Structural Similarity Index 인자를 이용한 방사선 분할 조사간 환자 체위 변화의 자동화 검출능 평가: 초기 보고 (Feasibility of Automated Detection of Inter-fractional Deviation in Patient Positioning Using Structural Similarity Index: Preliminary Results)

  • 현대 방사선치료는 고선명 X선 투사영상을 이용하여 환자 및 종양의 위치를 확인하는 기술이 요구되지만, 3차원 영상 촬영을 위한 피폭량 및 영상정보의 급격한 증가는 환자에게 추가적인 부담이 될 수 있다. 본 연구에서는 영상의 구조 정보를 효과적으로 추출할 수 있는 Structural similarity (SSIM) 인자를 도입하여 매일 촬영하는 환자의 2차원 X선 영상간 차이를 자동 분석하여 환자의 위치 정확성의 검증 가능성을 제시하였다. 먼저 종양을 모사하기 위하여 구형 전산 팬텀의 크기와 위치를 변화시키면서 각각의 투사 영상을 시뮬레이션하고, SSIM 인자를 통해 영상 간 차이를 검출하여 분석하였다. 또한 12일간 매일 촬영한 방사선 치료 환자의 2차원 X선 영상들 간 차이를 동일한 방법으로 검출하였다. 그 결과 산출된 팬텀 변화에 따른 SSIM 값은 0.85~1 범위로, 관심영역(ROI)을 영상 전체가 아닌 팬텀으로 한정하였을 때는 0.006~1 범위로 나타나서 ROI 적용 시 민감도가 크게 상승하는 것을 확인하였다. 또한 임상 영상의 SSIM은 0.799~0.853 범위의 값을 나타냈으며 영상 간 차이가 SSIM 분포 상에 검출되는 것을 확인하였다. 본 연구결과는 소요 시간 및 피폭 등의 우려로 매일 사용하기 어려운 3차원 영상기법 대신 간단한 2차원 영상만을 이용하여 객관적이고 정량적인 환자 위치 정확성의 자동 평가 기법을 제공할 수 있을 것으로 기대된다.

St. Jude 기계 판막을 이용한 승모판막 치환술의 20년 장기성적 (Twenty-year Experience of Mitral Valve Replacement with the St. Jude Medical Mechanical Valve Prosthesis)

