increased the heart rate significantly from 30 minutes after the adminstration compared to the T-Z-S group and T-Z-D group (p<0.05). 5. The decreases in th, body temporature were observed from 30 minutes in the T-Z-S group(p<0.05) and 40 minutes in th, T-Z-D group(p<0.05), after the adminstration. On the other hand, there was no hypothermia in the T-Z-Y group. 6. In the all experimental groups of the T-Z-S, T-Z-D and T-Z-Y, there were no specific findings on the electrocardiograph incept slight shift to the tachycardia in all cases. 1. We could not find any differences in the blood chemistry between all experimental groups (T-Z-S, T-Z-D and T-Z-Y). 8. the inhibition of the lymphocyte blastogenesis shown in the T-Z-S with 3 hours decreasing and thereafter restoring to the normal values up to the point 5 hours were not occurred in the T-Z-D and T-Z-Y groups. With the above results, we could conclude that both doxapram and yohimbine can be clinically used as recovery agents towards anesthesia by tiletamine- zolazepam fi:on the efficacy point of view, but yohimbine is more recommendable in this case if considering the recovery time and lymphocyte blastogenesis.

  • As the average life span have been lengthened and the rate of senile population have been raised, chronic degenerative diseases incident to aging has been increased rapidly and become a social problem. With this social background, recently, the facts that oxygen radicals(OR) have toxic effects on Central Nervous System and Peripheral Nervous System and cause neuropathy such as Parkinson's Disease, Alzheimer Disease have been turned out, and accordingly lots of studies on the mechanism of the toxic effects of OR on nerves, the diseases caused by OR and the approaches to curing the diseases have been made. The purpose of this study is to examine the toxic effects caused by Glucose Oxidase(GO) and the effects of herbal extracts such as Chilbokyeum(CBY) on the treatment of the toxic effects. For this purpose, experiments with the cultured cell from the cerebrums of new born mice were done. The results of these experiments were as follows. 1. GO, a oxygen radical, decreased the survival rate of the cultured cells on NR assay and MTT assay 2. GO, a oxygen radical, increased lipid peroxidation and the amount of LDH. 3. CBY have efficacy of decreasing lipid peroxidation. 4. CBY have efficacy of decreasing the amount of LOH. From the above results, It is concluded that Chilbokyeum has marked efficacy as a treatment for the damages caused in the GO-mediated oxidative process. And Chilbokyeum is thought to have certain pharmacological effects on controlling over aging and treating Dementia. Further clinical study of this pharmacological effects of Chilbokyeum should be complemented.

Study on Antifungal Activity of Aloe arborescens M. for a Potential Bio-Pestcide (알로에(Aloe arborescens M.)의 항진균성 검정을 통한 생물농약 가능성 탐색)

  • Antifungal activity reaction to Aloe peel and juice extracted by several solvents and concentrations against three plant pathogenic fungi as collectrichum gloeosporioides, Fusarium oxysporum, and F. solani were investigated. The Antifungal activity of Aloe extracts varied with the kinds of solvents. In case of C. gloesporioides and F. solani, it's the highest in water, second in methanol, and third in ethanol. It showed that Antifungal activity of Aloe peel and juice extracts based on concentration was much more excellent in juice extracts than peel extracts against C. gloeosporioides, F. solani. Effect of Antifungal against F. oxysporum by juice extracts did not appear at a concentration of $100{\mu}l/L$ and then shows 9.5% of inhibition rate at $100,000{\mu}l/L$ while against C. gloeosporioides it starts to react at $100{\mu}l/L$ and then reaches at 29.3% of the highest inhibition rate at a concentration of $100,000{\mu}l/L$ out of three plant pathogenic fungi. The effect of control efficacy against Fusarium wilt caused by F. oxysporum by juice extracts of foliar spray tends to increase and the more the concentration of juice extracts increases, the higher the effect of control efficacy against Fusarium wilt disease. Foliar spray at a concentration of 10g/L has a 62% of control efficacy compared with control treatment, which foliar spray of Aloe juice extracts can be the more effective method in order to prevent Fusarium wilt disease caused by F. oxysporum.

