Predictors of massive transfusion protocols activation in patients with trauma in Korea: a systematic review

  • Purpose: Massive transfusion protocols (MTPs) implementation improves clinical outcomes of the patient's resuscitation with hemorrhagic trauma. Various predictive scoring system have been used and studied worldwide to improve clinical decision. However, such research has not yet been studied in Korea. This systematic review aimed to assess the predictors of MTPs activation in patients with trauma in Korea. Methods: The PubMed, Embase, Cochrane Library, Research Information Sharing Service databases, KoreaMed, and KMbase were searched from November 2022. All studies conducted in Korea that utilized predictors of MTPs activation in adult patients with trauma were included. Results: Ten articles were eligible for analysis, and the predictors were assessed. Clinical assessments such as systolic and diastolic blood pressure, shock index (SI), prehospital modified SI, modified early warning system (MEWS) and reverse SI multiplied by the Glasgow Coma Scale (rSIG) were used. Laboratory values such as lactate level, fibrinogen degradation product/fibrinogen ratio, and rotational thromboelastometry (ROTEM) were used. Imaging examinations such as pelvic bleeding score were used as predictors of MTPs activation. Conclusions: Our systematic review identified predictors of MTPs activation in patients with trauma in Korea; predictions were performed using tools that requires clinical assessments, laboratory values or imaging examinations only. Among them, ROTEM, rSIG, MEWS, SI, and lactate level showed good effects for predictions of MTPs activation. The application of predictors for MTP's activation should be individualized based on hospital resource and skill set, also should be performed as a clinical decision supporting tools.

  • Beam-column joints in the frame structure are at high risk of brittle shear failure which would lead to significant residual deformation and even the collapse of the structure during an earthquake. In order to improve the damage issue and enhance the recoverability of the beam-column joints, a sector lead rubber damper (SLRD) has been developed. The SLRD can increase the bearing capacity and energy dissipation capacity, and also demonstrating recoverability of seismic performance following cyclic loading. In this paper, the hysteretic behavior of SLRD was experimentally investigated in terms of the regular hysteretic behavior, large deformation behavior and fatigue behavior. Furthermore, a parametric analysis was performed to study the influence of the primary design parameters on the hysteretic behavior of SLRD. The results show that SLRD resist the exerted loading through the shear capacity of both rubber parts coupled with the lead cores in the pre-yielding stage of lead cores. In the post-yielding phase, it is only the rubber parts of the SLRD that provide the shear capacity while the lead cores primarily dissipate the energy through shear deformation. The SLRD possesses a robust capacity for large deformation and can sustain hysteretic behavior when subjected to a loading rotation angle of 1/7 (equivalent to 200% shear strain of the rubber component). Furthermore, it demonstrates excellent fatigue resistance, with a degradation of critical behavior indices by no more than 15% in comparison to initial values even after 30 cycles. As for the designing practice of SLRD, it is recommended to adopt the double lead core scheme, along with a rubber material having the lowest possible shear modulus while meeting the desired bearing capacity and a thickness ratio of 0.4 to 0.5 for the thin steel plate.

  • A feasibility test for accelerating composting of sewage sludge irradiated with electron beam(3kGy) was investigated. Dried wood chip and leaves were used as a bulking agent and carbon source, respectively. The test variables included changes in temperature, organics and nutrients, and bioactivities from experimental and control composters. Results indicates that the temperature rose up to $60^{\circ}C$ within 1 day and maintained high temperature above $50^{\circ}C$ for more than 5 days in the irradiated sludge cake composter. It resulted in the fast degradation of organics during the initial 5 days, showing that approximately 70% of total amount of carbon degraded within 20 days was destroyed. It is likely that the composting of electron beam irradiated sludge cake is able to reduce it's maturing period significantly.

Bacillus siamensis 3BS12-4 Extracellular Compounds as a Potential Biological Control Agent against Aspergillus flavus

  • Aspergillus flavus, the primary mold that causes food spoilage, poses significant health and economic problems worldwide. Eliminating A. flavus growth is essential to ensure the safety of agricultural products, and extracellular compounds (ECCs) produced by Bacillus spp. have been demonstrated to inhibit the growth of this pathogen. In this study, we aimed to identify microorganisms efficient at inhibiting A. flavus growth and degrading aflatoxin B1. We isolated microorganisms from soil samples using a culture medium containing coumarin (CM medium) as the sole carbon source. Of the 498 isolates grown on CM medium, only 132 bacterial strains were capable of inhibiting A. flavus growth. Isolate 3BS12-4, identified as Bacillus siamensis, exhibited the highest antifungal activity with an inhibition ratio of 43.10%, and was therefore selected for further studies. The inhibition of A. flavus by isolate 3BS12-4 was predominantly attributed to ECCs, with a minimum inhibitory concentration and minimum fungicidal concentration of 0.512 g/ml. SEM analysis revealed that the ECCs disrupted the mycelium of A. flavus. The hydrolytic enzyme activity of the ECCs was assessed by protease, β-1,3-glucanase, and chitinase activity. Our results demonstrate a remarkable 96.11% aflatoxin B1 degradation mediated by ECCs produced by isolate 3BS12-4. Furthermore, treatment with these compounds resulted in a significant 97.93% inhibition of A. flavus growth on peanut seeds. These findings collectively present B. siamensis 3BS12-4 as a promising tool for developing environmentally friendly products to manage aflatoxin-producing fungi and contribute to the enhancement of agricultural product safety and food security.

