Factors that Affect on the Necessity of Establishment of Eco-friendly Hospital (친환경 병원 도입필요성에 영향을 미치는 요인)

  • The purpose of this research study is to analyze the influence on the necessity of establishment of eco-friendly hospital by evaluating hospital employees' awareness and their satisfaction level of working environment. Questionnaire investigation was conducted in April 2014 throughout five different medium-size hospitals in Seoul, Gyeonggi and Chungcheongbuk provinces. 213 sets of survey were used to conduct Frequency Analysis, T-test, Dispersion Analysis(ANOVA) and Structural Equation Modeling Analysis. The research concluded that employees with higher level of education, working position and longer term of working experience were more aware about Eco-friendly hospital. In addition, male, doctor, and employees with higher level of education and working position were more satisfied with their working environment. And male, employees with higher level of education and working position were more aware about necessity of establishment of eco-friendly hospital. According to the Structural Equation Modeling Analysis, the awareness of Eco-friendly hospital and the satisfaction level of working environment were the varying factors that influence the necessity of establishment of Eco-friendly hospital. Therefore, it is crucial for hospitals that are preparing for establishing Eco-friendly hospital to raise employees' level of understanding of Eco-friendly hospital and their working environment satisfaction level.

Convergence Research on Periodic Changes in the Quality Assessment of Surgical Prophylactic Antibiotics (수술 예방항생제 적정성 평가의 주기별 융합 변화 연구)

  • This study was conducted to provide the base line for the use of prophylactic antibiotics and the standard of antibiotic use and the evaluation based on analysis of current use, quality assessment and periodic changes of prophylactic antibiotics in a university hospital. We chose the year 2008, 2009, 2010, 2012, and 2014 as the study periods since the primary evaluation on the first year of implementation in 2007. For this study, healthcare benefit quality assessment data from the Health Insurance Review and Assessment Service was used. We analyzed the differences each year of treatment period; one category for initial treatment timing, three categories for selecting antibiotics, and two categories for the length of treatment from multiple evaluation indices of gastric surgeries, colorectal surgeries, and cholecystectomy. The analysis revealed the followings: Regarding length of antibiotics treatment, total days of treatment decreased down to 1.5, 1.5, and 0.6 days in the year 2014, from 13.5, 12.8, and 6.9 days in the year 2007, for gastric surgeries, colorectal surgeries, and cholecystectomy. Based on these outcomes, the efforts to devise efficient delivery and distribution of the recommendations or indices which medical staff could adhere for quality improvement of the prophylactic antibiotics would be necessary. Also implementing monitoring system to help the hospitals to acknowledge their own faults may be helpful.

Manchester coding of compressed binary clusters for reducing IoT healthcare device's digital data transfer time (IoT기반 헬스케어 의료기기의 디지털 데이터 전송시간 감소를 위한 압축 바이너리 클러스터의 맨체스터 코딩 전송)

  • This study's aim is for reducing big data transfer time of IoT healthcare devices by modulating digital bits into Manchester code including zero-voltage idle as information for secondary compressed binary cluster's compartment after two step compression of compressing binary data into primary and secondary binary compressed clusters for each binary clusters having compression benefit of 1 bit or 2 bits. Also this study proposed that as department information of compressed binary clusters, inserting idle signal into Manchester code will have benefit of reducing transfer time in case of compressing binary cluster into secondary compressed binary cluster by 2 bits, because in spite of cost of 1 clock idle, another 1 bit benefit can play a role of reducing 1 clock transfer time. Idle signal is also never consecutive because the signal is for compartment information between two adjacent secondary compressed binary cluster. Voltage transition on basic rule of Manchester code is remaining while inserting idle signal, so DC balance can be guaranteed. This study's simulation result said that even compressed binary data by another compression algorithms could be transferred faster by as much as about 12.6 percents if using this method.

Medical care utilization status and quality of life in diabetes mellitus patients (당뇨병환자의 의료이용 현황과 삶의 질)

  • This study aims to identify utilization status of medical care and factors to affect quality of life in diabetes mellitus patients. The research selected 6,146 adults aged over 50, who appeared with EQ-5D, from the Korean health panel. We analyzed states of medical care utilization using descriptive statistics. Multiple regression analysis was used to examine the main factors associated with quality of life in diabetes patients. The result shows that quality of life(EQ-5D) in diabetes is significantly associated with gender, age, insurance type, education level, household income, diabetes ambulatory medical cost and the number of ambulatory medical utilization for other diseases. Females, higher age, lower levels of education, medicaid, the lower household income, the higher diabetes ambulatory medical cost, and the higher the number of ambulatory medical utilization for other diseases were the significant factors of lower quality of life. Therefore, it is necessary to develop effective social programs and individualized approach to improve the quality of life in diabetes patients. In the future, these findings can be used as important data for health care policy and assessment.

Changes in the Training Conditions of Residents by Enforcement of Medical Residents Act (전공의법 시행에 따른 전공의 수련환경 변화)

  • The Medical Residents Act was enacted in December 2016 to protect the rights of residents, and to ensuring the safety of patients, and nurturing good medical human resources. This study analyzed the changes of training conditions according to the enforcement of The Medical Residents Act by comparing the results of two surveys conducted in 2015(1,793 Residents) and 2017(1,768 Residents). As a result, Residents worked over 80 hours per week on average('15=92.4h, '17=87.3h) and they worked twice as many times as 36 hours('15=89.4h, '17=70.1h). Female residents' leave before and after childbirth('15=78.5day, '17=82.2day), Preparation of Standard training contract('15=19.3%, '17=40.8%), Delivery of training contract('15=12.4%, '17=36.1%) did not comply with the regulations. The training conditions of the residents is directly related to the safety of patients and the public's health. National support is needed for the support of substitute workforces, fair training evaluation conditions and incentives based on the evaluation results, labor costs for residents and supervising medical specialists, and the cost of making training programs.