  • 배경: 전북대학교 흉부외과에서 1984년 5월 이후 시행한 St. Jude 기계판막을 이용한 승모판막 치환술의 장기 성적을 후향적 방법으로 고찰해보고자 한다. 대상 및 방법: 1984년 5월부터 1996년 12월까지 전북대학교 흉부외과에서 St. Jude 기계판막을 이용하여 승모판막 치환술을 받은 총 95명의 환자를 대상으로 하였으며 추적관찰 기간은 2004년 5월까지였다. 결과: 환자의 나이는 19에서 69세의 범위를 보였다. 평균 추적관찰 기간(mean${\pm}$standard deviation)은 $10.6{\pm}4.2$ 년이었다. 조기 사망은 4.2% (4/95) 이고 만기 사망 환자는 9명이었으며 5년, 10년, 20년 총 생존율은 각각 $90.5{\pm}3.0%,\;87.9{\pm}3.4%\;83.2{\pm}4.6%$를 보였으며 판막관련사망이 없을 확률은 각각 $95.5{\pm}2.1%,\;94.3{\pm}2.4%\;91.0{\pm}3.9%$를 보여주었다. 7명의 환자에서 혈색전층으로 인한 합병증이 발생하였고(1.05%/patient-year) 15명의 환자에게서 의의 있는 항응고제관련 출혈이 발생하였다(3.56%/patient-year). 추적관찰 기간 중 기계판막의 구조적 결함은 없었으며 모든 합병증이 발생하지 않을 확률은 5년, 10년, 20년 동안에 각각 $82.0{\pm}3.9%,\;71.3{\pm}4.8%\;42.4{\pm}10.5%$였다. 결론: St. Jude기계판막은 내구성이 좋으며 판막에 관련된 합병증의 빈도가 낮아 안전하게 사용할 수 있는 판막이라 생각하며 다만 기계판막을 가지고 있는 환자에서 항응고제 복용과 용량 조정에 관한 적정한 추적관찰로 항응고제 관련 출혈의 빈도를 더욱 낮추어야 한다고 생각한다.}3mm$였다. 괴사된 부위에서 살아 있는 세포를 발견할 수 없었다. 결론: 냉동 후 해동과정은 폐조직의 전도율을 변화시키며 이는 이차 냉동 시 폐조직의 온도를 더욱 떨어뜨리고 세포파괴의 범위를 넓힌다., $3{\mu}mol/L,\;6{\mu}mol/L,\;9{\mu}mol/L$의 농도로 첨가하여 심근세포로의 분화를 유도하였다. 4주간의 배양이 끝난 후 각 실험 조직군의 세포들을 cell blocks으로 만든 후에, CD34, heavy myosin chain, troponin T, SMA에 대한 항체를 이용하여 면역형광염색을 시행하였고, 형광염색에 반응을 보인 세포들의 대략적 비율을 비교하였다. 결과: CD34, heavy myosin chain에 대하여는 모두 음성이었으나, SMA에 대해서는 $3{\mu}mol/L,\;6{\mu}mol/L$의 복강 내 지방조직에서 $5{\sim}10%$ 정도의 양성반응을 보였고, troponin T에 대하여는 피하지방조직과 복강 내 지방조직 모두에서 $6{\mu}mol/L$$9{\mu}mol/L$에서 $10{\sim}15%$ 정도의 양성 반응을 보였다. 결론: 본 연구 결과, 지방조직에서 추출한 중간엽 줄기세포는 심근세포로 분화할 수 있는 가능성이 충분히 있는 것이 확인되었으나, 분화유도 비율면에서는 만족할 수준이 되지 못하였으며, 심근세포로의 분화는 복부 피하지방과 복강 내 지방조직에서 모두 가능한 것으로 판단되었다. 대형조각 작품들 - 예를 들어 대형화된 미니별 조각이나 개념미술, 또는 대지예술 등 -은 풍경의 실재가 아니기 때문에 환경으로부터 구분된다고 언급하고 있다. 이들 조각은 더 이상 만져지는 실체이거나 점유하는 공간의 상징언어를 지닌 조각의 범주에 한정되지 않게 된다.

Qualitative and quantitative analysis of the saponins in Panax notoginseng leaves using ultra-performance liquid chromatography coupled with time-of-flight tandem mass spectrometry and high performance liquid chromatography coupled with UV detector

  • Background: Panax notoginseng leaves (PNL) exhibit extensive activities, but few analytical methods have been established to exclusively determine the dammarane triterpene saponins in PNL. Methods: Ultra-performance liquid chromatography coupled with time-of-flight mass spectrometry (UPLC/Q-TOF MS) and HPLC-UV methods were developed for the qualitative and quantitative analysis of ginsenosides in PNL, respectively. Results: Extraction conditions, including solvents and extraction methods, were optimized, which showed that ginsenosides Rc and Rb3, the main components of PNL, are transformed to notoginsenosides Fe and Fd, respectively, in the presence of water, by removing a glucose residue from position C-3 via possible enzymatic hydrolysis. A total of 57 saponins were identified in the methanolic extract of PNL by UPLC/Q-TOF MS. Among them, 19 components were unambiguously characterized by their reference substances. Additionally, seven saponins of PNL-ginsenosides Rb1, Rc, Rb2, and Rb3, and notoginsenosides Fc, Fe, and Fd-were quantified using the HPLC-UV method after extraction with methanol. The separation of analytes, particularly the separation of notoginsenoside Fc and ginsenoside Rc, was achieved on a Zorbax ODS C8 column at a temperature of $35^{\circ}C$. This developed HPLC-UV method provides an adequate linearity ($r^2$ > 0.999), repeatability (relative standard deviation, RSD < 2.98%), and inter- and intraday variations (RSD < 4.40%) with recovery (98.7-106.1%) of seven saponins concerned. This validated method was also conducted to determine seven components in 10 batches of PNL. Conclusion: These findings are beneficial to the quality control of PNL and its relevant products.

