Comparative Analysis between Multilevel Sectioning with Conventional Haematoxylin and Eosin Staining and Immunohistochemistry for Detecting Nodal Micrometastases with Stage I and II Colorectal Cancers

  • Management of patients with stage II colorectal carcinomas remains challenging as 20 - 30% of them will develop recurrence. It is postulated that these patients may harbour nodal micrometastases which are imperceptible by routine histopathological evaluation. The aims of our study were to evaluate (1) the feasibility of multilevel sectioning method utilizing haematoxylin and eosin stain and immunohistochemistry technique with cytokeratin AE1/AE3, in detecting micrometastases in histologically-negative lymph nodes, and (2) correlation between nodal micrometastases with clinicopathological parameters. Sixty two stage I and II cases with a total of 635 lymph nodes were reviewed. Five-level haematoxylin and eosin staining and one-level cytokeratin AE1/AE3 immunostaining were performed on all lymph nodes retrieved. The findings were correlated with clinicopathological parameters. Two (3.2%) lymph nodes in two patients (one in each) were found to harbour micrometastases detected by both methods. With cytokeratin AE1/AE3, we successfully identified four (6.5%) patients with isolated tumour cells, but none through the multilevel sectioning method. Nodal micrometastases detected by both multilevel sectioning and immunohistochemistry methods were not associated with larger tumour size, higher depth of invasion, poorer tumour grade, disease recurrence or distant metastasis. We conclude that there is no difference between the two methods in detecting nodal micrometastases. Therefore it is opined that multilevel sectioning is a feasible and yet inexpensive method that may be incorporated into routine practice to detect nodal micrometastases in centres with limited resources.

위암 환자의 골수에서 발견된 Cytokeratin 양성세포의 임상적 의의 (Cytokeratin-positive Cells in the Bone Marrow of Patients with Gastric Cancer)

  • 목적: 암 환자의 골수에서 발견되는 cytokeratin 양성세포와 암의 재발과의 상관관계에 대해서는 알려진 바가 많지 않다. 이에 위암환자의 골수에서 발견되는 cytokeratin 양성세포가 위암 환자의 재발과 생존율을 예측할 수 있는지 알아보고자 하였다. 대상 및 방법: 1998년 6월부터 2000년 7월까지 경북대학교병원 외과에서 원발성 위암으로 수술받은 환자 419명을 대상으로 하였다. 수술 직전 장골능선에서 골수를 흡인하여 단핵구를 분리하고 항 cytokeratin 항체를 이용하여 면역세포화학적 염색을 하였다. 결과: Cytokeratin 양성세포는 219예(52.4%)에서 발견되었고, 위암의 침윤깊이(P=0.021), 병기(P=0.026)에 따라서 통계학적으로 유의한 차이가 있었으나, 암의 위치, 육안형, 림프절전이, 원격전이, 분화도에 따라서는 유의한 차이가 없었다. 골수의 cytokeratin 양성세포 유무에 따른 5년 생존율은 유의한 차이가 없었고(P=0.248), 재발여부, 재발부위도 유의한 차이가 없었다. 결론: 위암 환자의 골수에서 cytokeratin 양성세포 유무는 예후인자로 사용되기 어렵고 재발양상을 예측하기도 어렵다.

개 세르토리세포종 케이스에서 면역조직화학적 마커를 통한 진단 (Diagnosis of immunohistochemical marker expressed by a canine Sertoli cell tumor case)

  • Sertoli cell tumor (SCT) of the testicle arises from the supporting cells within the seminiferous tubules. SCT is common in dogs, especially in cryptorchid testicles, but also has been reported in the stallion, ram, cat, and bull. Sertoli cell tumor sample was collected from 7-years male german shepherd. In this study, SCT arose from one testicle. Sample size is approximately 1.7 cm in diameter and it has a round form. In the microscopic, cells within the tumor variably resemble Sertoli cells (SCs) that normally populate the seminiferous tubules and interstitial area. There is abundant stroma of dense, mature fibrous connective tissue in SCT. In the immunohistochemical staining, cytokeratin AE1/AE3 was not expressed in the control and SCT. S-100 protein was expressed by SCs, germ cells and fibrous connective tissue of SCT. Melan A was expressed by leydig cells (LCs) of SCT. A study by using S-100 and melan A in canine SCT was almost never carried out. S-100 and melans A is considered to suggest for diagnosis and pathogenesis of canine SCTs. Inhibin-alpha and Vimentin were well known as the marekers of SCTs. Also, they were expressed by Sertoli cells and LSs of SCT in this study.

