INFLUENCE OF ATTACHMENT RELATIONSHIP WITH PARENTS ON REJECTION SENSITIVITY AND PEERRELATION IN SCHOOL-AGE CHILDREN (학령기 아동들의 부모에 대한 애착관계가 거부민감성 및 또래 관계에 미치는 영향)

  • This study aims to reveal whether attachment relationship with parents can differ from children being accepted and rejected in peer relations, and whether children's rejection sensitivity can differ from children having secure and insecure attachment relationship with parents. Subjects were 218 students in fifth and sixth grade of primary school. Inventory of Parent and Peer Attachment(IPPA), Children's Rejection Sensitivity Questionnaire(CRSQ), and peer relation sociometrics were used as measurements. The results showed that children in secure attachment with parents had lower scores on the subscales of CRSQ, such as anxiety, anger, belief, and psychological reaction, than children in insure attachment relationship with parents. And IPPA scores were higher in children being accepted in peer relation than children being rejected. These results can be considered that early attachment relationship with parents may influence children's rejection sensitivity and peer relation.

Early Results of the Sternocostal Elevation for Pectus Excavatum (Sternocostal elavation술의 누두흉 교정효과)

  • Thirteen sternocosal elevations for pectus excavatum were performed on twelve patients between Jan 1989 and Sep. 1991. Eleven among the twleve patients were male. The subjects ranged in age from 2 years to 20 years, with a mean age of 8.33 years[SD=4.80 years], Eleven patients were below 15 years. The early postoperative evaluation by Humphreys` criteria except keloid formation was excellent in all the patients. The lower vertebral index[LVI], configuration index[CI], and upper vertebral index[UVI] were measured on the lateral chest roentgenogram. The LVI and CI improved significantly from 0.31$\pm$0.06 and 1.14$\pm$0.06 preoperatively to 0.25$\pm$0.03 and 0.78$\pm$1.10 postoperatively. The degrees of depression, flatness, and asymmetry and funnel index were measured at the most deformed portion on the computed tomgram performed pre- and postoperatively. The degrees of depression and funnel index improved significantly from 3.32$\pm$0.73 and 5.77$\pm$1. 54 preoperatively to 1.85$\pm$0.14 and 2.96$\pm$0.43 postoperatively, There was no significant change in the degree of flatness however, Six postoperative morbidities occurred in five patients Three were superficial wound infection, two hemopneumothorax, and one reoeration, Reoperation was performed due to forward displacement of the distal sternum below the posterior sternotomy, The findings of this study suggest that the sternocostal elevation is an excellent primary method for the pectus excavatum of children and young adolescents.

The Reliability of Eye Screening Physical Examination in School and Living Environment Characteristics of Young Children with Amblyopia (유소년 약시환자의 생활환경 특성과 학교신체검사 안검진의 신뢰성)

  • Considering that few studies had paid attention to the living characteristics and social environment surrounding amblyopes, the present study surveyed the parents of children aged 3 to 12 and diagnosed-with amblyopia to analyse the time when the children were first diagnosed-with amblyopia, the characteristics of their living environment and the socio-environmental factorsand to delve into correlations. For the purpose of this study, 104 parents of patients in 4 hospitals in Seoul, Gyeonggi and Incheon consented to participate in the survey from September 2 to November 23: 2013. As for the time when the children were first diagnosed with amblyopia: the age of 4 accounted for the highest percentage(28.8%). The older the parents were: the later the children were diagnosed with amblyopia (p<.01). The higher the education of parents, the earlier the diagnosis of amblyopia(p<.05). The present findings-will be conducive to decreasing the number of patients who have no choice but to live-with low vision for life by missing some timely visual acuity tests and ophthalmological examination and particularly to developing a social safety net that can realize national medical welfare for the low-income families and the socially disadvantaged class.With the help of information technology.

