파노라마 촬영의 피폭선량에 관한 문헌분석 연구 (A Literature Review and Analysis of Dosimetry in Panoramic Radiography)

  • 치과파노라마 촬영은 구강에 대한 선량의 증가와 안전관리가 필요하여 이에 대한 안전관리 및 장치관리에 대한 실태의 조사와 연구가 필요하다. 치과파노라마 촬영에 대한 기본 데이터 확보와 파노라마 촬영에 대한 국내 및 국외의 문헌에 대한 보고와 조사가 전무하여 이에 대한 조사 및 선량의 측정이 필요하다. 파노라마 촬영에 따른 유효선량, DAP, DWP를 분석하였고, ICRP에서 규정한 흡수선량과 가중계수 및 논문에서 국외의 진단참고준위를 참고하였다. 파노라마 촬영은 방사선의 피폭에 따른 환자에게 위험도를 수반하므로 진단참고 준위를 참조하여 환자의 방사선 방어를 최적화해야 한다. 또한 파노라마 방사선 촬영은 부위에 따라 흡수선량의 차이가 있어 선량에 가능한 노출되지 않도록 해야 한다.

방사선조사가 백서 협점막에 미치는 영향에 관한 전자현미경적 연구 (AN ELECTRON MICROSCOPIC STUDY ON THE EFFECTS OF IRRADIATION ON THE BUCCAL MUCOSA OF RAT)

  • The author studied the acute reaction of cobalt-60 irradiation to buccal mucosa in rats and difference of the effects of single versus fractionated exposure. 195 Sprague Dowley strain rats, weighing about 120gm, were used in this experiment. 3 rats served as controls and the remaining 192 rats were divided into six groups of 32 rats each. Experimental group Ⅰ, Ⅱ, Ⅲ were received a single dose of 15Gy, 16.5Gy, 18Gy and group Ⅳ, Ⅴ, Ⅵ were received two equal sized fractionated dose of 9Gy, 9.75Gy, 10.5Gy at 4 hour intervals, respectively. The experimental groups were irradiated with cobalt-60 teletherapy unit, Picker model 4M 60 (Field size, 12x5 cm, SSD, 50㎝, Dose rate, 222cGy/min, Depth, 1㎝). The animals were sacrificed at 1, 2, 3, 6, 12 hours, 1, 3, 7 days after irradiation and the changes of the irradiated buccal mucosa were observed by electron and light microscopy. The results were as follows: 1. A single exposure was more damaging than fractionated exposure, and as the radiation dose increased, the changes of cell organelles became faster, but the healing of radiation-induced damage in fractionated exposure was faster than in single exposure. 2. The radiation-induced changes of the basal cells were the most prominent in 18Gy-single exposure group, and the least in 18Gy-fractionated exposure group. 3. Electron-microscopically, there appeared nuclear changes, swelling of mitochondria and rough endoplasmic reticulum, decrease of free ribosome, presence of vesicles, widening of intercellular space, and loss of basal lamina. The early remarkable changes were partly loss of nuclear membrane and swelling of mitochondria. 4. Light-microscopically, derangement and pyknosis of basal cells, hydropic changes of spinous cells, enlargement of granular cells, indistinctness of basement membrane, and proliferation of epithelium were observed.

  • All patients who will Undergo irraidiation of the oral cavity cancer will need dental before and during Radiotherapy. The extent of the region and the presence of numerous critical normal tissues(mucosa, gingiva, teeth and the alveolar ridge, alveolar bony structure, etc) in the oral cavity area, injury to which could result in serious functional impairment. Therefore I evaluate the Usefulness of custom-made intraoral shielding device before and during Radiotherapy in oral cavity cancer. 구내 촬영용 방사선 장치의 촬영조건에 따른 선량변화 분석 (Analysis of Condition Changing on Dose Variation using Intraoral Radiation Devices)

