Analysis on the Dermatosrugical Prescriptions in BangYakHapPyun(方藥合編) (方藥合編 皮膚外科 處方에 대한 分析)

  • Subjects : We tried to analysis dermatosurgical prescriptions including 477 WonBang(元方) prescriptions for SangJungHaTong(上中下統) introduced by HwangDoYeon(黃道淵). Methods : Prescriptions in BangYakHapPyun(方藥合編) is generally categorized into SangTong(上統), JungTong(中統), HaTong(下統) which are called PoJe(補劑), HwaJe(和劑), KongJe(功劑) respectively. This study classified and analyzed major diseases and symptoms appeared in dermatosurgical prescription and composition of medicine, as well as in BangYakHapPyun(方藥合編). Results and conclusions : The results of examining dermatosurgical prescriptions in WonBang(元方) of SangJungHaTong(上中下統) in BangYakHapPyun(方藥合編) are as follows; 1. The proportion of dematosurgical prescriptions was SangTong(上統) $\frac{10}{126}$(7.9$\%$). JungTong(中統) $\frac{22}{181}$(12.1$\%$), and HaTong(下統) $\frac{16}{163}$(9.8$\%$), which means that JungTong(中統)(HwaJe 和劑) takes up relatively the largest portion. 2. As for SangTong(上統), upper level herbs used in medicine are Glycyrrhiza uralensis(甘草), Paeonia japonica(白芍藥), Angelica gigas(當歸). Astragalus membranaceus(황기). Ginseng(人蔘), Poria cocos(복령), Atractylodis macrocephalae rhizoma(白朮). Cinnamon(肉桂), Rehmaniniae radix preparat(熱地黃). And these herbs are the components of Sipjundaebo-tang(十全大補湯), one of the most well-known medicine for weak energy and blood(補氣血). 3. As for JungTong(中統), in addition to medicine for weak energy and blood. Ledebouriella seseloides(防風) that removes ill elements on skin surface and Pung(風) called "wind". Limonium tetragonum(桔梗) that eliminates discharges and sputum, Angelica dahurica(白芷) that removes discharge and suppress tumor are applied. Other herbs are Ostericum koreanum(羌活). Skullcap(황령),Schizonepeta tenuifolia(荊芥), Aurantii fructus(地殼), Cimicifuga heracleifolia(升麻), Bupleurum falcatum(柴胡), Lonicerae flos(金銀花). These herbs are more effective for wind-calming treatment. cooling down fever, clearing skin irritation, detoxication. removal of tumor and discharge than replenishing energy and blood. 4. As for HaTong(下統), Angelica gigas(當歸) and Ledebouriella seseloides(防風), the two major herbs for SangTong(上統) and JungTong(中統), are mostly used. In addition, Skullcap(黃芩), Gardenia jasminoides(梔子), Eisenia bicyclis(大黃) are other major components and their key efficacy is to lower fever and KongHa(功下). 5. Herbs applied for SangTong(上統), JungTong(中統), and HaTong(下統) in large quantity are Glycyrrhiza uralensis(甘草) that harmoniously combine different herbal elements and Poria cocos(복령) that discharges humidity and watery elements out of body, removes humid and hot elements, and strengthen gastrointestinal system. Based on this, it is inferred that prescriptions for this study focus largely on treatment of humid and hot elements. In the composition of this prescription, Angelica gigas(當歸), Paeonia japonica(白芍藥), and Cnidium officinale(川芎) are taking up relatively large proportion, which are basic herbs for Samul-tang(四物湯). Therefore, it is incurred here that the concept of "replenishing blood" bears importance in dermatosurgical treatment. 6. As for herb medicines used for more than two types of prescriptions of SangTong(上統), JungTong(中統), and HaTong(下統), most of them are simultaneously used for SangTong(上統) and JungTong(中統), or for JungTong(中統), and HaTong(下統) except for Atractylodis macrocephalae rhizoma(白朮) and Gleditsia sinensis(조각자). This finding implies that prescription or treatment that are simultaneously applied are replenishing and harmonizing, or harmonizing and attacking while replenishing and attacking never go together.

