The Effects of Supervisor Supports on Novice Counselor's Psychological Burnout : The Mediating Effects of the Countertransference Management Ability (수퍼바이저의 지지가 초심상담자의 심리적 소진에 미치는 영향 : 역전이 관리능력의 매개효과)

  • The purpose of this research was to verify the mediating effects of countertransference management ability and each of its subscales, i.e. self-insight, self-integration, empathy ability, anxiety-management, and conceptualizing skills, in the effect of supervisor supports on psychological burnout of novice counselors. For this purpose, 202 novice counselors who have experience of supervision less than 3 years in counseling experience were studied. The results of the analysis were as follows : First, The supervisor supports were positively related to countertransference management skill and all its five subscales. Additionally, supervisor supports, countertransference management skill, and its five subscales were proved to negatively related to psychological burnout. second, The verification of mediating effects of countertransference management skill in the relationship between supervisor support and psychological burnout demonstrated the complete mediation. third, The self-integration, empathy ability, anxiety-management completely mediated the relationship between supervisor supports and psychological burnout. and The self-insight, conceptualizing skills partially mediated the relationship between supervisor supports and psychological burnout. These results confirmed that countertransference management skill is a strong influential factor between supervisor supports and psychological burnout. Implications for clinical practice and directions for future research were also discussed.

Investigation of the Relationships Between Death Recognition, Terminal Care Stress, and Terminal Care Performance in Terminal Hospital Nurses (요양병원간호사의 죽음인식, 임종간호스트레스와 임종간호수행간의 관계 조사)

  • The purpose of this study is to investigate the relationships between death recognition, terminal care stress, terminal care performance, and other influencing factors of terminal care performance in terminal care hospital nurses. Two hundred forty nurses working in 11 hospitals for the elderly located in G, N, C city were surveyed. They were asked to complete three questionnaires: one on death recognition, one on terminal care stress, and a final one on terminal care performance. The data collected were analyzed using descriptive statistics, Pearson's correlation, and stepwise regression using SAS 9.1. The findings suggest that the nurses' own mental and occupational stress was the cause of many difficulties in the practice of end-of-life care, where the nurse will often experience the death patients, which results in increased dysentery stress. There was a negative correlation between species stress and terminal care performance. Death recognition and terminal care stress were correlated with terminal care performance. The results suggest that the longer the clinical experience, the higher the quality of terminal care performance. Therefore, it is suggested that terminal care should be performed considering the longevity of the careers career and their death tendency. Further to this study, it is necessary to find out how to reduce stress and to evaluate other variables affecting the care of the patient.

The Effect of the Periods of Bed Rest on the Postlumbar Puncture Headache in Pediatric Oncology Patients (소아혈액 종양환자의 요추천자 후 침상안정시간이 천자 후 두통에 미치는 영향)

  • Bed rest is recommended to prevent postlumbar puncture headaches(PLPHA), but the period of bed rest varies in the literature from 6 hours to 24 hours. In clinical practice the period of bed rest varies but nursing methods for adults and children have little difference. In Seoul National University Hospital, children have been given at least 6 hours bed rest after a lumbar puncture. Pediatric oncology patients require a lumbar puncture for an initial diagnosis, follow up treatment or administration of chemotherapeutic agent. But it is difficult for young children to lie supine or to refrain from their usual activities in any way, and unpleasant problems related to a shortage of beds often occurs during discharge or in an outpatient setting. The purpose of this study is to substantiate the preventive effect of PLPHA by the period of bed rest, to identify the other factors that influence PLPHA, and to use the nursing methods proper to children. The subjects were 65 children, ages 1-17, undergoing treatment in the children's cancer center at SNUCH during the period June 1, 1995, to Aug. 31, 1995. The team nurses asked questions about PLPHA of the parents and children in order to fill out a questionnaire. The data were evaluated by percent, t-test, Chi-square test and Mann-Whitney U test. Result; 1. There was no significant difference relating the bed rest time spent to the occurrence of postspinal headaches (t-test). 2. There was a significant risk of PLPHA in the children who were irritable before procedure and/or had experienced previous PLPHA(p<0.05, ${x^2}-test$). 3. The following factors were not found to be associated with increased risk of PLPHA: previous puncture experience, giving analgesics, the choice of puncturist, inpatient/outpatient status, gauge of needle, purpose, the amount of CSF removed, gender, diagnosis, the number of peripheral WBCs, previous lumbago experience after LP, position after bed rest, age, the number of aural puncture at the time. A longer period of bed rest is unlikely to be more effective to prevent PLPHA and seems impractical. A shorter period will save time and effort. Perhaps it will also allay some of the fears which surround LP. So 1 hour bed rest after LP is suggested and nursing methods for emotional support should be investigated to reduce PLPH.

