The Nurse Staffing in Intensive Care Units based on Nursing Care Needs: A Multicenter Study (중환자 간호요구도에 근거한 중환자실 간호사 배치수준 산정 : 다기관 연구)

  • Purpose : The aim of this study was to propose appropriate nurse staffing of adult intensive care units considering patients' nursing care needs according to the Workload Management System for Critical Care Nurses (WMSCN). Methods : In a cross-sectional survey conducted in September 2017, 1,786 patients' WMSCN scores, surveys from 2,145 nurses, and administrative data from 118 units in 41 hospitals were analyzed. The means (standard deviations) of the aforementioned scores and nursing hours per patient day were presented. Nurse-to-patient ratios and nurse-to bed ratios for staffing to meet patients' nursing care needs were calculated. Results : The mean WMSCN scores were 109.50±17.17 in tertiary hospitals and 96.38±19.26 in general hospitals. Nursing hours per patient day were 12.47±2.80 in tertiary hospitals and 11.01±2.45 in general hospitals. Nursing hours per patient day correlated with WMSCN scores. Nurse-to-bed ratios required for the provision of ICU nursing care ranged from 1: 0.36 to 1: 0.48. Conclusion : Our findings provide evidence that current ICU nurse staffing is insufficient for meeting patients' nursing care needs. We suggest adjusting the legal standards for adequate nurse staffing considering these needs.

  • This narrative inquiry examines the experience of mentally ill patients who used the psychiatric day hospital care service. The researchers in this inquiry conducted in-depth interviews three times with four patients who used day hospital care and data was also collected from additional conversations when patients were on outpatient visit after being discharged from the day hospital. The inquiry uses the narrative research method to describe the experience of using day hospital services as follows; a springboard to reconnect to the world, a place to be empowered and set the mind at ease, and a guide to direction in life. This inquiry is meaningful in that it deals with the in-depth stories of the mentally ill patients who experienced the day hospital care. Based on the results of the inquiry, the following policy suggestions are made; institutional support for reducing social isolation of psychiatric patients, activation of various support systems and further program development.

Nutrition Survey of Young Children of Day Care Centers in the Rural Area of Hongcheon-gun, Kangwon Province (강원도 홍천군 농촌유아원 어린이의 영양실태조사)

  • A untrition survey of early childhood was undertaken among 102 rural young children, aged 3 to 6, attending three day care centers of Hongcheon-gun, Kangwon province, in July of 1987, to investigate dietary and nutritional status. The results were obtained as follows: Mean values of height and arm circumference ranged from 96 to 97% of the KSRI's standards. However, mean value of weight met 91% of that standard. Mean value of hematocrit was 37.3±3.5%. According to the criterion established by the WHO, 3% of the subjects were proven to be anemic. Mean value of urinary urea nitrogen/creatinine ratio was 13.8±7.6; the higher the age lower the ratio was shown. Intake of energy and nutrients ranged from 63 to 88% of the RDAs. Carbohydrate provided 68% of total energy intake; protein accounted for 14%; fat provided 18%. Energy intake was divided among breakfast, lunch supper and snacks in a percentage ratio of 21:35:26:18. The survey clarified that the day care feeding largely supplemented the inadequate dietary intakes of these young children at home. Family factors, anthropometric measurements, biochemical results were positively correlated with nutrient intakes. Authors with this study can be contributed, as a reference, to develop the community nutrition programs as well as improving the quality of day care feeding.

  • The purpose of this study was to assess the nutrition education and training needs as a basis for program planning for day care children. Nutrition achievement test developed by American National Dairy Council and Iowa state University was modified and used for measuring the level of basic knowledge of food and nutrition for day care children. The results of this study can be summarized as follows 1. Over 70% of mothers indicated their children had a certain degree of food behavior and feeding problems in terms of unbalanced food habit, irregular meal period. 2. Mean score for the nutrition achievement test for day care children was 9.3. 3. The nutrition achievement test score for children with high socio-economic status was significantly higher than those of children with middle and lower group. 4. Inspection of the difficulty difference indices showed that over the 50% of the children had more items that were easier in the content of physiological facts than in the food handling and social psychological aspects of foods.

