A Study of the Co-Administration of Herbal and Western Medicines to Hospitalized Patients with Osteoarthritis (골관절염 환자의 한약 양약 투여 현황 및 안전성 연구)

  • Objectives: The combined use of herbal and Western medicine is increasing. However, herbal medicine is highly likely to interact with Western medicine making it important to understand the effects of co-administration. This study investigates the ratio of patients who take Western medicine with herbal medicine, the types of medicines commonly prescribed together, and the results of hospital examinations. Methods: We investigated patients who were hospitalized at Kyung Hee University Korean Medical Hospital for at least one day from January 1, 2010 to December 31, 2017. There were some inclusion criteria. First, we chose patients aged 19 and over. Second, we chose patients who were diagnosed with osteoarthritis (OA) with diagnosis codes M13, M15, M17 according to KCD-7. Third, patients had liver function tests, renal function tests, and general hematology tests performed at least two times during hospitalization. Results: Among a total of 131 OA patients, 32 (24.4%) patients were treated with herbal-Western medicine combination therapy. The most commonly prescribed herbal medicine was Daegalwhal-tang, and the most commonly prescribed Western medicine was celecoxib. In the laboratory findings, all liver function tests, renal function tests, and general hematology tests showed no difference compared to admission day. There were also no differences between herbal medicine single treatment and herbal-Western medicine combination treatment. Conclusions: From these results, we suggest that herbal medicine single treatment and herbal-Western medicine combination treatment for OA patients does not cause adverse effects.

  • In the process of simplifying the complex fate of the plant into a binary state, the information loss is inevitable. To minimize the information loss, the quantification of plant safety status has been formulated through the combination of the probability density function arising from the sensor measurement and the membership function representing the expectation of the state of the system. Therefore, in this context, the safety index is introduced in an attempt to quantify the plant status from the perspective of safety. The combination of probability density function and membership function is achieved through the integration of the fuzzy intersection of the two functions, and it often is not a simple task to integrate the fuzzy intersection due to the complexity that is the result of the fuzzy intersection. Therefore, a methodology based on the Algebra of Logic is used to express the fuzzy intersection and the fuzzy union of the arbitrary functions analytically. These exact analytical expressions are then numerically integrated by the application of Monte Carlo method. The benchmark tests for rectangular area and both fuzzy intersection and union of two normal distribution functions have been performed. Lastly, the safety index was determined for the Core Reactivity Control of Yonggwang 3&4 using the presented methodology.

The Effect of Thoracic Cage Mobilization and Breathing Exercise of Respiratory Function, Spinal Curve and Spinal Mobility in Elderly with Restrictive Lung Disease

  • The purpose of this study was to examine the effects of combined respiratory physical therapy on respiratory function, spinal curve and spinal mobility for community-dwelling elderlies with restrictive lung diseases. In total, 10 patients participated in an 8-week intervention program of thoracic cage mobilization and breathing exercise in combination. The results of the study are as follows: for respiratory function, the forced expiratory volume in 1 second (FEV1), forced vital capacity(FVC), and FEV1/FVC were significantly improved to $.30{\pm}0.31{\ell}$, $.46{\pm}.42{\ell}$, and $18.10{\pm}11.39%$, respectively (p<.05). For spinal curve, the thoracic curve and the lumbar curve were improved significantly to $-2.20{\pm}1.40^{\circ}$ and $-1.20{\pm}1.14^{\circ}$, respectively (p<.01). For spinal mobility, the thoracic flexion ($3.40{\pm}2.99^{\circ}$), thoracic extension ($3.50{\pm}1.43^{\circ}$), lumbar flexion ($4.50{\pm}4.74^{\circ}$), and lumbar extension($-1.50{\pm}1.84^{\circ}$) were all significantly improved (p<.05). These findings indicate that thoracic cage mobilization and breathing exercise in combination improve the respiratory function, spinal alignment, and spinal mobility in elderly people with restrictive lung diseases.

