A Qualitative Study on the Daily Help Substitution for the Household Work (가사도우미에 의한 가사노동대체, 문화기술적 사례 연구)

  • The purpose of this ethnography is to describe and analyze the employment and substitutional use of daily help for the household work. Ten housewives and one educator of daily help from Gwangju City were interviewed with an unstructured questionnaire. They experience many difficulties from this daily help employment and use, even though they gain some basic help from their service. The diversity of the routes to seek out this daily help means that there is no proper route. And the daily help's sudden quit causes many realistic and psychological problems. Frequent disagreement between the user's demand and help's implement comes from communication failure. Three different management patterns of daily help were issued ; 'conservative-dominant', 'rational-mandatory', 'powerless-dependent'.

Effects of Self-Help Program in Daily Living and Self-Care Activities, Flexibility, Grip Strength and Depression in Patients having Chronic Arthritis (자조관리과정이 만성관절염 환자의 일상생활과 자기간호활동, 유연성, 악력 및 우울에 미치는 효과)

  • The purpose of this study was to examine the effects of self-help program on patients with chronic arthritis. This study was performed from 13th of August to 29th of October 2002. 30 arthritis patients with an average age of 56 years were participated in the self-help program. At the completion of 6 weeks, 23 patients completed both pretest and posttest, overall dropout rate was 23%. Outcome measures were status of daily living, self-care activities, flexibility, grip strength and depression. Data were analysed by percentage, paired t-test, Pearson's correlation coefficient. There were statistically significant in self-care activities, flexibility, grip strength after the self-help program. However no significant differences were found in daily living and depression. There were significant relationships between daily living and depression. There were significant relationships between right flexibility & left flexibility and right and left grip strength. In conclusion self-help program was proved to be an effective nursing intervention to increase the self-care activities, flexibility and grip strength. These results of this study suggested that the follow-up program after the self-help program should be run to help the physical psychological wellbeing of arthritis patients.

Responsibilities and Difficulties of Caregivers of Cancer Patients in Home Care

  • Background: Having been known as a virulent disease in 1970s, cancer is now onsidered a chronic disease and 64% of cancer patients live for five years after diagnosis. Home care has gradually gained more importance and it is a great burden on the shoulders of caregivers. Caregivers have to undertake the responsibility of the cancer patient's home management, and organize care and arrange health care services according to the ever-changing condition of patients. Caregivers should be prepared for home care so they can provide accurate and complete care to patients. This descriptive study aims to investigate challenges that caregivers encounter in the home care of patients and the reasons for these challenges. Materials and Methods: The research group consisted of caregivers of outpatients in a daily treatment center in a university hospital. The research sampling consisted of 137 voluntary caregivers of patients who attended the Daily Treatment Center for control, chemotherapy or other supportive cares services between January-June, 2011. Data were collected with face-to-face interviews in the Daily Treatment Center. Ethics Committee approval was taken university hospital; caregivers and their patients were informed about the research and their approval was taken as well. Results: It was found that 54.01% of caregivers help patient's nutrition, 50.36% help medicine use, 26.28% help oral hygiene, 26.28% help to meet urinary needs and 51.82% help to change clothes, 69.34% of caregivers help to change bed sheets, 38.69% help the patient to communicate with their environment and 71.53% help to bring the patient to hospital or outside. Conclusions: This study, it was found that caregivers experience challenges due to following factors: patient nutrition, medicine use, oral and body hygiene, colostomy maintenance and stomach tube feeding, concern of dropping the patient, feeling incompetency in body temperature and fever control, fatigue, and lack of personal time.

A Study of Activities of Daily Living(ADL) in Patients with Rheumatoid Arthritis (류마티스성 관절염 환자의 일상생활활동 조사연구)

  • This study attempts to provide the rudimentary data that might help patients with Rheumatoid Arthritis manage an independent life by the analysis of their activities of daily living(ADL). The subjects were forty patients who had been diagnosed Rheumatoid Arthritis and under drug therapy from RA clinics of the two university hospitals in Pusan. Data were collected by performing an individual interview for each of them during June 15th through July 15th 1998. Questionaires were imployed to investigate the general characteristics of patients. the traits of Rheumatoid Arthritis, and actual conditions of their activities of daily living. The questionaires utilized especially for the study of ADL is the version of Lee Eun Ok's tool which modified and complemented Katz scale (Katz, Down, Cogh and Grots 1970) and Bathel index(Bathel. 1973). It's consisted of 31 questions that each question is measured by 0-3points. The high score in the questionaire signifies the high independency in the ADL. The data were analyzed by SPSS/Win in terms of frequency, percentage, mean, and standard deviation, close connection with the contents of the personal interview. Total mean of the ADL shows $1.55{\pm}0.68$, indicating the overall activities of daily living is generally dependent. It also indicates that patients, in the condition of dependent activity. tends either to have recourse to drugs or to take negative strategy, getting help from inside or outside family member. Hence, it is necessary that patients are exposed to the introduction and instruction of self-help devies that might help them manage a comparatively independent ADL, easing their paychological burden.

