Is FDG -PET-CT A Valuable Tool in Prediction of Persistent Disease in Head and Neck Cancer

  • Objectives: To evaluate accuracy of FDG-PET CT in prediction of persistent disease in head and neck cancer cases and to determine prognostic value of metabolic tumor response. Materials and Methods: Between 2009 and 2011, 46 patients with squamous cell carcinoma of head and neck receiving PET-CT were treated with definitive radiotherapy, with or without chemotherapy. There were 29 nasopharyngeal, 11 hypopharyngeal, 3 oropharyngeal and 3 laryngeal cancer patients, with a median age of 50.5 years (range 16-84), 32 males and 14 females. All patients were evaluated with PET-CT median 3-5 months (2.4-9.4) after completion of radiotherapy. Results: After a median 20 months of follow up, complete metabolic response was observed in 63% of patients. Suspicious residual uptake was present in 10.9% and residual metabolic uptake in 26.0% of patients. The overall sensitivity, specificity, positive predictive value and negative predictive value of FDG-PET-CT for detection of residual disease was 91% and 81%, 64% and 96% respectively. Two year LRC was 95% in complete responders while it was 34% in non-complete responders. Conclusions: FDG PET CT is a valuable tool for assessment of treatment response, especially in patients at high risk of local recurrence, and also as an indicator of prognosis. Definitely more precise criteria are required for assessment of response, there being no clear cut uptake value indicating residual disease. Futhermore, repair processes of normal tissue may consume glucose which appear as increased uptake in control FDG PET CT.

Impact of Preoperative Serum Levels of CA 125 on Epithelial Ovarian Cancer Survival

  • Background: CA125 is very helpful in treatment monitoring and detection of epithelial ovarian cancer (EOC) recurrence. However there is controversy as to its accuracy and optimal usage. What is the impact of the CA125 levels before primary surgery treatment to the survival of patients? This study aimed to detect any association of preoperative serum levels with prognosis and survival in EOC patients. Materials and Methods: Our cohort comprised EOC patients in Dr. Sardjito Hospital, Yogyakarta, Indonesia, who complied with follow up. To explore the effect of preoperative CA125 levels and other variables on survival Cox's regression models were applied. Results: A total of 90 cases of EOC who had surgery were available for follow up. The level of CA125 proved to be a prognostic factor for overall survival of EOC patients, with an adjusted HR of 4.10 (p = 0.03). Adjuvant chemotherapy was another prognostic factor, 1 - 2 cycles having an adjusted HR of 0.17 (p = 0.04) and 3 - 8 cycles HR 0.39 (p = 0.06). Other factors such as age of patients adjusted HR 1.54 (p = 0.32), moderate differentiation (adjusted HR 1.61, p = 0.51) poor differentiation (adjusted HR 3.41, p = 0.15), and stage of disease (adjusted HR 1.98,p=0.27) were statistically not significant. However, this might have been because the power of the study was low. Conclusions: Preoperative level of CA125 is a prognostic factor for overall survival in EOC patients. The best cut-off for prognostic classification of CA125 serum level is 70 U/ml.

Epstein-Barr Virus-Positive Diffuse Large B-Cell Lymphoma: is it different between Over and Under 50 Years of Age?

