한의치료를 통해 인지 장애, 배뇨 장애, 보행 장애에 개선을 보인 뇌의 신생물 및 신생물 주변의 뇌실내출혈을 동반한 뇌내출혈 환자 한방 치험 1례에 대한 증례 보고 (Case Report of Intracerebral Hemorrhage with Intraventricular Hemorrhage around Neoplasms and Neoplasms in the Brain in a Patient with Cognitive Impairment, Dysuria, and Gait Disturbance Improved by Korean Medicine)

  • Objectives: The purpose of this clinical case study was to report the effect of Korean medicine treatment on a stroke patient with cognitive impairment, dysuria, and gait disturbance. Methods: The patient was treated with Korean medicine, including herbal medicine, acupuncture, moxibustion, and cupping. We administered the Korean version of the Mini-Mental Status Examination (MMSE-K) Global Deterioration Scale (GDS) and Manual Muscle Test (MMT) and investigated the frequency of nocturia to evaluate the effectiveness of the treatment and improvement in symptoms. Results: After Korean medicine treatment, the patient's symptoms were significantly improved. according to the MMSE-K, frequency of nocturia, and MMT results. Conclusions: This clinical case study suggests that Korean medicine treatment, including Wuchasinkihwan-gagam could be an effective treatment for stroke patients with symptoms of cognitive impairment, dysuria, and gait disturbance.

  • A 9 year-old male Maltese with dysuria, pollakiuria, and stranguria was brought to the Veterinary Teaching Hospital, Chonnam National University. A right lateral abdominal radiograph was shown that 3 uroliths existed at the caudal aspect of the os penis and 5 uroliths existed in the passage of os penis. Uroliths were extracted by voiding urohydropropulsion and cystotomy in Maltese dog. The complication of cystotomy was hematuria, which might persist up to 2 days postoperatively but dysuria was not observed. So we applied voiding urohydropropulsion and cystotomy, and got a good prognosis. Gross examination of extracted uroliths were separated one from another but it was identified the same componet of calcium oxalate by stone quantitative analysis.

  • Ileovesical fistula is a rare condition in children. A case of 13 year-old female with ileovesical fistula caused by an ingested foreign material is presented. She had dysuria and lower abdominal pain for one month. There was no history of medico-surgical illness such as Crohn's disease or diverticulitis. Preoperative imaging study showed a movable calcified object in the pelvic cavity and air bubbles in the bladder. At laparotomy a bezoar-like mass was found at the antimesenteric border of the terminal ileum adherent to the dome of bladder. Segmental resection of the ileum and partial cystectomy were performed.

위축신(萎縮腎)환자 한방치료 후 배뇨장애 호전 1례 (A Clinical Case Report of Renal Atrophy Patient with Dysuria)

  • Object : This study was designed to evaluate the effects of oriental medicine therapy on a renal atrophy Patient. Methods : The patient was hospitalized from July 31. 2009 to Aug 27. 2009. Patient was treated with herbal medicine, acupuncture and moxa therapy. Result & Conclusion : After the treatment, the symptoms and IPSS, DITI results improved. From These results suggest that oriental medicine is an effective treatment for renal atrophy Patient. But more clinical case reports are needed.

홍의약침 단일요법이 여성 하부요로증상에 미치는 임상적 효과에 대한 후향적 설문조사 연구 (A Retrospective Questionnaire Survey on the Clinical Effects of HongYi Pharmacopuncture Monotherapy on Female Lower Urinary Tract Symptoms)

  • Objectives: The purpose of this study was to investigate the clinical effects of HongYi pharmacopuncture monotherapy on female voiding dysfunction. Methods: Korean medical practitioners who used HongYi pharmacopuncture to treat female patients complaining of dysuria were surveyed. They performed a retrospective chart review of 31 female patients who visited their Korean medical clinic for dysuria. General characteristics, marital status, urologic medical history, International Prostate Symptom Score (IPSS), International Consultation on Incontinence Modular Questionnaire-Female Lower Urinary Tract Symptoms (ICIQ-FLUTS), and related adverse events were examined. Results: Of 31 cases received, 29 were selected for analysis. Two cases were excluded because treatment was not continued through four weeks. In all cases, IPSS and ICIQ-FLUTS scores were significantly decreased after treatment. Adverse reactions occurred during treatment in four cases. Conclusions: These results suggest that treatment by HongYi pharmacopuncture is effective in improving lower urinary tract symptoms in women. Further studies will be needed for evaluation of clinical responses, to evaluate the safety and efficacy of HongYi pharmacopuncture treatment for female patients complaining of dysuria.