Effect of nutritional supplement formula on catch-up growth in young children with nonorganic faltering growth: a prospective multicenter study

  • BACKGROUND/OBJECTIVES: Inadequate nutrition in infants and young children affects physical growth and neurocognitive development. Therefore, early nutritional intervention is important to promote catch-up growth in young children with faltering growth. The aim of this study was to evaluate the effect of nutritional supplementation with a pediatric concentrated and balanced nutritional supplement formula on promoting growth and improving nutritional status in children with nonorganic faltering growth. SUBJECTS/METHODS: Children aged 12-36 months whose body weight-for-age was < 5th percentile on the Korean Growth Charts were enrolled. Children born premature or having organic diseases were excluded. Children were instructed to consume 400 mL of formula per day in addition to their regular diet for 6 months. Pediatricians and dietitians educated the parents and examined the subjects every 2 months. Anthropometric parameters were measured at baseline and at 2, 4, and 6 months, and laboratory tests were done at baseline and 6 months. The good consumption group included children who consumed ≥ 60% of the recommended dose of formula. RESULTS: Total 82 children completed the 6-month intervention. At baseline, there were no significant differences in all variables between the good consumption and poor consumption groups. Weight and weight z-scores were significantly improved in the good consumption group compared to the poor consumption group at the end of the intervention (P = 0.009, respectively). The good consumption group showed a significant trend for gaining weight (P < 0.05) and weight z-score (P < 0.05) compared to the poor consumption group during 6 months of formula intake. The concentration of blood urea nitrogen was significantly increased in the good consumption group (P = 0.001). CONCLUSIONS: Nutritional supplementation with a concentrated and balanced pediatric nutritional formula along with dietary education might be an effective approach to promote catch-up growth in children with nonorganic faltering growth.

A deep learning approach to permanent tooth germ detection on pediatric panoramic radiographs

  • Purpose: The aim of this study was to assess the performance of a deep learning system for permanent tooth germ detection on pediatric panoramic radiographs. Materials and Methods: In total, 4518 anonymized panoramic radiographs of children between 5 and 13 years of age were collected. YOLOv4, a convolutional neural network (CNN)-based object detection model, was used to automatically detect permanent tooth germs. Panoramic images of children processed in LabelImg were trained and tested in the YOLOv4 algorithm. True-positive, false-positive, and false-negative rates were calculated. A confusion matrix was used to evaluate the performance of the model. Results: The YOLOv4 model, which detected permanent tooth germs on pediatric panoramic radiographs, provided an average precision value of 94.16% and an F1 value of 0.90, indicating a high level of significance. The average YOLOv4 inference time was 90 ms. Conclusion: The detection of permanent tooth germs on pediatric panoramic X-rays using a deep learning-based approach may facilitate the early diagnosis of tooth deficiency or supernumerary teeth and help dental practitioners find more accurate treatment options while saving time and effort

A Study on Factors Related to Sleep Disordered Breathing in Children (어린이의 수면 호흡 장애 관련 위험인자)

  • The aim of this study was to investigate the risk factors associated with sleep disordered breathing (SDB) by comparing intraoral factors, body mass index (BMI), and medical history with pediatric sleep questionnaire (PSQ) findings. Seven hundred eighty-seven subjects aged between 7 to 11 years old were included. Their caregivers were asked to complete questionnaires. Oral manifestations including Angle's classification, overjet, and Brodsky tonsil grade were examined. Children with PSQ scores of more than 0.33 points were classified into the SDB high-risk group. Among the 787 subjects, 34 (4.3%) were classified into the SDB high-risk group. Children with allergic rhinitis, atopic dermatitis, excessive overjet, or large tonsil size had a significantly higher risk for SDB versus those without. Also, there was a significant difference in SDB risk according to BMI status. Gender, gestational age, breastfeeding, and Angle's classification were not associated with SDB. Children at high risk for SDB were predisposed to tonsillar hypertrophy, allergic rhinitis, obesity, and atopic dermatitis. Children with these factors could be candidates for early intervention to prevent the progression of SDB.

