Mesenteric pseudocyst is rare. This term is used to describe the abdominal cystic mass, without the origin of abdominal organ. We presented a case of mesenteric pseudocyst of the small bowel in a 70-year-old man. Esophago-gastro-duodenoscopy showed a 3.5 cm sized excavated lesion on the posterior wall of angle. Endocopic biopsy confirmed a histologic diagnosis of the poorly differentiated adenocarcinoma, which includes the signet ring cell component. Abdominal computed tomography scan showed a focal mucosal enhancement in the posterior wall of angle of the stomach, a 2.4 cm sized enhancing mass on the distal small bowel loop, without distant metastases or ascites in rectal shelf, and multiple gallbladder stones. The patient underwent subtotal gastrectomy with gastroduodenostomy, segmental resection of the small bowel, and cholecystectomy. The final pathological diagnosis was mesenteric pseudocyst. This is the first case report describing incidentally detected mesenteric pseudocyst of the small bowel in gastric cancer patients.

Arcuospathidium is a haptorian ciliate genus composed of 18 species, and only one species has been reported in Korea. Here, we identify two unrecorded Arcuospathidium subspecies by morphological observation of both living and protargol-impregnated specimens with the small subunit ribosomal RNA (18S rRNA) gene sequence. These subspecies, Arcuospathidium cultriforme cultriforme (Penard, 1922) Foissner, 1984 and A. cultriforme scalpriforme (Kahl, 1930) Foissner, 2003, were isolated from various terrestrial habitats in July and August 2013, respectivley. Arcuospathidium cultriforme cultriforme is similar to A. cultriforme scalpriforme by a knife-shaped body, a twisted-shaped macronucleus, number of dorsal brushes, position of dorsal brushes, and shape of macronucleus but former mainly differs from the body length to oral bulge length ratio (27-38% vs. 41-53%), extrusome (one types vs. three types), cyst shape (roughly faceted wall vs. smooth surface and thin wall) and number of somatic kinety rows(18-30 vs. 30-44). Additionally, we analyzed the 18S rRNA gene sequences of two A. cultriforme subspecies and compared them with the sequences from GenBank to confirm their identification at the molecular level. As the results of genetic analysis, the 18S rRNA gene sequence of the Korean A. cultriforme cultriforme population is most similar to that of Austrian population. Also, the sequence of the Korean A. cultriforme scalpriforme population is most similar to that of another population with some nucleotide differences.
We report a rare case of cerebellar liponeurocytoma with an unusually aggressive histopathology. A 49-year-old man presented with a four-month history of headache, vertigo, and progressive swaying gait. Magnetic resonance imaging showed a $3{\times}3.5cm$ sized relatively well-demarcated round mass lesion in the fourth ventricle, characterized by high signal intensity on T2-weighted images. Postcontrast images revealed strong enhancement of the solid portion and the cyst wall. The patient underwent suboccipital craniectomy and tumor removal. The pathologic diagnosis was cerebellar liponeurocytoma. Adjuvant radiotherapy was offered due to concerns related to the high proliferative index (Ki-67, 13.68%) of the tumor. At the last routine postoperative follow-up visit (12 months), the patient complained of no specific symptom and there was no evidence of tumor recurrence. However, longterm follow-up and the analysis of similar cases are necessary because of the low number of reports and the short follow-up of cases.
Spinal schwannomas are usually extramedullary intradural tumors and their intramedullary localizations are thought to be extremely rare. A 60-year-old woman complaining spastic quadriparesis, voiding difficulty and dyspnea was admitted. Her cervical MRI revealed an intramedullary mass in the cervicomedullary junction with multiple cyst which extended from lower cervical to C3 spinal cord. The mass showed a low signal on T1WI, high signal on T2WI with an wall enhancement. The patient underwent a suboccipital craniectomy and C1-2 laminectomy and the cystic tumor was totally removed through a midline myelotomy. The tumor was proved as an intramedullary schwannoma by pathologic examination. The Intramedullary presence of a tumor arising from the cells of the nerve sheath is unusual, because the central nervous system fibers do not contain the Schwann cell. There have been several hypotheses, but none has been accepted universally. This rare tumor is considered as a curable benign neoplasm, and an accurate intraoperative diagnosis and surgically total removal are essential.
Echinococcosis is a multisystem disease and has propensity to involve any organ, an unusual anatomical site, and can mimic any disease process. Primary peritoneal echinococcosis is known to occur secondary to hepatic involvement but occasional cases of primary peritoneal hydatid disease including pelvic involvement have also been reported. We report here 1 such case of primary pelvic hydatidosis mimicking a malignant multicystic ovarian tumor where there was no evidence of involvement of the liver or spleen. Our patient, a 27-year-old female, was detected to have a large right cystic adnexal mass on per vaginal examination which was confirmed by ultrasonography. Her biochemical parameters were normal and CA-125 levels, though mildly raised, were below the cut off point. She underwent surgery and on exploratory laparotomy, another cystic mass was found attached to the mesentery of the small gut. The resected cysts were processed histopathologically. On cut sections both large cysts revealed numerous daughter cysts. Microscopic examination of fluid from the cysts revealed free scolices with hooklets and the cyst wall had a typical laminated membrane with inner germinal layer containing degenerated protoplasmic mass. The diagnosis of pelvic hydatid disease was confirmed and patient was managed accordingly. Hydatid disease must be considered while making the differential diagnosis of pelvic cystic masses, especially in endemic areas.
