Multiple-life policies pay a benefit on the first death or the last death among the group of lives. In practice, the future lifetime random variable of policy holders has been considered to be independent, but it is more rational to take into account the correlations among the policy holders. In this paper, the Gaussian copula is applied to re ect the correlations among policy holders and then to diversify the common shock of the multiple life policies which follows an exponential distribution. Five case studies demonstrate its usefulness of using copula in calculating the premiums of the multiple-life policies including the common shock.
To determine its usefulness and safety of extracorporeal shock-wave lithotripsy in common bile duct and pancreatic duct stones, we analyzed the results of 13 patients with common bile duct stones and 6 patients with pancreatic duct stones which were removed by endoscopic procedures using the balloon or basket, who was performed the extracorporeal shock-wave lithotripsy using the ultrasonography for stone localization with a spark gap type Lithotriptor(Dernier MPL 9000, Germany). Fragmentation and complete clearance of the common bile duct and pancreatic duct stones were obtained in 19 of 19 patients(100%). Apart from transient attacks of fever in 2 of 13 patients with common bile duct stones(15%) and mild elevation of serum amylase and lipase in 2 of 6 patients with pancreatic duct stones(33%), no other serious side effects were observed. In our experiences, extracorporeal shock-wave lithotripsy is a safe and useful treatment for endoscopically unretrievable common bile duct and pancreatic duct stones.
This research proposes comprehensive models for analyzing common cause failures (CCF) due to cumulative shocks and to assess system reliability under the CCF. The proposed cumulative shock models are based on the binomial failure rate (BFR) model. Six kinds of models are proposed so as to explain diverse cumulative shock phenomena. The models are composed of the initial failure probability, shape parameter, and the total shock number. Some parameters of the proposed models can not be explicitly estimated, so we adopt the Expectation-maximization (EM) algorithm in order to obtain the maximum likelihood estimator (MLE) for the parameters. By estimating the parameters for the cumulative shock models, the system reliability with CCF can be assessed sequentially according to the number of cumulative shocks. The result can be utilizes in dynamic probabilistic safety assessment (PSA), aging studies, or risk management for nuclear power plants. Replacement or maintenance policies can also be developed based on the proposed model.
Heat shock proteins are a class of molecular chaperones that can be found in nearly all organisms from Bacteria, Archaea and Eukarya domains. Heat shock proteins experience increased transcription during periods of heat induced osmotic stress and are involved in protein disaggregation and refolding as part of a cell's danger signaling cascade. Heat shock protein, Hsp20 is a small molecular chaperone that is approximately 20kDa in weight and is hypothesized to prevent aggregation and denaturation. Hsp20 can be found in several strains of Proteobacteria, which comprises the largest phyla of the Bacteria domain and also contains several medically significant bacterial strains. Genomic analyses were performed to determine a common evolutionary pattern among Hsp20 sequences in Proteobacteria. It was found that Hsp20 shared a common ancestor within and among the five subclasses of Proteobacteria. This is readily apparent from the amount of sequence similarities within and between Hsp20 protein sequences as well as phylogenetic analysis of sequences from proteobacterial and non-proteobacterial species.

And then vibration responses experimental results and shock response of double stage elastic mounting system with complicated damped foundation structure like common-bed or raft in ships foundation structure model which adopts the above damped structure as intermediate foundation were compared. As a result it is found that F.E.M could be effectively used in analyzing the vibration and shock response of double and multi-stage elastic mounting system with complicated damped foundation structures.
Hemorrhagic shock is a common cause of death in emergency rooms. Since the symptoms of hemorrhagic shock occur after shock has considerably progressed, it is difficult to diagnose shock early. The purpose of this study was to improve early diagnosis of hemorrhagic shock using a survival prediction model in rats. We measured ECG, blood pressure, respiration and temperature in 45 Sprague-Dawley rats, and then obtained a logistic regression equation predicting survival rates. Area under the ROC curves was 0.99. The Hosmer-Lemeshow goodness-of-fit chi-square was 0.86(degree of freedom=8, p=0.999). Applying the determined optimal boundary value of 0.25, the accuracy of survival prediction was 94.7%

