A Case of a Dieulafoy Lesion Treated using Coil Embolization in a Child (코일 색전술로 치료한 소아 Dieulafoy 병변 1예)

  • A dieulafoy lesion, which is an unusual cause of gastrointestinal bleeding that can be fatal in children. Dieulafoy lesions are characterized by an abnormally large eroded submucosal artery that is commonly located in the lesser curvature of the proximal stomach. In most cases, permanent hemostasis is achieved by endoscopic epinephrine injection, however, some patients require other endoscopic treatment modalities, embolization or surgery. We report here a case of a Dieulafoy lesion in an 11-year-old boy who had recurrent bleeding from the lesion in the duodenal bulb after endoscopic epinephrine injection and surgical ligation, that was successfully treated using coil embolization.

A Study on the Optimal Amino Acid Pattern at the Proximal Duodenum in Growing Sheep

  • Nine crossbred castrated lambs fitted with rumen and duodenum cannula and fed a diet of hay and concentrate formulated with ground corn and soybean meal as main ingredients were used to assess the duodenal ideal amino acid pattern. Three synthetic amino acid mixtures with different profile of essential amino acids were duodenally infused in order to get three different amino acid patterns flowing into the duodenum. The mixtures were designed to have similar amino acid profile as rumen microbial protein (Pm), casein (Pc) and modified muscle amino acid (Pmm). Results showed a lower urine nitrogen excretion (p=0.05), a higher nitrogen retention (p=0.04) and bodyweight gain with treatment Pmm. The modified muscle amino acid pattern also promoted a lower ratio of Gly to other amino acids in plasma (Gly/OAA) and a higher RNA and RNA/DNA concentration in the liver of the sheep. Meanwhile, the urea concentration in plasma was reduced and the insulin concentration was increased with Pmm treatment. No differences in glucose and growth hormone concentration in plasma were found among three treatments. All results obtained indicate that the modified muscle amino acid pattern (Lys 100%, Met+Cys 39%, Thr 76%, His 41%, Arg 72%, Leu 158%, Ile 81%, Val 105%, Phe 81% and Trp 13%) was the best for growing sheep.

  • Background: The complex anatomy of the pancreaticobiliary duct was crucial in management of pancreatic and biliary tract disease. Materials and Methods: Fresh specimens of pancreas, common bile duct (CBD), and duodenum were obtained en bloc from autopsies of 160 patients. Results: Ninety-three male and 67 female patients were included. The length of the pancreas ranged from 9.8-20 cm (mean, 16.20 +/- 1.70 cm). The intrapancreatic portion of the CBD showed patterns of three types: most common (85.30%) was type A, in which the anterior surface of the common bile duct was totally covered, while its posterior surface was partially covered, by the pancreatic parenchyma. On dissection of the accessory duct of Santorini, the accessory duct was traceable to the duodenal wall in 67.6%. The anatomy of the Wirsung-choledochus confluence was grouped into five different types. The common channel was found in 75.60% and its length varied from just a common junction (so-called "V-type" anatomy) to 15 mm (Y-type-b). Separate papillae (so-called "II-type") were found in 15.3% of specimens. Conclusions: Several important points regarding the anatomy of the pancreaticobiliary junction and pancreatic ductal system were illustrated in this study.

  • Peptic ulcer disease complicated with gastric outlet obstruction is rare in children. Even though NSAIDs have been reported to cause various adverse events, they are still regarded as safe and, therefore, widely utilized in children. In the past, pediatric patients who were at risk of seizure due to high fever, were treated with dipyrone (Metamizole sodium) injection which inhibits cyclooxygenase-1 and inhibit prostaglandin, to weaken the gastro-duodenal defensive mechanism. A case of an infant with multiple esophagogatroduodenal ulcers and bleeding caused by NSAID complicated with gastric outlet obstruction is reported in this paper.

Isolated Common Hepatic Duct Injury after Blunt Abdominal Trauma

  • Extrahepatic bile duct injury is commonly associated with hepatic, duodenal, or pancreatic injuries, and isolated extrahepatic bile duct injury is rare. We report a patient who presented with an isolated extrahepatic bile duct injury after blunt trauma. A 50-year-old man was referred to our hospital after having suffered a fall down injury. His laboratory findings showed hyperbiliribinemia with elevated aspartate aminotransferase and alanine aminotransferase level. Initial abdominal computed tomography (CT) showed a mild degree of hemoperitoneum without evidence of abdominal solid organ injury. On the 3rd day of hospitalization, the patient complained of dyspnea and severe abdominal discomfort. Follow-up abdominal CT showed no significant interval change. Owing to the patient's condition, Emergency laparotomy revealed a large amount of bile-containing fluid collection and about 1 cm in size laceration on the left lateral side of the common hepatic duct. Primary repair of the injured bile duct with T-tube insertion was performed On postoperative day (POD) 30, endoscopic retrograde cholangiopancreatography showed minimal bile leakage and endoscopic sphincteroplasty and endoscopic retrograde biliary drainage were performed. On POD 61, the T-tube was removed and the patient was discharged.