  • 본 연구는 구강 내 방사선 촬영 시 동일조건 하에서 촬영장비의 기종을 다르게 한 경우와 동일 기종에서 노출조건을 일정하게 하였을 경우 획득되는 방사선의 선량변화를 알아보고자 하였다. 장비의 기종에 따른 변화를 알아보기 위해 3대의 각기 다른 촬영 장치를 이용하여 상악 전치부, 소구치부, 대구치부의 3가지의 촬영조건을 이용하여 선량을 측정하였고 장치의 재현성 검사를 위해서 각 장치에 동일한 조건으로 3회 조사하여 선량변화를 알아보았다. 선량의 측정은 반도체 측정기를 사용하였으며, 필름의 농도값을 획득하여 진단에 유효적인 범위에 만족하는지 분석해 보았다. 같은 촬영기를 사용하여 실험한 결과 각 부위별로 선량의 차이는 최대값과 최소값이 적게는 3배 많게는 10배의 선량 차이를 보였다. 흑화도의 범위 또한 적절한 범위인 0.25~2.0를 초과한 2.10~2.95의 허용범위를 보였다. 동일한 촬영기로 특정 부위를 같은 조건에서 촬영하더라도 측정된 선량값은 큰 차이를 보였으며 필름의 농도 분석의 결과 적합한 농도 분포를 보이지 않았던 것으로 나타났다. 이는 환자의 재촬영을 발생시킬 수 있으며 진단시 오류를 일으킬 수 있어 환자의 피폭에 직 간접적인 피해를 일으킬 수 있다. 따라서 치과 방사선 촬영장치 또한 정기점검이 필요할 것으로 생각되며 표준화된 촬영조건과 현상조건이 제공되어야 한다고 사료된다.

Effects of radiation therapy on the dislocation resistance of root canal sealers applied to dentin and the sealer-dentin interface: a pilot study

  • Objectives: This study evaluated and compared the effects of radiation therapy on the dislocation resistance of AH Plus and BioRoot RCS applied to dentin and the sealer-dentin interface. Materials and Methods: Thirty single-rooted teeth were randomly assigned to 2 groups (n = 15 each): AH Plus (Dentsply DeTrey) and BioRoot RCS (Septodont). Each group was subdivided into control and experimental groups. The experimental group was subjected to a total radiation dose of 60 Gy. The root canals of all samples were cleaned, shaped, and obturated using the single-cone technique. Dentin slices (1 mm) were sectioned from each root third for the push-out test and scanning electron microscopy (SEM) was done to examine the sealer-dentin interface. The failure mode was determined using stereomicroscopy. Bond strength data were analyzed by the independent t-test, 1-way analysis of variance, and the Tukey post hoc test (α = 0.05). Results: Significantly lower bond strength was observed in irradiated teeth than non-irradiated teeth in the AH Plus group (p < 0.05). The BioRoot RCS group showed no significant reduction in bond strength after irradiation (p > 0.05) and showed a higher post-irradiation bond strength (209.92 ± 172.26 MPa) than the AH Plus group. SEM revealed slightly larger gap-containing regions in irradiated specimens from both groups. Conclusions: The dislocation resistance of BioRoot RCS was not significantly changed by irradiation and was higher than that of AH Plus. BioRoot RCS may be the sealer of choice for root canal treatment in patients undergoing radiation therapy.

Metal artifact reduction algorithm의 두경부 CT에 대한 적용 가능성 평가 (Evaluating applicability of metal artifact reduction algorithm for head & neck radiation treatment planning CT)

