Epigenetic role of nuclear S6K1 in early adipogenesis

  • S6K1 is a key regulator of cell growth, cell size, and metabolism. Although the role of cytosolic S6K1 in cellular processes is well established, the function of S6K1 in the nucleus remains poorly understood. Our recent study has revealed that S6K1 is translocated into the nucleus upon adipogenic stimulus where it directly binds to and phosphorylates H2B at serine 36. Such phosphorylation promotes EZH2 recruitment and subsequent histone H3K27 trimethylation on the promoter of its target genes including Wnt6, Wnt10a, and Wnt10b, leading to repression of their expression. S6K1-mediated suppression of Wnt genes facilitates adipogenic differentiation through the expression of adipogenic transcription factors PPARγ and Cebpa. White adipose tissues from S6K1-deficient mice consistently exhibit marked reduction in H2BS36 phosphorylation (H2BS36p) and H3K27 trimethylation (H3K27me3), leading to enhanced expression of Wnt genes. In addition, expression levels of H2BS36p and H3K27me3 are highly elevated in white adipose tissues from mice fed on high-fat diet or from obese humans. These findings describe a novel role of S6K1 as a transcriptional regulator controlling an epigenetic network initiated by phosphorylation of H2B and trimethylation of H3, thus shutting off Wnt gene expression in early adipogenesis.

Association of Demographic and Socioeconomic Factors With Risk Factors for Chronic Kidney Disease

  • Objectives: The goal of this study was to examine the association of various demographic and socioeconomic factors with risk factors for chronic kidney disease (CKD). Methods: We used nationally representative pooled data from the Korea National Health and Nutrition Examination Survey (KNHANES), 2007-2013. We estimated the glomerular filtration rate (GFR) using the Modification of Diet in Renal Disease equation. We defined CKD as a GFR <60 mL/min/$1.73m^2$, and 1304 of the 45 208 individuals included in the KNHANES were found to have CKD by this definition. The outcome variable was whether individual subjects adhered to the CKD prevention and management guidelines recommended by the Korea Centers for Disease Control and Prevention. The guidelines suggest that individuals maintain a normal weight, abstain from alcohol consumption and smoking, manage diabetes and hypertension, and engage in regular exercise in order to prevent and manage CKD. Results: This study found that individuals with CKD were more likely to be obese and have hypertension or diabetes than individuals without CKD. In particular, male and less-educated CKD patients were less likely to adhere to the guidelines. Conclusions: Although the prevalence of CKD, as indicated by the KNHANES data, decreased from 2007 to 2013, the prevalence of most risk factors associated with CKD fluctuated over the same time period. Since a variety of demographic and socioeconomic factors are related to the successful implementation of guidelines for preventing and managing CKD, individually tailored prevention activities should be developed.

Convergence Exploration for Predictors of the Cardiovascular Disease Risk (한국 성인 남성의 심혈관질환 위험에 대한 예측 요인의 융복합적 탐색)

  • This study aimed to identify the risk factors for cardiovascular disease (CVD) among age groups using the Framingham risk score (FRS). The research design used was a cross sectional descriptive study using the Sixth Korean National Health and Nutrition Examination Survey from 2013-2015. Data from 5211 men, between the ages of 30-74 was analyzed. After adjusting for age, the result of logistic regression analysis showed that obesity (OR=2.51 95% CI=2.05-3.07), physical inactivity (OR=1.71, 95% CI=1.39-2.10), heavy alcohol drinking (OR=1.33, 95% CI=1.09-1.62), and dietary fiber intake (OR=0.99, 95% CI=0.98-0.99) were presented as predictors of CVD. Obesity was considered to be a particularly important predictor of CVD for young and middle-aged men. This result will be used for developing intervention relating to lifestyle modification for young and middle-aged men.

The Efficacy and Safety of Ephedra and Green Tea Mesotherapy on Localized Fat (피하지방 감량에 있어 경피침주요법의 유효성 및 안전성 평가)

  • Objectives : This study was performed to identify the efficacy and safety of ephedra and green tea mesotherapy on localized fat. Methods : Twenty-five healthy $20{\sim}35$ year overweight women (body mass $index{\geq}23kg/m^2$) were recruited and randomly assigned to two groups : experimental(mahuang and green tea) group (n=13), placebo(Normal saline) group (n=12). Total 8 times of herbarl mesotherapy treatment were performed once per week, 4 times were performed on right thigh (1st period), after wash-out for 1 week, the other 4 times were performed on left thigh (2nd period) without any modification of diet or exercise. Body composition, thigh circumferences were measured at each period before and after the intervention. Subcutaneous fat was measured by CT scan at before and after the 1st period of intervention and free fatty acid(FFA) was measured immediately before 1st intervention, 1 hour later, 24 hours later, and 7 days later. Satisfaction score and adverse events were evaluated. Results : In the subcutaneous fat, there were no significances, but in the circumferences. Experimental group had significant change when compared with placebo group (p<0.05). In FFA, experimental group has significant elevation at after 1 hour (p<0.05) evaluation. There were no serious adverse events in both groups. Conclusions : Experimental group had some significant findings in circumference, FFA, and had no serious adverse events comparing with placebo group, and no serious adverse events have been reported in both group. Nephropathy related to computed tomography in emergency department patients with serum creatinine <1.5 mg/dL (정상 혈청 크레아티닌을 가진 응급실 환자에서의 조영제 연관 신증)