The Effect of Self-Help Management Program by Strengthen Self-Efficacy on Self-Efficacy and the Activities of Daily Living in Stroke Patients (자기효능증진 자조관리 프로그램이 뇌졸중 환자의 자기효능과 일상생활동작 수행능력에 미치는 효과)

  • Purpose: This study examined the effect of a self-help management program which strengthens self-efficacy on the self-efficacy and the activities of daily living(ADL) in stroke patients. Methods: A quasi-experimental nonequivalent control group, pre and post design was used. The subjects consisted of 47 stroke patients. Twenty-six patients were assigned to the experimental group and 21 patients were assigned to the control group. The self-help management program by strengthen self-efficacy and general hospital care were carried out on the experimental group. General hospital care was performed on the control group. The data was collected before and 5 weeks after the program. All subjects were questioned about the self-efficacy and ADL before and after the program. Results: The scores of the self-efficacy and ADL in the experimental group were higher than those in the control group. Conclusion: The self-help management program by strengthen self-efficacy is effective in increasing the self-efficacy and ADL. Therefore this program may be a useful intervention to increase the ADL in stroke patients.

The Effects of Self-help Management Program for Post Stroke Patients at Home (재가뇌졸중 환자를 위한 자조관리 프로그램의 효과 연구)

  • Purpose: The purpose of this study was to determine the effects of 5 weeks self-help management program developed by Kim et al. (2000-b) and reinforced with music therapy for post stroke patients. Method: This was pre-experimental study and the subjects were 14 post stroke hemiplegic patients at home who were registered at the Gunsan Health Center. The program was applied for five weeks and two hours for each session composed of the preparation step, the main step and the finishing step. The contents of the program were ROM exercise, daily activity training, risk factors and aftereffects, nutrition management and stress management. To collect data, all subjects were questioned before and after the application of the program. Collected data were analyzed through frequencies, percentages, Wilcoxen signed rank test and Cronbach's alpha using SPSS-WIN program. Result: After the application of the self-help management program, a statistically significant increase was observed in the subjects' ability to perform activities of daily living (p=.039), ability to perform instrumental activities of daily living (p=.005), self-efficacy (p=.001), self-care behavior (p=.001) and quality of life (p=.001), and a statistically significant decrease was observed in depression (p=.012). Conclusion: The present self-help management program was found to be helpful in improving the subjects' physical and psychological functions after they were attacked by stroke, so to be an effective nursing intervention strategy for post stoke patients. Future researches need to reinforce and materialize music therapy and to develop and apply a self-help management program that includes not only post stroke patients but also their families. In addition, it is necessary to expand the scope of subjects and apply follow-up management in order to continue self-help meetings.

Effects of Tai Chi combined with Self-help Program on Arthritic Symptoms and Fear of Falling in Women with Osteoarthritis (골관절염 여성에게 적용한 타이치 운동과 자조관리 프로그램의 관절염증상 및 낙상공포감에 대한 효과비교)

  • Purpose: The purpose of the study was to compare the effects of the Tai Chi self-help program with a self-help program on pain, stiffness, physical functioning, and fear of falling in women with osteoarthritis. Method: Total of 82 women with Osteoarthritis recruited from outpatient clinics or community health centers were randomly assigned either to a Tai Chi self-help group or a self-help group. Thirty subjects in the Tai Chi self-help group and 39 in the self-help group completed posttest measures(pain, stiffness, physical functioning, and fear of falling) at 6 months. Results: After the 6 month study period, the subjects in the Tai Chi self-help group reported significantly less difficulties in performing daily activities(mean difference=-0.16 vs. 6.19 for the self-help), and less fear of falling(mean difference=-3.20 vs. 0.27 for the self-help) than those in the self-help group. There were no significantly differences in pain and stiffness between the groups. Conclusion: The Tai Chi combined with self-help program was more effective than the self-help only program in reducing difficulties in performing daily activities and fear of falling in women with Osteoarthritis. Whether these changes prevent fall episodes requires further study.

The Effects of Instrument-Activities Daily Living Training through Client-Centered Home Visitation on Cognitive Functions, Occupational Performance, and Instrument-Activities Daily Living among Elderly at the Cognitive Support Grade (클라이언트 중심 가정방문 일상생활훈련이 인지지원등급, 노인의 인지기능, 작업수행, 일상생활수행도에 미치는 영향)

