가정간호사 임상실무 훈련프로그램 개발과 평가를 위한 사전 연구 -정형외과 수술 환자를 중심으로- (A Preliminary Study on Development and Evaluation of Home Health Care Nurse Clinical Practice Program -Focused on Postoperative Orthopedic Patients-)

  • The clinical practice program for home care nurses was implemented in June 1994, to help to set up a hospital-based home care system in the Kwangju City area as a collaborative work between the Department of Orthopedic Surgery at Chunnam University Hospital and Chunnam University School of Nursing. Under the developed clinical practice strategy, the eight week training was given to five licensed home care nurses who had completed Part I and II of the home health care nursing practicum from June 1994. The purpose of this descriptive evaluation study was to identify the effectiveness of the clinical practice program for home care nurses specialized in the area of patient care for people with musculoskeletal function impairment. As a method in data analysis, data triangulation was used in the five home care nurse case evaluations. The variety of data analyzed include confidence score by home care nurse self-evaluation, patient and family member satisfaction scores, and competency score by preceptor evaluation. The study findings revealed that an increase rate in nursing performance didrate necessarily coincide with an increase not in competency score and also, not with the patient /family member satisfaction scores. And an order derived from the clinical performance scores of five home care nurses corresponded to those from three measurements-competency score, patient satisfaction score, and family member satisfaction score. However, it differed from the order associated with the confidence score. Consistency derived from the three objective evaluation methods may lead to the possibility that the level of competency measured by educator can be further explained by the levels of patient/family member satisfaction. The salient finding of this study was that, in case of nurse A who have had little clinical experience in the orthopedic patient care, there was a significant increase in the level of confidence and competency in subscale of professional skill with the home care clinical practice. Therefore, the effect of the clinical practice program would be successful for nurses who have had little experience in the area of specialization. The study results suggest that there might be some time difference in the development of cognitive sense (confidence) in performance and actual clinical performance (competency). In future research, relationships between the confidence and competency score, and between the confidence score and the patient satisfaction score should to be measured in different time frame to achieve a better explanation power of the study outcome.

숙지황 분말을 첨가한 국수의 품질 특성 및 항산화성 (Quality Characteristics and Antioxidant Activities of Noodles Added with Rehmanniae Radix Preparata Powder)

  • 본 연구는 숙지황 분말을 이용한 기능성 면류를 제조하기 위해서 숙지황 분말을 밀가루에 0, 2.5, 5, 7.5% 첨가하여 제조한 국수의 품질 특성 및 항산화성을 조사하였다. 숙지황 분말 첨가가 증가함에 따라 국수의 중량, 부피, 함수율은 감소하였고 탁도는 증가하였다. 색도는 숙지황 분말의 첨가량이 증가할수록 L값과 b값은 감소하고 a값은 증가하는 경향을 나타내었다. 수분 함량은 숙지황 분말의 첨가량이 증가할수록 감소하였다. 숙지황 분말 첨가량이 증가할수록 국수의 경도는 증가하였고, 탄력성, 응집성, 검성 및 씹힘성은 감소하였다. 숙지황 분말의 첨가 농도가 증가함에 따라 아밀로그래프상의 호화개시 온도는 증가하였고, 최고 점도와 최종 점도는 감소하였다. 숙지황 분말 첨가 농도가 높아질수록 polyphenol 함량은 증가하였고, DPPH radical 소거능에서 $IC_{50}$값이 감소하였으므로 숙지황 분말 첨가량이 증가할수록 항산화능이 높아지는 것을 알 수 있었다. 관능검사 결과 숙지황 분말 2.5% 첨가 국수가 전체적인 기호도 및 구입의사에서 가장 높게 평가되었다. 이상의 결과로 볼 때 숙지황 분말을 첨가한 기능성 국수의 개발이 가능하였고 기능성을 고려하였을 때는 숙지황 분말 7.5% 첨가가 가장 그 항산화 활성이 우수하였다. 하지만 제품 개발 특성상 관능적 특성을 크게 고려하였을 때, 숙지황 분말을 2.5% 첨가하여 국수를 제조하는 것이 가장 적합한 것으로 사료된다.