종합병원 간호사가 인식하는 환자교육 중요도와 수행도의 관계연구 (A Study on the Perception of the Importance and Performance of Patient Education of the Clinical Nurses)

  • The purpose of the study are to examine the perception of the importance and performance of patient education of the clinical nurse and find out the interfering factors in practicing patient education. The data were collected from convenient sample of 256 clinical nurses working in the nursing units of adult patients except the psychiatric unit, obstetric unit, dental surgical unit and intensive care unit of one University Hospital in Seoul from September 29 to October 2, 1998. Three measurement tools of self-report- questionnaires developed by researcher used. For the content validity of the questionnaires, two sessions of panel discussion and a pilot test were done and finally factor analysis was done with Varimax method. Analysis of data was done with SAS program using frequency, percentage, means, standard deviation, Pearson's Correlation Coefficients, t-test and ANOVA. The obtained results were as follows : 1. The surveyed nurses perceived the importance of patient education at higher level with mean score of 4.08 among 5 point than their perception of practice( mean score : 3.42). 2. There was positive significant correlation(r=.29, p=0.0001)between nurses' perception of the importance of patient education and it's practice 3. Among the teaching contents for patients, 'information of diagnostic procedure and operation' and 'orientation of hospitalization' were perceived most important. And 'preparation for discharge' and 'understanding of disease and health promotion' were perceived least important 4. Among the teaching contents for patients, 'orientation of hospitalization' and 'information of diagnostic procedure and operation' were perceived highly performable. And 'understanding of disease and health promotion' and 'preparation for discharge' were perceived least performable. 5. Three types of interfering factors were identified as patient-factor, situational factor, nurse-factor. The mean degree of impediment with the interfering factors was at average level(3.09 among 5). The patient and situational factors of impediments were more interfering than nurse- factor for teaching patients. 6. In older age(p<.05), married state (p<.05), higher educational status (p<.01), higher clinical experience (p<.01) and higher position(p<.01), the score of perceived importance of patients education was more high. 7. In older age(p<.01), higher clinical experience(p<.001) and surgical unit (p<.01), the score of perceived performance of patients education was more high. In conclusion, in order to activate patient education practice in the clinical setting, the continuing education for patients education should be more emphasized and the effective teaching methods and materials should be developed to help patient teaching. And an organizational support such as budgeting for patient education and reimbursement system should be administrated.

Potential of the Quantitative Trait Loci Mapping Using Crossbred Population

  • In the process of crossbreeding, the linkage disequilibria between the quantitative trait loci (QTL) and their linked markers were reduced gradually with increasing generations. To study the potential of QTL mapping using the crossbred population, we presented a mixed effect model that treated the mean allelic value of the different founder populations as the fixed effect and the allelic deviation from the population mean as random effect. It was assumed that there were fifty QTLs having effect on the trait variation, the population mean and variance were divided to each QTL in founder generation in our model. Only the additive effect was considered in this model for simulation. Six schemes (S1-S6) of crossbreeding were studied. The selection index was used to evaluate the synthetic breeding value of two traits of the individual in the scheme of S2, S4 and S6, and the individuals with high selection index were chosen as the parents of the next generation. Random selection was used in the scheme of S1, S3 and S5. In this study, we premised a QTL explained 40% of the genetic variance was located in a region of 20 cM by the linkage analysis previously. The log likelihood ratio (log LR) was calculated to determine the presence of a QTL at the particular chromosomal position in each of the generations from the fourth to twentieth. The profiles of log LR and the number of the highest log LR located in the region of 5, 10 and 20 cM were compared between different generations and schemes. The profiles and the correct number reduced gradually with the generations increasing in the schemes of S2, S4 and S6, but both of them increased in the schemes of S1, S3 and S5. From the results, we concluded that the crossbreeding population undergoing random selection was suitable for improving the resolution of QTL mapping. Even experiencing index selection, there was still enough variation existing within the crossbred population before the fourteenth generation that could be used to refine the location of QTL in the chromosome region.

