The Judicial Precedent Analysis of Medical Litigation in the field of Pediatric Ophthalmology (소아안과 영역에서 발생한 의료소송의 판례 분석)

  • Objective : The purpose of this study is to describe the characteristics of medical malpractice related to pediatric ophthalmology and to identify the causes and potential preventability of medical litigation in Korean medicine. Methods : A study was performed by analysing 8 cases of lawsuit in the year between 1968 and 2011, which were selected among the medical dispute cases involving pediatric ophthalmology. Results : The eight closed claims occurring in the field of pediatric ophthalmology were founded in the data for medical malpractice. One claim was supreme court decision, two claims were high court decisions and five claims were district court decisions. Conclusions : While malpractice claims occurring in the field of pediatric ophthalmology were uncommon, they resulted in a high rate and amount of indemnity payments. For reduction of medical disputes, improvement of clinical trials and clinical medical cares is emphasized, and informed consent is also important.

A case of Sinusitis Patient Improved by Kami-Hyunggyeyungyotang (가미형개연교탕으로 치료한 부비동염 치험 1례)

  • Objectives : The purpose of this study is to evaluate the effect of oriental medical treatment on a sinusitis patient Methods : We treated a 56-years old man patient with sinusitis. His symptoms are Ozena, Postnasal drip, Cough, sputum, nasal obstruction, rhinorrhea and Headache. The patient was treated with herb-med(Kami-Hyunggyeyungyotang,加味荊芥連翹湯), acupuncture, aroma oil therapy. During about a month, the patient was treated only with oriental medical treatment and without any of western medicine including antibiotics. We confirmed the progress of treatment with subjective symptoms, physical examination and radiographic inspection(PNS image). Results & Conclusions : After one month, Combined application of the oriental treatment including Kami-Hyunggyeyungyotang showed remarkable effects on subjective symptoms, physical examination and radiographic inspection(PNS image). This study show that Kami-Hyunggyeyungyotang effects on sinusitis.

A Case of Erythema multiforme Caused by Acute Tonsillitis (급성 편도선염에 의해 촉발된 다형홍반 환자 치험 1례)

  • Objectives : The purpose of this study is to report the oriental medical treatment on erythema multiforme caused by acute tonsillitis. Methods : Patient who has been ill Acute tonsillitis suffers from erythema, edema, pruritus, burning sensation of systemic skin. The patient was admitted to Dunsan oriental hospital for 7 days and was treated with oriental medicine(acupuncture, herbal-medicine, and vapor treatment). To evaluate the results of this treatment, the improvement of symptoms(erythema, pruritus, burning sensation) was identified by patient's subjective expression and in lesion-symptom improvement. Results : The symptoms caused by acute tonsillitis on total skin were improved. Conclusions : This study shows that the oriental medical treatment effects on the erythema multiforme caused by acute tonsillitis. Also, the public suggests that the correlation between the skin and tonsil.

  • Objectives : This study was carried out to analyze the trends of clinical studies of allergic contact dermatitis published in the Journal of Korean Medical Ophthalmology & Otolaryngology & Dermatology. Methods : We studied 10 research papers on allergic contact dermatitis published in JKOOD. We analyzed about experiment methods, pharmacodynamic effects, therapeutic effects in published papers. Results : 1. The number of searched journal is 10 papers. 2. All experiments was performed using DNCB primed mouse. To demonstrate therapeutic effect of drugs, previous researches observed dermatological and histopathogical conditions, erythema, melasma, white blood cell count, Lymphocyte ratio to white blood cell. 3. Most of drugs have heat-clearing and detoxifying effects. 4. Every drugs have positive effect in treating allergic contact dermatitis. Conclusions : Figuring out how experiment proceeded and tendency of drugs in researching allergic contact dermatitis is significance of this paper. Further research about drugs and their treating methods should be carried out to improve curative effectiveness for allergic contact dermatitis.

Analysis of Tranditional Chinese Medicine for Otitis Media with Effusion in Chinese Journals (삼출성 중이염에 대한 중의학 임상 논문 분석)

  • Objectives : The purpose of this study is to investigate the trend of Tranditional Chinese Medicine for Otitis Media with Effusion(OME) in Chinese journals. Methods : Chinese National Knowledge Infrastructure(CNKI) and Wanfang med online were used to search Chinese studies which were published from January, 2010 to April, 2020. Results : Among Chinese studies, Exterior-releasing medicinal(解表藥) and Heat-clearing medicinal(淸熱藥) were the most frequently used. The herbs which used the most frequently are Bupleuri Radix(柴胡), Acori Graminei Rhizoma(石菖蒲). All of studies have reported that Tranditional Chinese Medicine is effective for Otitis Media with Effusion. Conclusions : In analysis of selected studies, Tranditional Chinese Medicine is more effective than Western Medicine Treatments. Recurrence rates and side effects of OME can be reduced by cotreatment of Tranditional Chinese Medicine and Western Medicine Treatments.

