경화된 상아질의 접합 양상에 관한 주사전자현미경적 연구 (A SCANNING ELECTRON MICROSCOPIC STUDY OF BONDING ASPECTS TO THE SCLEROTIC DENTIN)

  • The changes of microstructures, morphology of sclerotic dentin and bonding aspects generated by an adhesive resin was investigated. Incisors and premolars showing natural cervical abrasions were collected and conditioned with 10 % phosphoric acid or 10 % maleic acid. The sclerotic dentin specimens were then rinsed and blot-dried and applied with dentin adhesive (All Bond 2) to the conditioned dentin surface. To examine the morphologic change of the sclerotic dentin specimen after etching and bonding procedure, the treated specimens were examined by SEM. To analyze the chemical composition of sclerotic dentin and crystals occluding dentinal tubules, the sclerotic dentin specimen was powdered and examined with X-ray Diffractometer. To investigate the Ca/P weight percent ratio within the dentinal tubules, the sclerotic dentin specimen was fractured perpendicularly to the long axis of the tooth from the center of cervical abrasion lesion and then examined with EDX(Energy Dispersive X-ray) microanalyzer. The results were as follows : 1. The increased width of peritubular dentin and the depositions of the irregular amorphous materials within the dentinal tubules were showed in the sclerotic dentin specimens. 2. After the treatment of sclerotic dentin specimen with 10 % phosphoric acid or 10 % maleic acid, the lateral side of tubules rather than cross-sectional tubule openings was showed exclusively at the incisal and gingival incline of the specimens. 3. After the treatment of sclerotic dentin specimen with 10 % phosphoric acid or 10 % maleic acid, the hybrid layer was not formed evidently and the resin tag was not formed or shortly penetrated into the tubules with the thinner diameter. 4. According to the results of XRD analysis of the sclerotic dentin specimen, Hydroxyapatite and Octacalcium phosphate were predominent, however, Whitlockite crystals were rare. 5. The mean Ca/P weight percent ratio analysed from 5 fractured sclerotic dentin specimens was $2.322{\pm}0.170$ at the intertubular dentin, $1.826{\pm}0.051$ within the dentinal tubule.

Microabrasive로 처리한 상아질표면에 대한 복합레진의 결합강도에 관한 연구 (A STUDY OF THE BOND STRENGTHS OF COMPOSITE RESIN TO DENTIN SURFACES PREPARED WITH MICROABRASIVE)

  • The bond strengths of composite resin to tooth dentin vary with the methods of cavity preparation and surface treatment. Recent developments in techniques of dentinal surface treatment have renewed interest in microabrasive as a means of tooth preparation, The purpose of this study was to determine the effects of a new method of cavity preparation on the bond of composite resin to dentin. Freshly extracted 144 healthy human third molars were used in this study. The dentin surfaces prepared with #600 SiC abrasive paper were divided into control and air abrasion groups according to the method of dentin surface preparation using different combinations of delivery pressure, time, and acid etching. The shear bond strengths were measured after the composite resin (Clearfil Photo Bright) was bonded to prepared dentin surfaces by light-curing using a dentin bonding system (All-bond 2), In addition, the average surface roughness was measured to investigate the effect of differently prepared dentin surfaces on the shear bond strengths. The surface changes of prepared dentin and the debonded dentin surfaces were observed with SEM (S-2300, Hitachi Co., Japan). The following results from this-study were obtained ; 1. There was no significant difference of shear bond strengths according to the changes of delivery pressure and time. 2. The shear bond strengths were lower than the control in the air abraded-only groups, but those of the additional acid-etched groups were higher than the control. 3. The shear bond strengths to all air-abraded surfaces were increased by acid etching. 4. The correlation between shear bond strengths and surface roughness was not certain, although the mean surface roughness of all air-abraded surfaces has increased evidently while it has slightly decreased for additional acid etching. 5. On SEM examination, the dentinal tubules were almost occluded in the air abraded-only groups, but those were opened in the additional acid-etched groups. 6. The debonded surfaces were showed adhesive failure mode in the air abraded- only groups, while those were showed mainly the mixed and cohesive failure mode in the additional acid-etched groups. These results suggest that the layer produced during cavity preparation or surface treatment with air abrasion must be removed for maximum bond strength of composite resin to dentin.