슈나우져 개에서 발생한 전립선 선암종 (Prostatic Adenocarcinoma in a Schnauzer Dog)

  • 배뇨곤란, 혈뇨 및 간헐적인 구토를 보이는 8살의 중성화된 수컷 슈나우져가 병원에 내원하였다. 투시기 하에서 양성 요도 조영을 실시 하였고, 전립선 주위 요도에서 조영제가 누출 되는 것을 확인 하였다. 또한 전립선 요도부터 방광 삼각까지의 충만 결손이 관찰 되었다. 수술적인 교정이 부분적인 방광 폐쇄로 인해 실시 되었고 수술 중 주변 장기로 전이 소견이 확인되어 안락사를 실시 하였다. 주변 조직의 전이를 확인 하고 위하여 Cytokeratins (AE1/AE3) 를 이용하여, 주위 림프 조직을 염색하여, 전립선의 상피 세포 유래의 세포가 림프절에 전이 되었음을 확인 하였다. 본 증례는 수컷 개의 전립선 선암종의 발생 증례로 임상 소견, 영상 진단, 조직 병리학적인 자료를 기술 하였다.

결합조직형성소원형세포종양의 압착도말 세포학적 소견 -1예 보고- (Imprint Cytology of a Desmoplastic Small Round Cell Tumor -A Case Report-)

  • Desmoplastic small round cell tumor (DSRCT) is a rare malignant mesenchymal neoplasm. It mainly involves the abdominal or pelvic peritoneum of male adolescents. We report here the imprint cytologic features of a case of DSRCT occurring in the intraabdominal cavity of a 21-year-old man. A microscopic examination showed moderate cellularity. The tumor cells were singly arranged and arranged in clusters. The cells had round to oval nuclei with finely granular chromatin, inconspicuous nucleoli and scanty cytoplasm. Some tumor cells showed nuclear molding, and some cells had an epitheloid appearance with a large amount of lightly eosinophilic cytoplasm. A rosette-like pattern was present. Spindle-shaped, fibroblastic stromal cells were occasionally found. The tumor cells were immunoreactive for the markers cytokeratin (AE1/AE3), epithelial membrane antigen (EMA), desmin, vimentin and neuron specific enolase (NSE).

병리적 병기 1기의 비소세포폐암 환자에서 면역조직화학염색에 의한 림프절 미세전이 관찰 (Immunohistochemical Detection of Lymph Nodes Micrometastases in Patients of Pathologic Stage I Non-small-cell Lung Cancer)

  • 연구 배경 : 병리적 병기 I기의 비소세포 폐암 환자를 대상으로 림프절 미세 전이의 빈도와 임상적 의의를 알아보고자 연구를 하였다. 방 법 : 수술적 절제를 시행한 29예의 환자에서 총 806개의 파라핀 포매 상태로 보관된 종격동 림프절을 이용하였다. 미세 전이를 확인하기 위하여 림프절 파라핀 불럭으로 2개의 절편을 만들었고 첫 번째 절편은 H & E 염색을, 두 번째 절편은 cytokeratin AE1/AE3 항체를 이용하여 암세포의 미세 전이를 확인하고자 면역 조직화학 염색을 하였다. 결 과 : 대상 환자의 병리적 병기 IA는 10예 이었고 IB는 19예이었다. 면역 조직화학 염색을 시행한 파라핀 블록 수는 총 246개이었고 림프절 수는 총 806개이었다. H & E 염색으로 총 806개의 림프절을 관찰하였으나 암세포의 미세 전이는 관찰되지 않았다. Cytokeratin AE1/AE3 항체를 이용한 면역 조직화학 염색에서 총 806개의 림프절 중 3개(0.37%)에서 관찰되었다. 대상 환자의 평균 관찰 기간은 34.2개월이었고 관찰 기간 중 9예에서 사망이 관찰되었다. 사망 원인은 폐암 진행: 4예, 수술 후 합병증: 3예, 동반 질환: 2예 이었다. 질병의 진행으로 사망한 환자 모두에서 림프절 미세 전이 소견이 관찰되지 않았으며, 림프절 미세 전이가 관찰된 3예의 환자 모두 생존 중이다. 결 론 : 림프절 미세 전이를 발견을 위한 면역 조직화학 염색은 시행한 806개의 림프절중 0.37%에서만 미세전이가 관찰되어 임상적 상용에 어려움이 있을 것으로 생각된다.

말티즈 개에서 발생한 갑상선수질암 (Medullary Thyroid Carcinoma in a Maltese Dog)

  • 11세령, 체중 2.67 kg, 암컷 Maltese의 기관 인근 경부에서 3주 전부터 종괴가 촉진되어 동물병원에 내원하였으며, 방사선 및 초음파 검사 결과 기관 및 미주신경 주위에 불투과성의 종괴가 확인되었다. 외과적으로 절제된 단일한 종괴의 크기는 $3.5{\times}2{\times}0.8cm$였다. 병리조직학적으로 큰 종양 병소가 정상 갑상선 조직과 혼재되어 있었다. 종양 병소는 형질세포와 같은 형태를 보이는 종양세포들이 모여 있는 다양한 크기의 소포로 구성되어 있고, 이 소포들은 미세한 섬유혈관중격에 의해 구획되어 있었다. 종양세포들은 인근 결합조직과 피막으로 침습하는 경향을 나타내었다. 면역조직화학염색을 실시한 결과 종양세포들은 사이토케라틴(AE1/AE3), 크로모그래닌 A, 신경원 특이 에놀라제(NSE)에 대하여 강한 양성 반응을, vimentin에는 음성 반응을 보이고 있었다. 육안병변, 병리조직학적 및 면역조직화학 염색의 특징을 토대로 본 증례는 갑상선수질암으로 진단되었다.