  • Objectives: This study was carried out to analyze and understand the trends of research papers published In the Journal of Oriental Medical Surgery?phthalmology & Otolaryngology Society(JOMSOOS). Methods: We studied the 308 research papers that had been published in JOMSOOS from 1988 Vol. 1. No. 1. to 2002 Vol. 15. No. 2. Our study was developed through the four stages in order to analyze the papers; 1) we analyzed all the papers overall to classify them into three categories; original article. review article and case report. 2) we classified the original articles in terms of methodology. 3) we also analyzed the case reports according to the sort of disease each paper dealt with. 4) we had another statistical approach to each paper to figure out the distribution of diagnoses in detail. Results: We have got the following outcomes from our analysis of the papers in terms of the four stages. 1. Overall Analysis. 1) Classification of 308 research papers between 1988 and 2002: 137 original articles(44.48$\%$), 111 review articles(36.04$\%$), 56 case reports(18.18$\%$). 2) Used language: Korean(99.03$\%$). English(0.97$\%$). 3) The Number of Authors: 2 persons(42.86$\%$). 3 persons(29.87$\%$), 1 person(14.61$\%$). 2. Original Article Analysis 1) Classification of 137 original articles in terms of methodology: 90 experimental studies(65.69$\%$)46 descriptive studies(33.58$\%$), 1 analytic study(0.73$\%$). 2) Classification of the original articles according to the use of statistical methods: No statistical methods(36.42$\%$), Descriptive methods only(1.99$\%$), Not defined(23.18$\%$), t-test(24.50$\%$), ANOVA(3.97$\%$), Multiple comparison(2.65$\%$), Non-parametric test(2.65$\%$), Other methods(1.32$\%$). 3) Classification of 46 descriptive articles in terms of diseases: otorhinolaryngology(43.48$\%$), dermatology(23.91$\%$), ophthalmology(13.04$\%$), facial palsy(13.04$\%$). 4) Classification of descriptive articles in terms of the number of patients: the highest was 'more than 26 but less than 50 persons'(19 articles - 41.30$\%$). 5) Classification of descriptive articles in terms of the period for patients observation: the highest was the time 'more than 9 but less than 12 months(34.78$\%$)' Out of the 34.78$\%$, the number of articles with the patients observed for more than 12 months was 13(28.26$\%$). 3. Case Report Analysis 1) Classification of 56 case reports in terms of the sort of disease: dermatology(44.64$\%$), ophthalmology(19.64$\%$), otorhinolaryngology(14.29$\%$), facial palsy(8.93$\%$). 2) Classification in terms of the number of patients: 1 person(50$\%$), 3 persons(16.07$\%$), 2 persons(14.29$\%$). 4. Diagnosis Distribution of Each Disease. 1) Studies regarding ophthalmology : the percentage of 'strabismus' cases was the highest(33.33$\%$). 2) Studies regarding otorhinolaryngology : nasal inflammation(34.48$\%$), tinnitus(20.69$\%$). 3) Studies regarding dermatology: the percentage of 'allergic skin disease' was the highest(33.33$\%$). Conclusions: We analyzed the trends of research papers that have been published in JOMSOOS in detail. We came to understand the trends of the research through this study. However, we acknowledge that we only adopted the quantitative method out of various possible analysis methods. For further studies, we strongly urge to adopt the qualitative methods as well.

Protective Effect of Green Tea Extract on Amyloid $\beta$ peptide-induced Neurotoxicity (아밀로이드베타 펩타이드 유도성 신경세포독성에 대한 녹차 추출물의 보호 효과)