Physiological Evaluation of Korean Mountain Ginseng and Korean Mountain Ginseng Leaf Tea (장뇌삼 및 장뇌삼엽차의 생리활성평가)

  • When extracts of KMG (Korean mountain ginseng) leaf tea and fermented KMG leaf tea were compared, the fermented KMG leaf tea extract showedhigher activity at each stage of density. Among the material groups, the KMG extract hadthe least profound SOD-like activity, and similar SOD-like activities were noted in the fermented KMG, KMG leaf tea, and fermented KMG leaf tea extracts. With regard to nitrite scavenging ability at a pH of 1.2, the KMG, fermented KMG, and KMG leaf tea groups exhibited similar results, and at pH 3.0, the KMG and KMG leaf tea extract groups exhibited more profound nitrite scavenging ability compared to the fermented groups. In the case of HeLa cell treatments, the KMG and fermented KMG leaf tea extracts exhibited cancer cell propagation restraint rates in excess of 30%, at a density of 1 mg/mL. And MCF-7 cells treated with fermented KMG and KMG leaf tea showedsimilar propagation restraint rates at more than 27% of cancer cells, at a density of 1 mg/mL. Among the materials, the KMG extract hadthe lowest cancer cell propagation restraint rate at 21%, and the fermented KMG leaf tea extract had the highest rate at more than 70%.

Enhanced Control Efficacy of a Fumigant, Chlorine Dioxide, by a Mixture Treatment with Carbon Dioxide (이산화탄소 혼합 처리를 통한 이산화염소 훈증 처리 효과 제고 기술)

  • Fumigation using chlorine dioxide ($ClO_2$) has been regarded as a potential control technique against stored grain insect pests. The control efficacy can be enhanced with increase of opening rates of spiracles by facilitating the toxic gas delivery to internal body. To test this hypothesis, this study used the Indianmeal moth, Plodia interpunctella, which was known to be susceptible to $ClO_2$, and analyzed the opening rate of spiracles. A total of 9 pairs of spiracles were observed in P. interpunctella larvae, in which one pair was located on the prothorax and eight pairs were on the abdomen. Within the body, the spiracles were connected to longitudinal and transverse tracheal trunks. The open spiracles were determined by the dye-infiltration method. Based on this method, the opening rate of spiracles increased up to about 60% with increase of ambient temperatures. Furthermore, exposure to carbon dioxide stimulated the opening rate of spiracles up to about 95%. In contrast, exposure to $ClO_2$ prevented the spiracle opening and resulted in only 25% of spiracles in an opening state. The addition of carbon dioxide to $ClO_2$ treatment rescued the opening rate of spiracles as much as the carbon dioxide single treatment. Based on this modulatory activity of carbon dioxide against spiracles, the combined treatment of $ClO_2$ with carbon dioxide resulted in significant increase of its toxicity against P. interpunctella compared to a single $ClO_2$ treatment.

Efficacy of Aprepitant in Patients with Advanced or Recurrent Lung Cancer Receiving Moderately Emetogenic Chemotherapy

  • Aims and Background: To evaluate the efficacy of a combination of aprepitant and conventional antiemetic therapy in patients with advanced or recurrent lung cancer receiving moderately emetogenic chemotherapy (MEC). Methods: Patients with advanced or recurrent lung cancer who were treated with MEC regimens at the Department of Respiratory Medicine, Fukuoka University Hospital, were included and classified into the following groups: control group (treatment: 5-HT3 receptor antagonists + dexamethasone) and aprepitant group (treatment: 5-HT3 receptor antagonists + dexamethasone + aprepitant). The presence or absence of chemotherapy-induced nausea and vomiting (CINV) was evaluated according to the Common Terminology Criteria for Adverse Events (CTCAE) v4.0; patients with grade 1 or above were considered positive for CINV. Food intake per day, completion of planned chemotherapy, and progression-free survival (PFS) achieved by chemotherapy were investigated. Results: The complete suppression rate of nausea in the aprepitant group was significantly higher than that in the control group (p = 0.0043). Throughout the study, the food intake in the aprepitant group was greater than that in the control group, with the rate being significantly higher, in particular, on day 5 (p = 0.003). The completion rate of planned chemotherapy was also higher in the aprepitant group (p = 0.042). PFS did not differ significantly, but tended to be improved in the aprepitant group. Conclusions: The aprepitant group showed significantly higher complete suppression of nausea, food intake on day 5, and completion of planned chemotherapy than the control group.