  • The purpose of this study is to examine the differences in young children's multiple intelligence and social behaviors based not only on Montessori and general programs of day care centers but also on their gender and social- demographic factors. The subjects were 329 young children (182 boys and 147 girls aged from four to five) from two day care centers in Cheongju city. The instruments used in this study were HAPI (Hillside assessment of Perceived Intelligence) to measure the surveyee's multiple intelligence and Park & Lee's scale to measure their social behaviors. The frequencies, percentiles, mean, standard deviations, Cronbach a, t-teat, F and LSD were used to analyze the collected data through SPSS WIN program. The results of this study are as follows: 1) As for young children's multiple intelligence, there are differences according to young children's gender, order of birth, parents' level of education and teacher's working period out of social-demographic factors. 2) As for the young children's social behavior, there are differences according to young children's gender, mother's level of education and teacher's working period out of social-demographic factors.

A Study on Parents' Actual Uses and Satisfaction toward the Occupational Therapy Service at the Day Care Center for Infants with Disabilities - Focusing on Daegu, Kyongbuk area - (장애아전담보육시설을 이용하는 부모들의 작업치료 서비스 이용실태 및 만족도 조사 - 대구·경북지역을 중심으로 -)

  • Objective : The study is to suggest basic data for the expansion and the necessity of the occupational therapy at the day care center for infants with disabilities through investigating actual use and satisfaction of parents who use the day care center for infants with disabilities in Daegu Kyonbuk area. Methods : From May 15 to 29, 2009, 105 parents using the day care center for infants with disabilities were surveyed. Results : Terms of age group, parents using the day care center for infants with disabilities are mostly 30s and 40s. In terms of academic background, there were 58% of secondary school graduates and 38.9% of college graduates. Parents with experiencing 3 years of the occupational theory had 37.1%, which held the largest group. 75% of the objects agreed that infants with disabilities received enough occupational therapy, while they pointed out that the financial burden for therapy would be the major factor in the therapy. Conclusion : In order to provide a qualified occupational therapy in accordance with each individual's purpose at the day care center for infants with disabilities, it would be necessary to expand occupational therapy, to increase the demand for the occupational therapy from parents, and to secure enough number of therapists and financial support from the government and municipalities.

A Validation Study for the Infant/Toddler Environment Rating Scale(ll) (영아/걸음마기 아동 환경 평정 척도 타당화 연구 (ll))

  • This study is the second study which was aimed to validate the Infant/ Toddler Environment Rating Scale(ITERS). The test-retest reliability included 10 infant/toddler classes among 30 national/public and private child care centers in Inchon city which had been observed in the first validation study. The Spearman's correlation coefficient for test-retest reliability was ranged from .52 to .84. In addition, two raters observed 10 family day care centers in Inchon city, where two observers independently rated each centers on the ITERS during a single visit. The Spearman's correlation coefficient for interrater reliability was ranged from.72 to .99 For the second measure of content validity, experts who were working in day care centers and who were graduate students, lecturers, or professors majoring child studies rated the appropriateness of each ITERS items for the provision of high-quality programs for infants and toddlers, using a 1(low) to 5(high) scale. The mean rating was ranged from 2.52 to 4.52. In sum, when the rating scores were analyzed by the type of child care centers, national/public child care centers have better quality than private or family child care centers.

Changes in Parenting Support Policy and Opening Day Nurseries to Local Communities in Japan (일본 자녀양육지원정책의 변화와 보육소의 지역사회 개방(開放))