Purpose: This study was conducted to investigate the characteristics of drug induced anaphylactis and anaphylactic shock in patients who were admitted to the emergency department Methods: We retrospectively collected the data on patients with drug induced anaphylaxis and who were admitted to the emergency department from January 2001 to June 2009. The study group was divided into the non-shock and shock groups according to whether the systolic blood pressure more than 90mmHg. The initial demographic data, the causes of drug-induced anaphylaxis, the clinical manifestations, the treatment and the prognosis were reviewed for 72 patients. Results: The mean age of the study subjects was $47.9P{\pm}14.2$ years old and there were 40 male patients and 32 female patients. There were 26 patients in the non-shock group and 46 in the shock group. The mean age was older in the shock group than in the non-shock group ($51.5{\pm}15.1$ vs $42.5{\pm}10.6$, p-0.002). A history of drug allergy was more common in the shock group, but no difference was found for the comorbid chronic diseases between the two groups. Radio-contrast media was the most common cause, followed non-steroidal anti inflammatory drugs and antibiotics, but there is no difference in the causes between the two groups. The symptoms of cyanosis, syncope, sweating and dizziness were more frequently manifested in the shock group. The administration of intravenous fluid and injection of subcutaneous epinephrine at the emergency department were more frequent in the shock group than in the non-shock group. Conclusion: For the patients who were admitted to the emergency department with drug induced anaphylaxis, the mean age was older and the symptoms of cyanosis, syncope, sweating, dizziness were more frequent in the anaphylactic shock patients than in the non-shock group. More treatments were given at the emergency department to the anaphylactic shock patients.

In the field of nuclear reactor safety study, common cause failures (CCFs) became significant contributors to system failure probability and core damage frequency in most Probabilistic risk assessments. However, it is hard to estimate the reliability of such a system, because of the dependency of components caused by CCFs. In order to analyze the system, we propose an analytic method that can find the parameters with lack of raw data. This study adopts the shock model in which the failure probability increases as the shock is cumulated. We use two-step Expectation and Maximization (EM) algorithm to find the unknown parameters. In order to verify the analysis result, we perform the simulation under same environment. This approach might be helpful to build the defensive strategy for the CCFs.
Urinary tract infections are among the most common infectious diseases and are the major causes of mortality and morbidity. These diseases result in many severe hospitalizations each year. Severe sepsis and septic shock are common and life-threatening medical conditions, and large cases are associated with urinary tract infection. The medical term "severe sepsis" is defined as sepsis complicated by hypotension, organ dysfunction, and tissue hypoperfusion, whereas "septic shock" is defined as sepsis complicated either by hypotension that is refractory to fluid resuscitation or by hyperlacteremia. A recent multicenter-study in Korea reported that the rate of in-hospital mortality associated with severe sepsis and septic shock was > 34%. Among the causative diseases, urogenital tract infection showed a high correlation. Moreover, it is very important that clinicians detect severe sepsis and septic shock early and treat them properly. The principles of initial treatment include provision of sufficient hemodynamic resuscitation and early administration of appropriate antibiotic therapy to mitigate uncontrolled infection. Initial resuscitation includes the use of vasopressors and intravenous fluids, and it is a key to achieve the target of initial resuscitation. Supportive care in the intensive care unit, such as glucose control, stress ulcer prophylaxis, blood transfusion, deep vein thrombosis prophylaxis, and renal replacement therapy, is also significant. We have summarized the key components in the treatment of severe sepsis and septic shock in patients with urinary tract infection. Urologists should be aware that appropriate early treatment is necessary to prevent fatal outcomes in these patients.
The heat shock transcription factor (HSF) family consists of at least three members in mammals and regulates expression of heat shock proteins in response to heat shock and proteotoxic stresses. Especially, HSF1 is indispensable for this response. Members of this family are also involved in development of some tissues such as the brain and reproductive organs. However, we did not know the molecular mechanisms that regulate developmental processes. Involvement of HSFs in the sensory development was implicated by the finding that human hereditary cataract is associated with mutations of the HSF4 gene. Analysis of gene-disrupted mice showed that HSF4 and HSF1 are required for the lens and the olfactory epithelium, respectively. Furthermore, a common molecular mechanism that regulates developmental processes was revealed by analyzing roles of HSFs in the two developmentally-related organs.