린코마이신과 남은 음식물의 첨가.급여가 육계의 생산성 및 혈액의 성상에 미치는 영향 (Effects of Dietary Supplemental Lincomycin and Food Waste on Performance and Blood Components of Broiler Chicks)

  • These studies were conducted to investigate the effect of dietary supplemental lincomycin at different food waste supplements on performance and blood components of broiler chicks. Diets were based corn, soybean meal and contained 3,100㎉/kg ME, 21% CP for the rest two weeks. Three levels of dietary lincomycin(LM ; 0, 2.2, 4.4 mg/kg) and food waste(FW : 0, 7.5, 15%) were added for both experiments. Weight gain, feed intake and feed conversion ratio(FCR) were measured for five weeks. Blood components, liver weight and abdominal fat ratio to body weight were examined at the end of experiment. Metabolizable energy of food waste used in formulating diet prior to feeding trials were measured 2,504(Expt 1) and 2,734㎉/kg(Expt 2), respectively. In Expt 1, weight gain of birds fed 4.4mg/kg LM was significantly higher than that of no LM treatment(P〈0.05). It also tended to increase in 7.5% food waste treatment compared to that of control but was not significantly different. FCR of chicks fed 2.2mg/kg LM supplement was significantly improved compared to that of other treatments. However, chicks fed food waste supplements had higher FCR than control as a dietary FW level increased(P〈0.05). There were no significance in blood components of all treatments. Liver to body weight ratio of birds fed 7.5% FW supplements decreased significantly(P〈0.05), but was no consistency in LM treatments. In Expt 2, weight gain of chicks fed 15% FW was not significantly different from the control and 7.5% FW. It tended to increase as dietary supplemental LM increased. It also increased in no LM treatment with 15% FW compared to that of control. However, the birds fed 2.2mg/kg LM with 15% FW exhibited higher weight gain than dther treatments. As FW supplement increased, the feed intake increased significantly(P〈0.05). There were no significance in blood components except serum calcium. Weights of liver were not affected by dietary FW or LM. The results of these experiments indicated that chicks fed 4.4mg/kg LM supplements maximized growth rate of all treatments and improved FCR in 2.2mg/kg LM.

한반도 관속식물 IUCN 적색목록 평가와 문제점 (An Assessment and Review of IUCN Red List for Vascular Plants in Korean Peninsula)

  • IUCN의 적색목록은 전 세계 규모에서 가장 신뢰받는 멸종, 희귀 동식물종의 평가와 보전 정보를 제공하고 있다. 2016년 7월까지 한반도에 분포하는 식물중 IUCN 적색목록 평가를 받은 종은 236종으로, 5종이 멸종위기종(CR), 20종이 위기종(EN), 9종이 취약종(VU)으로 확인되었다. 나머지 189종은 약관심종(LC)으로 평가되었다. 환경부는 2011년 국가적 색목록 평가라는 명목 하에 IUCN 적색목록과 내용, 형식이 다른 정성적 기준에 따라 평가 결과를 제시하였다. 이와는 별개로 환경부는 멸종위기종 I급과 II급 목록을 57종으로 수정 제시하였다. 이 목록은 IUCN의 적색목록과 비교시 일부 공통종이 있으나 공통종도 보전상의 지위가 일치하지 않는다. 이런 평가 내용의 불일치의 이유는 남한이라는 제한된 지역 수준의 평가로는 전 세계 규모의 평가와는 결과가 일치할 수 없기 때문이다. 평가에 필요한 정보가 부실하게 제시되거나 정보가 없는 상태에서 자의적 평가가 이루어진 경우도 있다. 국가 단위의 종 관리를 위해서는 멸종과 절멸에 대한 엄밀한 용어 적용과 남한에 국한된 제한적 시각의 편향된 결과를 배제하고, 분류학적 실체에 근거한 종 선정, 평가를 위한 개체군의 크기, 분포, 개체수, 개체군 증감에 대한 구체적 자료 제시와 수집이 필요하다. 환경부의 현재 멸종위기동식물 목록은 '국내절멸위기목록'으로 변경하고, 전 세계 단위의 IUCN적색목록 평가종을 '멸종위기목록'으로 이원화해서 관리할 필요가 있다. 현재 국가 기관에서 제시하는 몇 백 종의 과도한 목록 보다는 우선 평가 대상의 종을 순차적으로 늘려가면서, 평가 및 종관리에 내실을 다지는 것이 필요하다.

