An Ecological Analysis of Early Child Care Arrangements (1) (영유아 양육환경에 대한 생태학적 연구 (1) - 가정과 어린이집을 중심으로 -)

  • The home environments, daily activities, caregiving behaviors of mothers and teachers, infant's verbal behaviors, attachment to mothers and teachers of day care infants were compared to those of home-reared infants. 30 two-year-old home-reared infants and their mothers were observed at home by the time sampling method and 31 two-year-old day care infants and their mothers and teachers were observed at home or at the center. The HOME scale, the behavioral checking lists for caregiving behaviors and for the infants' verbal behaviors, and the Attachment Q-set were used for the home and center observations. It was found that the characteristics of early environments of day care infants were different from those of home-reared infants, but that infants' attachment to the mothers were not different. However, day care infants' attachment security scores to their mothers were higher than to their teachers.

Causes of Transfer of Neonates (Born after ≥34 Weeks of Gestation) to the Neonatal Intensive Care Unit Owing to Respiratory Distress and their Clinical Features

  • Purpose: Respiratory morbidity is the most common problem among neonates admitted to neonatal intensive care units. Therefore, the aim of this study was to make a differential diagnosis between transient tachypnea of the newborn (TTN), respiratory distress syndrome (RDS), and pneumonia through comparison of clinical features and test results. Methods: This retrospective study was conducted in 86 infants with TTN, RDS, or pneumonia. These were infants who had respiratory distress, were born after ${\geq}34$ weeks of gestation, and transferred to the neonatal intensive care unit of Kosin University Gospel Hospital between June 1, 2011 and June 30, 2016. Results: The numbers (percentage) of infants with TTN, RDS, and pneumonia were 51 (59.3%), 20 (23.3%), and 15 (17.4%), respectively. Late-preterm and early-term newborns accounted for 65.1% of the infants. Tachypnea was observed in 74.4% of the neonates. The median age at admission was 4 hours (0 to 116) after birth. The infants with RDS had significantly lower birth weights, pH levels, base excess and oxygen saturation levels at admission, longer duration of total ventilator therapy, and hospital stay than those in the other two groups. The infants with pneumonia showed significantly high initial high-sensitivity C-reactive protein levels and significant chest radiographic findings. Conclusion: Early differential diagnosis for TTN, RDS, and pneumonia is challenging because they show similar respiratory symptoms at an early stage. Clinical features and test results can be used to determine the etiology of respiratory distress and early antibiotic treatment.

Evaluation of the safety and efficacy for the technique of removing VFB from the bronchial tree in infants and early childhood using Fogarty balloon catheter. (Fogarty balloon catheter를 이용한 영유아 기관지 식물성 이물 제거술의 의의)

  • Background and Objectives: Vegetable Foreign bodies (VFB) in the bronchial tree may be complicated by fragmentation, slippage and impaction during the removal with forceps. This study is to evaluate the safety and efficacy for the technique of removing VFB from the bronchial tree in infants and early childhood using Fogarty balloon catheter. Materials and methods : The subjects consisted of 18 infants and early childhood (7-22 months old) with VFB in the bronchial tree from January 1991 through October 1998. The authors first attempted removal of VFB with forceps and if that failed, removed VFB with Fogarty arterial embolectomy catheter under the ventilating bronchoscopy and general anesthesia. Results: We removed 6 VFB with forceps. could not remove anymore, and so removed 12 VFB with Fogarty catheter. In 8 VFB of less than 24 hours, we could remove 6 VFB with forceps and 2 VFB which could not be removed with forceps were removed with Fogarty catheter. In 10 VFB of more than 24 hours, we could not remove with forceps and removed with Fogarty catheter. Conclusions : VFB in the bronchial tree of infants and early childhood can usually be removed with forceps. But we think that Fogarty balloon catheter technique is a easy, safe method for the removal of bronchial VFB of more than 24 hours, fragmentation, impaction, lower bronchus and too round or slippery to remove with forceps in infants and early childhood.

Taurine Level in Human Milk and Estimated Intake of Taurine by Breast -Fed Infants during the Early Period of Lactation (수유 초기 모유 중 타우린 함량과 영아의 섭취량 추정)

  • Taurine is only supplied to the infants from the breast-fed or formula milks because the enzyme activities of taurine biosynthesis are limited in early stages of infants . The objectives of present study were to quantitate the contents of tarurine in human milk and to estimate the intake of taurine by breast-fed infants during early period of lactation. Thirty -three lactating women, volunteered and delivered in R hospital in Serou. were recruited. Milk samples were collected every day at B1-B5 day(from 1 to 5 day dafter start of milk secretion), 15th and 30th day postpartum. Taurine contents were determined by HPLC equipped with RF-detector. The intake of taurine by infants was estimated by multiplization with the infant milk intake reported in our laboratory. The content of taurine was 406$\pm$174nmol/ml at B1-B5 day, and then gradually decreased to 359$\pm$125nmol/ml at 15th day and to 304$\pm$94nmol/ml at 30th day postpartum. The estimated intake of taurine was almost same as 24-25mg/day at B1-B5 day, and 15th , 30th days postpartum . This results was due to the increase of the intake of milk by infants.

Joint Attention and Language Development in Infants from Multi-Cultural Families (다문화 가정 유아들의 함께 주의하기와 언어발달)

  • Joint attention, language development, and the relationship between these two variables were compared in infants from multi-cultural and Korean families. Joint attention was observed in both the Early Social Communication Scale (ESCS) and in infant-mother free play. Language development was evaluated by means of the MacArthur-Bates Communicative Development Inventory-Korean. There were no group differences in initiating and responding to joint attention in ESCS. However, in infant-mother free play, joint attention episodes were less and shorter in duration with infants from multi-cultural families than in Korean infants. The size of both the expressive and receptive vocabulary was also smaller in infants from multi-cultural families than in Korean infants. In terms of Korean infants, mean duration of joint attention episodes in free play showed a significant positive correlation with the size of the expressive vocabulary and initiating joint attention in ESCS also showed a significant positive correlation with the size of receptive vocabulary. However, none of the measures of joint attention indicated a significant relationship with the size of either expressive or receptive vocabulary in infants from multi-cultural families.