A Bayesian approach to replacement policy following the expiration of non-renewing combination warranty based on cost and downtime (비재생혼합보증이 종료된 이후의 비용과 비가동시간에 근거한 교체정책에 대한 베이지안 접근)

  • This paper considers a Bayesian approach to replacement policy following the expiration of non-renewing combination warranty. The non-renewing combination warranty is the combination of the non-renewing free replacement warranty and the non-renewing pro-rata replacement warranty. We use the criterion based on the expected cost and the expected downtime to determine the optimal replacement period. To do so, we obtain the expected cost rate per unit time and the expected downtime per unit time, respectively. When the failure times are assumed to follow a Weibull distribution with uncertain parameters, we propose the optimal replacement policy based on the Bayesian approach. The overall value function suggested by Jiang and Ji (2002) is utilized to determine the optimal replacement period. Also, the numerical examples are presented for illustrative purpose.

  • In order to analyze lateral control in the forward manuever of a tractor- trailer combination , a human operator model and a kinematic vehicle model were utilized for the operator/vehicle system. By combining the vehicle and operator models, a mathematical model of the closed-loop operator/vehicle system was formulated. A computer program was developed so as to simulate the motion of the tractor-trailer combination . In order to verify the operator/vehicle system model, the results of the field trials were compared with the simulated results. There was found to be reasonably good agreement between the two.

  • In this paper we present combined estimator of capture-recapture and line transect model using bivariate detection function and detection probability according to objects being in cluster population. Here bivariate detection function use distance and cluster size. The simulation shows that combined estimator approaches the more true value the larger size parameter. Therefore this estimator using the bivariate detection function is more efficient in estimate the population size and density by size parameter.

  • This paper presents a combination of the generalized Cascade Correlation and generalized Recurrent Cascade Correlation learning algorithms. The new network will be able to grow with vertical or horizontal direction and with recurrent or without recurrent units for the quick solution of the pattern classification problem. The proposed algorithm was tested learning capability with the sigmoidal activation function and hyperbolic tangent activation function on the contact lens and balance scale standard benchmark problems. And results are compared with those obtained with Cascade Correlation and Recurrent Cascade Correlation algorithms. By the learning the new network was composed with the minimal number of the created hidden units and shows quick learning speed. Consequently it will be able to improve a learning capability.

Modelling CO2 and NOx on signalized roundabout using modified adaptive neural fuzzy inference system model

  • Air quality and pollution have recently become a major concern; vehicle emissions significantly pollute the air, especially in large and crowded cities. There are various factors that affect vehicle emissions; this research aims to find the most influential factors affecting $CO_2$ and $NO_x$ emissions using Adaptive Neural Fuzzy Inference System (ANFIS) as well as a systematic approach. The modified ANFIS (MANFIS) was developed to enhance modelling and Root Mean Square Error was used to evaluate the model performance. The results show that percentages of $CO_2$ from trucks represent the best input combination to model. While for $NO_x$ modelling, the best pair combination is the vehicle delay and percentage of heavy trucks. However, the final MANFIS structure involves two inputs, three membership functions and nine rules. For $CO_2$ modelling the triangular membership function is the best, while for $NO_x$ the membership function is two-sided Gaussian.

  • In this paper a simple measurement technique for mechanical-acoustic transfer functions is proposed . The mechanical-acoustic transfer functions, generally , are measured through mechanical excitations ; impact hammers or shakers. Recently , by virtue of vibro-acoustical reciprocity principle, they are measured through acoustic excitations : loudspeakers. This kind of test needs to measure the volume velocity , the radiation characteristics of a sound source. Because the volume velocity of the sound source is changed by driving signal , it is difficult to measure it. However , the new method in this paper needs not to measure the volume velocity of a sound source by combination of mechanical and acoustic excitations. Moreover, this method has the methodological advantages, such as usage of a general loudspeaker for the reciprocal excitation, no sptatial limitations for measurements of mechanical-acoustic transfer functions.

End Stage Renal Disease caused by IgA Nephropathy : One Case Report (IgA 신증에 의한 말기 신질환에 대한 증례보고)

  • This case is to report the effect of renal function of chronic kidney disease(CKD) caused by IgA nephropathy. A 37-year-old man visited a Korean medicine hospital, who has been diagnosed with end stage renal disease(ESRD), 5 stage of CKD, caused by IgA nephropathy, has had no improvement of western medical treatment, and wanted to be treated using Korean medicine before renal transplantation. The decrease of creatinine value, the increase of glomerular filtration rate(GFR), and the decrease of CKD stage (5 to 4) was observed after combination treatment of Ikkigeonbiisuhwalhyeoltang and saam acupuncture was applied. This case report is suggested that combination treatment of acupuncture and herbal medicine could be effective to renal function of CKD in spite of a single case.
