  • Purpose: This study was conducted to compare health-related characteristics and self-care behavior between a controlled group and a non-controlled group of hypertension patients in a customized home visiting health service. Methods: This study was conducted as cross-sectional research. The subjects were 1,317 hypertension-controlled patients and 555 non-controlled ones. Data were collected using a structured questionnaire and analyzed by descriptive statistics, $x^2$-test, and t-test using the SPSS WIN 15.0 program. Results: Sociodemographic characteristics and self-care behavior were not significantly correlated between the groups. According to health-related characteristics, the health status was significantly higher in the hypertensioncontrolled group than in the non-controlled group. The cholesterol level was significantly lower in the hypertension-controlled group than in the non-controlled group. Conclusion: Attention should be paid to patients who are not in good health status in order to control their hypertension. Also, it is necessary to educate the patients in order for them to manage their hypertension and cholesterol.

  • The purpose of this study is to evaluate the feasibility, advantages/disadvantages of patient-controlled sedation (PCS) compared to anesthesiologist-controlled sedation (ACS) during neurolytic pain block and regional anesthesia. Forty patients were divided randomly into two groups of 20 patients each. Group 1(ACS) received 0.01 $mg{\cdot}kg^{-1}$ intravenous midazolam and 0.5 ${\mu}g{\cdot}kg^{-1}$ fentanyl intravenously by anesthesiologist just before, 30, and 60 minutes after the procedure to acheive sedation; Group 2 (PCS) patients self-administered a mixture of midazolam (0.4 mg) and fentanyl ($20{\mu}g$) using a syringe type infusion pump (Terumo, Japan) to acheive sedation. Considering the dermographics of patients, the types and durations of procedure performed, the level of average sedation the comfort level were similar in both groups. But the doses of midazolam and fentanyl administerd in group 2 were smaller than those in group 1 (p<0.01). Patients in PCS group showed their level of sedation more proper than did those in ACS group. However, patients in ACS group rated their level of comfort higher than did those in PCS group. The findings of this study indicate that PCS using a combination of midazolam and fentanyl is a fafe and effective technique. More studies are, however, needed to determinc the best choice of drug(s), doses, lock-out intervals, and possible use of continuous infusion with patient-controlled sedation.

The Comparison of the Effects of Nicardipine and Remifentanil on Fluctuation of Blood Pressure during Controlled Hypotension (유도 저혈압하 양악 교정술 시 Nicardipine과 Remifentanil의 혈역학적 비교)

  • Background: Controlled hypotension is used to reduce bleeding and improve surgical conditions during surgery. Nicardipine and remifentanil have been used to induce controlled hypotension in healthy patients. This study compared controllability of nicardipine and remifentanil on hemodynamic stability during controlled hypotension. Methods: Forty healthy patients scheduled for orthognathic two jaw surgery were randomly allocated to nicardipine (group N) and remifentanil (group R) group. After induction of anesthesia, group N (n = 20) was infused with nicardipine to induce hypotension and group R (n = 20) was infused with remifentanil. Mean arterial pressure (MAP) and heart rate (HR) were measured over 5 minute interval. Surgical field rating and blood loss were measured after surgery. Results: HR was lower in group R than group N. MAP was maintained at 50-60 mmHg during controlled hypotension but the fluctuation of blood pressure and HR were more frequent in group N than group R. There were no significant differences in the surgical field rating and blood loss between the groups. Conclusions: Nicardipine and remifentanil enabled controlled hypotension and provided good surgical conditions, but the fluctuation of hemodynamic parameters was more frequent in nicardipine group.

Improving English communicative competence through individualized WBI (WBI 수준별 수업을 통한 영어 의사소통능력 향상 방안)

  • The purpose of this study is to increase students' interest in the subject of English, which has been led by some mannerism, and to improve the students' basic scholastic ability and self-controlled problem solving, through adopting an individualized Web Based Instruction(WBI) as a concrete and practical learning method. In order to achieve this, an individualized Web Based Instruction(WBI) homepage has been developed to be applied experimentally to the field of middle school education for 8 months, and a research has been made on its effects on the students' interest in English along with the improvement of English. Two groups which showed almost the same interest were selected as a controlled group and an experimental one. Individualized Web Based Instruction was applied to the experimental group, while the controlled group was not instructed according to student ability. The controlled group was taught using CD added with the textbook. As a result of T-test, the students in the experimental group showed more improvement in communicative competence than those in the controlled group.

  • The purpose of this article is to study the effect of finger play teaching method on the reproduction of children's English words. The hypotheses are as follows: 1) there will be the difference of the reproduction of children's English words between the experimental group(that is taught by finger play teaching method) and the controlled group (that is taught by only oral teaching method). 2) there will be the difference of children's retention capacity of reproducing English words between the experimental group and the controlled group, The researcher made the time series data The measure tool was the check list for reproduction test of children's English words that was made by Run-Soon Oh and J. Eden(2004). The researcher analyzed the data using the Repeated Measures ANOVA and the Analysis of Covariance. The findings of research are as follows: 1) the more the frequency of teaching increases, the more there are the difference of reproducing English words according to frequencies and groups. The reproduction capacity of the experimental group is better than that of the controlled group. A degree of statistical significance is p<.001. 2) The difference of retention capacity of reproducing English words between the experimental group and the controlled group is a statistical significant, p<.05. The experimental group is better than the controlled group.