  • Hearing loss is the most common birth defect among infants. Yet most hearing-impaired children are not identified until 1 to 3 years of age - which is well beyond the critical period(6 month) for healthy speech and language development. However, if a hearing-impairment is identified and treated in its early stages, child's speech and language skills will be comparable to his or her normal- hearing peers. For these reasons, hearing screening at birth and routinely throughout childhood is extremely important. Auditory brain-stem response(ABR) is nowadays one of the most reliable diagnostic tools in the early detection of hard of hearing. In this study, we developed the system that detects auditory evoked potential using micro-processor. Furthermore, it is required to develop a portable system to measure in not only soundproof room but also nursery for infants.

  • To study the eruption pattern of the mandibular first permanent molar, the author took 263 cases of Oblique Cephalogram from age 3 to 6 years old children and observed the vertical and mesiodistal directional change and tooth axis change. The following results were obtained.; 1. The eruption pattern of the mandibular first permanenl molar was changed at about 60~66 months or calcification stage IX. 2. At the early stage, the path of eruption of the mandibular first permanent molar directed upward and forward and after calcification stage IX it changed to the direction of upward. 3. The height of the alveolar bone of the upper part of the mandibular first permanent molar was almost equal to that of the interseptal bone of the first and second deciduous molar, but the height showed gradual descent afterwards. 4. At the early stage, the distance from the distal end of the mandibular second deciduous molar to the anterior portion of the ascending ramus was 1.2~1.4 times larger than the mesio-distal diameter of the mandibular first permanent molar, but at the later stage it was enlarged 1.7~1.9 times larger than the mesiodistal diameter of the mandibular first permanent molar.

Clinical Characteristics of Odontoma in Children : A Retrospective Analysis of 105 Cases (유년기 치아종의 임상적 특징 : 105증례의 후향적 분석)

  • We investigate the clinical characteristics of odontoma in childhood and adolescence. A retrospective analysis was performed in 105 patients under 15 years old who were diagnosed with odontoma in Kyungpook National University Dental Hospital, the department of pediatric dentistry between 1 January 2008 and 31 December 2013. The ratio of compound odontoma, immature odontoma and complex odontoma were 67.6%, 21.9% and 10.5%, respectively. Odontoma was more prevalent in males and the ratio of males to females was 1.5:1. The age of patients ranged from 3 to 14 years old with an average age of 9.3 years old. The odontoma was more frequently located in the maxilla (73.3%) than in the mandible(26.7%). In 98 cases, it was treated by surgical removal. Among adjacent teeth impacted by odontoma, 26 cases (34.7%) of successional permanent teeth were erupted when only odontoma were removed and 6 cases (8.0%) were erupted when surgical exposure was performed. Orthodontic tractions were performed in 43 cases (57.3%). In conclusion, odontoma could cause complications such as impaction of teeth. If they are found early and treated properly, high eruption success of successional permanent teeth can be obtained. The occurrence frequency of immature odontoma are higher than that of complex odontoma, and they are observed intensively in anterior maxillary teeth. Therefore, it is considered that they are important causes of dental impaction in anterior maxillary teeth.

General Anesthesia for Dental Treatment of a Pediatric Patient with Marfan Syndrome (마판 증후군 소아 환자의 전신마취 하 치과치료)

  • Multiple caries in a pediatric patient often requires dental treatment under general anesthesia, especially when the patient is suffering from a systemic disease. The patient was a 6-year-old boy with Marfan syndrome and needed extensive dental treatment. Marfan syndrome is an inherited disorder resulting from mutations in Fibrillin-1 gene. Patients are known to have mainly cardiovascular, ocular, and musculoskeletal problems. Although clinical symptoms of the syndrome are age-realted, thus hindering early diagnosis of the disease in young children, our patient had been confirmed by a gene study at a younger age. Medical history of the patient revealed moderate to severe mitral regurgitation and aortic root dilatation, which required mitral valve replacement surgery with a mechanical valve. As a result, the patient was taking warfarin post-operatively and changes in medication had to be made before the dental treatment. Also, prophyalctic antibiotics had to be given before the treatment for prevention of (to prevent the) infective endocarditis. With careful control of the medications and bleeding tendency, general anesthesia and the treatment were done successfully without any complications.
