This paper presents a design of a speech to sign language converter for deaf and hard of hearing people. The device is low-cost, low-power consumption, and it can be able to work entirely offline. The speech recognition is implemented using an open-source API, Pocketsphinx library. In this work, we proposed a context-oriented language model, which measures the similarity between the recognized speech and the predefined speech to decide the output. The output speech is selected from the recommended speech stored in the database, which is the best match to the recognized speech. The proposed context-oriented language model can improve the speech recognition rate by 21% for working entirely offline. A decision module based on determining the similarity between the two texts using Levenshtein distance decides the output sign language. The output sign language corresponding to the recognized speech is generated as a set of sequential images. The speech to sign language converter is deployed on a Raspberry Pi Zero board for low-cost deaf assistive devices.
Objectives: The purpose of this analysis was to assess the maternal and child health program in Lao PDR, which was implemented to reduce maternal and child mortality in Laos. Target areas for the project included 2 provinces (Xiengkhuang, Houaphan) with all 18 districts within them. The beneficiaries of this project included 121,000 childbearing women and 62,000 children under 5 years old (LSIS, 2012). Methods: In this study, it was done for efficiency evaluation of the assistance projects of the health sector of maternal and child health promotion program in Laos that took place from 2010 to 2013. We conducted a cost-benefit analysis for the evaluation of the effectiveness of that program was being carried out effectively. Results: This evaluation adopted the Cost-Benefit analysis approach. Key findings of the Cost-Benefit analysis are the following: The Benefit-Cost Ratio of 1.012, Net Present Value of 84,250,000 Korean Won, an estimated Internal Rate of Return of 10.12%. These findings suggest that project activities were efficiently implemented. Conclusion: As a result, maternal and child health project in Laos has been analyzed that there was economic efficiency. Therefore, It is considered necessary and continued support expansion of program in the future. The direction of maternal and child health project in Laos, approach and community policy support must be included. Background: A small but significant proportion of cases with atypical squamous cells of undetermined significance (ASCUS) may harbour CIN 2-3, or even invasive carcinoma. Although immediate colposcopy, HPV-DNA testing or expectant management are three recommended options in ASCUS triage, a consensus does not currently exist on which one of these approaches is the most efficient. In this study, we aimed to compare the performance and cost of immediate colposcopy and colposcopy based on the human papillomavirus (HPV) testing for detecting histologically confirmed high-grade cervical intraepithelial neoplasia (CIN) in women with ASCUS. Materials and Methods: Records of 594 women with an index Papanicolaou smear showing ASCUS were retrospectively analyzed. Women in the immediate colposcopy arm were referred directly to colposcopy (immediate colposcopy group, n=255) and those in the HPV triage arm were proceeded to colposcopy if the high-risk HPV (hrHPV) test was positive (HPV triage group, n=339). High grade CIN (CIN2+) detection rate and treatment costs were compared between the groups. Results: The detected rate of CIN2+ was higher in the HPV triage group compared to immediate colposcopy group (8% vs. 1.6%, p=0.011). In the HPV triage group, the total cost, cost per patient, and the cost for detecting one case of high grade CIN were higher than the immediate colposcopy group (p<0.001). Conclusions: In women with ASCUS cytology, HPV DNA testing followed by colposcopy is more costly than immediate colposcopy, but this approach is associated with a higher rate of CIN2+ detection. This findings suggest that HPV DNA testing combined with cervical cytology could reduce the referral rate to colposcopy.
Background: Punjab state is particularly reporting a rising burden of cancer. A 'door to door cancer awareness and early detection campaign' was therefore launched in the Punjab covering about 2.67 million population, wherein after initial training accredited social health activists (ASHAs) and other health staff conducted a survey for early detection of cancer cases based on a twelve point clinical algorithm. Objective: To ascertain unit cost for undertaking a population-based cancer awareness and early detection campaign. Materials and Methods: Data were collected using bottom-up costing methods. Full economic costs of implementing the campaign from the health system perspective were calculated. Options to meet the likely demand for project activities were further evaluated to examine their worth from the point of view of long-term sustainability. Results: The campaign covered 97% of the state population. A total of 24,659 cases were suspected to have cancer and were referred to health facilities. At the state level, incidence and prevalence of cancer were found to be 90 and 216 per 100,000, respectively. Full economic cost of implementing the campaign in pilot district was USD 117,524. However, the financial cost was approximately USD 6,301. Start-up phase of campaign was more resource intensive (63% of total) than the implementation phase. The economic cost per person contacted and suspected by clinical algorithm was found to be USD 0.20 and USD 40 respectively. Cost per confirmed case under the campaign was 7,043 USD. Conclusions: The campaign was able to screen a reasonably large population. High to high economic cost points towards the fact that the opportunity cost of campaign put a significant burden on health system and other programs. However, generating awareness and early detection strategy adopted in this campaign seems promising in light of fact that organized screening is not in place in India and in many developing countries.