  • Despite a recent increased nation's attention given to improving end-life care, we professionals need to be more critical and reflective on our realities surrounding hospice palliative care. The aim of this paper is to suggest that palliative care models can be used for patients/families in the last phase of life and examine whether they are appropriate for caring them in congruence with philosophy of hospice. The hospice experience model (HEM) of Eagan & Labyak and the developmental model of Byock are introduced and examined for their congruence with philosophy of hospice in applying to clinical practice. The HEM as a patient/family value-directed end of life care model emphasizes three principles; unique experience of patient/family, interactions/relationships among multiple dimensions of personhood and between family, and personal growth and development in the face of suffering through a life-completion. The developmental model stipulates dying as the last stage of living, a stage of life cycle in which patients/family may have growth through life-completion in multidimensional relationships of personhood. The model includes the developmental landmarks and tasks for life-completion as the framework to guide a means of professionals' to recognize their opportunity to grow. The landmarks and tasks include worldly and social affair, individual relationships, intrapersonal, and transcendent dimension. The models could work as appropriate palliative care models for patients/families in the last stage of living. The professionals need to be encouraged to apply the models to end of life care setting.

The Effect of Education on Motor Skill Assessment Tool for Occupational Therapists (작업치료사를 위한 운동성 평가도구의 교육효과)

  • Objective : The purpose of this study is to investigate the effect of intensive one-day training on Movement Assessment Battery for Children-2 screening developmental coordination disorder. Methods : We provided intensive one-day training which combined lecture and practice on MABC-2 assessment tools for 33 trainees. Interrater reliability was calculated by using Intraclass correlation coefficient (ICC) with the raw score of results scored simultaneously by experts and trainees of MABC-2. Results : The 33 female trainees with an average of five years and three months of experience and less experience using motor skill assessment tools scored raw score while watching the performance scenes of normal children. The results of the ICC between experts and training participants in MABC-2 obtained a high confidence 0.95 and above (p<.001). Conclusion : Clinicians who participated in one-day training seems to use MABC-2 effectively. Through this, one-day training seems to be effective in acquiring new motor skill assessment tools.

Effects of Nursing Students' Knowledge, Attitude and Nursing Professionalism on Confidence in Performance of Patient Safety (졸업학년 간호대학생의 환자안전 지식, 태도 및 간호전문직관이 환자안전 수행자신감에 미치는 효과)

  • This research study was conducted to investigate the effect of knowledge, attitude and nursing professionalism on the confidence of the performance of nursing students who had experience with clinical practice and also to provide basic data for the development of nursing students' curriculum. The subjects who participated in this study were 286 students in the 4th year of nursing at two colleges in the Daegu and Gyeongbuk regions. The research data was analyzed using the SPSS 22.0 program. Confidence in performance of patient safety was higher for the women than for men, and for the students of an older age and higher academic achievement. Confidence in performance of patient safety was positively related to knowledge (r=.25, p=.000), attitude (r=.39, p=.000), nursing professionalism (r=.33, p=.000) and all these had statistical significance. On the multiple regression analysis, the coefficient of determination ($R^2$) was .49 and the explanatory power of the model was 49.2% (F=24.04, p=.000). The most important factor affecting confidence in performance of patient safety was the experience of having undergone patient safety education. Based on these results, it is necessary to seek various educational methods to expand the concept of patient safety from the beginning of the undergraduate course work. Especially, we think that various education strategies such as simulation education methods or information videos are needed to develop scenarios related to patient safety.

Current Trends in the Treatment of Ankle Arthritis: Analysis of the Korean Foot and Ankle Society (KFAS) Member Survey (족관절 관절염 치료 동향: 대한족부족관절학회 회원 설문조사 분석)