Spontaneous Peripheral Ameloblastic Odontoma in a Male Sprague-Dawley Rat

  • Peripheral ameloblastic odontoma is a rare variant of odontogenic tumor occurring in the extraosseous region. The present report describes a spontaneous tumor in male Sprague-Dawley (SD) rats. The clinically confirmed nodule in the right mandibular region was first observed when the rat was 42 weeks and remained until the terminal sacrifice date when the animal was 48 weeks of age. At necropsy, a well demarcated nodule, approximately $2.5{\times}2.0{\times}2.0cm$, protruded from the ventral area of the right mandible. The nodule was not attached to mandibular bone and was not continuous with the normal teeth. Histopathologically, the tumor was characterized by the simultaneous occurrence of an ameloblastomatous component and composite odontoma-like elements within the same tumor. The epithelial portion formed islands or cords resembling the follicle or plexiform pattern typical of ameloblastoma and was surrounded by mesenchymal tissue. Formation of eosinophilic and basophilic hard tissue matrix (dentin and enamel) resembling odontoma was observed in the center of the tumor. Mitotic figures were rare, and areas of cystic degeneration were present. Immunohistochemically, the epithelial component was positive for cytokeratin AE1/AE3 (CK AE1/AE3), and the mesenchymal component and odontoblast-like cells were positive for vimentin, in the same manner as in normal teeth. On the basis of these findings, the tumor was diagnosed as a peripheral ameloblastic odontoma in an extraosseous mandibular region in a SD rat. In the present study, we report the uncommon spontaneous peripheral ameloblastic odontoma in the SD rat. We also discuss here the morphological characteristics, origin, histochemical, and immunohistochemical features for the diagnosis of this tumor.

슈나유저 개의 소장에 샘암종과 평활근육종의 동시 발생 1례 (Co-existence of Intestinal Adenocarcinoma and Leiomyosarcoma in a Schnauzer Dog)

  • A 7-year-old castrated male Schnauzer was presented with melena and inappetence. Laboratory examination revealed mild anemia. Abdominal ultrasonography showed abnormal enlargement of intestinal segment and a oval mass with soft tissue density. After surgical resection of the enlarged intestine including the mass, histopathologic examination showed that the mass was tentatively diagnosed as synchronous occurrence of gland cell- and mesenchymal cell-origin tumors. Subsequently, immunohistochemistry showed positivity to cytokeratin AE1/AE3 in the gland cells and positivity to ${\alpha}-smooth$ muscle-specific actin, but negative expression of c-Kit, suggesting the co-existence of adenocarcinoma and leiomyosarcoma. Follow-up examination after 3-year of the surgery confirmed that the dog remained healthy and did not show recurrence of the tumors.

Cytokeratin의 RT-PCR 및 면역조직화학적 분석을 이용한 구강편평세포암종의 임파절 미세전이 진단과 예후인자 효용성 평가 (DIAGNOSIS OF MICROMETASTASIS IN LYMPH NODE AND CLINICAL EVALUATION OF PROGNOSTIC FACTOR OF ORAL SCC USING RT-PCR AND IMMUNOHISTOCHEMISTRY FOR CYTOKERATIN)

  • Purpose: The lymph node status assessed by conventional histological examination is the most important prognostic factor in patients undergoing surgery for oral squamous cell carcinoma. The presence of lymph node metastasis has a strong adverse impact on patient survival even after extended radical resection. Despite these findings, tumour recurrence is not rare after surgery, even when histological examination shows no lymph node metastasis. Recently, molecular-genetically and immunohistochemically demonstrated micrometastasis to the lymph nodes has been shown to have a significant adverse influence on survival in patients with squamous cell carcinoma and histologically negative nodes. The present study sought to determine the incidence and clarify the clinical significance of molecular-genetically and immunohistochemically demonstrated nodal micrometastases and to correlate these data with the stage of oral cancer. Methods: Lymph nodes systematically removed from 71 patients who underwent curative resection between 1998 and 2003 with head and neck squamous cell carcinoma were examined molecular-genetically to detect cytokeratin 5 mRNA with RT-PCR and immunohistochemically to detect cells that stained positively for cytokeratins with the monoclonal antibody cocktail AE1/AE3. The postoperative course and survival rates were compared among patients with and without micrometastases, after numerical classification of overt metastatic nodes. Results: micrometastases were detected in 43(60%) of 71 patients by RT-PCR and 26(36%) of 71 patients by immunohistochemistry. By RT-PCR analysis, patients exhibiting a positive band for CK 5 mRNA had a significantly worse prognosis than those were RT-PCR negative. By immunohistochemistry, the presence of micrometastasis did not predict patient outcome. Conclusion: Micrometastases detected by RT-PCR may be of clinical value in identifying patients who may be at high risk for recurrence and who are therefore likely to benefit from systemic adjuvant therapy.