  • Background: Epstein-Barr virus (EBV) positive diffuse large B-cell lymphoma (DLBCL) of the elderly is an entity introduced in the latest WHO classification of lymphoid tumors and defined in patients older than 50 years without prior lymphoma or immunodeficiency. However, recently it has also been seen in patients under 50. There is thus debate as to whether these are separate entities. Materials and Methods: In this retrospective study, we analyzed de novo DLBCL admitted to our institute over a period of two years. Clinical data included age, sex, nodal and extranodal presentation. The results of an immunohistochemistry (IHC) panel were also reviewed. IHC findings were mainly used to sub-classify DLBCL as germinal center vs. non germinal center types. IHC for identification of LMP-1 (latent membrane protein) and in situ hybridization for detection of EBV-encoded RNA (EBER) was performed. EBV prevalence, clinical data and IHC findings were compared between patients under and over 50 years of age. Results: Out of 95 DLBCL, 11.6% were EBV positive (7.5% and 14.5% in the young and old groups). We did not find any significant differences in IHC subclasses and clinical data between EBV positive DLBCL (EBV+DLBCL) of young and old groups. Conclusions: EBV+DLBCL are not exclusive to patients older than 50 years. With regard to clinical data as well as IHC subclasses, no differences were evident between EBV+DLBCL of young and old groups. Our suggestion is to eliminate any cut off age for EBV+DLBCL.

High-definition Video Enhancement Using Color Constancy Based on Scene Unit and Modified Histogram Equalization (장면단위 색채 항상성과 변형 히스토그램 평활화 방법을 이용한 고선명 동영상의 화질 향상 방법)

  • As high-definition video is broadly used in various system such as broadcast system and digital camcorder the proper method in order to improve the quality of high-definition video is needed. In this paper, we propose an efficient method to improve color and contrast of high-definition video. In order to apply the image enhancement method to high-definition video, scale-down video of high-definition video is used and the parameter for image enhancement method is computed from small size video. To enhance the color of high-definition video, we apply color constancy method. First, we separate the video into several scenes by cut detection method. Then, we apply color constancy to each scene with same parameter. To improve the contrast of high-definition video, we use union of original image and histogram equalized image, and weight is calculated based on sorting of histogram bins. Finally, the performance of proposed method is demonstrated in experiment section.

One-Step Simultaneous Immunochromatographic Strip Test for Multianalysis of Ochratoxin A and Zearalenone

  • Individual immunochromatographic assays (ICG) for ochratoxin A (OTA) and zearalenone (ZEA) were optimized and used in the development of a one-step simultaneous immunochromatographic assay (OS-ICG) for the rapid multianalysis of two mycotoxins in corn samples. The nitrocellulose membrane of the OS-ICG was treated with OTA-bovine serum albumin (BSA), ZEA-ovalbumin (OVA), and anti-mouse IgG in the OTA test, ZEA test, and control zones, respectively. Monoclonal antibody-gold conjugates (OTA3 MAb-gold and ZEA2C5 MAb-gold) were sprayed onto the conjugate pad. The visual detection limits were 2.5 and 5 ng/ml for OTA and ZEA, respectively, and the results were obtained within 15 min after starting the analysis. An efficient, simple, and rapid extraction method using 30% MeOH/PBS was established and validated by analyzing the corn samples spiked with OTA/ZEA mixtures (0/0, 5/10, 10/20, and $20/30\;{\mu}g/kg$). The cut-off values of the OS-ICG for the spiked corn were 5 and $10\;{\mu}g/kg$ for OTA and ZEA, respectively. Natural corn samples were analyzed by OS-ICG, direct competitive enzyme-linked immunosorbent assay (DC-ELISA), and HPLC. Results of the OS-ICG were in good agreement with those obtained by DC-ELISA and HPLC. The developed OS-ICG offers a rapid, easy-to-use, and portable analytical system and can be used as a convenient qualitative tool for the on-site simultaneous determination of OTA and ZEA in cereals, food, and agricultural products in one analytical cycle.