  • Purpose : This study aims to investigate the effect of instrument-activities daily living training through client-centered home visitation on the cognitive functions, occupational performance, and instrument-activities daily living of elderly at the cognitive support grade(Grade6). Methods : The subject of this study was a 66-year-old woman living in G Metropolitan City, who has been diagnosed with Alzheimer's and mild dementia. The study period was from March 17, 2020 through June 12, 2020, and the A-B-A' design, among the individual case experiments, was adopted as the study design. For the data analysis, descriptive statistic and visual analysis using graph were used for the change of cognitive functions, occupational performance, and instrument-activities daily living. Results : The instrument-activities daily living provided through client-centered home visitation improved the subject's cognitive functions, occupational performance(performance, satisfaction) and instrument-activities daily living. Conclusion : This study showed that daily life training through client-centered home visitation can help elderly people at the cognitive support grade select for themselves the problems of daily life caused by cognitive decline and practice specific action plans, thereby enabling them to maintain and improve the cognitive functions necessary for the performance of activities, such as comprehension, memory, and thinking skills. In addition, it is thought that the activities based on the subject's preferences, performance, and sense of importance assured the subject of feelings of motivation and the possibility of participation, and had a positive effect on the subject's performance speed and rate. With the above in mind, Instrument-activities daily living client-centered home visitation is proposed as a potential practical intervention program for individuals. It can help elderly people at cognitive support grade to maintain and improve their functions, thereby delaying the progress of their condition to severe dementia.

Effects of 5 Weeks Self-Help Management Program on Reducing Depression and Promoting Activity of Daily Livings, Grasping Power, Hope and Self-Efficacy (재가 뇌졸중환자를 위한 5주간의 자조관리프로그램의 효과에 관한 연구)

  • The purpose of this study was to determine the effects of 5 weeks self-help management program on reducing depression, promoting Activity of Daily Livings(ADLs), Instrumental Activity of Daily Livings(IADLs), grasping power, hope and self-efficacy for post stroke patients visiting public health care centers in Seoul. This was pre-experimental study, and the subjects were 27 post stroke hemiplegic patients. The program was composed of five sessions and each session had health education on stroke, ROM exercise and recreation. Data were collected from May to November, 2000, and all subjects were asked to complete the Questionnaires, be measured vital signs and grasping power. Data were analyzed with frequency, percent, paired t-test, and Pearson's correlation coefficient using SAS(version 6.12) program. The results were as follows : 1) The scores of ADLs were increased from 27.04 to 28.22 after program, and that was statistically significant(p=.005). 2) The scores of IADLs were increased from 18.70 to 19.78 after program, and that was statistically significant(p=.004). 3) The grasping power of right hand were increased from 21.87kg to 26.93kg after program, and that was statistically significant(p=.002). But the grasping power of left hand were statistically insignificant(p=.919). 4) The scores of depression were decreased from 39.63 to 35.30 after program, and that was statistically significant(p=.030). 5) The scores of hope were increased from 30.89 to 34.15 after program, and that was statistically significant(p=.002). 6) The scores of self-efficacy were increased from 67.70 to 76.37 after program, and that was statistically significant(p=.000). According to the results of this study, the scores of Activity of Daily Livings(ADLs), Instrumental Activity of Daily Livings (IADLs), hope, and self-efficacy and the grasping power were improved and depression was reduced in post stroke patients participating in self-help management program. Therefore we recommend to use self-help management programs as a nursing intervention for the post stroke patient.

A Correlational Study on Activities of Daily Living, Self-efficacy, Stroke Specific Qualify of Life and Need for Self-help Management Programs for Patients with Hemiplegia at Home (재가 뇌졸중환자의 일상생활활동, 자기효능감, 삶의 질, 자조관리프로그램요구도와의 관계에 관한 연구)

  • The purpose of this study was to identify levels of activity of daily living, self-efficacy. stroke specific quality of life and need for self-help management program for patients with hemiplegia in the home. Data were collected from June to November, 2000 and subjects were 88 poststroke patients who lived in Seoul and Kyunggi-do. The questionnaire consisted of 5 scales: activities of daily living, self-efficacy, stroke specific qulaity of life and need for a self-help management program. Data were analyzed using frequencies, percent, paired t-test, and Pearson's correlation coefficient with the SAS(version 6.12) program. The results are as follows ; 1) Most of subjects were Partially independent in ADL, but they needed assist once to do dressing, bathing meal preparation and house keeping work. 2) The mean self-efficacy score was 54.89(range : 1 to 80) and the individual differences were large. 3) Subjects responded that they were satisfied on the stroke specific quality of life scale totaled 65.8%. This value is comparatively low, especially for social role(51.4%), family functioning(58.3%) and mood (62.2%). 4) The highest needs for self-help management programs were for physical therapy, stress management, and range of motion exercise and the lowest needs were for elimination management and training, family counseling, and speech therapy. 5) On the demographic variables, sex showed significant differences for the dependent variables. Females had higher scores than males for IADL, self-efficacy, stroke-specific quality of life, and need for self-help management. 6) Age had high negative correlation with ADL, self-efficacy and stroke specific quality of life. Age was also correlated with need for self-help management. In conclusion, there was a high correlation for ADL, Self-efficacy and Quality of life in poststroke patients of home. The patient with a stroke also had a strong need for self-help management programs especially physical therapy and stress management. Therefore rehabilitation programs based on self-efficacy enhancement need to be developed in order to promote independent living for patients with hemiplegia.