  • Amyloid $\beta$ peptide ($A{\beta}$) is known to increase oxidative stress in nerve cells, leading to apoptosis that is characterized by free radical formation and lipid peroxidation. Neurodegenerative diseases such as Alzheimer's disease (AD) are characterized by large deposits of $A{\beta}$ in the brain. In our study, neuronal protective effects of green tea, along with water activity (0.813), and leaf storage periods (fresh leaf, or leaf stored for up to 4 weeks) were investigated. We measured protective effects against $A{\beta}$-induced cytotoxicity in neuron-like PC12 cells. Powdered green tea was extracted with distilled water at $70^{\circ}C$ for 5 min, and this extract was freeze-dried and stored at $-20^{\circ}C$ until use. In cell viability assays using 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT), the fresh extract, and that obtained after 1 week of leaf storage, showed the best protective effects against $A{\beta}$-induced neurotoxicity. As oxidative stress causes membrane breakdown, the protective effect of green tea extracts was investigated using lactate dehydrogenase (LDH) and trypan blue exclusion assays. LDH release into the medium was inhibited (by 20-25%) in all tests. In addition, all green tea extracts (fresh, or stored before extraction for up to 4 weeks) showed better cell protective effects ($93.3{\pm}1.8-96.2{\pm}2.4$) than did vitamin C ($91.0{\pm}1.6$), used as a positive control. The results suggest that effectiveness of green tea extracts falls with prolonged leaf storage.

Physicochemical Comparison of Two Different Shark Meats Used for Preparation of Dombaeki (돔배기용 상어육의 이화학적 성분 비교)

  • Dombaeki, a traditional salted shark meat, has been widely used as a customary religious food in the Daegu-Gyeongbuk area of Korea. Two different sharks, Sphyrna zygaena (SZ) and Isurus oxyrinchus (IO) are traditionally used to prepare Dombaeki. Chemical components, lipid classes, fatty acid levels, and sterol compositions of meats prepared from the two sharks were investigated. There were no significant differences in chemical composition between the two dried shark meats. Major amino acids in shark meat were leucine, lysine, arginine, proline, isoleucine, valine, phenylalanine, glutamic acid, and asparagine, which together accounted for about 40% of total amino acids. Levels of amino acids in IO meat were higher than in SZ meat. Major fatty acids in the two shark meats were palmitic ($C_{16:0}$), stearic ($C_{18:0}$), oleic ($C_{18:1}$), arachidonic ($C_{20:4}$), and docosahexanoic (DHA, $C_{22:6}$) acids, which accounted for about 80% of total fatty acids. Notably, IO meat showed higher amounts of DHA (31.8%) and eicosapentanoic acid (EPA, 1.8%) than did SZ meat. The two shark meats showed similar dry weight levels of total lipids, with triacylglycerols, free fatty acids, sterols, and phospholipidscomprising on average 5.0, 2.0, 13.0 and 63.0% of total lipids, respectively. The dominant classes of phospholipids were mainly phosphatidylcholine (PC) and phosphatidylethanolamine (PE). PC content in the two shark meats was higher than that of PE, although the differencewas not great. The major fatty acids in phospholipids were myristic, palmitoleic, stearic, and docosanoic acids. Total volatile basic nitrogen content and the pH of SZ meat were lower than those of IO meat, whereas the Hunter's 'a' and 'b' values of SZ meat were higher than those of IO meat. These results suggest that shark meat may be useful as a functional food to prevent several degenerative diseases.

Effects of Fructus and Semen from Rosa rugosa on Osteoimmune cells (해당화의 과육 및 종자 추출물이 골형성 관련 세포에 미치는 영향)

  • Rosa rugosa has been used as a folk medicine with various pharmacological properties for a long time in Asia. We investigated effects of fructus extracts of Rosa rugosa (RRF) and semen extracts of this herb (RRS) on bone forming cells (osteoblastic and pre-osteoclastic cells) to evaluate the pharmacological possibilities in a variety of bone-related disease. RRF showed significant effect on proliferation of osteoblastic cells in dose-dependent manners at 72 hrs and $100\;{\mu}g/m{\ell}$ of RRS was effective at 48 and 72 hrs. RRF and RRS did not decreased production of TNF-$\alpha$ but NO by pre-osteoclastic cells under inflammation circumstance indeced by LPS. We also investigated the effects of RRF and RRS on the mitogen-induced lymphocyte proliferation in the old and young mice in ex vivo systems. RRF and RRS significantly enhanced proliferative effects of untreated and ConA-treated splenocytes from the old and young mice. But, RRS at $500\;{\mu}g/m{\ell}$ increased LPS-induced TNF-$\alpha$ production in pre-osteoclastic cells and reduced LPS-stimulated lymphoblastogenesis in the old and young at $1000\;{\mu}g/m{\ell}$. These results indicate that RRF has beneficial effects on osteoarthritis and give further possibilities for the immunomodulating effects not only in old that has more frequent bone related diseases but also in young.