  • 목 적 : 두경부 치료계획 CT영상에서 dental implant로 인한 metal artifact 발생 시 O-MAR(Metal artifact Reduction for Orthopedic Implants)(ver. 3.6.0, Philips, Netherlands)를 적용할 수 있을지 여부를 평가하고자 한다. 대상 및 방법 : 모든 CT영상은 Brilliance Big Bore CT(Philips, Netherlands)에서 관전압 120kVp, 2mm 두께로 촬영하였으며, O-MAR를 이용하여 Metal artifact reduction 후 전산화치료계획장비(Eclipse ver 10.0.42, Varian, USA)로 원본영상과 비교, 분석하였다. O-MAR의 기본적인 성능 테스트를 위해 Metal artifact가 발생하지 않은 영상과 발생한 영상에서 O-MAR 적용시, HU 변화를 검증하기 위해 원통형 팬텀과 cerrobend 막대, 불균질 팬텀을 이용하여 실험하였다. 각각의 원본 영상과 O-MAR 적용 영상에서 관심영역 내 HU 변화를 측정하였다. 이를 바탕으로 본 연구의 주목적인 dental implant로 인한 metal artifact 발생 영상을 재현하기 위해 팬텀을 제작하여 사용하였고, 실제 임상 환자 영상에 O-MAR를 적용한 영상과 원본 영상 그리고 artifact 부분을 보정한 영상의 선량 분포를 SNC Patient(Sun Nuclear Co., USA)로 비교하였다. 결 과 : 두경부에서의 metal artifact를 재현한 원본 영상과 O-MAR 적용영상의 선량 분포를 비교한 결과 gamma passing rate 는 2 mm / 2% 기준으로 99.8%, 일치를 보였다. 실제 임상 환자 영상을 바탕으로 O-MAR 적용 전후 영상과 density corrected CT 영상에 동일한 조건으로 치료 계획을 수립하여 선량 분포를 비교한 결과는 98.5% 일치로 비교적 높은 gamma passing rate를 보였다. 전체적인 선량 분포 차이는 모두 2% 이내로 팬텀 실험과 실제 임상 환자 영상 실험에서 비슷한 결과로 나타났다. 하지만 선량 편차가 적더라도 국소적으로 집중되어 있는 것은 문제의 소지가 될 가능성이 있다. 화질 개선 면에서는 모든 실험에서 O-MAR 적용영상이 원본에 비해 개선됨을 알 수 있었으나, 두경부 metal artifact를 재현한 팬텀 영상 air cavity 내에서 최대 HU 값이 상승하는 경우가 생겼고, 환자 영상에서는 air cavity가 tissue로 잘못 보정되는 경우 또한 발견할 수 있었다. 결 론 : 업체에서 제시한 사용제한 사항인 피부 근처와 저밀도 영역이 공존하는 두경부에서 O-MAR의 사용 가능성을 확인해 본 결과, 원본의 왜곡과 보정이 동시에 일어났다. 심지어 팬텀 실험보다 더 심한 artifact가 생긴 환자의 경우 air cavity가 tissue로 잘못 보정되는 경우도 발생하였다. 결과적으로 아직까지는 O-MAR 알고리즘이 air cavity와 photon starvation artifact를 정확히 구분하지 못하는 것으로 보인다. 선량 측면에서의 영향은 임상에서 배제될 만큼 큰 차이를 보이지는 않았다. 임상에서 원본과 O-MAR 적용 영상을 비교하며 작업한다면 contouring, artifact 보정작업, DRR 화질 개선 등에 도움을 받을 수 있을 것으로 사료된다.

국내 의료기관에서 측방두부규격방사선촬영시 임상에서의 촬영조건 및 환자 선량 (Radiographic examination protocol and patient dose in lateral cephalometric radiography in Korea)

  • Purpose : To survey the radiographic examination protocol for lateral cephalometric radiographic examinations and to measure their patient doses in Korea and to compare the dose according to the size of hospital, the type of image receptor system, and the installation duration. Materials and Methods : The radiographic examination protocols (kVp, mA, and exposure time) for lateral cephalometric radiography were surveyed with 61 cephalometric radiographic equipments and their patient dose-area product (DAP) measured with a DAP meter (DIAMENTOR M4-KDK, PTW, Freiburg, Germany) for 51 cephalometric radiographic equipments. The radiographic examination protocols and patient doses were compared according to the size of hospital (university dental hospital, dental hospital, and dental clinic), the type of image receptor system (film-based, DR and CR type) and the installation duration, respectively. SPSS 12.0.1 for Windows (SPSS Inc., Chicago, USA) was used for independent t-test and ANOVA test. Results : The average protocols were 77.0 kVp, 12.7 mA, 6.2 second for cephalometric radiography. The average patient dose (DAP) was $128.0mGy\;cm^2$ and 3rd quartile dose (DAP) $161.1mGy\;cm^2$ for cephalometric radiography for adult male. There was no statistically significant difference at average patient DAP according to the size of hospital, the type of image receptor system, and the installation duration, repectively. Conclusion : The average patient dose was $128.0mGy\;cm^2$ and the third quartile patient dose $161.1mGy\;cm^2$ for lateral cephalometric radiography for adult male in Korea.