  • 상아질 접착력을 높이기 위해서는 적절히 습윤한 상아질 표면을 얻는 것이 중요하다. 이런 방법에는 정확한 지침이 없으며, 주관적이고 술자에 따라 많은 차이가 있다. 본 연구에서는 적절히 습윤한 상아질을 얻는 방법에 관한 연구의 일환으로, 유치의 산부식 후. 여러 가지 방법을 이용해 상아질을 건조시킨 표본의 조직상을 획득하여 상아질 표면의 차이를 비교하고자 하여 다음과 같은 결론을 얻었다. 1. 유치 상아질을 산부식하면, 도말층이 제거되고 상아세관이 노출되며, 관간상아질과 관주상아질이 탈회되고, 교원섬유가 풍부한 이행층이 노출되는 것을 확인할 수 있었다. 2. 산부식된 상아질이 과도하게 건조되면 탈회된 상아질이 수축하여, 관간상아질이 붕괴되어 높이가 점차적으로 낮아지고 상아세관의 직경은 조금씩 감소되는 것으로 나타났다. 3. 유치 산부식 후, 10cm 거리에서 20초 동안 압축 공기로 건조시키면 상아질이 과도하게 건조, 탈수됨을 확인할 수 있었다. 4. 유치 산부식 후, 10cm 거리에서 3초 동안 압축 공기로 건조시키거나 마른 cotton, 젖은 cotton, microbrush, absorbent tissue paper로 건조시키면 상아질이 적절하게 건조됨을 확인할 수 있었다.

  • A variety of surface pre-treatments have been advocated to prepare the dentin prior to placement of a bonding agent. The purpose of this study was to evaluate the effect of various dentin conditioners upon the degree of resin impregnation to the dentinal tubules and the shear bond strength of a new dentinal bonding agent (Scotchbond 2) used in conjunction with a visible light cured composite (Silux). The healthy eighty human molars extracted due to periodontal or orthodontic reasons were used and randomly divided into five groups. All the grinded dentin surfaces were conditioned with 3% $H_2O_2$, Cavity Cleanser (Columbus/Bayer), Dentin Conditioner (GC Inter. Corp.), Scotchprep (3M Co.) according to the manufacturer's directions. The specimens were then demineralized in 10% HCl for 20 sec. and 24 hrs. in order to observe the resin tags in Hitachi X-450 scanning electron microscope at 25KV. Also, shear strengths were obtained using an Instron Testing Machine with a cross head speed of 1 mm/min. The following results were obtained ; 1. In group treating with Dentin Conditioner and Scotchprep, the resin strings were formed on most of the surfaces and penetrated more than $50{\mu}m$ into the tubules. 2. The inner surface of resin treated with Cavity Cleanser, indicating the resin strings formed partly and penetrated about in depth of $30{\mu}m$. 3. In control and experimental group treated with 3% $H_2O_2$, the resin tags were not formed, if any, penetrated shortly. 4. Shear bond strengths in groups treating with Dentin Conditioner and Scotchprep were statistically significant increase than with 3% $H_2O_2$. (P<0.01). 5. The Scotchprep treatment group was significantly higher in shear strength than groups treated with no conditioning and Cavity Cleanser.(P<0.01) 6. Shear bond strengths evaluated were gradually increase in proportion to the tag length of resin impregnation.