The Efficacy of Induction Chemotherapy in Stage III Non-Small Cell Lung Cancer (제 3기 비소세포 폐암에서 유도 화학 요법의 효과)

  • Purpose : This study was performed to analyze the efficacy of induction chemotherapy fellowed by radiation therapy in locally advanced non-small cell lung cancer Materials and Methods : Eighty patients with locally advanced non-small cell lung cancer treated from 1989 to 1995 at Pusan Paik hospital were analyzed retrospectively. Twenty-one patients were treated with induction chemotherapy followed by radiation therapy and Fifty-nine Patients were treated with radiation therapy alone. Chemotherapy regimen consisted of cisplatin-based combination (2 or 3 drugs). All patients were treated by Co-60 or 6 MV linear accelerators. Radiation dose ranged from 50 Gy to 80 Gy (median 64.8 Gy). We evaluated response rate, survival rate, and pattern of failure in both treatment groups. Results : Overall response rate in induction chemotherapy group and radiotherapy alone group were 48% and 45%, respectively. Of the 80 patients, 46 patients were evaluable for pattern of failure. Initial failure pattern in induction chemotherapy group was as follows: 8 (67%) at locoregional, 4 (33) in distant metastasis. Radiation alone group was 21 (71%) and 5 (29%), respectively. Results showed no difference of distant failure between induction chemotherapy group and radiation alone group. The 1 and 2 year survival rate in induction chemotherapy group were 43% and 14%, respectively and in radiotherapy alone group, 31% and 7%, respectively (p=0.135). Conclusion : In stage III non-small cell lung cancer, induction chemotherapy and radiation therapy showed increased tendency in survival with no statistical significance Induction chemotherapy seems to have no effect of decreasing distant failure and no survival advantage compared with radiotherapy alone.

Quantitative Evaluation of the First Order Creatine-Kinase Reaction Rate Constant in in vivo Shunted Ovine Heart Treated with Oxandrolone Using Magnetization Transfer 31P Magnetic Resonance Spectroscopy (MT-31P-MRS) and 1 H/31P Double-Tuned Surface Coil: a Preliminary Study

  • Purpose: Children born with single ventricle physiology demonstrate poor growth rate and suffer from malnutrition, which lead to increased morbidity and mortality in this population. We assume that an anabolic steroid, oxandrolone, will promote growth in these infants by improving myocardial energy utilization. The purpose of this paper is to study the efficacy of oxandrolone on myocardial energy consumption in these infants. Materials and Methods: We modeled single ventricle physiology in a lamb by prenatally shunting the aorta to the pulmonary artery and then postnatally, we monitored cardiac energy utilization by quantitatively measuring the first order reaction rate constant, $k_f$ of the creatine-kinase reaction in the heart using magnetization transfer $^{31}P$ magnetic resonance spectroscopy, home built $^1H/^{31}P$ transmit/receive double tuned coil, and transmit/receive switch. We also performed cine MRI to study the structure and dynamic function of the myocardium and the left ventricular chamber. The spectroscopy data were processed using home-developed python software, while cine data were analyzed using Argus software. Results: We quantitatively measured both the first order reaction rate constant and ejection fraction in the control, shunted, and the oxandrolone-treated lambs. Both $k_f$ and ejection fraction were found to be more significantly reduced in the shunted lambs compared to the control lambs, and they are increased in oxandrolone-treated lambs. Conclusion: Some improvement was observed in both the first order reaction rate constant and ejection fraction for the lamb treated with oxandrolone in our preliminary study.