불확도를 고려한 Class I 부정교합 환자의 측방두부방사선영상 계측값 (Lateral Cephalometric Measurements of Class I Malocclusion Patients with Uncertainty)

  • 본 연구의 목표는 측방 두부계측방사선 사진의 분석에 이용되는 소프트웨어의 소급성 확보와 측정값의 불확도를 계산하는 것이다. 또한 이를 이용해 소아에서 교정치료를 위한 계측값의 참고 표준을 얻기 위한 기반을 마련하고자 하였다. 교정치료를 위해 서울대학교 치과병원 소아치과에 내원한 6세에서 13세 사이 환아 중 1급 부정교합으로 진단 받은 환아 100명의 데이터를 수집하였다. 소급성 확보를 위해 방사선 촬영이 가능한 phantom 장비를 제작하였으며, 현재 사용중인 계측 소프트웨어를 이용하여 phantom 장비의 길이와 각도를 측정하여 소프트웨어의 교정값을 계산하였다. 불확도 계산을 위해 100명의 측방두부방사선영상 계측값과 반복측정에 의한 불확도와(A형 불확도) 최소분해능과 두부의 위치에 의한 불확도를(B형 불확도) 계산하였다. 이를 통해 합성표준불확도를 얻었으며 최종적으로 확장불확도를 계산하였다. 본 연구 결과 현재 사용중인 측방두부방사선사진 계측 프로그램이 높은 정확성과 신뢰도를 가지는 것을 확인하였다. 또한 교정값을 이용하여 계측값을 교정하였으며, 6 - 13세 한국인 소아에서의 교정계측치의 불확도를 계산하여 1급 부정교합 환아의 교정계측값의 95% 신뢰도를 가지는 분포범위를 제시하였다.

간호교육에서 교수매체 활용에 관한 연구 (A Study on the Use of Instructional Media in Nursing Education)

  • The use of instructional media in nursing education was investigated using a descriptive research methodology. Data were collected from 199 professors teaching in the areas of Fundamental Nursing (48 subjects), Adult Nursing(56 subjects), Pediatric Nursing(49 subjects), and Community Health Nursing(46 subjects). 120 professors were from 3-year college of nursing and 79 professors were from 4-year college of nursing, Data were analyzed using descriptive statistics (mean, standard deviation), and ANOVA with SPSS $PC^+$ program. The results are as follows. 1) The general attitude of the subjects toward the use of instructional media was positive(mean : 3.75). However, from the ANOVA result the subjects from 4-year college of nursing had more positive attitudes in the areas of media utilization and supply system. Most subjects had high interest in the areas of effectiveness of the media, and media literacy in using instructional media. 2) OHP(mean was 3.76) and VTR(mean was 3.36) were the most used instructional media in nursing education. These media were efficiently supplied by the school. However, other media like CD-ROM, Opaque Projector, and LCD or beam Projector were not sufficiently provided by the school. 3) The main reasons to use instructional media were that the use of media is effective to raise students' attention and learning motivation. 4) Insufficiency of the media and environment to use media were the causes of the lack of using instructional media. 5) The use of PC communication of internet, LCD or beam Projector, and Computer Graphics was low. The reasons were that the subjects did not have enough knowledge and skills to use these media, and there was lack of media or environment. 6) In general, environment to use media of the 3-year college of nursing was worse than that of 4-year college of nursing. However, there was no significant differences between the two groups in the use of media related to their position, and subject. On the basis of the study results professors teaching nursing have positive attitudes to use instructional media but the lack of media supply or facility to use media limited the use of instructional media.

성인여성용 브래지어 원형 개발연구(제2보) -패턴그레이딩을 중심으로- (A Study on the Development of Basic Brassiere Pattern for Adult Women(Part II)-Focusing on Pattern Grading-)

  • The purpose of this study was to develop grading method of the basic brassiere patterns therewith, provide for 12 size's brassiere patterns and prove fitness of their's and shape. The collected data were statistically processed using the SAS 6.12 for Means, Standard Deviation, etc. The results of this study can be summarized as follows; 1. The basic sizes adopted at the first design stage (see fig.1-2) were 75AA, 75A and 75B each of which was designed as upper foundation basic pattern in reference to the cup size. At the second stage, each of the upper body foundation basic pattern was used to design lower cup, upper up, front panel and wing in their order(see fig.3-6). At the third stage, the other sizes were graded in reference to the three brassiere patterns (75AA, 75A and 75B) to produce 12 brassiere sizes(see Tab. 7-8). In order to develop the basic patterns of adult women's brassieres, 33 women were selected per 11 size combination(3 women per size combination), and then, their brassiere fitting was teated. As a result, the fronts of the basic patterns were found to have such problems as undone front center, poor levelling of upper cup's bottom line or lower cup's top line, poor shape of upper cup's outside top line and shoulder strip position. On the other hand, on the rear side of the basic patterns, the central part of the wing was slightly lifted, while the wing width and straps'interval did not befit the ful1-cup brassiere. The result of this first brassiere fitting test showed that the functionality and aesthetics of the basic pattern brassieres were more or less satisfactory, and thus, the problems were addressed for the second teat. In the second test, the aesthetics, breast-shaping effect and functionality of the basic pattern brassieres were much improved. 2. We were to present the final brassiere sizing system to extend the interior length of breast and the exterior length of breast on the basis of fitting test. Thereby, 75B$.$80A$.$80B$.$85A$.$85B's the interior lengths of breast were enlarged to 0.7cm, the rest size was enlarged to 0.5cm and the exterior length of breast was enlarged to 0.5cm regardless of sizes. 3. Wind loads and load-effects of large scale wind turbine tower with different halt positions of blade