The objective of this study is to collect basic data which can lead effective direction in performing nutritional education, by investigating 273 homemakers' knowledge and attitudes about nutrition. This study was performed using questionnaire about the knowledge and the attitudes of nutrition according to the age, education level, occupation, food cost, and total income. Results were summarized as follows; 1) sources in the knowledge of nutrition appeared by order of T.V., cookbooks, magazines newspapers, mothers, friends, lecturers in cooking school, and nutritionist. 2) A level in the knowledge of nutrition which scord an average marks 154.7(the highest marks 210) was higher compared to other studies. Informations about food composition scord lower marks than other studies. 3) Attitudes about nutrition showed high score in the aspects that nutrition has an important role in health, and desirable eating habits were formed in childhood. 4) The knowledge of nutrition had no significant differences with age, occupation, and food cost whereas it had significant correlation with education level and total income. The higher the level in education and total income, the higher the level in the knowledge of nutrition. 5) The attitudes about nutrition had no correlation with age, occupation, but it had a significant correlation with education level, food cost and total income. 6) There was an significant correlation between the knowledge and attitudes about nutrition. In order to promote children's nutrition status, nutrition education for mothers must be accomplished in the first place. Development of T.V. program for children's nutrition education is desirable.
Objectives: The economic evaluation of health promotion programs has increasingly become an imperative activity for securing public fund or budget. The purpose of this study is to conduct an economic evaluation for the healthy school canteen program using the contingent valuation method(CVM). Methods: To estimate the benefit from the healthy school canteen program, double-bounded dichotomous choice method as a sort of willingness to pay was employed. Four hundred parents who lived in Seoul and have students at middle or high schools, were administrated by semi-constructive questionnaire containing the necessary information for benefit estimation. Cost estimation of healthy school canteen program was made referring to three types of pilot programs. Finally, the benefit against the cost was worked out according to the three levels of estimated cost. Results: Cost estimate is 8,488 and 9,311 won depending on the two senarios about how to invest on the program, respectably. The results of benefit estimate shows that the average cost willingness to pay(WTP) for healthy school canteen program is 21,275 won(16,963-59,838 won, 95% CI) and total benefits turned out 14.7 billion won. According to the cost-benefit analysis, average benefit-cost ratio is from 2.3 to 3.6. Conclusions: Healthy school canteen program could be economically accepted, and government can take consideration of expanding beneficiaries of healthy school canteen program for health promotion.
This study investigated the performance of hygiene management according to the capacity and food cost of foodservice in kindergartens as a measure of kindergartens foodservice hygiene management. Dietitians from the education office visited 50 kindergartens under the control of Dongbu District Office of Education during January, 2011. Kindergartens were 11 public and 39 private institutions. Over half of kindergartens (n=27) provided foodservice to 50~99 children. About 21 kindergartens had a foodservice cost per student per day of 2,000 won. Regarding personal hygiene, 'whether to wear an accessory or have a manicure' showed the best performance. 'Whether to have their health examined once every 6 months or keep their health records in 2 years' was rarely performed. For food materials, 'whether to buy food appropriate for the quality control standard of food materials' showed the highest performance. The highest performances for storage management of food materials and handling of food was 'whether to store goods within butlery at intervals more than 30 cm from the ground' and 'whether to heat and cook food more than $74^{\circ}C$', respectively. The highest performance for distribution of food and management of facilities was 'the hygienic management of cooking tools and facilities' and 'the proper installation of air-conditioning, heating and ventilation facilities', respectively. The results of this study show that capacity and food cost had the strongest effects on performance of personal hygiene. Especially, smaller facility size could increase performance of foodservice management.
Fresh-cut foods have been widely used in the school foodservice industry due to their convenience and saving effects of labor and working hours. This study evaluated the effectiveness of using fresh-cut foods in school foodservice with respect to cost efficiency, customer satisfaction, employee satisfaction, and productivity. First, in terms of cost efficiency, the cost of the fresh-cut food per meal price significantly increased by 2.6% via the new production plan. Meanwhile, the costs of labor and food wastes significantly decreased by 3.0% and 0.3%, respectively, after implementing the new plan. Second, customer satisfaction towards foodservice significantly increased, from 2.94 points prior to the new plan to 3.45 points. Third, foodservice productivity such as of the number of meals per full-time equivalent employee increased by 5.7 meals, from 143.0 meals to 148.7 meals after intervention. The productivity index of work hours for producing a meal also increased and was reflected by a work time reduction of 0.77 minutes, specifically from 4.25 minutes to 3.48 minutes. The labor cost per meal also decreased by 29.9 won, from 331.91 won to 301.97 won, but there was no significant difference. Through these results, we identified that using pre-process foods in school foodservice has positive effects on labor saving, customer satisfaction, and employee work satisfaction.
Generally, it is difficult to operate moveable-weir due to the expensive cost of the facility management and manpower consumption. Also, when it is installed in a remote area, there is a problem that the cost of connection for power system increases as well as the operating cost. Therefore, this paper proposes an optimal design algorithm to replace an existing power system with an Eco-friendly power supply system for movable-weir using PCS, PV module and lithium-ion battery. Also, this paper proposes a modeling method of environment-friendly power supply system for a movable-weir based on the PSCAD/EMTDC S/W and implements 5[kW] prototype environment-friendly power supply system. As a result of the performance test using the S/W modeling and the prototype system, it is confirmed that the proposed system has stable characteristics in the independent operation mode and the interconnection mode.