  • Background: Contrast-induced nephropathy (CIN) can cause serious adverse effects. To reduce the occurrence of CIN related computed tomography (CT) in emergency patients, we assessed the respective roles of serum creatinine (SCr) alone and estimated glomerular filtration rate (eGFR) as an early predictor for CIN related CT. Methods: For patients with SCr <1.5 mg/dL who underwent CT in emergency department (ED) between September 2012 and October 2013, we assessed the prevalence of CIN and its adverse effects. The Modification of Diet in Renal Disease Study (MDRD) and Cockcroft-Gault (CG) formula was used for the calculation of eGFR. Practical calculation was performed by electronic medical record (EMR) system for MDRD and internet calculating service for CG. And we investigated the prevalence of CIN in eGFR $<60mL/min/1.73m^2$ before CT. Results: A total of 1,555 patients were enrolled. The prevalence of CIN after CT was 4.6% and it showed correlation with renal deterioration, increased in-hospital mortality, and prolonged hospitalization. Despite baseline SCr <1.5 mg/dL, among enrolled patients, 11.3% as MDRD equation and 29.5% as CG formula were $<60mL/min/1.73m^2$ and in this condition, the prevalence of CIN was significantly high (odds ratio was 2.87 [1.64-5.02] as MDRD equation and 2.03 [1.26-3.29] as CG formula). Conclusion: Just SCr <1.5mg/dL was not appropriate to recognize preexisting renal insufficiency, but eGFR using MDRD equation was useful in predicting the risk of CIN related CT in ED. Using EMR, calculation of eGFR can be easier and more convenient.

U-health Service Model for Managing Health of Chronic Patients in Multi-platform Environment (멀티플랫폼 환경의 만성 질환자 건강관리를 위한 유헬스 서비스 모델)

  • U-health services have been progressed as treatment and management for specific diseases and prevention services for providing the behavior management to customers according to the increase in chronic patients. The conventional U-health services provide required services and bio-information monitoring only through remote diagnoses and counsels and that represent limitations in preventing and managing metabolic syndrome patients like chronic patients. Thus, in this study a multi platform based U-health service model for managing the health of chronic patients is proposed. The multi-platform based U-health service model can provide continuous health information, diet, and exercise services regardless of the location of customers through PCs and smart phones. In addition, it is able to provide prescription services to doctors and nurses using a CDS (Clinical Decision Support) module based on clinical information. Doctors can identify the life pattern of patients through a behavior modification program and provide customized services to patients. The U-health service model provides effective services in multi-platform environments to customers and that will improve the health of chronic patients.

Evaluation of Renal Function Using the Level of Neutrophil Gelatinase-Associated Lipocalin is Not Predictive of Nephrotoxicity Associated with Cisplatin-Based Chemotherapy

  • Background: For early detection of renal damage during the usage of cisplatin based chemotherapy, changes in renal function should be monitored carefully. In recent years, neutrophil gelatinase-associated lipocalin, a small polypeptide molecule, has shown promise as a marker of acute renal failure. The aim of this present study was to assess possible risk prediction of cisplatin-induced nephrotoxicity using serum NGAL. Materials and Methods: A total of 34 consecutive patients with documented serum creatinine at least 24 hours before every cycle of cisplatin-based chemotherapy were included in the study. Demographic and medical data including age, performance status, tumor characteristics and comorbid diseases were collected from medical charts. Renal function was evaluated at least 48 hours before the treatment and at the end of the treatment based on the Modification of Diet in Renal Disease (MDRD) formula. Before and after cisplatin infusion serum NGAL levels were measured for the first and 3rd cycles of chemotherapy. Results: The median age of the study population was 54 (32-70) years. Fifteen patients (41.1%) were treated on an adjuvant basis, whereas 19 patients (58.9%) were treated for metastatic disease. There was no correlation of serum NGAL levels with serum creatinine (r=0.20, p=0.26) and MDRD (r=-0.12, p=0.50) and creatinine clearance-Cockcroft-Gault (r=-0.22, p=0.22) after cisplatin infusion at the end of the 3rd cycle of chemotherapy. Conclusions: In our study, serum NGAL levels were not correlated with the cisplatin induced nephrotoxicity. Further prospective studies are needed to conclude that serum NGAL level is not a good surrogate marker to predict early cisplatin induced nephrotoxicity.