  • Purpose: Variations in the preferred conservative and operative treatment methods for ankle arthritis may be observed in the practice of orthopedic surgeons. This study is based on the Korean Foot and Ankle Society (KFAS) member survey and aims to report the current trend and changes in the management of ankle arthritis over the last few decades. Materials and Methods: A web-based questionnaire containing 24 questions was sent to all KFAS members in July 2021. The questions were mainly related to the preferred techniques and clinical experience in osteotomy, arthrodesis, and total ankle arthroplasty (TAA) for patients with ankle arthritis. Results: Sixty-three out of 550 surgeons (11.5%) responded to the survey. The responses to 6 out of the total of 24 questions (25.0%) achieved the levels needed to reflect a tendency. Answers that showed a tendency were related to the following: a surgical approach for arthrodesis (anterior approach), use of auto-bone graft for arthrodesis (iliac bone), a nonunion rate of more than 10% following arthrodesis, main reason related to unsatisfactory results after arthrodesis (nonunion or delayed union), the main reason to change total ankle prosthesis (unstable supply of prosthesis), the unusualness of revision TAA and conversion of fusion to TAA. Diversity was found in several aspects of treatment (degree of experience and satisfaction in supramalleolar osteotomy, fixation method for arthrodesis, preferred TAA prosthesis and longevity, degree of experinece, annual number of TAA operation. Conclusion: This study proposes updated information with regard to the current trends in the management of ankle arthritis in Korea. Both consensus and variations in the approach to patients with ankle arthritis were identified through this survey. With an increasing preference for TAA, the need for the development of a prosthesis optimized for Koreans and a stable supply of prostheses were the suggestions made by the study.

A Qualitative Study of the Parenting Experience of Adolescents (청소년기 자녀 어머니의 양육경험에 대한 질적 연구)

  • This study was conducted to explore parenting experiences for mothers of adolescent children. Participants in the study were 7 primary caregivers of children in the first to third grades of middle school in the Seoul area, and in-depth interviews were conducted from September to November 2019. After recording the interviews of each participant, the transcripts of verbal words and documents that were the summary of the interview were collected as data. In this study, the participants' perceptions of experiences and actions were analyzed by Smith & Osborn (2003)'s interpretative phenomenological method, which allows researchers to make meanings. As a result of data analysis, 6 major topics and 25 sub-themes were derived. The results derived from the parenting experiences of mothers of adolescents are largely six types: the implications of spending time with their children, unnatural act, the aesthetics of expression, the way to recharge energy, the mental heritage that they want to leave for their children, and resilience. The research results of this study can be presented so that they can learn the healthy responses and interaction of care-givers in a preventive educational dimension, as it provides both generality and specificity of how mothers should interact with their adolescent children in the field of clinical practice.

Investigating Professional Competency and the Needs of Training for Occupational Therapists Using Sensory Integration Interventions (감각통합중재를 사용하는 작업치료사의 실무 역량에 대한 인식 및 교육 요구도)

  • Objective : The aim of this study was to determine the priority competency by analyzing the importance, performance, and educational requirements of occupational therapists to develop a competency in performing sensory integration intervention. Methods : Occupational therapists were surveyed by gender, age, educational background, work location, occupational therapy experience, and sensory integration therapy experience. The difference was investigated through the importance-performance analysis of competency, and the priority of the competency was investigated using the Borich demand analysis method. Results : The therapists recognized professional competency as the most important, whereas performance was the least important. In all sub-competencies, the importance was high, but the performance was low. As a result, the education requirement was highest for professional competency. The importance-performance analysis revealed that professional competency was the area requiring the most urgent improvement. As a result of the Borich demand analysis, statistically significant differences between the importance of all competencies and the actual performance. The most significant difference was found in the professional competency group. Conclusion : The occupational therapists in this study who performed sensory integration interventions recognized professional competency as the most important but most lacking in actual clinical practice. The results of this study will be used as guidelines for developing competency-based sensory integrated intervention curricula.

Research for Recognition Levels of Students in Occupational Therapy on the Function of the Dementia Care Center (치매안심센터 기능에 대한 작업치료 전공 학생들의 인식 수준 조사)

  • The purpose of this study was to investigate the recognition levels of students in occupational therapy on the function of the dementia care center. The study included 357 students in occupational therapy and the study was conducted from September 2019 to December 2019 based on the questionnaire about the function of the dementia care center. The major results of this study were as follows: Among the types of projects of the dementia care center, 'consultation and registration management' scored the highest. There were significant differences in recognition levels of 7 projects of dementia care center according to 'age' among general characteristics. There were significant differences in recognition levels of 6 projects except 'a visiting model' according to 'grade'. There were significant differences in recognition levels of 'rest area business for dementia' and 'support for families with dementia' according to 'region'. There was a significant differences in recognition levels of 'dementia care village' according to 'experience of using dementia care center'. There were significant differences in recognition levels of 'consultation and registration management', 'support for families with dementia', 'dementia care village' according to 'experience of clinical practice'. This study will provide basic data for the study of community based rehabilitation and help to establish the curriculum of occupational therapy.