  • This 54-year-old Korean coal miner soBered from continuous watery diarrhea and weight loss after corticosteroid treatment(${\beta}-methasone$, 4 mg daily for 1 week) due to hip-bone fracture in January 1991. Except for the short therapy f steroid, no other histories were contributory. The malabsorption syndrome was aggravated while the case was treated under the impression of amebiasis or intestinal tuberculosis. AIDS antibody test by EIA was negative and quantitative analysis of serum immunoglobulins was in normal ranges. Nine months after the onset of symptoms, the case was diagnosed as malabsorption syndrome caused by complected and aggravated infection by Strengyleides stercoralis, Isospora and cytomegalovirus in the small intestine, which were proved by stool examination and duodenal biopsy. His clinical course became worse even after high-dosaged and prolonged albendazole treatment for strongyloidiasis with supportive quid therapy. The patient was discharged in hopeless status in November, 1991 and died after one week at home.

A human case of Echinostoma hortense (Trematoda: Echinostomatidae) infection diagnosed by gastroduodenal endoscopy in Korea

  • A human Echinostoma hortense infection was diagnosed by gastroduodenoscopy. An 81-year-old Korean male, living in Yeongcheon-shi, Gyeongsangbuk-do and with epigastric discomfort of several days duration, was subjected to upper gastrointestinal endoscopy. He was in the habit of eating fresh water fish. Two live worms were found in the duodenal bulb area and were removed using an endoscopic forcep. Based on their morphological characteristics, the worms were identified as E. hortense. The patient was treated with praziquantel 10 mg/kg as a single dose. The source of the infection in this case remains unclear, but the fresh water fish consumed, including the loach, may have been the source. This is the second case of E. hortense infection diagnosed by endoscopy in Korea.

  • Oral rehydration solution (ORS) is safe and effective for the prevention and treatment of dehydration in children. It has been commercially available as a small packaging unit that needs to be taken with a specified amount of water. Intake of incorrectly formulated ORS results in side effects, such as electrolyte imbalance and upper gastrointestinal (GI) disturbance. We experienced a case of severe GI hemorrhage from gastric and duodenal ulcers in a previously healthy child following intake of incorrectly formulated ORS. GI hemorrhage in children is often life threatening and reaching a diagnosis may be challenging. Commercially manufactured packets of powdered oral rehydration salts have been widely used and GI hemorrhage associated with an improperly diluted ORS has been rarely reported. Caution and education for proper preparation of ORS are imperative.

  • A 12-years-old Maltese (castrated male) dog with jaundice and purulent discharge from the shoulder was referred to the Veterinary Medical Teaching Hospital of Chungnam National University. A physical and laboratory examination revealed that the mucosa of the conjunctiva and the sclera were icteric, and the liver functional enzyme level and the total bilirubin level were increased. Duodenal caudal deviation caused by suspected masses in diameter of about 2 cm was shown by radiographic and ultrasonographic examinations. Using fine needle aspiration (FNA) and computed tomography (CT), the dog was diagnosed with alimentary lymphoma. The dog was treated with chemotherapy. Hematuria was discovered and dysuria occurred because of sterile hemorrhagic cystitis, which is a unique complication resulting from the use of cyclophosphamide.

Chongkukjang Mucilage Stimulates Immunohistochemical Activities of Gastrointestinal Tract in Rats

  • We investigated the effect of a viscous substance from chongkukjang (chongkukjang mucilage) on immunohistochemical reactions in rat gastrointestinal (GI) tracts, Rats fed a steady diet of chongkukjang mucilage showed an increase in the immunoreactive densities of gastrin and serotonin in the pyloric region of their stomachs and duodenal villi, The number of gastrin and serotonin immunoreactive cells was significantly higher in the experimental group than in the control group. Feeding on dietary chongkukjang mucilage increased the immunohistochemical densities of $CD4^+$ and $CD8^+$ lymphocytes in the mucosa and submucosa of the rats' gastroduodenal region. The universal nitric oxide synthase (uNOS)-immunoreactive neurons and nerve fibers were strongly stained in the vascular walls of the submucosa and myenteric plexus in rats fed the test diet. The results indicate that the intake of chongkukjang mucilage could increase mucosal immune activity, gastrointestinal motility, and blood circulation in the GI tract.