  • The purpose of this study was to evaluate the effect of dentin pretreatment with Dentin Conditioner, Ultra-Etch, conditioner of Fuji Plus cement on the shear bond strength of resin-reinforced glass ionomer cements to dentin and analyze the fractured surfaces. To evaluate the bond strength, the extracted human teeth which had uniform area of exposed dentin were cemented with conventional glass ionomer cement, 3M $RelyX^{TM}$ Luting (Vitremer luting cement), Fuji Plus cement after dentin pretreatment. The shear bond strength was measured using the Universal testing machine (Instron Co., USA) with a crosshead speed of 1mm/m. The effect of dentin pretreatment was evaluated by observing pretreated dentin surfaces under the scanning electron microscope, measuring the shear bond strength and observing the fractured surfaces under the scanning electron microscope. The results were as follows : On the SEM observation of surface morphology, the specimens treated with Dentin Conditioner. Ultra-Etch and conditioner of Fuji Plus cement were removed the smear layer and funneled dentinal tubules in dentin surfaces. In $RelyX^{TM}$ Luting cement group, shear bond strength of pretreated group was significantly higher than control group. In Fuji Plus cement group and Fuji I group, regardless of the type of pretreatment agents, there was tendency of increase in the shear bond strength. On the SEM observation of fractured surfaces, as the shear bond strength increase, it were shown thicker cement layers and were not shown dentinal tubules According to these results. it were shown that dentin pretreatment have much effect on bonding states.

상아질에 적용된 재 습윤제가 미세인장 결합강도에 미치는 영향 (EFFECT OF REWETTING AGENT ON DENTINAL MICROTENSILE BOND STRENGTH)

  • This study investigated that the effect of rewetting agent on dentinal microtensile bond strength(${\mu}TBS$). Human molars were sectioned to expose the superficial dentin surfaces. Samples were divided into two groups according to type of adhesives-Single Bond (S) and One-Step (0)], and again subdivided into five groups by different dentin surface treatment-dry for 15s (D), blot dry (BD) or dry for 15s, and rewet with different rewetting agents [distilled water (DW), Gluma Desensitizer (GD) and Aqua-Prep (AP)] for 30s. After application of adhesive, composite resin was built up on the bonding surface. Each tooth was sectioned to obtain stick with $1\textrm{mm}^2$ cross sectional area and the ${\mu}TBS$ was determined by EZ test. In the S group, the mean ${\mu}TBS$ of GD, AP, and BD group was significantly higher than that of DW and D group (p < 0.05), In the O group, the mean, ${\mu}TBS$ of AP, GD, BD and DW group was significantly higher than that of D group (p < 0.05). The data suggested that Gluma Desensitizer and Aqua-Prep could be successfully used as rewetting agents, and Distilled water could be acceptable in aceton based adhesive system only.

  • The purpose of this study was to examine the influence of several root canal sealers on the discoloration of internal surface after root canal obstruction. Twenty four sound human premolars, extracted for orthodontic or prosthodontic purposes, were randomly selected and divided into eight groups. Extracted premolars were prepared, and the following seven materials were introduced into the pulp cavities: AH 26, Fuji ionomer (Type I) cement, N2, Oxypara "Murakami", Kerr sealer, PCA sealer, and G-C's Propac ZOE cement. After 7 weeks of incubation; the discolored tooth crowns were hemisectioned, and the internal staining patterns were examined. Then, with an association of observed values the mean intensity scores and percentage of coverage scores of the internal staining patterns in teeth attained by two observers using for Chisquare test were analyzed. The results were as follows: 1. All the experimental premolar's crown showed various ranged discoloration of internal surface. 2. There was no significance between the association scores of two observers participated into this experiments:% coverage scores (P > 0.05) and intensity (P > 0.05) 3. The crowns filled with PCA sealer, AH 26, and Fuji ionomer cement was visible within a depth of one third of dentin. (P> 0.05) 4. For N2, Kerr sealer, and G-C's Propac cement, A slight dentinal staining was recorded, which penetrated up to half way into the dentin. (P> 0.05) 5. It was noticed that the teeth filled with Oxypara "Murakami" were discolored more than two thirds of the dentinal layer. 6. On the control group, there was no discoloration.