  • This study reviews recent changes in parenting support policy in Japan, and examines the roles and functions of day-nursery as the base for parenting support in the local community. The purpose of this study is to provide implications for the direction of the parenting support policy and child-care facilities for the local communities in South Korea. The analysis concentrates on the official national reports such as the Declining Birthrate White Paper and Japan's National Guidelines for Care and Education at Day Nursery by the Japanese Cabinet Office. The study finds that the Japanese government has introduced comprehensive medium and long-term policies for all the members of the community with recognizing the changes in the domestic and foreign child-rearing environments and social needs. These policies have also helped to improve the social awareness of parenting. The role of the day-care center has been raised as a significant part of the support for child-rearing in the local community due to the revision of the Child Welfare Act for nursery school teachers in 2001 and the official announcement of Japan's National Guidelines for Care and Education at Day Nursery in 2008. The study also concludes that the social responsibility of the nursery center has been strengthened by being utilized as a public resource for the community. From the findings, we can elicit the conclusions and implications that the roles and functions of child care centers as a significant base for parenting support in the local communities need to be considered. We also listen to the voices of stakeholders such as parents and staff of child care centers. Last but not least, we would understand the social role and importance of bringing up children by cooperation of the community members, and promote the role of child care centers as a central place providing and communicating information on child-rearing in the local communities.

A Study on the Effectiveness of Care of Patients with Alzheimer s Disease According to Residence Arrangement and Types of Services (치매노인의 거주형태 및 서비스유형에 따른 간호관리의 효과분석)

  • The problem of care of patients and families with Alzheimer's disease has become a conscious raising social policy issue in Korea. The government of the Republic of Korea has become cognizant of the situation and has begun searching for ways to remedy it. Thus, there is a need for a comprehensive under-standing of the situation in which patients and their families are struggling and the enormous problems of care. With a realization of the urgent need, this study was done to investigate the situation and the care needs of families with patients with Alzheimer's Disease, and to compare the effectiveness of services utilized by the families in terms of cost and effects on patient's conditions and on family live. The Subjects for the study were 29 families with hospitalized patients, 25 families utilizing hospital outpatient clinics, 14 families utilizing day care facilities, and 16 families with homebound patients. A total of 84 families were interviewed by four trained interviewers using structured and semistructured questionnaires. The data produced from these interviews included : the patient's stage of Alzheimer's disease, patient's bizarre behavior, hours spent on patient care per day, family burden and quality of life, direct and indirect costs encountered in the care of patients, and the families' evaluation of the effectiveness of the services received. The data were analyzed to determine the relationships between family charactersistics, patient's conditions and services utilization. The effectiveness of each of the service entities was assessed through families evaluation and hoped for service and comparisons were made between services in terms of the cost-effectiveness ratios. After initial comparison of cost-effectiveness ratios, further analysis was done to compare between groups for incremental effectiveness for each incremental unit of cost to determine the most cost-effective service entities. The findings of the study are as fellows : 1. The choice of living arrangement and the types of services are a function of the stage of Alzheimer's condition and the economic status of the family. 2. Comparision of the cost of care showed that most expenses were encountered in by families with hospitalization, families using outpatient services, and families using day care services in that order. The least expense was involved in the care of homebound patients. The economic burden felt by families was in the same order as expenses. 3. The average number of hours spent on daily patient care was 9.9 hours for the outpatient clinic users, 9.7 hours for homebound patients, and 5.4 hours for day care users. 4. There were significant differences in the patient's conditions (CDRL), bizarre behaviors and the families's burden by living arrangement and /or types of service. However, no significant difference was found between groups in the family's quality of life. 5. The families rated the services of day care center as most effective for the care of the patients and families, except for a few families who had experienced some improvement in the patient's conditions. The outpatient clinic users expressed psychological comforts mainly in that the patient was being taken care of. For those hospitalized patients, families expressed the comfort of being relieved of the burden of care and that the patient is being professionally cared for. Form the analysis of the costs, hours of patient care, patient's bizarre behaviors, family's quality of life and burdens, and family's evaluation of services, it is concluded that up to the mid stage of Alzheimer's condition, the utilization of day care center services is found to be the most cost-effective, and toward the end stage of the Alzheimer's disease, it is hoped that there will be a establishment of long term or short term in-patient facilities for the protection of patients and preservation of the integrity of families for less cost. Thus. it was concluded that the family centered system of care is the most effective for Korea with systematic support systems developed for the care of patients and their families according to the needs of families as the patient's condition deteriorates.