  • In order to investigate the influence of different blade positions on aerodynamic load and wind loads and load-effects of large scale wind turbine tower under the halt state, we take a certain 3 MW large scale horizontal axis three-blade wind turbine as the example for analysis. First of all, numerical simulation was conducted for wind turbine flow field and aerodynamic characteristics under different halt states (8 calculating conditions in total) based on LES (large eddy simulation) method. The influence of different halt states on the average and fluctuating wind pressure coefficients of turbine tower surface, total lift force and resistance coefficient, circular flow and wake flow characteristics was compared and analysed. Then on this basis, the time-domain analysis of wind loads and load-effects was performed for the wind turbine tower structure under different halt states by making use of the finite element method. The main conclusions of this paper are as follows: The halt positions of wind blade could have a big impact on tower circular flow and aerodynamic distribution, in which Condition 5 is the most unfavourable while Condition 1 is the most beneficial condition. The wind loads and load-effects of disturbed region of tower is obviously affected by different halt positions of wind blades, especially the large fluctuating displacement mean square deviation at both windward and leeward sides, among which the maximum response occurs in $350^{\circ}$ to the tower top under Condition 8; the maximum bending moment of tower bottom occurs in $330^{\circ}$ under Condition 2. The extreme displacement of blade top all exceeds 2.5 m under Condition 5, and the maximum value of windward displacement response for the tip of Blade 3 under Condition 8 could reach 3.35 m. All these results indicate that the influence of halt positions of different blades should be taken into consideration carefully when making wind-resistance design for large scale wind turbine tower.

하진의 중국재현과 오리엔탈리즘 논쟁 (Representation of China in Ha Jin's Works and the Controversy over Orientalism)

  • Chinese American Writer, Ha Jin has been writing exclusively about the life in his native Communist China. His stories and poems are almost all about the Chinese people so far. In addition, the distinctive Chinese flavour and the inexorably repressive image of China in his works present an 'Other' to the American culture. Such kind of Chineseness can also be found in Ha Jin's works and his career as a writer. The continued demand for knowledge of China, which is created by China's increasingly important role in the globalized economy, sustains the country's position as an Other for America. In his early four novels, Ha Jin portrays a totally repressive image of Communist China, an image of which functions perfectly as a form of otherness for his American readers. In Ha Jin's portrayal, the Chinese masses are subjected to the Communist authority through its bureaucracy and state-economy mechanism, as well as through the godlike image of Mao Zedong. They are to follow the Communist conscience and subscribe to unity-in-difference. Deviation from the one-party rule is intolerable. In each of the novels, Ha Jin presents a specific system of repression. In In the Pond, confrontation against Party authority is contained by a process of complicity. In Waiting, the Party's power is upheld through a system of surveillance in which people act as agents, resulting in a web of power which paralyses love. The Crazed illustrates a play of power by Party officials which, against the backdrop of the Tiananmen Square Massacre, is full of craze itself, driving people either out of sanity or out of the country. War Trash exposes the Communist power's repression to the extreme by presenting a case of dishonour in those whose life is debased as trash by the Party. The repressive image of China produced in these stories, which span over half a century, makes Ha Jin's China a perfect Other for the West. To sum up, Ha Jin's novels construct a repressive image of China. In his novels, Ha Jin exposes the working of repression in particular systems. Through these systems, he problematizes the notion of personal autonomy for Chinese people and proposes for his western/American readers a solution which eventually turns into a re-presentation of American hegemony.