Er:YAG 레이저를 이용한 치아삭제 효과 (EFFECTS OF ER:YAG LASER ABLATION ON THE DENTIN)

  • Er:YAG 레이저는 파장이 물의 주흡수 정점과 일치하는 $2.94{\mu}m$의 방출파장을 가지기 때문에, 물분사를 하면서 사용했을 때, 다른 레이저보다 열손상은 더 작으면서 치아경조직을 보다 효과적으로 삭제한다고 알려져 있다. Er:YAG 레이저의 상아질 삭제 효과를 평가하고자 발거된 소구치와 대구치를 대상으로 상아질 시편을 제작하고 $2.94{\mu}m$의 pulsed Er:YAG 레이저(SDL-300EN, 삼성전자, 한국)를 handpiece형의 전달계를 이용한 비접촉식 방법으로, 물에 담궈진 시간과 레이저 조사시간에 따른 삭제량과 형태를 조사, 분석하여 다음과 같은 결과를 얻었다. 1. 건전 상아질에 대한 조사 결과물에 담군 시간이 증가할수록 삭제율이 증가하였다. 2. 건전 상아질과 우식 상아질에 150mJ, 10Hz의 조건으로, 30초간 레이저를 조사했을 때, 우식상아질에 비해 건전 상아질에서 surface irregularity가 증가하고 상아세관이 열리는 경향이 있었으며 crack이 관찰되었다. 또한 우식 상아질에 비해 많은 탄조각(ash flecks), crack이 관찰되었고 상아세관의 불연속성 (inconsistency)이 관찰되었다. 3. 우식 상아질에서 30초로 조사한 경우에서는 dark zone이 와동에 국한되어 나타났으며 작은 fleck들이 관찰되었고, 1분간 조사한 경우에서는 30초의 경우보다도 와동이 더 넓었으며, crack이 상아세관 주행방향과 직각으로 형성된 것을 관찰 할 수 있었다. 2분간 조사한 경우는 와동이 가장 넓었으며 crack이 더 많이 형성된 것을 볼 수 있었다.

  • The purpose of this was to evaluate the in vitro effects of different application modes of tetracycline HCI on dentin root surfaces. The study group was comprised of 30 teeth with advanced periodontal disease extracted before the start of periodontal therapy. The diseased root surface was vigorously root planed to expose dentin. The teeth were sectioned and cotton pellets. soaked in a saturated solution of tetracycline HCI, were either "placed" or "burnished" on the prepared root surface for 3, 5 min. respectively. Control surfaces were similarly treated with root planing only. Following the various treatments, teeth were fixed. dehydrated, critical point dried, and coated for scanning electron microscopic(SEM) evaluation. The surfaces of tetracycline HCI treated specimens considerably differed from specimens treated with root planing only. Results demonstrated that root-planed, no-tetracycline HCI specimens had amorphous, irregular surface which corresponded to a smear layer. Tetracycline HCI specimens exhibited collagen fibers and dentinal tubules exposed by the removal of the smear layer. Although some differences were seen in surface structures among some specimens. "burnished" application causes more extensive change than "placed" application, and application time of the tetracycline HCl did not seem to have any major impact on the surface morphology.

상아질의 표면 ZOE처리가 글래스아이오노머 시멘트의 전단강동에 미치는 영향에 관한 연구 (A STUDY ON THE BOND STRENGTHS OF GLASS IONOMER CEMENT TO THE ZOE SURFACE TREATED DENTIN)

  • The purpose of this study was to asses the shear bond strengths of 3 types of glass ionomer cement and 1 type of composite resin to dentinal surface with or without ZOE pretreatment. 80 extracted tooth specimens are divided into two groups; the control group (40 specimens) is not treated with ZOE and the other(40 specimens) is ZOE pretreated during 24 hours before bonding procedure. Shear bond strengths were measured with universal testing machine (Instron, Model 4301) and statistically processed by ANOVA and t-test. The results were as follows: 1. Bond strength of the ZOE treated experimental group showed lower than the control group, except chemical cured glass ionomer cement(p<0.05). 2. After ZOE surface treatment, the bond strength of composite resin was superior than glass ionomer cement and all experimental group was decreased (p<0.05). 3. It has nothing to do with ZOE surface treatment, that chemical curing glass ionomer cement was showed lowest bond strength.