  • Objective : The purpose of this study is to analyze the trends of allergic rhinitis related articles that have been published in the Journal of Korean Medicine Ophthalmology, Otolaryngology & Dermatology(JKOOD). Method : We studied 32 articles of allergic rhinitis published in JKOOD from 1988 to 2018. 24 articles are classified into two categories; treatment-related article and diagnosis-related article which include pattern identification. And we reviewed the other 8 articles briefly in discussion. Results : 16 treatment articles are classified into two categories; 13 case reports and 3 retrospective observation studies. In case reports, the studies are classified into three categories; 3 acupuncture group, 3 herbal medicine group or 7 herbal medicine with other parallel treatments group. Conclusions : Further studies are needed on allergic rhinitis including treatment, alternative medicine and methods of evaluation in order to develop a level of evidence.

A Literature Study of Korean Medicine for Sinusitis (비부비동염의 한의학적 문헌고찰)

  • Objectives : The objective of this study is to analyze the research of Korean medicine for the treatment of sinusitis. Methods : We searched domestic journals for this analysis with the title 'Sinusitis, Rhinosinusitis, Biyeon(鼻淵)'. We searched 'KISS, OASIS, Korean traditional knowledge portal'. Results : Total numbers of searched articles are 103. We selected 14 papers; 5 systematic reviews, 9 clinical trials. Systematic reviews deal with cause, treatment, herbal medicine and acu-points of sinusitis. Clinical trials deal with treatment cases of Korean medicine which are failed to treatment of western medicine which is antibiotics and surgery. Conclusions : Positive effect of Korean medicine on sinusitis is reported but cases are less and research cases are limited. We conclude that well-designed clinical studies are needed.

Clinical Study of Three Cases of Recurrent Peripheral Facial Nerve Palsy with Sequelae (말초성 안면마비 후유증과 재발을 겸한 환자 3례에 대한 임상적 고찰)

  • Objective : The purpose of this study is research on three cases of recurrence facial palsy with sequelae. Method : The authors observed patients by the House-Brackmann grade system and by the Scale of Peitersen for operated acupuncture treatment, herbal treatment and herbal acupuncture. Result : After the treatment, the House-Brackmann grades were decreased in two cases of three and the Scale of Peitersen grades were decreased in two cases of three. Conclusion : These result suggest that early admission with Korean medicine treatment could be more effective in recovery of recurrent peripheral facial nerve palsy with sequelae.

A Case Report of Verruca Vulgaris Patient, Treated only with Korean Medicine (한약 단독 치료를 이용한 심상성 사마귀 1례 보고)

  • Objectives : The purpose of this study is to report a case of verruca vulgaris patient, treated only with Korean medicine. Methods : A 6-years-old female patient was treated only with Korean medicine due to verruca vulgaris on left hand. Photographs and size measurements of lesions were used to evaluate the changes in symptoms. Results : After 3 months of treatment, the patient's lesions showed almost complete clearance and there were no adverse effects. Conclusions : According to the results, Korean medicine with Bojungikgi-tang Hab Saengmaek-san Gamibang could be an effective and safe treatment for verruca vulgaris in a pediatric patient.

  • Objective : This study was carried out to analyze and understand the trends of Atopic dermatitis papers Published in the Journal of Korean Oriental Ophthalmology & Otolaryngology & Dermatology(JKOOOD) Methods : We have reviewed 43 articles of atopic dermatitis which were published in JKOOOD during 1988-2008. All the papers are classified into three categories; original article, review article and case report. Results : 1. The number of journals associated with atopic dermatitis is 43 papers: 16 original articles(16/43%), 16 case reports(16/43%), 11 review articles(11/43%). 2. Classification of 16 original articles in terms of methodology: 11 experimental studies(11/16%), 5 descriptive studies (5/16%), 0 analytic studies(0%). 3. Classification of 16 case reports in terms of the number of patients: 1 person(8/16%), 2 persons(3/16%), 3 persons(2/16%). Conclusions : In this study, we analyzed the trends of Atopic dermatitis papers Published in the Journal of Korean Oriental Ophthalmology & Otolaryngology & Dermatology during 1988-2008. It is needed to adopt the qualitative methods of evaluation in order to develop a level of evidence.