The cytological and ultrastructural findings of Pneumocystis carinii(PC) obtained from rats by bronchoalveolar lavage (BAL) are described. All developmental forms of the PC organisms were obtained in the lavage fluid. Papanicolaou stain revealed conglomeration of PC as a foamy cast. The cystic walls of PC were well identified on Gomori's methenamine silver stain. Trophozoites and intracystic bodies were stained by Giemsa and Diff-Quik techniques. Some PC organisms were seen within the alveolar macrophages. Ultrastructurally, the cysts were almost circular in shape, and were nearly devoid of surface tubular extensions. The wall of the cyst was composed of an unit membrane, an intermediate electron lucent layer and an external electron dense layer The cysts frequently contained intracystic bodies, so called sporozoites. Occasionally empty or collapsed cysts with no intracystic bodies, and precysts were found. Trophozoites were variable in size and shape with abundant tubular extensions along the single electron dense pellicle. BAL is a useful method for concentrating the various morphologic forms of PC organisms, and is a rapid diagnostic method for PC pneumonia.
The metacercariae of an echinostomatid fluke were detected from a freshwater 6sh, Odontobutis obscura interrupta caught from the Seom river system, Wonseong-gun, Kangwon-do, and experimentally infected into rats to obtain the adult worms for the species identification. A total of 32 metacercariae was detected from 10 fishes (27.8%) out of 36 O. obscura interrupta caught from the river. The average size of metacercariae was $153.4{\times}149.0{\;}{\mu}{\textrm{m}}$. Also, it was uniquely specific that thickened membrane was formed around the metacercarial cyst wall. The rats orally infected with those metacercariae were sacrificed 20 days after infection to get the adult worms. The mean length and width of the adult worms were 0.75 cm and 0.13 cm, respectively. The number of collar spines on circumoral disk was 26 to 28, and the end group spines at lateral sides of the oral sucker were 4 on each side. The echinostomatid flukes observed in this study were all identified as Echinostoma hortense according to the morphologies of the cirrus sac, ovary, vitellaria and testes of the adult worms and the shape of eggs. On the other hand, O. obscura interrupta is one of the fresh-water fish that are preferably eaten raw by the rural inhabitants. Although no human case of 5. hortense infection by eating O. obscura interrupta has been experienced yet, the possibility is strongly suggested in this study.
In Gamak Bay, Paralytic Shellfish Poisoning (PSP) was first detected from seafoods in 2003, however the toxin source is unknown yet. In this study, we report potential PSP producers of toxic dinoflagellates, describing morphology and abundance of cysts isolated from surface sediment of Gamak Bay. The most abundant type in these cysts was characterized with ellipsoidal and transparent wall identical to Alexandrium catenella and/or A. tamarense. Germination experiment of the cysts revealed that all motile cells germinated were morphologically identified as A. tamarense. This result suggests that A. tamarense may relate to PSP contaminations in Gamak Bay. Moreover, bottom water temperature in Gamak Bay is favorable for germination of A. tamarense cysts. Further studies are required to carry out the PSP monitoring for preventing the risk of PSP events that may outbreak in future at Gamak Bay.
Introduction : Patients with temporomandibular disorder may have various ear-related symptoms. If an excessive load is applied to the ear area due to the skeletal shape of the mandibular condyle or malposition of the disc, an auditory problems may occur. Case report : The patient was referred to our clinic due to the suspicion of temporomandibular disorder from the local otorhinolaryngology clinic. A few days ago, his right ear could not be heard. MRI showed that the left TMJ disc was anterior displacement with reduction, the right TMJ disc was anteromedial displacement without reduction. Also Right mandibular condyle showed sclerotic bone change, subchondral cyst and was compressing the frontal wall of the ear on MRI view. Right TMJ arthroplasty was done under the diagnosis of right TMJ osteoarthritis and osteochondroma. Postoperative intermaxillary fixation was done with SAS screw and elastics for 2 weeks. One month after the operation, hearing and TMJ discomfort were recovered without any complications. Conclusions As seen in this case, hearing loss due to benign tumor-like lesions of the temporomandibular joint should be treated surgically to restore the TMJ function and hearing.
The umbilicus is an important aesthetic component of the abdomen; therefore, its absence is both cosmetically and psychologically distressing to the patient. However, loss of the umbilicus during abdominal surgical procedures is often unavoidable. Umbilical reconstruction is aimed at obtaining a natural, three-dimensional appearance. We propose a simple method for immediate umbilical reconstruction with good long-term results. This technique was used successfully on a patient who underwent tumor excision. A 49-year-old woman presented with a large mass, measuring 5.8×4.0 cm, on her umbilicus. The mass, an epidermal cyst, developed after laparoscopic uterine myomectomy 5 years earlier. Complete excision of the mass resulted in a large defect, and immediate umbilical reconstruction was planned. Our procedure involved apposing and anchoring two opposing flaps onto the abdominal wall, so that the umbilicus would retain its depth over a long period of time. Negative-pressure wound therapy was applied for 72 hours as a mild compressive dressing. No complications were encountered. The healing process was uneventful and the aesthetic outcome was pleasing; a natural-appearing navel was created. The patient was satisfied with the end result. This technique provides a permanent and sufficient depression for the umbilicus.
