Housing Needs of Korean Family with Demented Elderly (한국치매노인 가족의 주거환경 개선요구)

  • Most elderly victims of senile dementia in Korea are cared for at home. This creates hardship for their families. Recent research suggests that the physical environment can affect the treatment of dementia. In light of these findings, a prudent approach to the design of the housing environment of family with dementia sufferers is advisable. This study aims to determine the design factors that need to be considered for the housing environment of dementia, particularly for those suffering from Alzheimer's or Cerebrovascular dementia. For this study, and in-depth qualitative investigation method was employed. Principal caregivers were interviewed using various investigative tools, including objective measures, structured and non-structured questionnaires. The investigation results suggest that the need for housing environment improvement varies with the severity and type of dementia. The housing needs in this study, is expected to promote a focus on improved design management for the environment and housing of dementia and their families. The results also suggest the establishment of a welfare policy for senile dementia victims and their families.

The Effect of Cognitive Stimulation Training on Elderly Persons with Dementia (치매노인을 대상으로 한 인지 자극 훈련의 효과)

  • This study was conducted to test the effect of cognitive stimulation training on elderly persons with dementia. The design of the research was one group in pre-test-post-test design. The subjects were nine demented persons over sixty years, with mild to moderate cognitive impairment. Training was administered by research assistants on a one to one basis for thirty to forty minutes, three times a week for eight weeks. In order to evaluate the effect of cognitive stimulation training, we measured cognitive function before and after three training sessions each. Data were analyzed using descriptive statistics and a paired t-test analysis using a spss pc package, The results are as follows: 1) The recipients of the training program showed improvement in overall cognitive functioning. The MMSK - K score, recall levels of concepts, daily tasks, personal past history and performance of word fluency were significantly increased after training. 2) There was a significant improvement in cognitive functioning over the training period: recall levels of concepts, daily tasks, past personal history and performance of word fluency significantly increased over the training period progessively, The results suggest that cognitive stimulation training is effective in improving and maintaining overall cognitive function of elderly persons with dementia.

Importance of and Influencing Factors on Nurses' Caring Behaviors for Elders with Dementia (간호사가 인지한 치매노인 돌봄행위의 중요도 및 영향요인)

  • Purpose: The purpose of this study was to investigate the importance of nurses' caring behaviors and to identify influencing factors on nurses' caring behaviors for elders with dementia. Methods: Data were collected through questionnaires from 156 nurses in several demented elderly setting and analyzed by applying descriptive statistics, t-test, ANOVA, Scheffe-test, Pearson's correlation coefficient and stepwise multiple regression with SPSS/WIN 16.0. Results: The average score for importance of nurses' caring behaviors was 3.55 and the 'attentive to other's experience' factor was the highest level. The mean score of nursing professional value was 8.46 and job satisfaction, 3.86. Importance of nurses' caring behaviors had a significant positive correlation with nursing professional value, job satisfaction and nurse's age. Four significant variables influencing importance of nurses' caring behaviors were job satisfaction, nursing professional value, working area of nurses and cognition dysfunction of elderly. Conclusion: With the results of this study, importance of nurses' caring behaviors can be improved by intervening the factors affecting this importance. Based on the findings of this study, further nursing practice and research for nurses' caring the elders with dementia should be focused on job satisfaction, nursing professional value, working area of nurses and cognitive dysfunction of elderly.

The Effects of a Support Program for Family Caregivers of Elderly with Dementia on Empowerment and Attitudes toward Dementia (치매노인 가족 지지프로그램이 치매가족의 임파워먼트와 치매태도에 미치는 효과)

  • Purpose: This research was conducted to identify the effects of providing support programs to families of elderly with dementia on family empowerment and attitudes toward dementia. Methods: A nonequivalent control group pre- and post-test design was used. Participants were 49 family caregivers, experimental (24) and control (25), recruited from families through a dementia support center in Y district. This program implemented Haearim, a support program for family caregivers of demented elders developed by the National Institute of Dementia in 2016. Outcome measures were empowerment and attitudes toward to dementia. Data were analyzed with a ${\chi}^2-test$, independent t-test, Fisher's exact probability test, paired t-test, and repeated measures ANOVA with SPSS/PC version 20.0. Results: Family empowerment (F=6.84, p=.002) and family caregivers' attitudes (F=16.48, p<.001) toward dementia in the experimental group improved significantly more than that of the control group after intervention. Conclusion: Results indicate that support programs for families of elderly with dementia are effective in positively changing empowerment and attitudes toward dementia among family caregivers.