Disappearance of Hysteria(Conversion Disorder) and the Evolutionary Brain Discord Reaction Theory (히스테리아(전환장애)의 소실과 진화적 뇌신경 부조화 반응 가설)

  • Objectives : The author tried to find out reasons why and how hysteria(and conversion disorder) patient numbers, which were so prevalent even a few decades ago, have decreased and the phenotype of symptoms have changed. Methods : The number of visiting patients diagnosed with conversion disorder and their phenotype of symptoms were investigated through chart reviews in a psychiatric department of a University hospital for the last 12 years. Additionally, the characteristics of conversion disorder patients visiting the emergency room for last 2 years were also reviewed. Those results were compared with previous research results even if it seemed to be an indirect comparisons. The research relied on Briquet P. and Charcot JM's established factors of the vicissitudes of hysteria(and conversion disorder) which has been the framework for more than one hundred and fifty years since hysteria has been investigated. Results : The author found decreased numbers and changes of the phenotype of the hysteria patients(and conversion disorder) over the last several decades. The decreased numbers and changes of the symptoms of those seemed to be partly due to several issues. These issues include the development of the diagnostic techniques to identify organic causes of hysteria, repeated changes to the symptom descriptions and diagnostic classification, changes of the brain nervous functions in response to negative emotions, and the influence of human evolution. Conclusions : The author proposed that the evolutionary brain discord reaction theory explains the causes of disappearance of and changes to symptoms of hysteria(conversion disorder). Most patients with hysteria(conversion disorder) have been diagnosed in the neurological department. For providing more appropriate treatment and minimizing physical disabilities to those patients, psychiatrists should have a major role in cooperating not only with primary care physicians but with neurologists. The term 'hysteria' which had been used long ago should be revived and used as a term to describe diseases such as somatic symptom disorder, functional neurological symptoms, somatization, and somatoform disorders, all of which represent almost the same vague concept as hysteria.

Risk Factors of Socio-Demographic Variables to Depressive Symptoms and Suicidality in Elderly Who Live Alone at One Urban Region (일 도시지역의 독거노인에 있어서 우울증상 및 자살경향성에 영향을 미치는 인구학적 변인에 대한 고찰)

  • Objectives: To understand the risk factors of demographic data in geriatric depression scale, and suicidality among in elderly who live alone at one urban region. Methods:In 2009, 589 elderly who live alone(age${\geq}$65) were carried out a survey about several socio-demographic data, Korean version of the Geriatric Depression Scale(SGDS-K) and Suicidal Ideation Questionnaire (SIQ). Statistical analysis was performed for the collected data. Results: Mean age of elderly who live alone is 75.69(SD 6.17). 40.1% of participants uneducated, 31.4% graduate from elementary school, 12.9% graduate from high school, 11.7% graduate from middle school, 3.2% graduate from university. Religionless, having past history of depression or physical diseases, low subjective satisfaction of family situation, and not having any social group activity have significance to depressive symptoms of elderly who live alone. Having past history of depression, religionless, low subjective satisfaction of family situation have significance to suicidality. Especially, low subjective satisfaction of family situation and having past history of depression are powerful demographic factor both depressive symptoms and suicidality of elderly who live alone. Conclusions: When we take care elderly who live alone, we should consider many things, but especially the social support network such as family satisfaction and past history of depression for reducing or preventing their depression and suicide both elderly depression and suicide who live alone.

Effects of Schisandrae Fructus and Corni Fructus Extracts on the Proliferation and Expression of Prostatic Hyperplasia-inducing Factors in Dihydrotestosterone-stimulated LNCaP Human Prostate Carcinoma Cells (오미자와 산수유 추출물이 dihydrotestosterone가 처리된 LNCaP 인간 전립선 암세포의 증식 및 전립선 비대 유발 인자 발현에 미치는 영향)