마미증후군 환자의 침, 봉약침, 한약 복합치료 1례 (A Case of Cauda Equina Syndrome Cared with Acupuncture, Sweet Bee Venom Pharmacopuncture, Herbal Medicine Combined Treatment)

  • Objectives : This study was done to determine the effect of Korean medical treatment to patient diagnosed as cauda equina syndrome(CES) suffering from both lower limb dysesthesia, saddle dysesthesia, buttock dysesthesia, dysuria, dyschezia and gait disturbance. Methods : A patient with CES was treated with acupuncture, sweet bee venom pharmacopuncture, herbal medicine from February 2 to April 9 at Department of Acupuncture & Moxibustion, Daejeon Korean Hospital of Daejeon University. Patient received acupuncture treatment($CV_1$, $CV_3$, $CV_4$, $CV_6$, $ST_{36}$, $BL_{31}$, $BL_{32}$, $BL_{33}$, $BL_{34}$) 12 times a week, bee-venom pharmacopuncture($BL_{23}$, $BL_{28}$) was conducted twice a week, herbal medicine(Samilsingihwan decoction) was applied to patient thrice a day for nine weeks. The following symptoms were observed saddle, and lower limb dysesthesia, dysuria, dyschezia and gait disturbance. Results : After treatment, dysesthesia, dysuria, dyschezia and gaiting developed good outcome. But anal reflex remained still. Conclusions : Acupuncture, bee-venom pharmacopuncture, herbal medicine combined treatment might be effective to patients with CES.

  • There's a case of a 74-year-old male patient who was recommended urethral catheterization because of voiding problem which is diagnosed as diabetic cystopathy. According to the Differentiation of Symptoms(辨證), we classified his situation as kidney yang deficiency (腎陽虛衰), prescribed Uchasingi-hwan(牛車腎氣丸). and treated with moxibustion and electroacupuncture. After nine days of such treatments, he started voiding himself without catheterization, and the volume of residual urine decreased. So we report this case.

관상동맥 조영술 후 불편감에 영향을 미치는 요인 (Factors Predicting Patient Discomfort after Coronary Angiography)

  • Purpose: The purpose of this study was to identify the factors that predict discomfort after coronary angiography or percutaneous coronary intervention (PCI) among hospitalized patients. Methods: A total of 203 patients who underwent coronary angiography or PCI were recruited from C hospital located in S city, J province, from June through August 2008. The level of discomfort was measured and standardized by two instruments, discomfort questionnaire and the Visual Analogue Scale (VAS). Results: Stepwise multiple regression showed that the factors predicting the level of discomfort were type of angiography, gender, previous angiography, dysuria, pre-information, and sleep satisfaction, which together explained 30.6% of the total variance of the level of discomfort. Conclusion: Patients who had previous experience with these procedures, received a pre-information about the upcoming procedure, had no dysuria, and had slept well after the procedure were less likely to complain discomfort. Pre-informed education should be given by nurses to patients who will have an angiography or PCI to reduce their physical and emotional discomforts.

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine on a post-laparoscopic myomectomy patient's lower urinary symptoms (LUTS). Methods: The patient who diagnosed with uterine myoma had Laparoscopic myomectomy. After the operation, the patient complained of dysuria, nocturia, frequent urination, urgent urination and urinary retention. The patient received combination treatment of Korean medicine during 14 days of hospitalization. The treatment included Ohrim-san, acupuncture, moxibustion, and pharmacoacupuncture. The effects were evaluated through the Numeric Rating Scale (NRS), Overactive Bladder Symptom Score (OABSS), Frequency-Volume chart, and 5-Level version of EuroQol-Five Dimension (EQ-5D-5L). Results: After the treatment, the clinical symptoms such as dysuria, nocturia, frequent urination, urgent urination, urinary retention were improved. Also, the quality of life was enhanced. Conclusions: This case report shows that the combination treatment of Korean Medicine with Ohrim-san may be effective for treating lower urinary tract symptoms (LUTS) after laparoscopic myomectomy.

후복강에서 기원한 비혈관성 연부조직 육종 증례 (Soft Tissue Sarcomas Originating from Retroperitoneal Cavity in a Maltese Dog)

  • An 11-year-old spayed female maltese was presented for abdominal distention, dysuria and dyschezia. Panting and heart murmur was found and abdominal palpation was difficult due to increased abdominal pressure. A soft tissue mass, $6{\times}3cm$ in size, was identified radiographically in pelvic canal, displacing the descending colon to the medioventral direction and the urinary bladder cranially. On ultrasonography, the mass consisted of homogeneous hypoechoic parenchyma containing the focal hyperechoic region ($1.6{\times}1.5cm$). The mass had distinct margin and no connection with adjacent organs. It was considered as a mass originating from the retroperitoneal cavity. Additional diagnostic procedures were not advanced because of the owner's request, and only a surgical excision of the mass was performed to alleviate the dysuria and dyschezia. Histopathologic examination and immunohistochemistry determined the mass as a soft tissue sarcoma and excluded hemangiosarcoma and osteosarcoma, both are the most common types of the retroperitoneal tumors. This report described non-vascular soft tissue sarcoma originating from the retroperitoneal cavity in a maltese dog.