Effects of three different diets on the fatty acid profile and sensory properties of fresh Pecorino cheese "Primo Sale"

  • Objective: This study aimed to evaluate and compare the effects of three different diets on the fatty acids (FA) profile and sensory properties of a characteristic Italian fresh cheese: Pecorino "Primo Sale" (PS). Methods: Fifty-four sheep were divided into three feeding groups: total mixed ration (TMR) enriched with extruded linseed (TL), control diet with TMR without any integration (TC), and pasture (P). During cheese production, six cheeses per experimental group were produced each week, stored for 10 days at 4℃, and then analyzed for chemical composition, FA profile, and sensory properties. Results: Saturated fatty acids (SFA) were significantly higher in PS from group TC (82.11%) than in cheese from other two groups (P 75.48% and TL 66.83%). TL and P groups presented higher values of polyunsaturated fatty acids, 4.35 and 3.65%, respectively, than that of TC group (2.31%). The lowest SFA/unsaturated fatty acid ratio was found in TL and P groups, while the highest was found in the TC group. Vaccenic acid and conjugated linoleic acid (CLA) were higher in group P (p<0.05) than in groups L and TC. Sensory properties of cheese from group P received the highest scores for odor intensity and friability, while control group had a greater chewing consistency. Overall, all cheeses received good scores for acceptability. Conclusion: In conclusion, this study showed how the integration of extruded linseed improved the FA profile of fresh pecorino cheese PS preserving its sensory properties. Levels of CLA in the PS group achieved using this approach was not higher than that in a grazing diet. Cheeses from groups P and L contained a higher value of FA, with nutritional implications for humans, when compared with un-supplemented diet. Linseed may be a good feeding strategy when pasture is not available.

건강기능식품의 기능성을 중심으로 한 글루코사민의 관절건강 기능성에 대한 체계적 고찰 (Systematic Review of the Effect of Glucosamine on Joint Health while Focused on the Evaluation of Claims for Health Functional Food)

  • 글루코사민의 관절건강 기능성을 건강기능식품 재평가 기준에 맞추어 체계적 고찰을 실시하였다. 2012년 5월 기준 DB 검색을 통해 703건의 자료를 수집하여, 선정/제외 기준에 따라 선별한 결과 총 34건의 연구가 평가되었다. 34건 중 28건(4,225명)의 연구에서 통계적으로 유의한 개선 효과가 나타났으며(P<0.05), 일일 섭취량은 1.5~2 g으로 공전에서 제시되어 있는 섭취량 범위에 해당하였다. 글루코사민의 기능성 재평가 이후의 연구 결과는 16건이었고, 이 중 1,158명 대상 14건의 연구에서 유의한 관절건강 개선효과를 보였다(P<0.05). 연구대상자에 따른 분석 결과, 방사선 검사 기준에 따라 관절 연골의 손상 정도가 심한 대상자를 제외한 연구는 11건이었으며, 이 중 2,934명 대상 10건의 연구에서 글루코사민이 관절건강에 유의한 개선 효과를 보였다(P<0.05). 따라서 현시점에서 건강기능식품인 글루코사민의 관절건강 기능성은 인정될 것으로 판단되나 향후 연구 결과 추이를 지속적으로 지켜볼 필요가 있다.

