가미소풍활혈탕의 혈소판 응집억제 작용에 대한 in vitro 연구 (Anti-Platelet Aggregation Effect of Extract from Gamisopunghwalheol-tang in Vitro)

  • The study was designed to test anti-platelet effect and find out anti-platelet mechanism of extract from Gamisopunghwalheol-tang in vitro. The extract was investigated for the inhibition against the aggregation of human platelet suspensions induced from collagen by aggregometer. And also the extract was investigated for the inhibition against the aggregation of human platelet suspensions who is taking aspirin or clopidogrel induced from collagen by aggregometer. In collagen-induced platelet aggregation test, the extract significantly inhibited collagen-induced platelet aggregation in a concentration-dependent manner(p<0.05). The extract significantly inhibited collagen-induced platelet aggregation of human platelet who is taking aspirin or clopidogrel ing 0 mg/ml concentration(p<0.05). And the extract inhibited more ingpatients who is taking aspirin. These results show that the extract from Gamisopunghwalheol-tang has anti-platelet aggregation effect.

코리안 숏헤어 고양이의 제한성 심근병증 (Restrictive Cardiomyopathy in a Korean Domestic Short-haired Cat)

  • 3살된 코리안 숏헤어 고양이(몸무게 5.2 kg)가 복수, 흉수, 호흡곤란으로 인해 강원대학교 수의과대학병원에 진료의뢰 되었다. 진단검사상, 유미성 흉수와 출혈성 복수, 심장비대, PW 도플러 초음파와 TDI 초음파의 restrictive filling pattern, 미비하게 비대된 좌심실 자유벽이 관찰되었다. 심장초음파 진단소견을 토대로 제한성 심근병증으로 잠정진단 내리게 되었다. 본 환자는 흉수를 제거하고 furosemide, enalapril, sildenafil, clopidogrel을 포함한 약물치료를 하였다. 이것은 국내에서 처음으로 발표하는 제한성 심근병증의 증례이다.

Risk Factors of Red Blood Cell Transfusion in Isolate off Pump Coronary Artery Bypass Surgery

  • Background: Perioperative transfusion of red blood cell (RBC) may cause adverse effects. Bloodless-cardiac surgery has been spotlighted to avoid those problems. Off pump coronary artery bypass (OPCAB) surgery can decrease the transfusion. However, the risk factors of transfusions in OPCAB have not been investigated properly. Materials and Methods: One hundred and thirteen patients (male:female=35:78, mean age=$66.7{\pm}9.9$ years) who received isolated OPCAB were retrospectively analyzed from March 2006 to September 2007. The threshold of RBC transfusion was 28.0% of hematocrit. Bilateral internal thoracic arteries graft were used for 99 patients (87.6%). One hundred and three (91.1%) and 35 patients (31.5%) took aspirin and clopidogrel just before surgery. Results: Sixty-five patients (47.5%) received the RBC transfusion (mean $2.2{\pm}3.2$ units). Mortality and major complications were not different between transfusion and no-transfusion group. But, ventilator support time, intensive care unit stay and hospitalization period had been reduced in no-transfusion group (p<0.05). In multivariate analysis, patients risk factors for RBC transfusion were preoperative low hematocrit (<37.5%) and clopidogrel medication. Surgical risk factors were longer graft harvesting time (<75 minutes) and total operation time (<5.5 hours, p <0.05). Conclusion: We performed the transfusion according to transfusion guideline; over 40% cases could conduct the OPCAB without transfusion. There were no differences in major clinical results between transfusion and non-transfusion group. In addition, when used together with accurate understanding of transfusion risk factors, it is expected to increase the proportion of patients that do not undergo transfusions.

Transcranial Doppler emboli monitoring for stroke prevention after flow diverting stents

  • Objective: Flow diverting stents (FDS) are increasingly used for the treatment of intracranial aneurysms. While FDS can provide flow diversion of parent vessels, their high metal surface coverage can cause thromboembolism. Transcranial Doppler (TCD) emboli monitoring can be used to identify subclinical embolic phenomena after neurovascular procedures. Limited data exists regarding the use of TCDs for emboli monitoring in the periprocedural period after FDS placement. We evaluated the rate of positive TCDs microembolic signals and stroke after FDS deployment at our institution. Methods: We retrospectively evaluated 105 patients who underwent FDS treatment between 2012 and 2016 using the Pipeline stent (Medtronic, Minneapolis, MN, USA). Patients were pretreated with aspirin and clopidogrel. All patients were therapeutic on clopidogrel pre-operatively. TCD emboli monitoring was performed immediately after the procedure. Microembolic signals (mES) were classified as "positive" (<15 mES/hour) and "strongly positive" (>15 mES/hour). Clinical stroke rates were determined at 2-week and 6-month post-operatively. Results: A total of 132 intracranial aneurysms were treated in 105 patients. TCD emboli monitoring was "positive" in 11.4% (n=12) post-operatively and "strongly positive" in 4.8% (n=5). These positive cases were treated with heparin drips or modification of the antiplatelet regimen, and TCDs were repeated. Following medical management modifications, normalization of mES was achieved in 92% of cases. The overall stroke rates at 2-week and 6-months were 3.8% and 4.8%, respectively. Conclusions: TCD emboli monitoring may help early in the identification of thromboembolic events after flow diversion stenting. This allows for modification of medical therapy and, potentially, preventionf of escalation into post-operative strokes.