Identification of Active Agents for Reductive Dechlorination Reactions in Cement/Fe (II) Systems by Using Cement Components (시멘트 구성성분을 이용한 시멘트/Fe(II)의 TCE 환원성 탈염소화 반응의 유효반응 성분 규명)

  • Experimental studies were conducted to identify the active agents for reductive dechlorination of TCE in cement/Fe(II) systems focusing on cement components such as CaO, $Fe_2O_3$, and $Al_2O_3$. A hematite that was used to simulate an $Fe_2O_3$ component in cement was found to have degradation efficiencies (k = 0.641 $day^{-1}$) equivalent to that of cement/Fe(II) systems in the presence of CaO/Fe(II), only when it contained an aluminum impurity$(Al_2O_3)$. When the effect of $Al_2O_3$ content of hematite/CaO/$Al_2O_3$/Fe(II) system was tested, the mole ratio of $Al_2O_3$ to CaO affected the rate of TCE degradation with an optimum ratio around 1 : 10 that resulted in a rate constant of 0.895 $day^{-1}$. In the SEM images of hematite/CaO/$Al_2O_3$/Fe(II) systems, acicular crystals were also found that were also observed in cement/Fe(II) systems. Thus it was suspected that these crystals were reactive reductants and that they might be goethite or ettringite that are known to have acicular structures. An EDS element map analysis revealed that these crystals were not goethite crystals. A subsequent experiment that tested reactivities of compounds formed during the ettringite synthesis showed that ettringite and minerals associated with ettringite formation are not reactive reductants. These observations conclude that a mineral containing CaO and $Al_2O_3$ with a acicular structure could be a major reactive reductant of cement/Fe(II) systems.

Bioenergy and Material Production Potential by Life Cycle Assessment in Swine Waste Biomass (전과정 평가에 의한 양돈 바이오매스의 물질 및 에너지 자원화 잠재량 연구)

  • As a result of the growing livestock industry, varieties of organic solid and waste biomass are be generated in swine breeding and slaughtering stages. Anaerobic digestion is a promising alternative for the treatment of livestock waste biomass, as well as for the material recovery and energy production. Objectives of this study were to analyze the biochemical methane potential of swine waste biomasses that were generated from swine pen and slaughterhouse and to investigate the material recovery and methane yield per head. As pig waste biomass, swine slurry, blood, intestine residue, and digestive tract content were collected for investigation from pig farmhouse and slaughterhouse. The $B_{th}$ (Theoretical methane potential) and $B_0$ (Biochemical methane potential) of swine slurry generating in swine breeding stage were 0.525 and $0.360Nm^3\;kg^{-1}-VS_{added}$, the ratio of degradation ($B_0/B_{th}$) was 68.6%. $B_{th}$ of blood, intestine residue, and digestive tract content were 0.539, 0.664, and $0.517Nm^3\;kg^{-1}-VS_{added}$, and $B_0$ were 0.405, 0.213, and $0.240Nm^3\;kg^{-1}-VS_{added}$, respectively. And the ratio of degradation showed 75.1, 32.1, and 46.4% in blood, intestine residue, and digestive tract content. Material yield of swine waste biomass was calculated as TS 73.79, VS 46.75, TN 5.58, $P_2O_5$ 1.94, and $K_2O$ $2.91kg\;head^{-1}$. And methane yield was $16.58Nm^3\;head^{-1}$. In the aspect that slaughterhouse is a large point source of waste biomass, while swine farmhouse is non-point source, the feasibility of an anaerobic digestion using the slaughtering waste biomass need to be assessed in the economical aspect between the waste treatment cost and the profitable effect by methane production.

Adaptive Block Watermarking Based on JPEG2000 DWT (JPEG2000 DWT에 기반한 적응형 블록 워터마킹 구현)

  • In this paper, we propose and verify an adaptive block watermarking algorithm based on JPEG2000 DWT, which determines watermarking for the original image by two scaling factors in order to overcome image degradation and blocking problem at the edge. Adaptive block watermarking algorithm uses 2 scaling factors, one is calculated by the ratio of present block average to the next block average, and the other is calculated by the ratio of total LL subband average to each block average. Signals of adaptive block watermark are obtained from an original image by itself and the strength of watermark is automatically controlled by image characters. Instead of conventional methods using identical intensity of a watermark, the proposed method uses adaptive watermark with different intensity controlled by each block. Thus, an adaptive block watermark improves the visuality of images by 4$\sim$14dB and it is robust against attacks such as filtering, JPEG2000 compression, resizing and cropping. Also we implemented the algorithm in ASIC using Hynix 0.25${\mu}m$ CMOS technology to integrate it in JPEG2000 codec chip.