Transmission and Histochemical Detection of Mulberry Dwarf Mycoplasma in Several Herbaceous Plants (뽕나무 오갈병 마이코플라스마의 몇가지 초본식물에의 전염과 조직화학적 검정)

  • Transmission of mulberry dwarf mycoplasma (MDM) from diseased mulberry to 5 herbaceous plants (periwinkle, white clover, Ladino clover, red clover, and Chinese milk vetch) through insect vector, Hishimonus sellatus, was confirmed by symptom expression and microscopic evidences. The earliest symptom appearance was noticed on periwinkle in which incubation period was 25-30 days, while it ranged 35-40 days in the other plant species. The common symptoms of MDM infected plants were characterized by poor plant growth with accompanying discolorations of leaves (chlorosis with vein clearing in periwinkle, reddish in red clover, brownish in white and Ladino clovers, and yellowish in Chinese milk vetch). Mycoplasmal infections were diagnosed light microscopically by Dienes' and toluidine blue staining of hand-cut and Epon-embedded sections, respectively. In Dienes' stain, all the plants infected with MDM showed specific staining reaction in phloems. In toluidine blue stain, mycoplasmal existence was noted by granular appearance in sieve tubes which were confirmed to be mycoplasma-like organisms under an electron microscope.

Analysis of Acrylamide in Mainstream Cigarette Smoke and Effects of Total Nitrogen and Reducing Sugars on Acrylamide Content

  • Acrylamide has been found in many foods. Acrylamide in foodstuffs were analyzed by a GC/MS after bromination of acrylamide or by a LC/MS for underivatized acylamide. Time consuming and laborious clean up procedures is applied for the purification of the extract, in these methods. In this study, a simple and fast method without clean up step for the analysis of acrylamide in mainstream cigarette smoke was developed by using liquid chromatography-tandem mass spectrometry (LC/MS/MS) and the effects of tobacco leaf constituents on acrylamide content was observed. The analysis of acrylamide in mainstream cigarette smoke started to collect TPM (total particulate matter) from smoking and to extract by 0.1 % acetic acid solution and then to detect by liquid chromatography tandem mass spectrometry using electrospray in the positive mode. The recovery of acrylamide in 2R4F reference cigarette was 98 % and the reproducibility was 2.5 % and the limit of detection was 1.6 ng/mL. Reducing sugars and amino acids are considered to be main precursors of acrylamide in foodstuffs. Cut tobacco contain substantial amounts of reducing sugars and amino acid which may be explained the occurrence of acrylamide in mainstream cigarette smoke. The effects of reducing sugars and total nitrogen studied in an experiment with a various tobacco types. This result indicated that reducing sugars are not limiting factor for acrylamide formation, but the level of acrylamide in cigarette smoke was significantly correlated with the total nitrogen contents.

The Development of Landslide Predictive System using Measurement Information based on u-IT (u-IT기반 계측정보를 이용한 급경사지붕괴 예측 시스템 개발)

  • This paper has studied about the development and application of landslide collapse prediction real-time monitoring system based on USN to detect and measure the collapse of landslide. The rainfall measuring sensor, gap water pressure sensor, indicator displacement measuring sensor, index inclination sensor, water content sensor and image analysis sensor are selected and these are applied on the test bed. Each sensor's operation and performance for reliability verification is tested by the instrument which is installed in the field. As the result, u-IT based real-time landslide monitoring system which is developed by this research for landslide collapse detection could minimize life and property damages because it makes advance evacuation with collapse risk pre-estimate through real-time monitoring on roadside cut and bedrock slopes. This system is based on the results of this study demonstrate the effect escarpment plan are spread throughout.

Packet by attacks of a hacker was cut off, and ensured the normal inside and external network resources besides the packets which were normal by the results of active renewal.

  • This paper presents a built-in current sensor(BICS) that detects defects in CMOS integrated circuits using the current testing technique. This circuit employs a cross-coupled connected PMOS transistors, it is used as a current comparator. The proposed circuit has a negligible impact on the performance of the circuit under test (CUT) and high speed detection time. In addition, in the operation of the normal mode, the BlCS does not have dissipation of extra power, and it can be applied to the deep submicron process. The validity and effectiveness are verified through the HSPICE simulation on circuits with defects. The area overhead of a BlCS versus the entire chip is about 9.2%. The chip was fabricated with Hynix $0.35{\mu}m$ 2-poly 4-metal N-well CMOS standard technology.