Factors Influencing Cognitive Impairment of the Elderly Residents (재가노인의 인지기능장애에 영향을 미치는 요인)

  • Purpose: This study was aimed at finding factors influencing cognitive impairment which is one of the typical symptoms among the demented elderly. Methods: The number of subjects was 417 elderly residents aged over 65 yr in a community. A cognitive function and a depression level were measured using the Korean Version of Mini-Mental State Examination (MMSE-K) and the Korean Form of Geriatric Depression Scale (KGDS). Alcohol abuse was measured using the CAGE instrument. Chi-square test and Logistic regression analysis were conducted to identify factors affecting cognitive impairment. Results: The prevalence rates of cognitive impairment of the sample was 43.5%. Sex, age, educational level, perceived health and alcohol abuse were strong factors influencing cognitive impairment. However, the effects of smoking, living alone, depression, family history in dementia and stroke were not strong. Conclusion: Risk for cognitive impairment were increased by being female and older than 70 yr, having low education, perceiving health as poor, and drinking alcohol abusively. Therefore, a reinforcement system, continuous research and the development of proper programs should be preformed in order to prevent cognitive impairment.

A Study on the Cognitive Rehabilitation Project of the Elderly with Moderate Memory Dementia (기억학교 경증치매노인의 인지재활사업에 관한 연구)

  • The purpose of this study was to develop a cognitive rehabilitation program based on the cognitive activities of the elderly with mild dementia using the weekly care facilities in Daegu city memory and to provide useful welfare services for the elderly with mild dementia. In Korea, there is not much development of community-based integrated intervention program for demented patients who are still in the present situation. The results of this study suggest that the cognitive rehabilitation program for the elderly with mild dementia is suitable to be applied as a group in the case of community based protective editorials and may be a basis for management of mild dementia patients. It seems that there may be differences in the appropriate intervention methods and effects. Therefore, it is expected that the individualized customized strategy will maximize the effect. Therefore, more specific and individualized nursing intervention program should be concurrently developed.

The Effects of Wholeness Program Using Virtual Reality(VR) on Cognitive Function, Depression, and Activities of Daily Living for the Elderly with Demented (VR 활용 홀니스 프로그램이 치매노인의 인지기능과 우울 및 일상생활수행능력에 미치는 효과)

  • This study analyzed the effects of participating in wholeness programs according to virtual reality(VR) viewing status on the cognitive function, depression and activities of daily living of elderly with dementia. The subjects were 8 elderly persons aged 75 years or older who were diagnosed with dementia by a neurologist who were divided into an experimental group of 4 subjects that participated in a wholeness program after 15 minutes of VR viewing and a control group of 4 subjects that participated in a wholeness program without viewing VR. Data were collected through pre and post test, and analyzed in the wholeness program conducted at K1 and K2 Senior Welfare Centers in Seoul from March 6 to April 14, 2017, twice every week for 60 minutes. As a result, there was no effect on cognitive function and activities of daily living, but there was a significant difference in depression, and it was confirmed that wholeness program activity after VR watching decreased depression of elderly with dementia.