PCXMC 소프트웨어를 이용한 소아에서의 CBCT 환자선량 평가 (Dose estimation of cone-beam computed tomography in children using personal computer-based Monte Carlo software)

  • Objective: The purpose of the study was to calculate the effective and absorbed organ doses of cone-beam computed tomography (CBCT) in pediatric patient using personal computer-based Monte Carlo (PCXMC) software and to compare them with those measured using thermoluminescent dosimeters (TLDs) and anthropomorphic phantom. Materials and Methods: Alphard VEGA CBCT scanner was used for this study. A large field of view (FOV) (20.0 cm × 17.9 cm) was selected because it is a commonly used FOV for orthodontic analyses in pediatric patients. Ionization chamber of dose-area product (DAP) meter was located at the tube side of CBCT scanner. With the clinical exposure settings for a 10-year-old patient, DAP value was measured at the scout and main projection of CBCT. Effective and absorbed organ doses of CBCT at scout and main projection were calculated using PCXMC and PCXMCRotation software respectively. Effective dose and absorbed organ doses were compared with those obtained by TLDs and a 10-year-old child anthropomorphic phantom at the same exposure settings. Results: The effective dose of CBCT calculated by PCXMC software was 292.6 μSv, and that measured using TLD and anthropomorphic phantom was 292.5 μSv. The absorbed doses at the organs largely contributing to effective dose showed the small differences between two methods within the range from -18% to 20%. Conclusion: PCXMC software might be used as an alternative to the TLD measurement method for the effective and absorbed organ dose estimation in CBCT of large FOV in pediatric patients.

Late side effects of radiation treatment for head and neck cancer

  • Patients undergoing radiation therapy for head and neck cancer (HNC) experience significant early and long-term side effects. The likelihood and severity of complications depends on a number of factors, including the total dose of radiation delivered, over what time it was delivered and what parts of the head and neck received radiation. Late side effects include: permanent loss of saliva; osteoradionecrosis; radiation recall myositis, pharyngoesophageal stenosis; dental caries; oral cavity necrosis; fibrosis; impaired wound healing; skin changes and skin cancer; lymphedema; hypothyroidism, hyperparathyroidism, lightheadedness, dizziness and headaches; secondary cancer; and eye, ear, neurological and neck structures damage. Patients who undergo radiotherapy for nasopharyngeal carcinoma tend to suffer from chronic sinusitis. These side effects present difficult challenges to the patients and their caregivers and require life-long strategies to alleviate their deleterious effect on basic life functions and on the quality of life. This review presents these side effects and their management.

파노라마 촬영시의 두경부 주요기관의 등가선량, 유효선량 및 위험도 (EQUIVALENT DOSE, EFFECTIVE DOSE AND RISK ASSESSMENT FROM PANORAMIC RADIOGRAPHY TO THE CRITICAL ORGANS OF HEAD AND NECK REGION)

  • The purpose of this study was to evaluate the equivalent and effective dose, and estimate radiation risk to the critical organs of head and neck region from the use of adult and child mode in panoramic radiography. The results were as follows. 1. The salivary glands showed the highest equivalent and effective dose in adult and child mode. the equivalent and effective dose in adult mode were 837μSv and 20.93μSv, those in child mode were 462μSv and 11.54μSv, respectively. 2. Total effective doses to the critical head and neck organs were estimated 34.21μSv in adult mode, 20.14μSv in child mode. From these datas, the probabilities of stochastic effect from adult and child mode were 2.50x10/sup -6/ and 1.47x10/sup -6/. 3. The other remainder showed the greatest risk of fatal cancer. The risk estimate were 4.5 and 2.7 fatal malignancies in adult and child mode from million panoramic examinations. The bone marrow and thyroid gland showed about 0.1 fatal cancer in adult and child mode from these examinations.