고초균에 의한 탈지대두 Grits(Defatted Soybean Grits) 발효물의 품질 특성 (Quality Characteristics of Defatted Soybean Grits Fermented by Bacillus subtilis NUC1)

  • 증자한 탈지대두 grits에 B. subtilis NUCI을 2% 접종하여 $40^{\circ}C$에서 24시간 발효하여 DSG 발효물을 제조하였다. 제조한 DSG 발효물 및 DSG의 단백질 함량은 $57.2{\sim}61.0%$로 청국장보다 높았으며, 지질은 함유하지 않았다. 그리고 DSG에서는 protease 및 $\alpha$-amylase의 활성이 없었으며, 점질물도 검출되지 않았으나, DSG 발효물은 청국장보다 더 큰 protease 활성, $\alpha$-amylase 활성, tyrosine 함량, 점질물 함량을 보였다. 또한 DSG 발효물은 DSG보다 유리아미노산 함량이 증가하는 경향이었고, phenylalanine, tyrosine, glutamic acid 등이 주된 유리아미노산으로 확인되었다. 그리고 isoflavone 함량도 DSG 발효물이 DSG보다 1.6배 더 증가되었다. 관능검사에서는 냄새 및 맛에 대한 기호도의 경우 DSG 발효물이 청국장보다 높은 값을 보여 DSG 발효물의 기호도가 더 좋은 것으로 나타났으나, 종합적인 기호도는 청국장이 DSG 발효물보다 조금 높게 나타났지만 큰 차이는 보이지 않았다.

혈부축어탕(血府逐瘀湯) 추출물(抽出物)이 고지방식이(高脂肪食餌)로 유발(誘潑)시킨 고지혈증(高脂血症) 흰쥐의 혈액(血液) 및 간(肝) 기능(機能)에 미치는 영향(影響) (Effects of Hyolbuchukeo-tang Extracts on Blood and liver of Hyperlipidemia Rats Induced by High Fat Diet)

  • Objectives: The purpose of this study was to investigate the effects of Hyolbuchukeo-tang extracts on the hyperlipidemia rats induced by high fat diet. Methods and Materials: In vitro; The extracts prepared for Hyolbuchukeo-tang by hot water extraction (HH), fermentation(HF) and UMPM extraction(HU) method. The extracts were examined for levels of polyphenol compounds, antioxidant activities, and inhibitory potencies for HMG-CoA reductase. In vivo; Sprague-Dawley male rats of weighing $150{\pm}5g$ were randomly divided into six groups ; normal control diet(NC), and high fat diet(HC), high fat diet with treated lovastatin of 10mg/kg(PC), high fat diet with Hyolbuchukeo-tang extracts; HH, HF and HU treated extracts of 300mg/kg, respectively. Also, we compared the effects of the extracts of HH, HF and HU on rats fed high fat diet for four weeks. Results: 1. The content of polyphenol compounds and electron donating abilities(EDAs) was the HF higher than HH and HU. The superoxide dismutase(SOD)-like activities were proportionate in consistency and they appeared highly from all extracts. The HMG-CoA reductase inhibition activities was highest activities in the HU. 2. The activities of serum GOT and GPT were significantly lower in the HH and HF groups. The level of serum triglyceride was significantly decreased in the HF group. HH and HU groups were significantly decreased in the atherogenic index(AI). The total cholesterol concentration in liver was significantly decreased in the HF group, and HU showed more significantly decreased in the triglyceride than of the lovastatin. Also, photomicrographs of liver tissue showed higher fat accumulation in the HC group than in the HH, HF and HU groups. Conclusions: These result suggest that the hyper-lipidemia caused by a high fat diet was effectively inhibited the administration of HF and HU.