A Study on Weight Control Attempt and Related Factors among College Female Students (여대생들의 체중조절 실태 및 관련 요인에 대한 연구)

  • The purpose of this study was to investigate the weight control attempts and related factors among 220 female college students in Seoul. Factors examined included body image, body satisfaction, interest in weight control, beliefs related to weight control, social norms, social expectations regarding subjects' body size and weight change. Data were analyzed using t-test and chi-square test(at $\alpha$=0.05). The average height and weight of subjects were 160.7cm, 52.2kg respectively. BMI and body fat(%) were 20.2 and 25.2%, respectively. One hundred seventy two students(78.2%) had attempted to control their weight, mainly to lose weight, and were categorized as the weight control attempt group. Subjects used diet modification as well as unhealthy method to control weight. 48.2% in the attempt group were underweight or normal weight(by BMI), suggesting that their weight control attempts were was unnecessary. Students in the attempt group described their own size as heavier than those in the no-attempt group(p<0.01) or other girls of their age(p<0.05) ; more dissatisfied with their body size(p<0.01), and showed more interest in weight control(p<0.01). Several differences in beliefs were also noted(p<0.001). The attempt group believed less strongly in the harmful effects or difficulties in weight control and believed more strongly in the advantages of weight control. With respect to social factors, the attempt group perceived that their family and friends wanted them to be smaller than they were(p<0.001), perceived that significant others felt that they should lose weight(p<0.05). These results suggest that educational programs for college female students should start focusing on the harmful effects of excessive dieting and information about desirable weight control methods. Students should be helped to have a correct body image. In addition, educational programs should incorporate strategies to change beliefs regarding weight control, as well as modifying social expectations from significant others.

Development of a Website-Based Nutrition Education Program for Female College Students and Young Women (여대생 및 젊은 여성층을 위한 영양교육 웹사이트 개발)

  • This study was designed to develop a website for providing nutrition information and education for college women and young women. The website focuses on two topics ; general nutrition and desirable weight control. This program is located at http : // www.ezydiet.co.kr. This program is composed of 5 major sections. The first two sections were designed to provide information ; one for general nutrition, and the other for providing information regarding weight control. The first section includes information regarding young adult women's diet, nutrition standards, such as the RDA or Korean Food Pyramid, drinking and smoking. The second section coveres information regarding desirable control, foods with high or low calories, eating disorders and popularly used dieting methods. The third section is for simple nutrition assessment, consisting of assessment of ideal body weight and obesity, energy requirements, and eating habits. The fourth section was designed to introduce and help users to apply behavioral modification techniques, such as monitoring, goal setting, stimulus control and reinforcement. The final section was designed for meal planning, by introducing a food exchange list and menu examples for one week. The characteristics of this web-based program are as follows ; 1) provide nutrition information systematically, 2) involve sections for the participation of the user, 3) include food pictures to help understanding of nutrition information, 4) include management modules for some sections to revise or update the information. One-hundred and ten female university students participated in the evaluation of this website. The evaluation results were favorable. About 90% of subjects rated that this program covers major topics 'well ' or 'very well', and that it was 'easy'to 'very easy'to understand the contents on website. Two-thirds of subjects rated quite positively on questions regarding attractiveness, overall quality and technical quality of website. In addition, about three-fourths of subjects answered that this website was helpful in increasing nutrition knowledge and in applying nutrition information into daily life. These results suggested the possibility of using a website as a means of providing nutrition information and education for young adult women.

A Retrospective Study on the Status of Obesity and Eating and Weight Control Behaviors of Elementary School Children in Inchon (인천지역 초등학생의 후향적 연구방법에 의한 비만이행추이 및 식생활습관과 체중조절행태)

  • Two hundred sixty six children were retrospectively surveyed from first to fifth grade to find out the status of obesity during four years. The obesity index was gained by height and weight data from student's health file and the data about eating behaviors and weight control behaviors, etc. were collected by questionnaires. Children assigned to one of the following groups : underweight, normal, overweight and obese. The results were as follows ; 1. Obese children were 18.6%(n=27) of boys and 10.8%(n=13) of girls at fifth grade. 2. Twenty eight children among forty obese children at fifth grade were also overweight or obese at first grade, and the other twelve was normal. 3. Breakfast skipping rate was significantly high among underweight and obese group of boys(p<0.01).The most children(87.6%) had prejudice for special foods. 4. The rate of agreement between obesity index and self recognition of body image was 60.7% in boys and 63.6% in girls. 5. Forty two point five percent of children have ever tried weight control as exercise(35.7%), reducing sugars and snacks(27%), fasting(23.8%), avoiding fatty foods(12.7%), taking diet food or drug(0.8%). In conclusion, the prevalence of children obesity is gradually increasing and children tried weight control without right guidances. In order to prevent progress into adulthood obesity, nutrition education should be followed with school foodservice including the right perception for obesity, proper weight control and eating behavior modification.