Comparison in Care Burden, Fatigue, and Life Contentment of Caregivers by Gender Relationship with Demented Elders (치매노인과 주부양자의 성별 관계에 따른 주부양자의 부양부담감, 피로 및 생활만족도 비교)

  • Purpose: This study was done in order to examine the differences in gender respect to care burden, fatigue, and life contentment of family caregivers of elderly with dementia. Methods: The setting of the study was a community-based dementia care center providing daycare services. One hundred ninety four caregivers were recruited for the survey. A structured questionnaire was used which included demographic information, care burden, fatigue, and general contentment scales. Caregivers were classified into 4 groups by gender relationship with care recipients. Results: The majority of the caregivers were middle-aged and elderly. More than twice as many were female rather than male caregivers. The results showed that women caregivers expressed more fatigue than men overall (p < .05). In the gender relationships, a different combination of man-woman appeared to have more burdens than that of a woman-woman group (p < .05). For fatigue, the man caregiver-woman elderly group showed a higher fatigue score than a woman-man group (p < .05). However, there was no significant difference in life contentment among groups. Conclusion: These results suggest that caregiver's gender and gender relationship with elders could be considerable factors when the nurses make a plan in the community for dementia nursing management services.

A Study on the Nutrient Intakes of Hospitalized Elderly Dementia Patients Receiving a Regular Diet

  • The purpose of the present study was to examine the nutrient intake status of elderly dementia patients. We surveyed the dietary intake of 50 demented elderly patients receiving a regular diet, who were hospitalized in a geriatric hospital in Yongin, Gyeonggi-do. The average age of the subjects was $79.6{\pm}6.5$ for the males and $80.5{\pm}6.3$ for the females. The average heights were $169.5{\pm}6.0\;cm$ (males) and $154.6{\pm}5.2\;cm$ (females), and the average weights were $58.5{\pm}7.4$ (males) and $51.7{\pm}8.9\;kg$ (females). Depending on the type of dementia, the male patients showed significant differences in their intakes of vegetable protein, fiber, total iron, and non-heme iron between the Alzheimer's disease (AD) and the vascular dementia (VD) groups, and the female patients showed significant differences in their intakes of total protein, phosphorus, zinc, and vitamin $B_6$ between the two groups. The male patients showed significant differences in their intakes of animal-source calcium and carotene according to ability to self-feed. According to physical activity, the male patients showed significant differences in vegetable fat and vitamin A intake, and the females showed significant differences in their intakes of total protein, animal protein, fiber, ash, total calcium, vegetable-source calcium, animal-source calcium, phosphorous, total iron, non-heme iron, heme iron, sodium, vitamins $B_1$, $B_2$, $B_6$, niacin, vitamin C, folate, and potassium. We found that the patients consumed excessive protein, but low amounts of calcium, vitamin $B_2$, and folate. Also, the patients' physical activity abilities appeared to affect their nutrient intakes.

Effect of Horticultural Therapy on the Stress and Serum Cortisol of Demented Elders (원예치료가 치매노인의 스트레스 및 혈중 코티졸 호르몬에 미치는 영향)

  • This study was conducted in order to examine the effects of horticultural therapy on stress in demented elders, and for this purpose, we analyzed stress-related physiological changes in demented elders under the care of an elderly facility who had not taken medication and consented to blood tests. The horticultural therapy program included only flower decoration, which was preferred by the participants, aiming ultimately to reduce stress. First, a one-to-one session was given for forming rapport and the horticultural therapist had many conversations about plants with the elders. Furthermore, the participants were induced to be absorbed into the horticultural activities, though a short while, so that they might forget troubles. Evaluation was made by measuring subjective stress and the blood cortisol hormone concentration before and after the horticultural therapy and examining differences between them. Subjective stress increased from 12.88 to 17.88 in the control group, but decreased significantly from 13.88 before the therapy to 6.38 after in the experimental group (p = 0.007). When the blood cortisol hormone level was measured as a physiological indicator of stress, it decreased significantly from $11.33{\mu}g{\cdot}L^{-1}$ before the horticultural therapy to $9.85{\mu}g{\cdot}L^{-1}$ after (p = 0.037). These results suggest that horticultural therapy reduces the blood cortisol hormone level and is effective in easing stress.