초등수학 도형영역에 제시된 정의에 관한 교사의 인식과 오류 (Teachers' conceptual errors related to the definitions in the area of geometry of elementary school mathematics)

  • Unlike ordinary situations, deifinitions play a very important role in mathematics education in schools. Mathematical concepts have been mainly acquired by given definitions. However, according to didactical intentions, mathematics education in schools has employed mathematical concepts and definitions with less strict forms than those in pure mathematics. This research mainly discusses definitions used in geometry (promising) course in primary schools to cope with possibilities of creating misconception due to this didactical transformation. After analyzing problems with potential misconceptions, a survey was conducted $\underline{with}$ 80 primary school teachers in Jeju to investigate their recognitions in meaning of mathematical concepts in geometry and attitudes toward teaching. Most of the respondents answered they taught their students while they knew well about mathematical definitions in geometry but the respondents sometimes confused mathematical concepts of polygons and circles. Also, they were aware of problems in current mathematics textbooks which have explained figures in small topics (classes). Here, several suggestions are proposed as follows from analyzing teachers' recognitions and researches in mathematical viewpoints of definitions (promising) in geometric figures which have been adopted by current mathematics textbooks in primary schools from the seventh educational curriculum. First, when primary school students in their detailed operational stage studying figures, they tend to experience $\underline{a}$ collision between concept images acquired from activities to find out promising and concept images formed through promising. Therefore, a teaching method is required to lessen possibility of misconceptions. That is, there should be a communication method between defining conceptual definitions and Images. Second, we need to consider how geometric figures and their elements in primary school textbooks are connected with fundamental terminologies laying the foundation for geometrical definitions and more logical approaches should be adopted. Third, the consistency with studying geometric figures should be considered. Fourth, sorting activities about problems in coined words related to figures and way and time of their introductions should be emphasized. In primary schools mathematics curriculum, geometry has played a crucial role in increasing mathematical ways of thoughts. Hence, being introduced by parts from viewpoints of relational understanding should be emphasized more in textbooks and teachers should teach students after restructuring this. Mathematics teachers should help their students not only learn conceptual definitions of geometric figures in their courses well but also advance to rigid mathematical definitions. Therefore, that's why mathematics teachers should know meanings of concepts clearly and accurately.

서비스 질 척도(서브퍼프)의 타당화 연구: 종합사회복지관을 대상으로 (Reliability and Validity of the Korean Version of SERVPERF : A Validation Study)

  • 본 연구는 서비스 질을 측정하기 위해 사용되고 있는 대표적인 척도인 서브퍼프(SERVPERF)를 종합사회복지관 이용자들을 대상으로 평가하는 것이 목적이다. 그 동안 서브퍼프가 일부 국내 연구들에서 사용되어 온 적은 있으나 한국어판 서브퍼프의 신뢰도 및 타당도에 대한 철저한 평가 없이 사용되어왔다. 특히 본 연구의 대상이자 대표적인 사회복지현장이라고 할 수 있는 종합사회복지관 이용자들을 대상으로 서브퍼프를 평가한 연구는 거의 전무하였다. 본 연구에는 서울과 경기도 소재 종합복지관 이용자 328명이 참여하였다. 확인적 요인분석을 실시한 결과 한국어판 서브퍼프는 국내외 선행연구들에서 보고된 바와 같이 5가지 차원(유형성, 신뢰성, 대응성, 보증성, 공감성)으로 구성된 척도로 확인되었다. 하위 요인별 내적 신뢰계수의 범위는 0.85~0.94로 각 요인이 동질적인 문항들로 구성되어 있음이 확인되었다. 수렴타당도 평가를 위해 만족도와의 상관관계를 분석하였는데, 한국어판 서브퍼프는 만족도 척도들과 모두 정적인 상관관계를 보였고, 판별타당도 평가에서는 서브퍼프와 인구사회학적 특성은 서로 상관관계가 없는 것으로 나타나 한국어판 서브퍼프의 개념구성타당도가 인정되었다. 마지막으로 한국어판 서브퍼프의 5가지 차원은 사회적 바람직성의 영향을 받지 않았다.