  • Opinion mining is a way to analyze the emotions in the text and is used to identify the emotional state of the author and to find out the opinions of the public. As you can analyze individual emotions through opinion mining, if you analyze the text by region, you can find out the emotional state you have in each region. The regional sentiment analysis can obtain information that could not be obtained from personal sentiment analysis, and if a certain area has emotions, it can understand the cause. For regional sentiment analysis, we need text data created by region, so we need to collect data through Twitter crawling. Therefore, this paper designs a Twitter data collection system for regional sentiment analysis. The client requests the tweet data of the specific region and time, and the server collects and transmits the requested tweet data from the client. Through the latitude and longitude values of the region, it collects the tweet data of the area, and it can manage the text by region and time through collected data. We expect efficient data collection and management for emotional analysis through the design of this system.

Factors affecting perception of marriage needs including health related variables : In 20-30s unmarried workers (건강관련 변수를 포함하여 결혼 필요성 인식 영향 요인 : 20-30대 미혼남녀 근로자를 대상으로)

  • The purpose of this study is to investigate the affecting factors on perception of marriage needs in 20-30s unmarried workers. This study used raw data from the survey on the actual social and economic conditions of the youth of 2016. Among the 2,534 subjects, 904 unmarried workers in their 20s and 30s were included in the analysis. Dependent variables were marriage needs, and independent variables were age, sex, height, weight, education level, monthly income, parents' economic level, and health-related variables were self-rate health, stress, regular exercise, and life satisfaction included. Statistical analysis was performed binominal logistic regression analysis. In results, there were no factors influencing perception of marriage needs in 20s men, and Education was a predictor in 20s women. In 30s men, the more regular exercise, the less the monthly income, the more positive the perception of marriage needs was. In the 30s women, the better the self-rate health, the more positive the perception of marriage needs was. These findings suggest that health may affect the marriage perception of 30s unmarried workers.

Trend Analysis of Medical Care Utilization among People with Autistic Spectrum Disorder Using National Health Insurance Data (자폐성장애인의 의료이용 경향분석 및 시사점 : 국민건강보험자료를 이용한 융복합적 접근)

  • The purpose of this study was to estimate the current status and trends of healthcare utilization among people with ASD. Using National Health Insurance open database, from 2010 to 2017. We analyzed the treatment prevalence for people with ASD, the pattern of healthcare utilization, the difference in medical care utilization according to age, and the type and location of main medical institutions. The main results of the study are as follows: First, the medical utilization has been continuously increasing from 2010 to 2017. The total amount of medical utilization is increased by 50% in 2017 compared to 2010, and the treatment prevalence was estimated to be 79.1% in 2017 and medical uses for the next three years is also increasing. Second, the pattern of medical care utilization varied widely according to age, especially after 20 years of age. Third, the types of medical institutions that were mainly used were 45.6% in the medical clinic and 35.9% in Seoul. The results of this study can be used as a minimum reference point of evaluating the effectiveness of government policy on future autistic disorders. However, further studies are required to increase the prevalence of treatment for autistic patients and to find out the difference in medical use according to age.

Effects of virtual reality training on upper extremity function and activities of daily living in patients with sub-acute stroke (가상현실 훈련이 아급성 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 효과)

  • The aim of this study was to investigate the effects of virtual reality training on upper extremity function and activities of daily living in patients with sub-acute stroke. The present study enrolled 18 patients with sub-acute stroke. All subjects were assigned into either the experimental group (n=9) or control group (n=9). Both groups received conventional occupational therapy for 30 minutes/day, 5 times a week, for 4 weeks. Additionally, the experimental group performed virtual reality training in each session for 30 minutes/day, and the control group conducted conventional occupational therapy in each session for 30 minutes/day. The outcome measures were performed through the Fugl-Meyer Assessment (FMA) and the Korean-modified Barthel Index (K-MBI) before and after intervention. In results, the experimental group showed significant improvements in the scores of FMA and K-MBI after intervention (p<.05). The control group showed significant improvements in the shoulder/elbow/forearm, wrist, and hand sub-domains of the FMA and K-MBI (p<.05). After intervention, the experimental group showed significantly greater improvements in the total score and in the wrist and hand sub-domains of the FMA than control group (p<.05). These findings suggest that virtual reality training may have positive effects on the improvements of upper extremity function in patients with sub-acute stroke.

Effect of tDCS on Cognitive Function of Patients With Stroke: A Systematic Review and Meta-Analysis (뇌졸중 후 인지장애에 대한 경두개 직류 자극: 체계적 고찰 및 메타분석)

  • Objective : This study aimed to analyze the effect of transcranial direct current stimulation (tDCS) on cognitive function recovery in patients with stroke. Methods : Data published in Korean and foreign academic journals from 2009 to 2019 were searched using the NDSL, RISS, PubMed, and CINAHL databases. A total of 11 experimental research articles were selected based on the inclusion and exclusion criteria. A qualitative assessment was conducted, and a meta-analysis of nine results from seven of the stuides was performed using the Comprehensive Meta-Analysis 3.0 program. Results : Based on the results of the meta-analysis, the attention and memory effect sizes were 0.725 and 0.796, respectively, which were both considered a "medium effect size". Statistically significant changes were observed in both the areas (p<0.05). Conclusion : The results of this study confirmed that tDCS can be useful in the rehabilitation of patients with stroke with limited cognitive function. In addition, the application methods differed, indicating that a formalized tDCS protocol is required.