The effects of multimedia fairytale activities on infants' prosocial actions (멀티미디어 동화 활동이 유아의 친사회적 행동에 미치는 영향)

  • The purpose of this study is to investigate effective teaching media for infants' teaching-learning process by comparing the effects of multimedia fairytale activities and picture fairytale listening activities on infants' prosocial actions. Following the purpose of this study, we studied any effect to infants' prosocial actions from multimedia fairytale activities by dividing in two groups. This study was conducted with 5-year-old infants (total 40; 20 experimental group & 20 comparison group) in D daycare center in Daejeon. Verifying data tool were time series corresponding t-test, independent t-test and multi-variant analyses. As a result, multimedia fairytale listening activities are more effective for infants' prosocial actions. It has meaningful difference in the interpersonal relationship skills and early childhood education adapt which is sub-area of infant's prosocial actions. So, for various and high-quality early childhood education, using multimedia teaching media is more effective than existing teaching media.

The Development of Gesture in the Early Communication of Korean Infants (한국 영아의 초기 의사소통 : 몸짓의 발달)

  • Korean infants' use of gesture was examined with 45 10-to 17-month olds. The mothers of infants were asked to check each word in the MacArthur Communicative Development Inventory-Korean (MCDI-K) vocabulary checklist if their infant had a gesture for a given word and to indicate what kind of early communicative behavior she showed in 5 different situations. The results show that infants in this study have 11 gestures, of which many are learned within the context of routines or games. Referential gestures were rarely reported. There was no positive correlation between the number of gestures and the number of expressive words. However, more qualitative measures on early communicative behaviors show that there was a positive correlation between "frequent use of gestures" and "try to communicate by verbal means".

Association between breastfeeding and early childhood caries: analysis of National Health Insurance Corporation's oral examination data for infants and toddlers (모유수유와 유아기 우식증과의 관련성: 국민건강보험공단 영유아 구강검진 자료 분석)

  • Objectives: The aim of this study was to investigate the effect of breastfeeding on the occurrence of early childhood caries in Korean infants and toddlers. Methods: Data on oral examinations of infants and toddlers of the National Health Insurance Service were analyzed. The study subjects were children who participated in both the first, second, and third oral examinations and the first general health examination in 2008-2017 (n=142,185). Based on the responses to the questionnaire, the subjects were classified into breastfeeding, formula feeding, and mixed feeding groups. The participants were monitored for the development of early childhood caries in three sequential oral examinations. Results: Based on the oral examination results conducted at 54-65 months old, the decayed-filled teeth index of the breastfeeding group was the highest (2.03±3.08), followed by the mixed (1.96±3.03) and the formula feeding groups (1.82±2.91). The Cox proportional hazard regression model including all the variables showed that the risk of developing dental caries was significantly lower in the formula (hazard ratio [HR], 0.85) and mixed feeding groups (HR, 0.91) than in the breastfeeding group. Conclusions: Breastfeeding children have a higher risk of early childhood caries; therefore, oral hygiene education and regular dental check-ups are necessary.

  • This study is a study on the perceptions of pre-primary early childhood teachers on teaching and learning methods for infants and toddlers in the early childhood teacher training process. This is a mixed study using word cloud analysis and qualitative case analysis on the subject, focusing on literature research and understanding of pre-primary early childhood teachers' 'teaching and learning methods for infants and toddlers'. The purpose of this study was to find out the meaning of a early childhood teacher through thoughts on teaching and learning methods for infants, difficulties, points to be learned, teaching competency to be good as a teacher, and experiences for teaching professionalism. Through the results of this study, it is expected to find a way to increase their sense of efficacy on teaching and learning methods when conducting classes for young children in the future, and to provide basic data for improving the quality of early childhood education.

The Effects of the Early Intervention of V{\ddot{o}}jta Therapy for the Infant with Central Coordination Disturbance (영유아 뇌성 운동발달 장애아에 대한 보이타치료법 조기적용의 효과)

  • The purpose of this study was to examine the effect of treatment when the V{\ddot{o}}jta therapy applied to the infant with central coordination disturbance in early stage. The subject were 10 cases : each one with the V{\ddot{o}}jta therapy applied to the group of the infants before 6 months old and the group of the infants over 6 months old. When the V{\ddot{o}}jta therapy was applied I analized the papper which was written about the infants who were diagnosed as moderate CCD and divided two groups which were the infants before 6 months old and the infants over 6 months old. These collected data were analyzed by using t-test. The results of study were as follow; 1. When I compared the locomotion stage after the V{\ddot{o}}jta therapy. t-test showed significant differences. The group of infants before 6 months old had the result 7 to over stage 7 and the group of infants over 6months old had the result 2(p<.05). 2. When I compared the result of the postural reaction after the V{\ddot{o}}jta therapy, 1-test showed significant differences. The group of infants before 6 months old had 6 cases was normalizes and the group of infants over 6 months old had 1 case was normalized(p<.05). 3. When 1 compared the duration of the V{\ddot{o}}jta therapy between the group of infants before 6 months old and the group of infants over 6 months old. t-test showed significant differences. The group of the infants before 6 months old had 5 cases for 1-6 months and 4 cases for 7-15 months, 3 cases for 16-24 months and the group of infants over 6 months old had 6 cases for 7-15 months and cases for 16-24 months, 1 case for 25-30 months(p<.01).