Studies on Photocatalytic Thin Films($TiO_2$, TiO-N) Manufactured by DC Magnetron Sputtering Method and it's Characteristics for Removal of Pollutants (DC 마그네트론 스퍼터링법을 이용한 광촉매박막($TiO_2$, TiO-N)제조 및 오염물질 제거에 관한 연구)

  • [ $TiO_2$ ] was deposited by DC magnetron sputtering on glass surface under various sputtering parameters such as discharge power($0.6{\sim}5.2\;kW$, substrate temperature($R.T{\sim}350^{\circ}C$), Ar and $O_2$ flow ratio with $0{\sim}50\;sccm$($Ar+O_2$ 90 sccm) and about 1 mtorr of pressure. TiO-N thin film was prepared under same sputtering conditions for $TiO_2$ thin film except flow ratio($Ar+O_2+N_2$ 90 sccm). The sheet resistance of thin films deposited under these parameters was measured to analyze electronic characteristic and thin film's thickness(${\alpha}$-step), surface roughness(AFM) and formation construction(FE-SEM, XRD) were also measured to draw optimal sputtering parameters. In order to evaluate photo-activity of thin film($TiO_2$, TiO-N) made in optimal parameters for removal of pollutants, toluene among VOCs and Suncion Yellow among reactive dyes were chosen to probe organic compounds for photo-degradation. It was shown that the photo-catalytic thin films had a significant photo-activation for the chosen contaminants and especially TiO-N thin film showed maximum efficiency of 33% for toluene(5 ppm) removal in visible-light range.

Preparation and Characterization of Synthetic Hydroxyapatite/Polyacrylic Acid Homogeneous Composite (합성 Hydroxyapatite/Polyacrylic Acid 균질복합체의 제조 및 특성)

  • Hydroxyapatite(HAp) powders were synthesized by wet-precipitation precess using $Ca(NO_3)_2{\cdot}4H_2O$ and $(NH_4)_2HPO_4$ and homogeneous composites of four type were prepared by mixing of synthetic HAp and Polyacrylic Acid(PAA). Ca/P ratio of synthetic HAps was determined using ICP analysis and the thermal property of HAp/PAA composites were investigated by TGA. Good crystalline HAp was obtained at pH 11 and $60{\circ}C$. The ratio of Ca/P in synthetic HAps was quantified in a range of 1.35~1.49, from which Ca-deficient HAp was obtained. The specific surface area of HAp/PAA composite increased with increasing the content of PAA and the weight loss of HAp/PAA composite at $800{\circ}C$ decreased in a range of 3.5~9.6% due to the degradation of PAA binder. From FT-IR analysis of HAp/PAA composite, it was confirmed that the ionic bond between ion of HAp and carboxyl group of PAA was formed.

The Effect of Trivalent Cation Doping on the Low Temperature Phase Stability of 2Y-TZP (3가 양이온 산화물이 첨가된 2Y-TZP의 저온 상안정성)

  • The phase stability and the Low Temperature Degradation(LTD) mechanism of Tetragonal Zirconia Polycrystals(TZP), sintered specimens of $Y_2O_3$-Stabilized Zirconia(2Y-TZP), doped with trivalent cations, were evaluated by investigating meachnical properties, Raman spectra, lattice parameter variation and the oxygen vacancy behavior under applied electric field. XRD observation was put forward on 2Y-TZP doped with trivalent cation whose ionic radii were larger than $Zr^{4+}(Sc^{3+},\;Yb^{3+},\;Y^{3+},\;Sm^{3+},\;Nd^{3+},\;La^{3+})$ up to 2 mol% and sintered at 1500 h for 1h. For $La^{3+}$ doping, the stability of tetragonal phase was degraded due to the formation of the pyrochlore phase $(La_2Zr_2O_7)$ as the dopant content increased above exceeded 0.5 mol%. As the dosage increased, tetragonal phase maintained for the case of $Sc^{3+}$, whose radius was similar to $Zr^{4+}$, on the other hand, the cubic phase was formed for the cases of $Yb^{3+},\;Y^{3+},\;Sm^{3+},\;Nd^{3+}$. As the radii of dopant cation increased, c/a ratio increased and it was experimentally observed that the amount of monoclinic phase decreased when the specimens were annealed at $220{\circ}C$ for 500 h.