The Effect of Self-controlled Feedback on Proprioception in Elbow Flexion of Healthy Subjects (정상성인의 팔굽 굽힘 시 자기통제 피드백이 고유수용성감각에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate effect of self-controlled feedback on proprioception in elbow flexion. METHODS: Thirty young adult volunteered to participate and were randomly assigned to one of three groups (self-controlled, yoked, control). Power and velocity in elbow flexion was measured by PRIMUS RS (BTE Tech., Hanover, U.S.A). Statistical analysis was used multivariate ANOVA to know effect of self-controlled feedback on proprioception in elbow flexion. Post hoc was used Scheffe. RESULTS: In acquisition phase to practice effect, variable errors in self-controlled group was significantly low scored more than yoked and control group. In retention phase to learning effect, variable errors in self-controlled group was significantly low scored more than yoked and control group. CONCLUSION: Self-controlled feedback was more effective on movement control when the learner could make a decision about receiving feedback after the trial. This seems to support the view that self-controlled feedback benefits learning, because learners can make a decision about feedback based on their performance on a given trial.

  • The purpose of this study was to examine the effects of self-controlled learning using the (KR) feedback schedule versus the yoked KR on the acquisition and retention of balance training for individuals with hemiplegics. Sixteen hemiplegics were randomly assigned to either a self-controlled or yoked KR group. All subjects were ambulatory with or without an assistive device. The self-controlled group was provided with feedback whenever they requested it, whereas the yoked group had no influence on the feedback schedule. All subjects performed 10 acquisition trials and 10 retention trials the day after acquisition. The data were analyzed using an independent t-test and a Mann-Whitney U test. Participants in the self-controlled group achieved significantly more effective learning than the yoked group during the acquisition and retention test except anterior/posterior (AP) body sway. These results suggest that a feedback schedule which is controlled by the individuals with hemiplegics may be more effective in balancing training than a yoked KR which is not controlled by the subject.

Efficacy and Safety of Combined Subacromial and Intravenous Patient-controlled Analgesia after Arthroscopic Rotator Cuff Repair

  • Background: This study investigated the efficacy and safety of combined subacromial and intravenous patient-controlled analgesia for control of postoperative pain after arthroscopic rotator cuff repair. Methods: Between May 2012 and August 2014, 60 patients who underwent arthroscopic rotator cuff repair with acromioplasty and received patient-controlled analgesia were studied prospectively. Cases were divided into 2 groups: combined subacromial and intravenous infusion group (group A, 30 cases) and solitary intravenous infusion group (group B, 30 cases). The visual analogue scale was used to record the patient's level of pain every 12 hours during postoperative 72 hours and the following 48 hours after the suspension of patient-controlled analgesia. Results: The mean preoperative visual analogue scale score was 7.8 in group A and 7.6 in group B, and the immediate postoperative visual analogue scale score was 7.9 and 8.1 for each group. At postoperative time (From 12 hours to 72 hours after operation), the scores of combined subacromial and intravenous infusion were significantly lower than those of solitary intravenous infusion. Significant difference in the frequency of supplemental analgesic injections was observed between group A and group B (p=0.008). However, no significant difference in complication rate was observed between the two groups (p=0.562). Conclusions: Combined subacromial and intravenous patient-controlled analgesia after arthroscopic rotator cuff repair is more effective than solitary intravenous infusion without significantly increasing complications. Therefore, combined subacromial and intravenous patient-controlled analgesia could be a effective pain control method.

Study on Effect of Pressure-Controlled Injection System in Dental Local Anesthesia (치과용 국소마취 주사시 압력조절형 주사장치 사용의 효과에 대한 연구)

  • The purpose of this study was to evaluate the difference on pain intensity and discomfort between pressure-controlled injection system and conventional syringe injection technique from the patients undergoing oral and maxillofacial surgery. In a prospective, randomized, case-controlled study, 60 patients were divided into two groups (n=30 in each). In experimental group, pressure-controlled injection system was applied. In control group, conventional syringe injection system was applied. Pain rating score (PRS) and visual analogue scale (VAS) were assessed. The average of VAS in experimental group ($16.67{\pm}15.07$) was smaller than that of control group ($25.63{\pm}22.21$), though there were no significant differences (P=0.072). In PRS, fifteen patients (50.0%) of experimental group answered that they experienced mild pain. However sixteen patients (53.3%) of control group answered that they experienced intermediate pain. From the results, pressure-controlled injection system may be an effective method to reduce pain during the dental local anesthetic procedure.

Effects of Gamiguibitang on the ovulation in rats (가미귀비탕(加味歸脾湯)이 백서(白鼠)의 배란(排卵)에 미치는 영향(影響))

  • Gamiguibitang(GMGBT) is used in amenorrhea and female infertility caused by ovulation disorder. An attempt was made to evaluate the influences of GMGBT on the serum concentrations of FSH, LH, estradiol(E2) and progesterone, the histological and optical changes of ovary of rats. The results of the study were as follows: 1. Blood FSH level significantly increased in experimental group controlled by four times quantity as compared with control group. 2. Blood LH level increased in experimental group controlled by four times quantity as compared with control group, which showed no efficacy. 3. Blood E2 level increased in experimental group controlled by four times quantity as compared with control group, which showed no efficacy. 4. Blood progesterone level significantly decreased in experimental group controlled by double quantity as compared with control group. 5. In optically observations of ovary, weight of ovary significantly increased in experimental group controlled by double quantity as compared with control group 6. In histological observations of ovary, ovulation significantly increased in experimental group controlled by both double and four times quantity as compared with control group. According to these results, it can be concluded that GMGBT influences the pituitary gland and ovary to increase the ovulation of rats.