Clinical and Echocardiographic Findings for Dilated Cardiomyopathy Induced by Taurine Deficiency in a Cat

  • A 6-year-old male domestic shorthair cat was referred to Gyeongsang National University Animal Medical Center for labored breathing. According to the patient's history, the client had fed him commercial dog foods. The patient's hematological, radiographic, and echocardiographic examinations were evaluated for diagnosis. Echocardiography results showed marked dilations of ventricles and atriums and mitral regurgitation. A systolic dysfunction was detected. Plasma taurine concentration was lower than the reference range. Based on these results, the patient was diagnosed with feline dilated cardiomyopathy associated with taurine deficiency. Treatment included feline commercial foods, taurine, digoxin, furosemide, and clopidogrel. Digoxin was changed to pimobendan when normal blood pressure was achieved. Clinical signs improved gradually and no abnormalities were detected on echocardiograms at 10 weeks following onset of treatment.

Successful long-term management of late-stage precursor-targeted immune-mediated anemia with prednisolone and mycophenolate mofetil in a dog

  • A 13-year-old neutered male Maltese dog presented to our hospital with lethargy and anorexia. Laboratory abnormalities included severe non-regenerative anemia (hematocrit, 12.9%; reticulocyte count 12.8 K/µL). The cytology of bone marrow revealed erythroid hypercellularity with mild myelofibrosis. Therefore, late-stage precursor-targeted immune-mediated anemia (PIMA) was diagnosed. Multimodal treatment including 2 immunosuppressant drugs (prednisolone and mycophenolate mofetil), antithrombic drug (clopidogrel), and blood transfusion was performed. The dog showed complete remission from PIMA, and the total duration of follow-up was 622 days. This is the first case report of canine PIMA managed successfully with prednisolone and mycophenolate mofetil in Korea.

원료의약품 분진의 폭발 위험성 평가 (Explosion Hazard Assessment of Pharmaceutical Raw Materials Powders)

  • 산업현장에서 취급되거나 가공되는 원료의약품 분진의 폭발 위험성은 항상 존재하며, 이로 인한 폭발사고가 자주 발생되고 있다. 본 연구에서는 원료의약품 시료 3종의 분진폭발특성을 측정하였다. 주요 폭발특성 측정값은 록소프로펜산은 평균 입경이 $5.31{\mu}m$이며, $P_{max}$는 8.4 bar, 최소점화에너지는 1 mJ < MIE < 3 mJ이며 최소점화온도는 $550^{\circ}C$이다. 클로피도그렐 캄포르술폰산염은 평균 입경이 $95.63{\mu}m$이며, $P_{max}$는 7.9 bar, 최소점화에너지는 30 mJ < MIE < 100 mJ이며 최소점화온도는 $510^{\circ}C$이었다. 리팜피신은 평균 입경이 $26.48{\mu}m$이며 $P_{max}$는 7.9 bar, 최소점화에너지는 1 mJ < MIE < 3 mJ이며 최소점화온도는 $470^{\circ}C$로 나타났다. 이들 값을 적용하여 폭연지수($K_{st}$)와 폭발지수(EI)의 폭발위험등급을 구하고, 원료의약품 분진의 폭발 위험성을 비교 검토하였다. 그 결과 폭발 위험성은 록소프로펜산과 리팜피신의 폭발등급은 St 2이고 폭발위험등급은 severe이며, 클로피도그렐 캄포르술폰산염의 폭발등급은 St 1이고 폭발위험등급은 strong으로 나타났다.

요크셔테리어 개에서 심장초음파를 이용하여 진단된 심장내 종괴 2 증례 (Echocardiographic Diagnosis of Intracardiac Masses in Yorkshire Terrier Dogs : 2 Cases)

  • 심장에 발생하는 원발성 그리고 전이성 종양은 개에서는 흔치 않다. 우리는 본 연구에서 심장초음파적 진단을 이용하여 요크셔테리어 2마리에서 발생한 심장내 종괴를 확인하였다. 첫째 증례에서의 종괴는 상행 대동맥과 폐동맥 사이에 위치하고 있었으며, 중증도의 대동맥 역류와 좌심실의 확장을 유발하고 있었다. 이 증례의 환자등급은 ISACHC II 등급의 심부전으로 분류되었다. 환자에 대한 심장처방으로는 furosemide, enalapril, pimobendan 그리고 경구적인 화학요법으로는 lomustine가 처방되었다. 둘째 증례에서의 종괴는 좌심방의 2/3를 차지하고 있었으며, 좌심방의 확연한 확장과 심각한 이첨판 역류 (~5 m/s) 를 유발하고 있었으나, 환자의 심부전 등급은 ISACHC Ib 등급으로 심각한 울혈성 심부전은 나타나지 않았다. 종괴의 성상은 cardiac myxoma로 추정되었으나, 조직생검검사는 보호자의 거절로 인하여 수행되지 않았다. 환자에 대한 심장처방으로는 ramipril, clopidogrel 그리고 기관지 확장제는 aminophylline이 처방되었다. 본 증례의 두 환자들은 여전히 생존해 있으며, 정기적인 검진을 받고 있다.