  • Schisandrae Fructus (SF) and Corni Fructus (CF) have been used for a long time for the prevention and treatment of various diseases. Although reports have highlighted the possibility of inhibiting the onset and progression of benign prostatic hyperplasia (BPH), studies on related mechanisms are still lacking. In this study, we investigated the potential of SF and CF in improving BPH by using a dihydrotestosterone (DHT)-induced in vitro BPH model using LNCaP prostate carcinoma cells. According to our results, water and ethanol extracts of SF and CF significantly inhibited the proliferation of LNCaP cells by DHT treatment and markedly downregulated the expression of DHT-induced BPH biomarkers and growth factors. They also regulated the expression of apoptosis regulatory factors and significantly reduced DHT-mediated oxidative stress. In addition, the protective effect on major factors involved in the pathogenesis of BPH was more effective in the ethanol extract treatment group than in the water extract group. Furthermore, the improvement effect on BPH was higher in the 1:1 combined treatment group than in the ethanol extract alone treatment group of SF and CF, and 60% ethanol extracts showed a better effect than 40% ethanol extracts. Therefore, our findings demonstrate that SF and CF can protect against BPH by preventing the hyperproliferation of prostate cells through the inhibition of the androgen signaling pathway, which was correlated with their antioxidant activities. Therefore, SF and CF extracts may be useful in the clinical treatment of BPH, and the combination of these two extracts can be synergistic.

  • Due to the spread of Corona 19 around the world, Infectious Disease Medicine and New Medical Devices such as Diagnostic Agent are being rapidly developed and launched, and for the fast supply and demand of these, each country has eased import regulations or has implemented policies for fast approval(NIDS, 2020). On the other hand, New Developed Medical Devices that are not related to New Infectious Diseases, they are still entering the market through strict licensing and licensing regulations, such as delay and cancellation in the test inspection process, etc. Therefore, This Study specialized in the government-managed laws encountered when New Medical Devices enter the market, derive Factors influencing the Strength of Regulations, analyzes the Strength of Regulations, and proposes a Regulatory Response Framework. The Research Method was conducted by Literature Research, was applied by Failure Mode and Effects Analysis(FMEA) Method, Expert Interview(1st): Idea Collection, Expert Interview(2nd): Validation, and Priority through the Application Process of FMEA Method. A Method of Quantifying the Intensity of Regulation was proposed by multiplying the Impact of the Influencing Factors for each stage of regulation and the Burden Impact for each type of Regulatory Affairs to find the Importance of the Regulatory Factors and multiplying the Severity of the Regulatory Impact. The Implications are that major overseas countries and the Korean government are actively responding with Special Regulatory Policies and Mitigation Policies for fast licensing of New Developed Medical Devices in accordance with Corona 19. It is expected that the direction for improvement of regulations and measures to respond to regulations will be implemented so that a more proactive and preemptive response to the regulatory process of the licensing policy for New Devices can be achieved.

  • On November 17, 2019, an infectious disease with symptoms of pneumonia, called the Wuhan virus at the time, occurred in Wuhan, China. Since then, the name has been changed to COVID-19, and the virus has spread all over the world, and the WHO has declared the highest warning level for infectious diseases, "Pandemic". The coronavirus has also caused great confusion in South Korea. This resulted in large infected people.The first confirmed cases occurred on January 20, 2020, and the number of infected patients is steadily increasing after experiencing several waves, and many corona confirmed cases are also occurring in 2021 after the year. As the whole world enters a pandemic, walls are created between people and people, companies and businesses, and countries and countries, and all growth stops or declines, including human relationships, domestic companies and industries, and foreign industries. As a result, society in general is experiencing a lot of stagnation. Among them, small and medium-sized enterprises (SMEs), which are the basis of all growth in Korea, and youth who are trying to contribute to the national development by entering society, are struggling to find jobs. Even before the coronavirus outbreak, the difficulty of job hunting and the prospect of small and medium-sized businesses were not very good. In this situation, as the country's overall economic situation is poor, the vitality of SMEs has decreased a lot, the prospects are not good, so jobs are reduced, and there are many difficulties due to reluctance to hire new employees. In this study, with 2019 before the corona era and 2020 after the corona era, we compare SMEs before and after the corona era and overall job search and job search activities through average difference analysis, and whether they are affecting through correlation analysis. Through this, it suggests a direction to increase job search through corporate and government policies after raising the prospects of SMEs first.
